Terapia via internet e relação terapêutica / Internet therapy and therapeutic relationship

A terapia via Internet ainda não foi aprovada pelo Conselho Federal de Psicologia,entretanto, pesquisas são recomendadas. A possibilidade técnica de oferecer tal serviço e a possibilidade de formação da relação terapêutica foram investigadas. Foi desenvolvido, por meio da utilização de softwares livres, um sistema de inscrição e psicoterapia breve realizada de forma assíncrona. A relação terapêutica foi analisada por meio do Working Alliance Inventory (WAI). Foi possível realizar o serviço de terapia via Internet e a relação terapêutica desenvolvida apresentou características semelhantes às escritas na literatura. Os clientes que se inscreveram (373) eram predominantemente do sexo feminino, com nível superior, usuários ativos de Internet e experientes no uso do computador. Os terapeutas que se inscreveram (20) apresentaram características demográficas similares às dos clientes, média experiência clínica e eram academicamente ativos. Nas 53 terapias criadas, foram encontradas diferenças significativas, referentes ao WAI, entre os clientes que abandonaram a terapia (19) e os que a completaram (29). Por outro lado, em relação aos terapeutas (15), não foram encontradas diferenças significativas entre os que atenderam os clientes que abandonaram a terapia e os que atenderam os clientes que a completaram. Contudo, foram encontradas diferenças significativas referentes ao tamanho das mensagens dos terapeutas, tendo sido essas mais extensas nas terapias que apresentaram melhor relação terapêutica. / The Brazilian Federal Council of Psychology has not approved Internet Therapy yet, however, research is recommended. The technical possibility of offering such a service and the possibility of developing therapeutic relationship were investigated. A system of registration and brief psychotherapy which was asynchronously carried out was created using free software. The therapeutic relationship was analyzed by using the Working Alliance Inventory(WAI). It was possible to conduct Internet therapy, and the therapeutic relationship presented characteristics similar to those described in the literature. Clients enrolled(373) were predominantly female, graduate, active Internet users and experienced in using computers. Therapists enrolled (20) presented demographic characteristics similar to the clients, medium clinical experience and were academically actives. In the 53 therapies created, there were significant differences, regarding the WAI, between clients who abandoned the therapy (19)and clients who completed it (29). On the other hand, in relation to the therapists (15), there were no significant differences between those whose clients abandoned the therapy and those whose clients completed it. However, there were significant differences regarding the size of the therapists' messages; how longer the messages, better the therapeutic relationship.

Em psicoterapia, à medida que o terapeuta cria as condições necessárias para que um relacionamento íntimo seja construído com o cliente, comportamentos problema, da mesma classe daqueles que são emitidos em seu ambiente natural, podem aparecer em sessão, podendo constituirse em respostas improdutivas para o processo terapêutico (sejam elas respostas agressivas ou de esquiva). O modo como o terapeuta responde a esses comportamentos pode ter efeitos diferentes na aliança terapêutica, que é tida como preditora de resultados terapêuticos. Estudos indicaram que terapeutas apresentariam tendência a responder a clientes resistentes ou hostis com contrahostilidade, frieza e distanciamento. Os terapeutas que obtiveram bons resultados em manejar tais situações realizaram intervenções interpretativas e não diretivas, focando na relação terapêutica. Entretanto, houve dificuldade de se encontrar trabalhos que tenham analisado o comportamento do terapeuta por meio de observação direta em situações que eliciam nele sentimentos negativos. O presente trabalho se propõe a investigar esse tipo de interação. Nesta linha, pretendeu-se verificar (1) se o terapeuta altera, no decorrer do processo terapêutico, sua resposta aos comportamentos do cliente que eliciam sentimentos negativos, demonstrando que está sensível ao impacto de suas intervenções no comportamento do cliente e vice-versa; e (2) se houve diminuição desses comportamentos do cliente, sendo, mesmo que indiretamente, uma medida de eficácia das intervenções terapêuticas. Para isso, foram filmadas sessões de três terapeutas analíticocomportamentais (T1, T2a e T2b) e uma estudante de psicologia (T3) e seus clientes. As terapeutas responderam a questionário pós-sessão durante todo o período de coleta de dados com o objetivo de identificar a ocorrência de sentimentos negativos nas sessões. Os comportamentos de terapeuta foram categorizados com o Sistema Multidimensional de Categorização da Interação Terapêutica. Para os comportamentos de clientes foram elaboradas categorias com o auxílio das transcrições dos áudios das supervisões. Os resultados indicaram que quatro comportamentos foram observados como eliciadores de sentimentos negativos para todas as terapeutas: Falta de Diálogo, Fala Superficial, Oposição e Atenuar. De maneira geral, as interações desse tipo corresponderam a um pouco mais de um terço do total da sessão. Houve uma prevalência das categorias Facilitação, Empatia e Solicitação de Relato nas intervenções de todas as terapeutas a esses comportamentos, confirmando dados da literatura para clientes difíceis. Facilitação e Empatia apresentaram muitas vezes padrões de ocorrência diferenciados, entendendo-se que Empatia é uma intervenção mais ativa que Facilitação. Ainda, a categoria Facilitação indicaria problemas na condução do caso, quando apresenta predominância muito acentuada entre as categorias ou quando tem percentuais muito baixos. Nas situações que produziram melhores resultados a Facilitação apresentava proporções próximas de outras categorias do terapeuta. As demais categorias foram agrupadas em Interventivas e apresentaram diferentes tendências dependendo do resultado da terapia. T1 apresentou tendência de aumento na categoria Interventivas no decorrer das sessões, sendo este o único caso que continuou em atendimento e que apresentou comportamentos de melhora expressivos. Sessions of three behavior-analytic therapists and a psychology student were videotaped. The therapists completed a questionnaire after each session in order to identify if any adverse situation had occurred. The therapist behaviors were coded with the Multidimensional System for Coding Behaviors in Therapist-Client Interaction. Clients behaviors categories were developed based on audio transcripts of supervisions. The results indicated that four behaviors were observed as eliciting negative feelings for all therapists: Lack of Dialogue, Superficial Talk, Opposition and Attenuation. In general, interactions of this type correspond to a third part of the session. There was a prevalence of categories Facilitation, Empathy and Request Report for all therapists in response to these behaviors, confirming literature data for difficult clients. Facilitation and Empathy often showed different patterns of occurrence, being Empathy a more active intervention than Facilitation. The category Facilitation indicates problems in conducting the case, when it presents very marked predominance among the categories or when it presents very low percentages. In situations with better results Facilitation had proportions similar to other therapist categories. The remaining categories were grouped into Interventional and showed different trends depending on the outcome of therapy. T1 tended to increase the Interventional category during the sessions, and it was the only case that has continued in therapy and showed significant improvement during sessions. The therapists responses differed according to client categories. It is possible to infer that each therapist deals best with some client behaviors than with others

Efeitos de workshop de psicoterapia analítica funcional sobre habilidades terapêuticas / Not informed by the author

Psicoterapia Analítica Funcional (FAP) tem como proposta a análise e intervenção de comportamentos que ocorrem durante a sessão, através de modelagem direta dos comportamentos do cliente. Para realizar a FAP de forma sistemática, alguns repertórios são requeridos do terapeuta, que o conhecimento teórico em si pode não ser capaz de produzir. Por isso, tem-se estudado formas de treino ao terapeuta FAP e sua eficácia. Além de leituras, a maneira mais comum de treino é por meio de supervisões, mas uma forma adicional tem sido utilizada mais recentemente, a de workshops experienciais, com objetivo maior de desenvolver as habilidades desejáveis ao terapeuta FAP. O objetivo do presente estudo foi investigar se esta forma de treino é capaz de produzir aquisições no repertório do terapeuta e se estas mudanças são mantidas três meses após a intervenção. Para isso, dois terapeutas participaram de delineamento experimental de linha de base múltipla, e passaram por um workshop (variável independente - VI). Foram utilizados para análise dos dados os instrumentos Functional Analytic Psychotherapy Rating Scale (FAPRS) e a Escala de Impacto da FAP (FAPIS). A análise dos dados mostrou que, após o workshop, ambos os terapeutas passaram a prover consequências (TRB2) aos comportamentos de melhora do cliente (CCR2) com maior eficiência, e um dos terapeutas passou a evocar mais comportamentos problema (CCR1) após o treino. A Cliente 1 emitiu mais CCR1 na fase experimental, enquanto o Cliente 2 passou a emitir mais CCR2 após a introdução da VI. Os dados da FAPIS mostraram que ambos os terapeutas relataram ter adquirido habilidades relacionadas à aplicação principalmente das Regras 2 e 3 da FAPA / Functional Analytic Psychotherapy proposes to analyze and do interventions in behaviors that occur during sessions, through direct shaping of clients behaviors. To do FAP in a systematic way, some repertoires are required of the therapist that theoretical knowledge may not be able to provide. Therefore, studies have been carried out on training methods for FAP therapists and its efficacy. Besides readings, the most common form of training is through supervisions, but a new additional way is through experiential workshops, whose goal is to develop desirable skills in FAP therapists. The aim of this study was to investigate if this form of training is capable of producing repertoire acquisitions to therapists and if these changes maintain three months after the intervention. For this, two therapists participated in a multiple-baseline experimental design, and did a workshop (independent variable IV). To analyze the data, two instruments were used: the Functional Analytic Psychotherapy Rating Scale (FAPRS) and the FAP Impact Scale (FAPIS). Results show that, after the workshop, both therapists provided consequences (TRB2) to clients improvement behaviors (CRB2) more effectively, and one of the therapists evoked more problem behaviors (CRB1). Client 1 emitted more CRB1 in experimental phase, whereas C2 emitted more CRB2. FAPIS data showed that therapists reported that they have acquired more FAP-related skills, especially the ones related to Rules 2 and 3

Análise dos comportamentos de terapeuta e cliente em um caso de transtorno de personalidade borderline / Analysis of therapists and patients behavior in a case of Borderline Personality Disorder

Clientes com Transtorno de Personalidade Borderline apresentam um alto índice de abandono de terapia. Entender o que ocorre durante as sessões entre terapeuta e cliente com este tipo de transtorno de personalidade pode contribuir para evitar futuros equívocos ou falhas na relação terapêutica, aumentando as chances de continuidade do processo terapêutico e diminuindo a probabilidade de abandono da terapia. O presente estudo teve como objetivo identificar as variáveis que estão relacionadas ao abandono de um caso de Transtorno de Personalidade Borderline. Participou do estudo uma terapeuta de orientação analítico-comportamental, com 12 anos de experiência clínica e uma cliente com 30 anos de idade, casada, sem filhos e com escolaridade superior completo. Um total de 13 sessões foi gravado em áudio, transcrito e categorizado segundo o Sistema Multidimensional para Categorização de Comportamentos da Interação Terapêutica. Foi feita análise sequencial de atraso (Lag sequential analysis). As sessões que compuseram os dados deste estudo foram entre a 9ª e a 22ª sessões, embora a 14ª sessão tenha sido excluída da análise de dados por ter sido uma sessão de casal. Os resultados mostraram que as categorias da terapeuta de maior porcentagem de ocorrência foram Facilitação, Solicitação de Relato e Empatia e as menos frequentes foram, Solicitação de Reflexão\", Aprovação e Recomendação. Reprovação foi a categoria que teve a menor frequência entre as demais categorias da terapeuta. Embora a categoria Interpretação tenha ocorrido em baixa frequência, sua duração foi grande. As categorias de maior porcentagem da cliente foram Relato e Estabelecimento de Relações entre Eventos. E as de menor porcentagem de ocorrência foram: Solicitação, Concordância, Oposição e Melhora. A categoria Metas não ocorreu nenhuma vez. Ao longo das sessões, foi observado um declínio na porcentagem de Estabelecimento de Relações entre Eventos e um aumento em Relato, no que se refere às categorias da cliente. Ao mesmo tempo, foi observada uma diminuição das categorias Empatia, Solicitação de Reflexão e Interpretação da terapeuta. As sequências que mais ocorreram foram Relato\" seguido por Facilitação\" e Facilitação\" seguida por Relato\". O abandono da terapia pareceu estar relacionado a diversos fatores: a) perda de oportunidades de aprovar e solicitar reflexão e interpretar, b) não dar atenção a relatos sobre queixas de doenças, exercendo função de invalidação, c) férias prolongadas da terapeuta e d) não flexibilidade da terapeuta em fazer mais uma sessão domiciliar em um momento de crise, repetindo assim, um comportamento de invalidação / Clients suffering from Borderline Personality Disorder show a high dropout rate in psychotherapy. The understanding of what occurs during the sessions between therapist and client suffering from this type of personality disorder may contribute to prevent future errors and failures, increasing chances of completing the therapeutic process and reducing drop-out probability. The current study had the objective of identifying the variables related to the drop-out in a case of Borderline Personality Disorder. Participated a female behavior-analytic therapist, with 12 years of clinical experience and a female client, 30 years old, married, no children, with a college degree. 13 complete sessions were recorded in video, transcribed and categorized according to the Multidimensional System for Categorization of Behaviors in Therapeutic Interaction. A lag sequential analysis was carried out. Data of this study refer to the 9th through the 22nd sessions, although the 14th session was excluded, since it was a couple session. Outcomes showed that the therapists categories in higher percentages were `Facilitation`, `Request of Report` and `Empathy` and the less frequent percentages were `Request of Reflection`, `Approval` and `Recommendation`. `Disapproval` was the therapists category appearing with lower frequency. Although `Interpretation` occurred in low frequency, it had a long duration. The clients categories with higher percentages were `Report` and `Establishment of Relationship between Events`. The lower percentages of occurrences were: `Request`, `Agreement`, `Opposition` and `Improvement`. The category `Aims` did not occur at any time. Throughout the sessions a decrease in percentage of the client\'s categories `Establishment of Relationship between Events` and an increase in \'Report\' were observed. At the same time a decrease in the therapists categories `Empathy`, `Request for Reflection` and `Interpretation` was noted. Sequences which occurred more frequently were `Report` followed by `Facilitation` and `Facilitation` followed by `Report`. The therapy drop-out seemed to be related to several factors: a) loss of opportunities by the therapist to approve, to request reflection and to interpret; b) lack of attention to reports on complaints of diseases, having an invalidation effect; c) therapists extended vacation and d) therapists refusal to attend an additional home session, in a critical moment, thus repeating the invalidation

Si l’efficacité des psychothérapies semble maintenant faire consensus, la recherche vient aujourd’hui interroger directement le processus psychothérapeutique. Notre travail s’inscrit dans cette démarche et se fonde sur l’étude de l’interaction verbale issue d’une psychothérapie émergente, la psychothérapie EMDR, Eyes Movement Desensitization and Reprocessing. La traduction française est désensibilisation et retraitement par les mouvements oculaires. Ce dispositif thérapeutique s’avère prometteur dans le champ du soin psychique, et est principalement identifié parmi les pratiques psychothérapeutiques comme étant celle où le thérapeute exerce une action physique sur son patient et ce, au moyen de stimulations bilatérales alternées (notamment par le biais de mouvements oculaires). L’EMDR bénéficie dans le champ de la recherche majoritairement d’études s’intéressant aux effets physiologiques en lien principalement avec les mouvements des yeux pendant le traitement. Or, en vertu du fait que l’EMDR s’agence aussi autour d’un protocole verbal, nous nous inscrivons dans le champ des études qui considèrent que la psychothérapie est avant tout un « événement de parole » (Labov et Fanshel, 1977). Ainsi, en nous appuyant sur l’étude du langage, à la fois tel qu’il est prévu par le protocole et tel qu’il est dispensé par le thérapeute en séance, nous venons compléter ces travaux en nous plaçant à un niveau de description clinique, à hauteur du sujet. Dans une première partie, nous nous situons sur le plan théorique, nous éclairons les phases 3 et 4, prototypes du protocole EMDR à la lumière de théories issues du champ de la pragmatique mais aussi à la lumière des théories issues de la psychanalyse. Dans une seconde partie de recherche empirique, nous examinons un corpus constitué de trois suivis psychothérapeutiques filmés et retranscrits. En nous inscrivant dans le réel de la pratique telle qu’elle se donne à entendre, nous mettons en œuvre une méthodologie d’analyse exploratoire et qualitative issue du paradigme de la pragmatique. Pour ce faire, nous nous référons aux travaux menés au sein de l’analyse linguistico-pragmatique des interventions du thérapeute et de la Logique Interlocutoire. Nous dégageons certaines caractéristiques de la matrice langagière qui viennent souligner son importance, son rôle et ses effets dans l’instauration d’un processus de changement qui apparaît bénéfique pour le patient / As the efficiency of psychotherapies is now widely recognized, research questions the psychotherapeutic process. In this vein, we review and study the verbal interactions resulting from an emerging psychotherapy, the Eyes Movement Desensitization and Reprocessing (EMDR) psychotherapy. This therapy is very promising in the field of psychic care. It is a psychotherapeutic practice in which the therapist exercises a physical action on his patient by means of alternating bilateral stimulations – notably by ocular movements. Thus far, research on EMDR has primarily examined the physiological effects related mainly to eye movements during treatment. Because EMDR also has a verbal protocol, we review it in the framework of studies that consider psychotherapy as a "speech event" above all (Labov and Fanshel, 1977). Based on a study of language, both as provided for in the protocol and as used by the therapist in session, we attempt to pursue these works by placing ourselves at a level of clinical description, at the level of the subject. In a first part, we follow a theoretical approach, and review phases 3 and 4 and prototypes of the EMDR protocol, in light of theories in the field of pragmatics and theories stemming from psychoanalysis. In a second part of empirical research, we examine a corpus consisting of three psychotherapeutic follow-ups, which have been filmed and transcribed. By placing ourselves into the reality of the practice, as it is commonly understood, we implement a methodology of exploratory and qualitative analysis emerging from the paradigm of pragmatics. To do this, we refer to the works carried out within the linguistic-pragmatic analysis of the interventions of the therapist as well as to interlocutory logic. We highlight some characteristics of the language matrix that demonstrate its importance, its role and its effects in the implementation of a process of change that appears to be beneficial for the patient
