Proposta de um método de projeto de próteses de membros superiores com a utilização da engenharia e análise do valor. / Proposal of an upper limb prosthesis design method, which uses the value analysis and engineering.

As próteses de membros superiores atualmente disponíveis no mercado não atendem a muitos dos requisitos dos seus usuários, em especial devido à funcionalidade insuficiente e a seu alto peso e custo. Este fato, aliado à importância do assunto, foi a motivação deste trabalho, que utiliza a Análise do Valor no projeto deste tipo de próteses. Para isto, propõe uma metodologia de projeto, que utiliza a mencionada Análise, e aplica a mesma às próteses de membros superiores com o objetivo de obter configurações das mesmas que contenham vantagens comparando-se com as disponíveis atualmente. A Análise do Valor trabalha com funções, e por isto, as próteses, neste trabalho, foram vistas através das funções que deveriam executar. Para a obtenção destas funções e das suas especificações, foram consultados usuários de próteses, de quem as opiniões foram consideradas as mais importantes. Após estas consultas, foi executado um brainstorming com voluntários para a obtenção de idéias de novos componentes e novos tipos de próteses. A partir do resultado das entrevistas com usuários e do brainstorming, e com base nas pesquisas realizadas, foram propostas novas configurações de próteses que procurassem atender ao máximo possível das especificações que tinham sido obtidas. Em suma, esta dissertação pretende fornecer mais subsídios para se entender o que é necessário para que as próteses de membros superiores tenham uma aceitação maior e auxiliem mais seus usuários. A metodologia proposta, por sua vez, mostrou que tem potencial para ser útil na obtenção de produtos cada vez melhores, visto que ela pode ser aplicada a produtos em geral, não somente às próteses. / The available upper limb prosthesis in the market at this moment don’t correspond to many of the users’ requirements, mainly because of the insufficient functionality and the high weight and cost. This fact, allied to the importance of the subject, was the motivation of this work, which uses the Value Analysis in this kind of prosthesis design. For this purpose, it proposes a design method, which uses the mentioned Analysis, and applies it to the upper limb prosthesis aiming to obtain this product configurations with advantages, comparing to the available prosthesis at this moment. The Value Analysis works with functions, and, because of that, the prosthesis, in this work, were seen through the functions they should execute. To obtain these functions and its specifications, prosthesis users were consulted – their opinions were considered the most important. After these consultations, it was executed a brainstorming with volunteers to obtain new prosthesis’ kinds and components ideas. With the interviews and brainstorming results, and supported by the research made, they were proposed new prosthesis configurations, which had the goal to correspond to the maximum possible of the obtained specifications. In short, this dissertation intend to provide more subsidies to comprehend what is necessary to the upper limb prosthesis to have a bigger acceptance and help more its users. The proposed design method, at its side, showed that has potential to be useful in the more and more better products obtaining, because it can be applied to the products in general, not only to the prosthesis.

Problematika využití protetických pomůcek u dětí / Deploying Protheses for Children

This thesis named: "Problems of usage of prosthetic aids for children" focuses on the nursing care specifics of the child patient after an amputation. The Amputation is defined as removal of the peripheral portion of the body with an interruption of a bone and the soft tissues which allows subsequent prosthetic solution. Prosthesis is externally applied aid, which replace missing or underdeveloped part of a limb eventually the whole limb. The aim of the Prosthetic centers is to offer to its clients the exact prosthetic solution which could fully replace the function of a missing limb and allow the child to live full life without any limitations. The intention of the thesis was to summarize the problematic of the upper and lower limbs amputation into one publication and also to focus this thesis on children and prosthetic aids for them. The reason is that in Czech literature there is no complex information which focuses on specifics for children. The list of synoptic distribution of the types of prosthetic aids was created based on the level of amputation. A large part of this thesis deals with the nursing care, including rehabilitation after amputation and prosthetic solutions. The possibility of the children´s active life with prosthetic aids was mentioned, also how they coped with the loss of a limb. And then the specifics of the nursing care and the attitude of the nursing staff to children and their parents were mentioned too. In the research part of the thesis three aims were set: Find out the limitations or the changes in the family´s life style which has child with prosthesis and the limitations in child´s active life with prosthesis. Ascertain the specifics and the lacks of the nursing care during preparation of children for the prosthetic aid. Based on these following aims the research questions were set: How was the life style of the family with child with prosthetic aid affected? What are the limitations in child´s active life with prosthesis? What are the specifics of the nursing care during preparation of the child for the prosthetic aid? The empirical part of my thesis was processed with qualitative data analysis using technique of the unstructured in-depth interviews. The first research group consisted of seven respondents. Five out of them were parents of the children without the limb and two out of them were children without the limb itself, who were old enough to give us the trustworthy information. The information from the prosthetic technicians was followed with these interviews. The prosthetic technicians clarified us the questions which resulted from reaching the respondents who cannot have all theoretical knowledge regarding the issue. We hope that the results from this thesis will be benefit not only for the medical staff to improve the quality of the nursing care, but also for the respondents who can find the information which was missing till this time.

Estudo das ligas tit?nio-zirc?nio resultantes do processo de fundi??o plasma-skull para aplica??es como biomateriais

The first stage was devoted to the fabrication of tablets from these alloys by Plasma-Skull casting method using a Discovery Plasma machine from EDG Equipamentos, Brazil. In a second stage, the chemical composition of each produced tablet was verified. In a third stage, the specimen were submitted to: as-cast microstructure analysis via optical and scanning electron microscopy (OM and SEM), x-ray dispersive system (EDS) chemical analysis via SEM, Vickers hardness tests for mechanical evaluation and corrosion resistence tests in a 0.9% NaCl solution to simulate exposition to human saliva monitored by open circuit potential and polarization curves. From the obtained results, it was possible to infer that specimens A1 (94,07 wt% Ti and 5,93% wt% Zr), A4 (77,81 wt % Ti and 22,19 wt % Zr) and A8 (27,83 wt% Ti and 72,17 wt% Zr), presented best performance regarding to corrosion resistance, homogeneity and hardness which are necessary issues for biomaterials to be applied as orthopedic and odontological prosthesis / Este trabalho teve como finalidade estudar uma s?rie de onze ligas met?licas bin?rias, tit?nio zirc?nio, quanto ? capacidade de resistirem ?s deteriora??es sem sofrerem modifica??es de suas propriedades iniciais de massa e superf?cie quando expostas a meios agressivos, como condi??o fundamental para que as mesmas sejam aplicadas como biomateriais. A primeira etapa tratou da confec??o das amostras das ligas em forma de pastilhas, empregando-se o processo de fundi??o Plasma Skull realizado na m?quina Discovery Plasma, produzida no Brasil pela EDG Equipamentos. Na segunda etapa, as amostras confeccionadas foram submetidas aos ensaios para a determina??o da composi??o qu?mica resultante de cada liga da s?rie ap?s a fus?o. E durante a terceira etapa foram feitos os procedimentos de avalia??es das amostras quanto ? homogeneidade e apresenta??o da microestrutura de cada uma das ligas no estado bruto de fus?o, como resultados dos ensaios metalogr?ficos por microscopia ?ptica e microscopia eletr?nica de varredura acoplada a espectroscopia por energia dispersiva de raios-x; quanto ? propriedade mec?nica de dureza medida em escala Vickers (HV); e quanto ?s propriedades qu?micas de resist?ncia ? corros?o, quando expostas em solu??o de cloreto de s?dio 0,9 %, para simular a saliva, empregando-se o monitoramento do potencial de circuito aberto e as curvas de polariza??o.
