Technicko-ekonomické aspekty platebního styku / Technical and economic aspects of payment transactions

Thesis describes technical and economic aspects of payment transaction using the empirical analysis on real data of payment institution. It focuses on the description of selected products provided by payment institution and empirically analyzes them on the basis of the available data in terms of the number of transactions in individual currencies, the ratio of product types utilization and clearing and settlement used by the payment institution. The thesis also describes the security requirements necessary for the conduct of payment transactions on the Internet for non-technically educated audiences. At the end of the thesis there are several recommendations for the development of payment institution based on empirical knowledge from data analysis, which can be used for further analysis of the payment institution.

No. of bitstreams: 0 Previous issue date: 2009 / O presente trabalho tem como objetivo explorar a associação entre a realização de ações de promoção da saúde e prevenção de doenças cardiovasculares e a utilização de serviços de saúde, além dos correspondentes custos na atenção à saúde dos beneficiários de uma operadora de plano de saúde, a Caixa de Assistência aos Funcionários do Banco do Brasil (CASSI). O método envolve um estudo de caso, de caráter descritivo, com a seleção de beneficiários com idade maior ou igual a 30 anos, de duas capitais, Curitiba e Goiânia, e posterior constituição de dois grupos: os cadastrados na Estratégia de Saúde da Família e em programas voltados para a prevenção doenças cardiovasculares até 31/12/2004, e os não cadastrados nos programas. Primeiramente, foram descritas as características gerais da operadora e dos programas desenvolvidos. Além disso, realizou-se a descrição do perfil demográfico dos indivíduos selecionados e dos fatores de risco para doenças cardiovasculares entre os cadastrados. Em seguida, foram realizadas três análises: a primeira refere-se à análise dos cadastrados, a fim de verificar a evolução dos fatores de risco para doenças cardiovasculares, no período de 2004 a 2007; a segunda diz respeito à avaliação do perfil de utilização de serviços, com os correspondentes custos, no mesmo período; e, na terceira análise, a utilização de serviços e os custos foram comparados entre cadastrados e não cadastrados nos programas. No que se refere ao perfil de saúde dos cadastrados, foram observadas reduções no IMC e colesterol total entre os obesos; na pressão arterial sistólica ediastólica entre os hipertensos; e na glicemia de jejum entre os diabéticos. Na análise da utilização de serviços pelos cadastrados, verificou-se uma redução do número total e da média de consultas por beneficiário e do custo médio. Os exames e procedimentos diagnósticos apresentaram aumento dos números totais e per capita, com a consequente elevação dos custos assistenciais. Em Curitiba, ocorreu aumento do número médio de internações por beneficiário e do custo médio, enquanto, em Goiânia, ocorreu redução desses números. No que tange à comparação entre cadastrados e não cadastrados, constatou-se a maior utilização de consultas e exames entre os cadastrados, o que era esperado. O custo médio com internações foi maior entre os não cadastrados nos programas em todos os anos analisados para Curitiba e em 2007 para Goiânia. Espera-se que este trabalho contribua para a diminuição da lacuna existente no que tange à ausência de estudos que avaliem os resultados obtidos através do desenvolvimento de programas de promoção da saúde e prevenção de doenças, apontando para o potencial da realização desses programas tanto na mudança do perfil de utilização de serviços quanto na melhoria do perfil de saúde dos indivíduos. / The aim of this study was to explore the association between health promotion actions and cardiovascular diseases prevention and the use of health services, besides the corresponding costs to the attention when it comes to the health of beneficiaries of a private health plan, the Assistance Fund to the Workers from Brazil Bank (CASSI). The method involves a case study, of descriptive character, with the selection of beneficiaries at the age above or equal 30, from two capitals, Curitiba and Goiânia, and later formation of two groups: those registered in the Health Family Strategy and in programs linked to the prevention of cardiovascular diseases up to 12/31/2004, and those not registered in the programs. First, the general characteristics of the private health plans and of the developed programs were described. Besides that, the description of the demographic profile of the selected individuals and of the risk factors to cardiovascular diseases among those registered in the programs was performed. Then, three analysis were carried out: the first refers to the analysis of those registered, with the aim of verifying the evolution of the risk factors to cardiovascular diseases, in the period from 2004 to 2007; the second is linked to the evaluation of the profile of services use, with the corresponding costs in the same period; and, in the third analysis, the use of services and costs were compared between those registered and not registered in the programs. When it comes to the health profile of those registered, reductions in the body mass index and total cholesterol among obese people were observed, as well as in the systolic and diastolic blood pressure among those with hypertension and fasting glucose among diabetic people. In the analysis of the use of services by those registered, a reduction of the total number of the average of appointments per beneficiary and of average cost were verified. The diagnostic procedures and exams showed increase of the total numbers and per capita, with the consequent increase of the assistential costs. In Curitiba, there was increase of the average number of hospitalizations per beneficiary and of the average cost, while, in Goiânia, there was reduction of such numbers. When it comes to the comparison between registered and not registered, a greater use of appointments and exams among the registered was proved, what was already expected. The average cost with hospitalizations was bigger among those not registered in the programs in all analyzed years for Curitiba and in 2007 for Goiânia. It is expected that this work can contribute to the diminishing of the gap that exists when it comes to the absence of studies that evaluate the results obtained through the development of health promotion and prevention from diseases programs, pointing at the potential of the development of such programs both in the change of the profile of services use, as well as in the improvement of the health profile of the individuals.
