Depressão puerperal: avaliação da relevância dada pelos profissionais de saúde antes e após intervenção educativa

The postpartum period has been described as a period of female life in which mental disorders are particularly frequent, which could reflect a stereotypical view of society and health professionals about the positive experiences arising from the myth of unconditional maternal love. This research aims to present strategies to enable health professionals through continuing education, thinking in the clinical situation of postpartum depression (PPD), allowing them to act preventively in the care of pregnant or postpartum women. The methodological design was action research focused on qualitative analysis, the first stage consisted of interviews applied to healthcare professionals involved in caring for pregnant and postpartum women, seeking to investigate how the approach was on the PPD in the Basic Health and Maternity of Penápolis / SP. Subsequently we perform activity in order of Continuing Education for professionals involved, and in the third stage we repeat the interviews with professionals participating in the study on the impact that the discussion on the subject caused in their care practices maternal health. We conclude that there was no specific approach on PPD in prenatal, with clear lack of suitable space for reflection involving matters that were not the technical aspects of obstetrical conditions. After the educational intervention, we saw important changes that were reflected in the discourses on new positions ahead to patients, staff and the best rapport, including management support to embrace concerns about PPD institutional form, determining the deployment routine for the diagnosis and for referral of patients with PPD and a new project due to the results of our research. Therefore it became clear the relevance of making on continuing education a current practice of the health team, and also to allow space for continuing education in order to listen to the reality of professionals, encouraging them to seek solutions and solving their conflicts before, thereby lowering anxieties and strengthening networking and relationship between the professionals themselves / A gestação, parto e puerpério constituem uma experiência humana das mais significativas, com forte potencial positivo e muito enriquecedora para todos que dela participam. O puerpério tem sido apontado como um período da vida feminina em que os transtornos mentais são particularmente frequentes, o que poderia refletir a visão estereotipada da sociedade e dos profissionais da saúde sobre as vivências positivas decorrentes do mito do amor materno incondicional. A presente pesquisa teve como objetivo apresentar estratégias para capacitar profissionais da área de saúde, através de educação continuada, a pensar na situação clínica de depressão puerperal (DP), permitindo que possam agir de forma preventiva no atendimento da gestante ou puérpera. O método utilizado foi pesquisa-ação focado em análise qualitativa cuja primeira etapa constou de entrevistas aplicadas aos profissionais de saúde envolvidos no atendimento de gestantes e puérperas buscando-se investigar como se fazia a abordagem sobre DP na nas Unidades Básicas de Saúde e na Maternidade de Penápolis/SP Posteriormente realizamos ação de Educação Continuada para os profissionais envolvidos e na terceira etapa foi realizada nova entrevista com os profissionais participantes do estudo sobre o impacto que a discussão sobre o tema teve em suas práticas de atenção a saúde da gestante. Concluímos que não havia nenhuma abordagem específica relativa a DP no pré natal, com clara ausência de espaço propício para a reflexão envolvendo assuntos que não fossem os aspectos técnicos médicos da gravidez. Após a intervenção educativa percebemos importantes mudanças que se refletiram nos discursos sobre novas posturas frente às pacientes, melhor entrosamento na equipe e, inclusive o apoio dos gestores para adotar a preocupação sobre DP de forma institucional, determinando a implantação de rotina para o diagnóstico e para o encaminhamento de pacientes com DP e de um novo projeto decorrente dos resultados de nossa pesquisa. Portanto ficou evidente a relevância de se fazer da educação continuada uma prática corrente à equipe de saúde, e também de possibilitar espaços de educação permanente visando a escuta da realidade dos profissionais, estimulando a eles que busquem soluções e resolubilidade diante de seus conflitos, diminuindo assim as angústias e fortalecendo o trabalho em rede e o vínculo entre os próprios profissionais

The meanings of sobreparto : postpartum illness and embodiment of emotions among Andean migrants in Santa Cruz de la Sierra, Bolivia

This thesis concerns a postpartum condition known as sobreparto among female Andean migrants in the lowland city of Santa Cruz de la Sierra, Bolivia. While sobreparto is a traditionally Andean illness, its occurrence in the lowland city of Santa Cruz opens up new dimensions of analysis. In addition to exposing transformations of the traditional understandings of health, illness, and the body, the study of this phenomenon in an atypical setting sheds new light on issues such as migration, social networks, biomedicalisation, or gender patterns. By means of narratives of lives interrupted by sobreparto, it is possible to locate this condition within a wider frame of life trajectories, exposing motifs beyond the temporarily dysfunctional body. I argue that the narratives of sobreparto can be used as a springboard for a study of transformations in the understandings of motherhood and womanhood, migration and social networks, as well as emotions. Looking at these processes through the lens of a postpartum illness also reveals the connections between the ill body, the troubled mind, and imperfect social relationships. On the one hand, sobreparto can be analysed at the micro-level – in terms of an understanding of the body, individual reproductive histories, or the availability of other people's support. On the other hand, sobreparto constitutes a commentary on phenomena occurring at the macro-level, such as large-scale internal migration in Bolivia or the increasing domination of biomedicine as a model of health and illness. The city of Santa Cruz offers a unique setting for scrutinising these changes using a traditionally Andean postpartum illness as a point of departure. Being much more than a postpartum bodily dysfunction, sobreparto, therefore, can be used as a lens through which it is possible to see the interplay of social and political macro- and micro-processes in people's lives at the time of reproduction.
