Psychiatric consultation-liaison at Dr George Mukhari Hospital in Ga-Rankuwa between January- December 2009Pillay, Nivendhiren January 2010 (has links)
Thesis (M Med (Psychiatry))--University of Limpopo (Medunsa Campus), 2010. / The aim of the study was to establish a profile of psychiatric illnesses that are encountered during consultation-liaison (C-L) psychiatry and to describe interventions undertaken. The objective was to identify referral patterns, establish the referral rate, and to profile the diagnoses & describe the actions taken by the consulting doctor. This was a retrospective cross-sectional descriptive study conducted on all inpatients that were referred to psychiatry from other disciplines in the hospital for the 12 month period January to December 2009. The study was conducted at Dr George Mukhari Hospital, which is a tertiary psychiatric unit which caters for mental health care users in the Garankuwa , Soshanguve and Mabopane region.
The study found that the referral rate to psychiatric C-L services was lower than at other institutions. Amongst the referred patients, there was a high incidence of organic brain syndrome diagnoses, but a low rate of depression.
The study proposed further investigation into the reasons behind the low rate of referral and low rate of depression, so as to improve C-L service delivery in the hospital.

Previous issue date: 2007 / Resumo: Introdução: Estudos atuais mostram que indivíduos com transtornos mentais graves têm uma menor expectativa de vida quando comparados à população geral. Realizou-se um estudo retrospectivo e descritivo da população de pacientes internados na Unidade de Internação Psiquiátrica do Hospital de Clínicas da Unicamp que foram a óbito no período de 18 anos. Objetivo: Descrever o perfil sócio demográfico e clínico dos casos de óbitos ocorridos na Unidade de internação Psiquiátrica do Hospital de Clinicas da Unicamp, comparando-os com uma amostra dos que receberam alta hospitalar. Método: Foi realizada uma análise descritiva de todos os casos de óbito num período de 18 anos, e uma comparação entre esses casos e um grupo sorteado de casos que receberam alta hospitalar. Resultados: Verificou-se que os pacientes que foram a óbito tinham o seguinte perfil: masculinos (58%), média de idade de 47,2 (DP=14,6) anos, procedentes da Região Metropolitana de Campinas (92%). Os diagnósticos psiquiátricos mais freqüentes no grupo de óbito foram de transtornos de humor e transtornos decorrentes do uso de álcool e drogas, e as causas de óbitos mais freqüentes foram doenças cardio-respiratórias (54%). Conclusões: Esses resultados sugerem que pacientes com transtornos mentais graves parecem morrer mais cedo que a população da Unidade Psiquiátrica do Hospital de Clínicas da Unicamp e a concentrarem-se nos grupo masculino, com transtornos do humor e transtornos relacionados a álcool e drogas / Abstract: Introduction: Current studies show that individuals with severe mental illness have lower life expectancy when compared to the general population. This is a retrospective and descriptive study of the inpatient population who died at the Psychiatric Unit of the University Hospital of Unicamp in an 18-year period. Objective: Describe the sociodemographic and clinical profile of the deceased patients hospitalized at the Psychiatric Unit of the University Hospital of Unicamp, comparing them to a sample of those who were discharged from the hospital. Method: A descriptive analysis of mortality and a comparison between these cases and a randomly allocated group of patients among those who were discharged. Results: It was found that the patients who died had the following profile: males (58%), mean age 47.2 (SD=14.6) years, single (54%), from Campinas great metropolitan area (92%). The most frequent psychiatric diagnoses among the patients who died were affective and alcohol/drug disorders, while the most frequent cause of death were cardiorespiratory diseases (54%). Conclusions: These outcomes suggest that patients with severe mental disorders tend to die earlier than the population from Psychiatric Unit of the University Hospital of Unicamp. The purpose of this dissertation is to evaluate an attempt to use the facilities of a general hospital with minimal psychiatric cover to deal with acute psychiatric conditions. This is in keeping with current psychiatric opinion in South Africa as expressed by Dr. C.W. Allwood and Dr G.A.D. Hart. This dissertation will examine the needs structure, difficulties and results of the provision of a service dealing with acute psychiatric disorders in a peripheral General Hospital and make suqqestions should similar satellite facilities be established at other peripheral hospitals. Although in principle. the concept of part-time specialist cover at a peripheral general hospital is not new in medical. surgical and gynaecological departments. the previous involvement of private psychiatrists within the teaching department has been in the form of out-patient department sessions only at academic and state psychiatric hospitals as is practiced in Cape Town. This project however allowed for the evaluation of emergency cases with subsequent referral to appropriate treatment centres if needed from a peripheral hospital. A consultation-liaison service within the general wards and the opportunity to conduct seminars, lectures and in-house training for all level of professional staff were also encompassed.

Patienters upplevelser av psykiatrisk vårdmiljö : En kvalitativ litteraturöversiktBostedt, Daniel, Backström, Olga January 2024 (has links)
Introduktion: Den psykiatriska vården har förändrats över tid med olika idéer för vad som är bäst för patienten. Dessa patienter har vårdats, och fortsätter att vårdas, i olika vårdformer och på olika instanser. Den psykiatriska heldygnsvården ska vara trygg, meningsfull, främja patientens återhämtning och minska behovet av tvångsåtgärder. Specialistsjuksköterskor inom psykiatrisk vård ansvarar för patientens vårdmiljö. Därmed är det av intresse att undersöka patienters upplevelser av vårdmiljön och vilka aspekter som har betydelse för deras återhämtning. Syfte: Syftet med uppsatsen är att beskriva hur patienter inom psykiatrisk heldygnsvård upplever aspekter i vårdmiljön som har betydelse för återhämtning. Metod: Litteraturöversikt baserad på 16 originalartiklar, som analyserades enligt kvalitativ innehållsanalys med induktiv ansats. Resultat: I resultatet uppstod tre huvudkategorier och sex underkategorier. Huvudkategorierna var Den fysiska vårdmiljön, Vårdpersonal och medpatienter som en del av den psykosociala vårdmiljön och Struktur och rutiner i vårdmiljön. Resultatet påvisar att patienternas upplevelser är överhängande negativa men positiva aspekter går att finna, båda dessa aspekter påverkar återhämtningsprocessen. Slutsats: Patienternas upplevelse av aspekter i vårdmiljön som har betydelse för deras återhämtning verkar vara multifaktoriell, både på ett psykosocialt, strukturellt och ett fysiskt plan. Patienterna är i behov av stimulerande aktiviteter, meningsfulla relationer, en hemlik inredning med naturinslag, en trygg omgivning och tydlighet kring rutiner och regler. Ett viktigt fynd som har uppkommit är vårdpersonalens betydelse. Det synes som att vårdpersonalens agerande har en betydande roll i patientens upplevelse av vårdmiljön och i patientens återhämtningsprocess. / Introduction: Psychiatric care has evolved over time with varying ideas on what's best for the patient. These patients have been, and continue to be, treated in different forms and at various facilities. Psychiatric inpatient care should be safe, meaningful, promote patient recovery, and reduce the need for coercive measures. Psychiatric specialist nurses are responsible for the patient's care environment. Therefore, it is of interest to investigate patients' experiences of the care environment and which aspects are important for their recovery. Aim: The purpose of the essay is to describe how patients in psychiatric health facility care experience aspects of the care environment that are important for recovery. Method: A literature review based on 16 original articles, analyzed using qualitative content analysis with an inductive approach. Results: In the result, three main categories and six subcategories emerged. The main categories were The physical care environment, Care staff and fellow patients as part of the psychosocial care environment and Structure and routines in the care environment. The results show that the patients' experiences are overwhelmingly negative, but positive aspects can be found, both of these aspects affect the recovery process. Conclusion: The patients' experience of aspects of the care environment that are important for their recovery appears to be multifactorial, both on a psychosocial, structural and physical level. The patients need stimulating activities, meaningful relationships, a homelike interior with elements of nature, a safe environment and clarity around routines and rules. An important finding that has emerged is the importance of the healthcare staff. It seems that the care staff's actions have a significant role in the patient's experience of the care environment and in the patient's recovery process.
