Universidade Federal do Cearà / A presente investigaÃÃo trata da anÃlise de dimensÃes concernentes à integralidade e à humanizaÃÃo do cuidado, no interior da EstratÃgia SaÃde da FamÃlia, a partir das percepÃÃes dos diferentes atores implicados, no MunicÃpio de Fortaleza-CE. A EstratÃgia SaÃde da FamÃlia â ESFsurgiu no cenÃrio nacional como instrumento para aumentar os nÃveis de eqÃidade do SUS â Sistema Ãnico de SaÃde e, principalmente, para alteraÃÃo do modelo de atenÃÃo à saÃde historicamente hegemÃnico. O espaÃo de observaÃÃo deste estudo compreende o MunicÃpio de Fortaleza, capital do Estado do CearÃ, em funÃÃo da expressiva ampliaÃÃo das equipes de SaÃde da FamÃlia e da cobertura da EstratÃgia SaÃde da FamÃlia e incorporaÃÃo de um conjunto de aÃÃes voltadas à efetiva alteraÃÃo do modelo assistencial. A proposta metodolÃgica desta investigaÃÃo se inscreve na abordagem qualitativa, alinhando-se ao enfoque crÃtico-interpretativo, uma vez que toma a subjetividade na interface com a materialidade a que se vincula, dialeticamente. Para a apreensÃo das narrativas dos informantes, foram realizadas entrevistas em profundidade, com conteÃdo gravado apÃs seu consentimento. Em momento posterior, essas entrevistas foram transcritas. TrÃs temas centrais constituÃram nossa Rede de Significados: A) Processo de transiÃÃo em Fortaleza; B) Aspectos da organizaÃÃo do trabalho no cuidado em saÃde; C) Qualidade do cuidado.Os resultados apontaram algumas incongruÃncias entre os aspectos organizativos/normativos do Sistema Ãnico de SaÃde e seus princÃpios doutrinÃrios, interpondo-se à efetiva alteraÃÃo do modelo assistencial e a consecuÃÃo de prÃticas de cuidado humanizadoras. Destacamos, tambÃm, o predomÃnio de prÃticas nÃo resolutivas na atenÃÃo bÃsica, atuando na geraÃÃo de padrÃes ineficientes de utilizaÃÃo dos serviÃos. No que tange Ãs competÃncias profissionais, foi possÃvel compreender as motivaÃÃes originais que conduzem as categorias profissionais na busca por seu espaÃo nos cenÃrios das prÃticas em saÃde. Quanto à integralidade do cuidado e à humanizaÃÃo, fez-se presente o reconhecimento da multidimensionalidade do sujeito e a predisposiÃÃo para uma compreensÃo ampliada acerca de suas necessidades, mas, por outro lado, foram apontados elementos normativos que cerceiam a prÃtica e produzem o afastamento de trabalhadores e de usuÃrios. Concluindo, ainda que seja necessÃrio criar condiÃÃes institucionais para que inÃmeras mudanÃas tenham um solo que lhes assegure viabilidade, consideramos corajosa a iniciativa de transformaÃÃo da assistÃncia no MunicÃpio de Fortaleza, caracterizando um bom comeÃo para empreitada de tal envergadura. / he present investigation deals with the analysis concerning the dimensions of completeness and humanization of care within the Family Health Strategy, through the perceptions of different actors involved in the city of Fortaleza-CE. The Family Health Strategy - ESFsurgiu on the national scene as a tool to increase levels of equity SUS - Unified Health System, and especially to change the model of health care historically hegemonic. The space observation of this study includes the city of Fortaleza, capital of Cearà State, due to the significant expansion of the Family Health teams and coverage of the Family Health Strategy and the incorporation of a set of effective actions aimed at changing the model care. The proposed methodology of this research falls in the qualitative approach, aligned with the critical-interpretive approach, since it takes the subjectivity at the interface with the material with which it binds, dialectically. For the apprehension of the narratives of the informants were interviewed in depth, content recorded after informed consent. Thereafter, these interviews were transcribed. Three main themes constitute our Network of Meanings: A) Transition process in Fortaleza, B) aspects of work organization in health care; C) Quality cuidado.Os results showed some inconsistencies between organizational aspects / regulations of the Unified Health and its doctrinal principles by bringing to the change in the effective model of care and the achievement of humanizing care practices. The article also highlights the predominance of non-resolving practices in primary care, working in the generation of inefficient patterns of use of services. In terms of professional skills, it was possible to understand the unique motivations that lead to the professional categories in the search for her place in the scenarios of health practices. As for the comprehensive care and humane, it was this recognition of the multidimensionality of the subject and the predisposition to a broader understanding about their needs, but on the other hand, were identified regulatory elements that limit the practice and produce the dismissal of workers and users. In conclusion, although it is necessary to create conditions so that many institutional changes have to be secure in a solo viability, we consider the bold initiative to assist in the transformation of the city of Fortaleza, featuring a good start to venture on such a scale.
