Patienter med borderline personlighetssyndrom ─ Upplevelse av bemötande i psykiatrisk vård : en systematisk kvalitativ litteraturstudie / Patients with borderline personality disorder ─Experience of treatment from health care professionals in psychiatric care : A qualitative systematic literature review

Bakgrund: Personer med diagnosen borderline personlighetssyndrom (BPS) återkommer ofta i slutenvårdspsykiatrin. Patienterna har problem med emotioner, kognitioner och relationsproblem där särskilt separationsångest skapar oro och konflikter med vårdpersonalen. Vårdpersonal kan ha svårt att bemöta patienter med BPS på grund av patienternas beteende, och det är därför av vikt att utöka kunskapen om hur patienter med BPS upplever den psykiatriska vården. Denna litteraturstudie ämnar sammanställa befintlig kunskap om hur patienter med BPS upplever den psykiatriska vården.  Syfte: Att belysa patienter med borderline personlighetssyndroms upplevelser av bemötande inom psykiatrisk vård.  Metod: En systematisk kvalitativ litteraturstudie utfördes enligt Howell Major och Savin-Badens (2010) metod. Tolv artiklar med högre vetenskaplig kvalitet hittades och analyserades. Resultat: Två huvudteman framkom: Upplevelsen av en stigmatiserande vård och upplevelsen av ett humant bemötande. Resultatet visade på i huvudsak negativa upplevelser av psykiatrisk vård där okunnighet hos personalen, bristande information och bristande kunskap kring diagnosen var framträdande. De positiva upplevelserna speglades i ett bemötande som gav patienterna förståelse, hopp och tillit från en engagerad personal.   Slutsats: Negativa attityder kan påverka vården och stödet för patienter med BPS, därför bör vården för dessa patienter innehålla förståelse, hopp och tillit för att återhämtning skall uppnås. Ansvaret hos sjuksköterskan är att handleda personalgruppen genom svårigheter för att tillgodose omvårdnaden. / Background: People diagnosed with borderline personality disorder (BPD) often return to inpatient psychiatry. Patients have problems with emotions, cognitions and relationship problems where separation anxiety in particular creates anxiety and conflicts with the care staff. Nursing staff may find it difficult to treat patients with BPD due to patients' behaviour, and it is therefore important to expand knowledge about how patients with BPD experience psychiatric care. This literature study aims to compile existing knowledge about how patients with BPD experience psychiatric care. Aim: The aim was to explore patients with borderline personality disorder's experiences of treatment in psychiatric care.  Method: A systematic qualitative literature study was conducted based on the Howell Major and Savin-Baden (2010) method. Twelve scientific articles of higher scientific standard were found and analysed. Results: Two main themes emerged: The experience of a stigmatizing care and the experience of a humane treatment. The included studies showed mainly negative experiences of psychiatric care where ignorance, lack of information and lack of knowledge about the diagnosis among staff were prominent. The positive experiences were reflected in a treatment that gave patients understanding, hope and trust from a committed staff. Conclusion: Negative attitudes can affect care and support of patients with BPS, therefore the care for these patients should include hope and confidence to achieve recovery. The nurse's responsibility is to guide the health care professionals through difficulties, with the aim to provide the best psychiatric nursing care for this patient group.

Spolupráce psychiatrických sester s rodinami duševně nemocných / Cooperation of psychiatric nurses with families of the mentally ill

I was inspired to choose this diploma thesis by reading an article written by an anxious family member of a person suffering from a mental illness. The stigma of a mentally ill patient does not affect only the patient themselves, it also extends to psychiatrists and nursing staff who work with these people, but it also affects their families and close relatives. For them mental illness is a burden from the psychological as well as social perspective; there are changes in family roles, the emotional climate is influenced too. The theoretical part focuses on the cooperation of nurses, families and a multidisciplinary team. It also deals with the personality of the nurse in psychiatric care, psychiatric rehabilitation limitations, communication and education in psychiatry. It also deals with the family of a mentally ill person and forms of support provided to families. The objective of this thesis was to determine whether psychiatric nurses involve families of the mentally ill in nursing care. The secondary objective was to determine whether the families of the mentally ill are aware of all the possibilities of psychiatric treatment in all areas of care. These objectives have been met. A quantitative research, the techniques of a questionnaire and a standardized interview were chosen for this work. The first research set consisted of 212 nurses working in psychiatric care; the second research set consisted of 33 families. Based on the results, it was found that nurses do not have time to educate families during their shifts. The nurses encourage families to participate actively in caring for their relatives and they have no problems in communicating with the families. Furthermore, the results indicate that families do have access to information and they are satisfied with the information from healthcare professionals; families use the information provided in the care of their relatives. Six hypotheses were set to obtain the research objectives, of which 5 hypotheses have been confirmed and a hypothesis has been refuted. This work also included an educational lesson that can help non-medical professionals to orientate in education. An educational lesson on ``Appropriate behaviour of family members of the mentally ill{\crqq}, which can be used in educating families, was prepared for family members.

Patienters erfarenheter av tvångsåtgärder inom psykiatri : En strukturerad litteraturöversikt / Patients' experiences with coercive measures in psychiatry : A structured literature review

Background: Every year, many patients with mental health problems are admitted to compulsory care under the Compulsory Psychiatric Care Act (LPT). There is a significant risk of patients injuring themselves or others. To prevent these risks, coercive measures which often infringe patients’ privacy and restrict their autonomy are employed. Therefore, it is crucial to consider patients' experiences of coercive measures to protect their privacy, autonomy and to reduce the need for coercive measure by providing person-centered care. Aim: The aim of the literature review was to describe adult patients' experiences of coercive measures in psychiatry. Method: Structured literature review with a qualitative approach is used. Fifteen scientific articles were identified using three databases: PubMed, CINAHL, PsycInfo. Two additional articles were found via Google Scholar and included. Data was analyzed using thematic analysis. Results: Participants described negative and to some extent positive experiences with coercion measures. Three themes were identified: Negative experience regarding health, vulnerability and autonomy, Positive experience of coercive measure and alternative measures, and participation and communication were described as the experience of the patients. Conclusion: Coercive measures have both negative and positive experiences where the negatives can be mitigated through effective communication with patients. In addition, nurses play a significant role in providing patients person-centered care in the context of coercive measures.
