Doença cronica, autocuidado e autonomia : estudo de um processo educativo junto a pacientes com Diabetes Melito tipo 2

A alta prevalencia do diabetes, aliada A possibilidade de complicações tardias muitas vezes incapacitantes e ao fato de se constituir em doença cronica que exige dos pacientes grande parcela de cuidados tornam necessAria uma abordagem educativa que facilite o lidar cotidiano com a doença. A vivencia profissional com pacientes diabeticos acompanhados no ambulatorio de um hospital-escola evidenciou uma baixa autonomia da maioria dos portadores de diabetes tipo 2, muitas vezes associada A escassez de conhecimentos sobre a doença e seus cuidados, motivando a proposição de um trabalho educativo que privilegiasse a escuta e o vInculo profissional de saUde¿paciente. O estudo foi realizado junto a usuArios do Ambulatorio de Diabetes tipo 2 da disciplina de Endocrinologia do Hospital das Clinicas da Unicamp, no periodo de maio de 2001 a dezembro de 2002. Envolveu catorze pacientes de 48 a 63 anos, sendo onze mulheres, com aproximadamente 4 anos de escolaridade e com mais de 10 anos de doença diagnosticada. Os participantes foram entrevistados no inIcio e ao tErmino do processo educativo. Esse orientou-se pelas necessidades expressas pelos pacientes por ocasiAo da primeira entrevista e ao longo do trabalho, apoiado na interação dialogica entre profissional/educador e os sujeitos do processo. Privilegiou-se a abordagem individual, a que se associaram atividades em grupo sob a forma de oficinas de nutrição. Na analise do processo educativo utilizou-se duas categorias, a relação dos pacientes com a doença e os cuidados que ela exige, e a autonomia/autocuidado, privilegiando a apropriação de conhecimentos sobre o cuidado alimentar, a insulinoterapia e a monitorização glicemica, assim como sua utilização no cotidiano. O processo educativo propiciou um repensar na relação dos pacientes com a doença e nas concepções relativas ao seu cuidado, em especial quanto A alimentação e A monitorização da glicemia capilar. A apropriação de conhecimentos sobre o diabetes, como aprendizado significativo para os pacientes, e sua repercussão no autocuidado foram observadas, ainda que de forma não homogenea entre os participantes. Secundariamente, observou-se melhora do controle metabolico dos pacientes do estudo. O estudo espera contribuir para a discussão a respeito da importancia da abordagem educativa no cuidado das doenças cronicas, em particular do diabetes, e, dentre as diferentes possibilidades de realiza-la, valorizando a questAo do vInculo e da escuta, de forma a propiciar uma melhor relação dos pacientes com a doença e uma ampliação do autocuidado / Abstract: This paper consists of the analysis of a patient centered educational process that seeks to increase the autonomy and self-care of Type 2 diabetes mellitus patients. An educational approach to enable an easier daily management of the disease is needed, due to the high prevalence of diabetes, the possibility of late and sometimes incapacitating complications and the fact of it being a chronic disease that demands a lot of care from the patient.The close professional contact with diabetic patients in the outpatient program of a school hospital, showed the low dimension of autonomy of the majority of Type 2 diabetes patients, many times associated to the lack of knowledge of the disease and its care. This motivated the elaboration of an educational paper that prioritized listening and the connection of healthcare professional-patient.The study was performed between May 2001 and December 2001, with patients from the Type 2 diabetes outpatient department of the endocrinology discipline of the Hospital das Clinicas of the Unicamp. Fourteen patients ages 48 to 63 were studied, their diabetes had been diagnosed for over 10 years and they had approximately four years of schooling. Eleven were female.The participants were interviewed at the beginning and at the end of the educational process. The patient¿s requirements were evaluated on the first interview and throughout the study, focusing on the dialogue between the professional educator and the subjects of the study. Individual approach was prioritized and group activities were also performed via nutrition workshops.Two categories were used to analyze the educational process, the relationship of the patients with their disease and the care required by the disease and the autonomy/self-care. Special focus was given to the patient¿s knowledge of the daily management of their diet, insulin therapy and blood glucose monitoring. The educational process led to a rethinking of the relationship of patients with their disease and of the concepts related to its care, mainly of the diet and blood glucose monitoring. Participants showed, though not in a homogenous way, improved knowledge of diabetes and of the importance of self-care. On a secondary level there was improvement of their metabolic control.This study hopes to contribute to a discussion about the importance of the educational approach in the care of chronic diseases, especially of diabetes, and of the different ways of management, with emphasis on the relationship and listening issues, providing a better relationship of the patients with their disease and improved self-care / Mestrado / Saude Coletiva / Mestre em Saude Coletiva

A angioplastia coronariana transluminal percutânea é um dos métodos mais comumente utilizados para o tratamento da mesma. Avanços tecnológicos possibilitaram a realização de procedimentos mais seguros e com reduzida permanência no ambiente hospitalar. Entretanto, ainda há a necessidade de intervenções educativas em relação aos fatores de risco modificáveis que contribuam para mudanças comportamentais, adesão ao tratamento e melhoria da qualidade de vida relacionada à saúde. Este estudo buscou avaliar a qualidade de vida relacionada à saúde em pacientes submetidos à angioplastia coronariana transluminal percutânea, utilizando-se as variáveis do questionário SF-36, e desenvolver folhetos explicativos sobre a angioplastia coronariana e sobre hábitos saudáveis de vida. Foram incluídos 45 pacientes internados na enfermaria de Clínica Médica do Hospital das Clínicas da Faculdade de Medicina de Botucatu – UNESP e que realizariam o procedimento pela primeira vez. A coleta de dados ocorreu em dois momentos no período de Janeiro a Maio de 2012. Os pacientes foram avaliados quanto à qualidade de vida antes (M1) e 30 a 40 dias após a angioplastia coronariana (M2). Foram elaborados folhetos explicativos sobre ACTP e sobre hábitos saudáveis de vida, que auxiliaram na orientação destes pacientes. Os participantes tinham em média 60,8 anos, eram predominantemente do sexo masculino (60%) e possuíam baixa escolaridade (média de 5,1 anos de estudo). Dentre os principais fatores de risco e comorbidades destacaram-se a hipertensão arterial sistêmica (77,8%), sedentarismo (71,1%) e dislipidemia (60%). As medianas dos escores do SF-36 foram maiores no M2, com diferenças estatisticamente significantes em todos os domínios, exceto Aspectos Emocionais. Percutaneous transluminal coronary angioplasty (PTCA) is one of the most commonly used methods for treating such disease. Technological advances have made possible to perform safer procedures with reduced stay in the hospital. However, educational interventions are still needed concerning modifiable risk factors that lead to behavioral changes, treatment adherence, and better quality of life in terms of health. This study aimed at assessing the quality of life in terms of health in patients submitted to percutaneous transluminal coronary angioplasty by using the variables of the SF-36 questionnaire and developing leaflets on coronary angioplasty and healthy lifestyles. Forty-five patients hospitalized in Botucatu Medical School Hospital – UNESP who were undergoing the procedure for the first time were included in the study. The data collection occurred twice from January to May 2012. The patients were evaluated regarding their quality of life before the coronary angioplasty (M1) and from 30 to 40 days after it (M2). Leaflets on PTCA and healthy lifestyles were designed to assist guiding such patients. The participants were in average 60.8 years old, mainly male (60%), and had low educational backgroun (in average, 5.1 years of study). Intervenção psicoeducativa e parâmetros imunológicos em pacientes HIV+

This study was designed to observe the effect of a psychoeducational intervention on CD4/CD8 and viral loads in the HIV+ patients as well as to promote a space where patients could learn more about the HIV virus and direct their attention to aspects that need to be dealt with for their well-being. This study was also undertaken to evaluate the prognostic value of IL-16 serum levels, in comparison to CD4 counts, CD8 counts, viral load and 02 microglobulin levels (J32-MG). Two groups of HIV+ patients, one of women infected heterosexually (n=7) and another of male homo/bisexuals (n=11) participated in psychoeducational encounters. Blood samples were drawn at baseline (T1), approximately 3 months after baseline and immediately after group intervention (T2) and approximately 6 months after baseline (T3) to perform CD4/CD8 and viral load exams. Laboratory data from 55 HIV-infected subjects and 10 AIDS inpatients were analyzed to compare prognostic values. Encounters were reported as informative. General study results revealed that patients who attended less than 7 sessions had significantly lower CD4 numbers, between T1 and T3 ( p = 0.0039), patients who attended more than 7 sessions had significantly lower viral loads between T2 and e T3 ( p = 0.0234). Patients who were not offered any session had stable CD4 counts and viral loads over study period. The median values, in general, did not differ neither according to clinical classification nor to time (in months) of patients' diagnosis. An inverse correlation was detected between 1L-16 levels and viral load (r =-.269) and between 02-MG and CD4 (r =-610) while a positive correlation was observed between 132-MG and viral load (r = 0.433). Three conclusions may be stated. In relation to the psychoeducational intervention and immunological parameters, a larger number of participants is needed for significant statistical analysis. This study is favorable to the adoption of j32-MG, as a more useful predictor than 1L-16 levels for AIDS progression. Athough, 1L-16 is not confirmed as an useful marker in I IIV infection it still warrants future studies especially in regard to its feasibility as a therapeutic weapon. / Este estudo teve como objetivo avaliar o efeito da intervenção psicoeducativa sobre os parâmetros tradicionais, ou seja exames de CD4/CD8 e carga virai, de acompanhamento imunológico, em pacientes HW+, bem como proporcionar um espaço onde os pacientes poderiam aprender mais sobre o vírus 1-11V e pudessem dirigir a sua atenção a alguns aspectos que contribuam para o seu bem-estar. Ademais, o estudo visou avaliar o valor prognóstico de níveis séricos de IL-16 em comparação as contagens de CD4 e CDS, valores de carga virai e níveis de (32 microglobulina 032- MG). Dois grupos de pacientes 1-11V+, um de mulheres infectadas heterossexualmente (n=7) e outro de homo/bissexuais masculinos (n=11), participaram de encontros psicoeducativos no Hospital das Clínicas da Universidade Federal de Uberlândia (FIC-UFU), no período de maio a agosto de 2001. Amostras de sangue foram colhidas em três momentos: no tempo de linha de base (TI), aproximadamente três meses depois do tempo de linha de base e imediatamente após a intervenção grupal (T2) e, aproximadamente seis meses depois do tempo de linha de base (T3) para realizar os exames de CD4/CD8 e carga virai. Para a comparação entre as provas imunológicas, foram analisados os dados laboratoriais de 55 indivíduos HTV+ e de 10 pacientes internados com AIDS (HC¬UFU). Os encontros foram avaliados como sendo proveitosos. O estudo em geral revelou que os pacientes que participaram de menos de sete sessões tiveram números de CD4 significativamente mais baixos no T1 e T3 ( p = 0,0039), os pacientes que freqüentaram mais que sete sessões tiveram log de carga virai significativamente mais baixos entre T2 e T3 ( p = 0,0234) e os pacientes que não foram oferecidos nenhuma sessão tiveram números de CD4 e log de carga viral estáveis durante o período do estudo. Em geral, os valores medianos não diferiram de acordo com a classificação clínica nem com o tempo de diagnóstico (em meses). Urna correlação inversa foi encontrada entre níveis de IL-16 e carga viral (r = -0,269) e entre f32-MG e CD4 (r = - 0,610), enquanto uma correlação positiva foi observada entre f32-MG e carga viral (r 0,433). O estudo levou a três conclusões. Primeiramente,em relação à intervenção psicoeducativa e observação da evolução imunológica, há necessidade de um maior número de participantes para uma análise estatística significativa. Em segundo lugar, sugere-se que a 132-MG é um parâmetro mais útil do que a IL-16 para monitorar a progressão da AIDS. Finalmente a IL-16, apesar de não ser confirmado como um marcador útil na infecção pelo HIV, ainda constitui interessante objeto, para futuros estudos principalmente no que diz respeito à sua viablidade como arma terapêutica. / Mestre em Imunologia e Parasitologia Aplicadas

No. of bitstreams: 1 Maria Aparecida Barbosa Rego.pdf: 1503924 bytes, checksum: 9f582589bf54cbb5312f49882238f735 (MD5) Previous issue date: 2004-12-16 / As a nurse of the Family Health Program we observe that diabetes bearers of the town of Nova Aurora State of Goias did not manage to reduce their glycemic levels even when receiving educational orientation Facing this issue this research was developed with the means of: analyzing an educational process for health along with diabetes bearers applying an approach between the theoretical assumptions of Paulo Freire and the methodological references from the Arch of Maguerez which was adapted by Bordenave e Pereira and analyze the results of the glycated hemoglobin before and after the implementation of the educational plan It is an action research with a quanti qualitative approach This study was approved by the Ethics Research Committee from the Federal University of Goias Nineteen people participated in the educative action and seventeen in the glycated hemoglobin test all diabetes bearers who signed a term of free and clarified assent and were informed concerning the intervention proposal An educative action was accomplished in six meetings The dosage of glycated hemoglobine occurred at the beginning of the educative action and five months after the end of it The results of this examination were analyzed by means of Student s t-Test The speeches of the group during the educative process were recorded and analyzed in accordance with Laurence Bardin In the light of reality the emergent categories related themselves to: feeding confrontation and feelings of success and frustration towards the illness The themes diet hypoglicemia / hyperglicemia and the physiopathology of the Diabetes mellitus were chosen as key points to be theorized In the stage of theorization of the diet categories emerged such as dependency on the health professional and autonomy / independence on the choice of one s food For the themes hypo / hyperglicemia the categories focused on the educational and liberating approach and the cognitive control that they had over these themes From the theorization of the physiopathology of the Diabetes mellitus the exposed categories were: absence of knowledge and knowledge at the common sense Changes of the subjects at the stage of solution hypothesis were observed when they elaborated educational posters which evidenced two categories: preventive measures of specific health promotion for the group and early diagnosis directed towards the community The application to the reality was accomplished through the accounts of the DM bearers as to changes in their abilities and attitudes and the attainment of autonomy and independence conquered through the control of cognitions the resultant actions of the educational practice were: the construction of teaching material which made it possible to share their knowledge with the group and the community; the capacity to establish an association between their previous knowledge to the acquired scientific knowledge during the educational intervention; changes of abilities and in the capacities of confrontation of the illness besides the changes in the alimentary habits As to the referent results of the glycated hemoglobine the t-Test shows an statistically significant difference with the reduction of the plasmatic levels of the dosed element The present research evidenced that educative action was capable to contribute to the construction of significant knowledge and for the attainment of autonomy and independence of the DM bearers facing their health problem and reduction in the index of glycated hemoglobine / Enquanto enfermeira do Programa Saúde da Família percebemos que os portadores de diabetes do município de Nova Aurora-GO não conseguiram reduzir seus níveis glicêmicos mesmo recebendo orientações educacionais Para o enfrentamento deste problema desenvolvemos esta pesquisa com objetivos de: analisar um processo de educação para a saúde junto a portadores de diabetes utilizando uma aproximação entre os pressupostos teóricos de Paulo Freire e metodológicos do Arco de Maguerez adaptado por Bordenave e Pereira e analisar os resultados de hemoglobina glicada antes e após a implementação do plano educativo Trata-se de uma pesquisa ação com abordagem quanti-qualitativa Este estudo foi aprovado pelo Comitê de Ética em Pesquisa da Universidade Federal de Goiás Participaram dezenove pessoas da ação educativa e dezessete do teste de hemoglobina glicada todas portadoras de diabetes que assinaram o termo de consentimento livre e esclarecido e foram informados acerca da proposta de intervenção Realizou-se a ação educativa em seis encontros A dosagem de hemoglobina glicada ocorreu no início da ação educativa e cinco meses após o término da mesma Os resultados deste exame foram analisados mediante teste T de Student As falas do grupo durante o processo educativo foram gravadas e analisadas de acordo com Laurence Bardin Na observação da realidade as categorias emergentes se relacionavam à alimentação ao enfrentamento e aos sentimentos de êxito e frustração frente à doença Os temas dieta hipoglicemia / hiperglicemia e a fisiopatologia do DM foram eleitos como pontos-chave a serem teorizados Na etapa de teorização da dieta surgiram as categorias: dependência do profissional de saúde e autonomia / independência para a escolha de seus alimentos Para os temas hipo / hiperglicemia as categorias focavam a abordagem educacional e libertadora e o domínio cognitivo que detinham acerca destes temas A partir da teorização da fisiopatologia do DM as categorias evidenciadas foram: a ausência de conhecimento e o conhecimento em nível do senso comum Observaram-se mudanças dos sujeitos na fase de hipóteses de solução quando construíram cartazes educativos que evidenciavam duas categorias: medidas preventivas de promoção da saúde específicas para o grupo e promoção da saúde e diagnóstico precoce direcionado à comunidade A aplicação à realidade foi efetivada mediante relatos dos portadores de DM quanto às mudanças em suas habilidades e atitudes e o alcance da autonomia e independência conquistadas por meio do domínio de conhecimentos As ações resultantes da prática educacional foram: construção de material educativo que possibilitou compartilhar saberes com o grupo e a comunidade; a capacidade de estabelecer associação de seus conhecimentos prévios aos conhecimentos científicos adquiridos durante a intervenção educacional; mudanças nas habilidades e atitudes de enfrentamento da doença além da mudança nos hábitos alimentares Quanto aos resultados referentes a hemoglobina glicada o test t mostra diferença estatisticamente significativa com diminuição dos níveis plasmáticos do elemento dosado A presente pesquisa evidenciou: que a ação educativa foi capaz de contribuir para a construção de conhecimentos significativos para o alcance da autonomia e independência dos portadores de DM frente a seu problema de saúde e diminuição no índice de hemoglogina glicada
