Physicians’ Work Environment and Health : A Prospective Controlled Intervention Study of Management Development Programs Targeting Female Physicians / KUPA : Kvalitet, utveckling och psykosocial arbetsmiljö

During the last decade, there has been a renewed interest in physicians´ working conditions at a time when health care is undergoing major structural and financial changes. Physicians report decreasing work satisfaction and at the same time increasingly contemplate on leaving their profession. Despite an increasing proportion of female physicians, they are underrepresented in management positions. Very often, female physicians also report worse personal health and less organisational influence than their male colleagues. Management programs have been viewed as an important vehicle to improve working conditions and career development for female physicians. The aim of the present thesis was to evaluate the effects on individual health, professional development, perception of organisational structure and career development. Fifty-two female physicians participated in structured 1-year management programs. The control group consisted of 52 physicians not participating in any formal management education during the study period. The female physicians were compared with 157 male physicians in order to evaluate possible gender and manager-reported differences on individual and organisational well-being. The study also assessed occupational predictors of individual well-being, health and professional development for managers. Participants in management programs reported a clearer organisational structure and improved professional development and influence. However, there were no significant effects with regard to health, sickness absenteeism, nor career development. Male and female physicians differed in several areas when assessed as a group, but stratifying for management level, most of the gender-related differences disappeared. When managers rated clearer organisational structure, this was associated with higher ratings on individual well-being and professional development. Management programs for female physicians might have some beneficial effects but these programs need to be better defined and targeted to the most important issues at hand to recruit future managers. Furthermore, these programs must be better integrated with other processes to create efficient and able managers.

Nustatyti šeimos gydytojų teikiamų akušerinės – ginekologinės pagalbos paslaugų apimtį pirminėje sveikatos priežiūros grandyje. 3. Nustatyti problemas, su kuriomis susiduria šeimos gydytojai, teikdami akušerinės – ginekologinės pagalbos paslaugas savo bendruomenės moterims. Tyrimo metodika. Tyrimo objektas: Kauno mieste dirbančių šeimos gydytojų požiūris į jų teikiamas akušerinės – ginekologinės pagalbos paslaugas. Tyrimo metodai: Anoniminė anketinė šeimos gydytojų apklausa. Anketa išplatinta visiems Kauno miesto šeimos gydytojams (n = 220). Atsakas – 90,9 proc. Statistinė duomenų analizė atlikta SPSS – 13 programa. Rezultatai. Moterų piktybinių navikų prevencijos vykdymo ir moterų raštingumo lytinės higienos ir šeimos planavimo klausimais priskyrimą šeimos medicinos institucijai teigiamai vertino 96 proc. visų respondentų, nėščiųjų stebėsenos - 58 proc., ginekologinių paslaugų - 64 proc. Privačiose pirminės sveikatos priežiūros įstaigose ( PSPĮ) dirbančių šeimos gydytojų teikiamų akušerinių – ginekologinių paslaugų apimtis (išskyrus klimakterinių simptomų diagnostiką) yra didesnė nei VŠĮ dirbančių šeimos gydytojų. To evaluate family physicians’ attitudes towards obstetric-gynecological services provided in primary health care settings. Objectives. 1. To evaluate family physicians’ attitudes towards changes in the organization of obstetric-gynecological servises. 2. To determine the extent of obstetric-gynecological services provided by family physicians in primary health care level. 3. To identify the problems family physicians encounter when providing obstetric-gynecological services . Methods. Anonymous questionnaire-based survey of family physicians was carried out in Kaunas city (n = 220). The response rate was 90.9%. Statistical data analysis was performed using SPSS v.13 software package. Results. The attribution of cancer prevention among women and women’s literacy in sexual hygiene- and family planning-related issues to the institution of family medicine was positively evaluated by 96% of respondents, the attribution of the monitoring of pregnant women – by 58% and the attribution of gynecological services – by 64% of respondents. The extent of obstetric-gynecological services (except for the diagnostics of menopausal symptoms) provided by family physicians working in private primary health care institutions (PHCI) exceeded that provided by family physicians working in public PHCI. Time and cost constraints are forcing marketers to search for new ways to maintain and grow brand awareness. The amount of time that doctors allow for representatives to market their products is becoming less and less and as a result products are not getting the exposure they used to. Organisations that find innovative ways to maintain exposure of their products may gain a competitive advantage over those organisations that rely on traditional marketing methods. The prolific increase in the use of the internet may provide pharmaceutical organisations with a complementary channel to market their products. The objective of this study is to determine the levels of internet technology adoption among general practitioners. The Technology Acceptance Model (TAM) is one of the most influential research models in studies of the determinants of information systems/ information technology (IS/IT) acceptance. In TAM, perceived usefulness and perceived ease of use are hypothesised and empirically supported as fundamental determinants of user acceptance of a given IS/IT. Using TAM, this study sets out to determine general practitioners’ intention to use the internet as a source of pharmaceutical information. The literature review provides an extensive evaluation on the development of TAM and its application in different technologies. Based on these findings, the researcher developed this study to investigate internet technology acceptance in the pharmaceutical industry. Use is made of the descriptive survey method and data is retrieved from a sample of 105 general practitioners in Kwazulu-Natal. The observation is made via the benefit of a questionnaire. The process of sampling is that of convenient sampling. The analysis is quantitative and makes use of statistical analysis appropriate for the data. Analysis of the survey results produces useful insights into the factors influencing internet technology adoption by general practitioners. When analysing the independent variables, respondents were not in strong agreement about the perceived usefulness nor the perceived ease of use of internet technology as a source of pharmaceutical information. However, positive results from the relationships between the independent (perceived usefulness and perceived ease of use) and dependent variables (attitude and intention to use) provide valuable data with which organisations may develop internet-based marketing strategies. Based on the survey findings, recommendations using the Beynon-Davies (2004) Internet Adoption Model are suggested.

Perceptions of knowledge transfer of foreign African doctors practicing in South African provincial hospitals.

The study examined the factors affecting the African foreign doctors' perception of knowledge transfer with special reference to South African provincial hospitals. The influence of three organisational factors (Interpersonal relationships, Language & communication and organisational culture) and the demographic variables on knowledge transfer were assessed. From these variables four hypotheses were formulated and tested. The study employed a cross-sectional study and a total of 62 African foreign doctors practicing in South African provincial hospitals completed a structured questionnaire. The findings indicated that interpersonal relationships, language and communication as well as organisational culture influenced knowledge transfer. Also, there was a variation on the influence of language and communication on knowledge transfer among different age groups in the organisation. Based on the research findings the results were discussed and compared and contrasted to previous research and the literature review.
