Oral Health-related Quality of Life in an Aging Canadian Population

The purpose of the study is to describe the impact of oral health-related quality of life (OHRQoL) on the lives of pre-seniors and seniors living in Nova Scotia, Canada. This cross-sectional study involved 1461 participants, grouped by age (pre-seniors [45-64] and seniors [65+]) and residential status (long-term care facility [LTC] or community). OHRQoL was measured using the 14-item Oral Health Impact Profile questionnaire. Approximately one in four pre-seniors and seniors reported at least one OHRQoL impact ‘fairly/very often’. Of those residing in the community, pre-seniors (28.8%) reported significantly more impacts than seniors (22.0%). Logistic regression revealed that for the community dwelling sample, those who were dissatisfied with their teeth or dentures were 5.16 times more likely to report an impact ‘fairly/very often’, which was the strongest indicator. Among the LTC sample, those who have poor perceived mouth health were 9.87 times more likely to report an impact.

A salutogenic perspective to oral health:sense of coherence as a determinant of oral and general health behaviours, and oral health-related quality of life

Abstract Dental diseases such as dental caries and periodontal disease could well be seen as being behaviour-related. The high prevalence of periodontal disease in the Finnish adult population mirrors the need for improving oral health behaviours in a comprehensive manner. Thus far, scant attention has been drawn to the underlying psycho-social factors that could, in part, explain oral health and oral health behaviours. Deficiencies in oral health behaviour may also be indicative of an individual's poor health behaviour in general. The aim of this study was to introduce the salutogenic approach, called sense of coherence, into the domain of oral health and health behaviour. The present study uses data from the nationally representative Health 2000 survey carried out in 2000–2001 by the National Public Health Institute of Finland. The subjects of this study numbered 4175 in article I, 4131 in article II, 4039 in article III, and 4096 in article IV, and were 30- to 64-year-old dentate men and women. The cross-sectional data was collected via home interviews, self-administered questionnaires, or clinical examinations. Sense of coherence was positively associated with oral health behaviours, such as dental attendance and tooth-brushing frequency. In addition to tooth-brushing frequency, sense of coherence was also positively associated with the level of oral hygiene. The association between sense of coherence and level of oral hygiene weakened only marginally after controlling for tooth-brushing frequency. A strong sense of coherence was strongly associated with a positive oral health-related quality of life (OHIP). Sense of coherence was also associated with all of the OHIP sub-scales, and the association was most evident in the psychological discomfort, psychological disability and handicap sub-scales. Among males, health behaviours seemed multidimensional, whereas they tended to be unidimensional among females. A strong sense of coherence was a common determinant of healthy behaviours in general, as well as of a good subjective health status. The present study recognizes the sense of coherence as a common health-promoting determinant of oral and general health behaviours, good oral health, and a good oral health-related quality of life. The results thus suggest that the role of psycho-social factors should not be underestimated in health promotion.

Factors associated with oral health-related quality of life during pregnancy: a prospective observational study

Methods: A follow-up study was carried out with pregnant Spanish women. Two oral examinations and an oral health-related quality of life (OHRQoL) assessment, using the OHIP-14 questionnaire, were performed in the first and third trimester of pregnancy. Data on sociodemographic characteristics, medical history, O’Sullivan Test measures, oral hygiene habits, history of caries, and periodontal status of participants were collected through structured medical-dental questionnaires. Results: A complete dataset comprising 246 pregnant women was available for analysis. Overall scores for negative impacts on the OHIP questionnaire were significantly higher during late pregnancy (74%). This indicated a deterioration in oral health-related quality of life amongst participants. Items describing “painful aching”, “self-consciousness”, “unsatisfactory diet”, and “interrupted meals” showed the greatest increase between the first and third trimester of gestation. Multivariate analysis showed that pre-gestational/gestational diabetes mellitus and poor oral hygiene habits during the first trimester of gestation were directly associated with worse oral health-related quality of life during the third trimester of gestation (hyperglycemia: OR 2.86; 95% CI 1.019–8.050: p = 0.043 / oral hygiene: OR 1.33; 95% CI 0.970–1.836; p = 0.076). Conclusions: In the present research, hyperglycemia during pregnancy and poor oral hygiene habits during the first trimester of gestation led to a higher risk of poor oral quality of life during late pregnancy. / Ministerio de Ciencia, Innovación y Universidades / Revisión por pares

Poucos estudos têm avaliado a estrutura dimensional do Oral Health Impact Profile-14 (OHIP-14). Análise sobre a dimensionalidade e a adequação do OHIP-14 pode ajudar a melhorar a interpretação deste instrumento. Os objetivos desta tese foram avaliar a dimensionalidade e testar um modelo conceitual de qualidade de vida relacionada à saúde bucal utilizando a escala OHIP-14. A tese foi organizada em 3 manuscritos. O primeiro manuscrito "Comparison of two assessment instruments of the quality of life in older adults" teve como objetivo investigar se existe convergência entre as dimensões da versão abreviada do questionário da Organização Mundial da Saúde sobre Qualidade de vida (WHOQOL-Bref) e do questionário Perfil do Impacto de Saúde Bucal-14 (OHIP-14). Neste estudo, foram avaliados 872 idosos do sul do Brasil. As dimensões dos questionários WHOQOL-Bref e OHIP-14 foram correlacionas por afinidade. Todas as correlações analisadas apresentaram baixa magnitude. Apesar dos questionários WHOQOL-Bref and OHIP-14 apresentarem dimensões relacionadas, eles medem as relações físicas, sociais e psicológicas de maneira diferente. O segundo manuscrito "Oral Health Impact Profile-14: a Unidimensional Scale?" teve como objetivo investigar a estrutura dimensional do OHIP-14. As amostras foram provenientes de dois estudos realizados no Brasil, um no Rio de Janeiro (N=504) e o outro em Carlos Barbosa (N=872). Análises Fatoriais Confirmatória e Exploratória foram realizadas para identificar as dimensões do OHIP-14. As análises fatoriais confirmaram um fator para ambos os estudos. Nossos resultados sugerem que o OHIP-14 é uma escala unidimensional. O terceiro manuscrito "Testing the applicability of a conceptual model of oral health-related quality of life in community-dwelling older people" teve como objetivo testar o modelo conceitual de Wilson e Cleary em relação à qualidade de vida relacionada à saúde bucal. Uma amostra aleatória de 578 idosos do sul do Brasil foi avaliada.O modelo conceitual de Wilson e Cleary foi testado usando a modelagem de equações estruturais, incluindo: edentulismo, sintomas, estado funcional, percepção de saúde bucal, qualidade de vida relacionada à saúde bucal e variáveis sociodemográficas. No modelo final, o edentulismo foi correlacionado com a insatisfação da aparência dos dentes (r = -0,25). O pior estado funcional foi correlacionado com pior percepção de saúde bucal (r = 0,24). A idade teve um efeito direto no OHIP-14 (r = -0,15). Houve um efeito indireto do sexo no OHIP-14 através do estado funcional (r = 0,12).Os resultados apresentados mostram que, para idosos brasileiros, variáveis como sexo e idade desempenham um papel importante para o entendimento conceitual de qualidade de vida relacionada à saúde bucal. Nossos resultados sugerem que o OHIP-14 não avalia o impacto das condições bucais na qualidade de vida numa perspectiva multidimensional, mas em uma única dimensão. The third manuscript "Testing the applicability of a conceptual model of oral health-related quality of life in community-dwelling older people" aimed to test Wilson and Cleary's conceptual model in relation to oral health-related quality of life. A random sample of 578 elderly Southern-Brazilians was evaluated. Wilson and Cleary's conceptual model was tested using structural equations modeling including: edentulism, symptom status, functional health, oral health perceptions, oral health-related quality of life, and sociodemographic variables. In the final model, edentulism was negatively correlated to dissatisfaction of appearance of their dental prostheses (r= -0.25). The worse functional status was correlated with poor oral health perception (r= 0.24). Age had a direct effect on OHIP-14 (r= -0.15). There was an indirect effect of sex on OHIP-14 via functional status (r= 0.12). The results showed that for elderly Brazilians variables such as sex and age are important in the conceptual understanding of oral health-related quality of life. Our findings suggest that the OHIP-14 may not evaluate the oral impact on quality of life on a multidimensional perspective, but in a single dimension. The present thesis demonstrates that there are direct and mediated pathways between clinical and nonclinical variables in relation to oral health-related quality of life.

The change in oral health related-quality of life among adolescents and their families after orthodontic treatment

Objective: Assess the changes in oral health-related quality of life (OHRQoL) of adolescents and their parents after overjet reduction. Materials and Methods: 53 patients between the ages of 11-18 years with increased dental overjet (≥ 6mm) and their parents were selected, of which 28 were pre- and 25 were post-treatment with dental overjet reduced to within normal limits. The data collection instrument was the Child Oral Health Quality of Life (COHQoL) Questionnaire. Results: Adolescents and their parents reported poorer quality of life before orthodontic treatment than after. The improvement in oral health-related quality of life was statistically significant for all health domains except for the social well-being domain. Parental reports on (OHRQoL) were in agreement with their children’s. No statistically significant differences were evident in (OHRQoL) between pre- and post-treatment groups. Conclusions: Adolescents with increased dental overjet ≥6mm experienced substantial psycho-social impacts. Adolescents with increased overjet can accurately recall the initial negative effects of the original malocclusion on their lives, even after a time lapse of five years. Orthodontic treatment significantly improves the perceived quality of life of orthodontic patients and their parents

Der Zusammenhang zwischen mundgesundheitsbezogener Lebensqualität und Depression bei prothetischen Patienten im Vergleich zur Allgemeinbevölkerung

In der vorliegenden Untersuchung sollte ein möglicher Zusammenhang zwischen mundgesundheitsbezogener Lebensqualität (MLQ) und Depression untersucht und bei prothetischen Patienten und Personen der Allgemeinbevölkerung vergleichend betrachtet werden. Es handelt sich um eine Querschnittsstudie, in welche 311 zahnärztlich-prothetische Patienten und 811 erwachsene Probanden der Bundesrepublik Deutschland einbezogen wurden. Zur Erfassung von MLQ und Depression wurden als standardisierte Instrumente zum einen die deutsche Version des Oral Health Impact Profile (OHIP) und zum anderen das Vereinfachte-Beck-Depressions-Inventar (BDI-V) eingesetzt. Die statistischen Zusammenhänge der Konstrukte wurden mit Hilfe von Korrelationsanalysen sowie Strukturgleichungsmodellen ermittelt. Die konfundierenden Variablen Alter und Geschlecht wurden dabei kontrolliert. In beiden Populationen wurde ein signifikanter Zusammenhang zwischen Depression und MLQ festgestellt. Diese gegenseitige Beeinflussung zwischen MLQ sowie deren Dimensionen und der Depression war jedoch zwischen den beiden Probandengruppen signifikant unterschiedlich stark ausgeprägt. Bei den prothetischen Patienten war der Zusammenhang nur halb so stark ausgeprägt wie bei den Personen der Allgemeinbevölkerung. Die OHIP-Dimension „Psychosozialer Einfluss“ der MLQ korrelierte in beiden Probandengruppen signifikant stärker mit Depression als die anderen Dimensionen. Folglich könnte sie auf eine mögliche Depression hinweisen und als zahnärztliches Diagnostikum eingesetzt werden. Zudem legt diese Studie nahe, dass eine eingeschränkte MLQ möglicherweise erst zeitversetzt zu depressiven Symptomen führen kann.

No modelo final, o edentulismo foi correlacionado com a insatisfação da aparência dos dentes (r = -0,25). O pior estado funcional foi correlacionado com pior percepção de saúde bucal (r = 0,24). A idade teve um efeito direto no OHIP-14 (r = -0,15). Houve um efeito indireto do sexo no OHIP-14 através do estado funcional (r = 0,12).Os resultados apresentados mostram que, para idosos brasileiros, variáveis como sexo e idade desempenham um papel importante para o entendimento conceitual de qualidade de vida relacionada à saúde bucal. Nossos resultados sugerem que o OHIP-14 não avalia o impacto das condições bucais na qualidade de vida numa perspectiva multidimensional, mas em uma única dimensão. A presente tese demonstrou que há caminhos diretos e mediados entre as variáveis clínicas e não clínicas em relação à qualidade de vida relacionada à saúde bucal. / A few studies have evaluated the dimensional structure of Oral Health Impact Profile-14. Further analysis on the dimensionality and the adequacy of OHIP-14 can help improve the interpretation of this instrument. The objectives of this thesis were to assess the dimensionality and testing a conceptual model of oral health-related quality of life using the OHIP-14 scale. The thesis was organized in three manuscripts. The first manuscript "Comparison of two assessment instruments of the quality of life in older adults" aimed to investigate if there is convergent validity between the dimensions of World Health Organization Quality of Life Questionnaire-Brief Version (WHOQOL-Bref) and Oral Health Impact Profile-14 (OHIP-14) questionnaires. In this study, 872 elderly Southern-Brazilians were evaluated. The dimensions of WHOQOL-Bref and OHIP-14 questionnaires were correlated by affinity. All correlations analyzed had a low magnitude. Despite the fact that WHOQOL-Bref and OHIP-14 instruments have related dimensions, they measure physical, psychological and social relations differently. The second manuscript "Oral Health Impact Profile-14: a Unidimensional Scale?" aimed to investigate the dimensional structure of the OHIP-14. Subjects were from Rio de Janeiro (N=504) and Carlos Barbosa (N=872) Studies in Brazil. Exploratory and Confirmatory Factor Analysis were performed to identify the dimensions of OHIP-14. The factor analysis confirmed one factor in both studies. Our findings suggest that the OHIP-14 is a unidimensional scale. The third manuscript "Testing the applicability of a conceptual model of oral health-related quality of life in community-dwelling older people" aimed to test Wilson and Cleary's conceptual model in relation to oral health-related quality of life. A random sample of 578 elderly Southern-Brazilians was evaluated. Wilson and Cleary's conceptual model was tested using structural equations modeling including: edentulism, symptom status, functional health, oral health perceptions, oral health-related quality of life, and sociodemographic variables. In the final model, edentulism was negatively correlated to dissatisfaction of appearance of their dental prostheses (r= -0.25). The worse functional status was correlated with poor oral health perception (r= 0.24). Age had a direct effect on OHIP-14 (r= -0.15). There was an indirect effect of sex on OHIP-14 via functional status (r= 0.12). The results showed that for elderly Brazilians variables such as sex and age are important in the conceptual understanding of oral health-related quality of life. Our findings suggest that the OHIP-14 may not evaluate the oral impact on quality of life on a multidimensional perspective, but in a single dimension. The present thesis demonstrates that there are direct and mediated pathways between clinical and nonclinical variables in relation to oral health-related quality of life.
