Esta síndrome mundialmente conhecida tem sido objeto de intensa discussão devido sua etiopatogenia incerta e tratamento controverso. O exame endoscópico é essencial instrumento no diagnóstico da HPIE e deve ser realizado 30 a 120 minutos após esforço físico intenso, tempo necessário para o sangue ser eliminado das vias aéreas distais. O objetivo deste estudo foi mensurar potenciais marcadores plasmáticos de lesão pulmonar como a enzima conversora de angiotensina (ECA) e endotelina tipo 1 (ET-1), antes de iniciar condicionamento físico (M0); e depois do teste de esforço máximo (M1) e depois de 15 minutos após fadiga (M15) em esteira de alta velocidade em cavalos de trote com e sem HPIE. Oito cavalos de trote (4 fêmeas e 4 machos castrados) com idade de 3 a 5 anos (média de 4 anos de idade) foram incluidos neste estudo. A severidade da HPIE foi estabelecida em graus de 0 a 4 depois de 15, 30, 60, 90 e 120 minutos após fadiga, pelo exame traqueobrônquico por endoscopia, e pela contagem manual de hemácias e concentração de hemoglobina no lavado broncoalveolar (LBA) coletado 90 minutos após fadiga, na 13° semana do experimento. EIPH is considered one of the greatest problems in athlete horses due to the high economical impact caused by the indiscriminate use of pharmaceutical products, veterinary costs and disposal of the animal. This worldwide syndrome has been object of intense discussions because of its unclear etiopathogeny and controversial treatment. Endoscopic exam is an essential instrument to diagnose HPIE and it must be performed 30 to 120 minutes after intense physical exercise, the necessary amount of time for the blood to be eliminated from distal airways. The objective of the present study was to measure potential markers of lung injury such as angiotensin-converting enzym (ACE) and endothelin-1 (ET- 1), before the beginning of training (M0); and after maximum effort testing on the treadmill (M1); and 15 minutes after fatigue (M15) on high speed treadmill in Standardbred horses with and without HPIE. Eight Standardbred horses (4 fillies and 4 geldings) aged 3 to 5 years old (average 4 years old) were included in this study. The severity of EIPH was scored in degrees from 0 to 4 after 15, 30, 60, 90 and 120 minutes after fatigue by tracheobronchoscopy exam and red blood cell count and hemoglobin concentration in BALF collected 90 minutes after fatigue on the 13th week of the experiment. Avaliação de índices ecocardiográficos de equinos sob efeito de furosemida / Echocardiographic evaluation of horses under furosemide effect

Muitos equinos participantes de provas de corrida, ao exame endoscópico, apresentam evidências de ocorrência de Hemorragia Pulmonar Induzida pelo Exercício (HPIE). A HPIE trata-se de um sangramento de origem pulmonar que ocorre em equinos durante atividade física intensa e pela alta prevalência, muitas vezes associada aos equinos de corrida. Considerando, baseado nos estudos realizados, que o principal mecanismo descrito no desenvolvimento dos quadros de HPIE envolve o aumento da pressão transmural, resultante do aumento da pressão pulmonar causada pelo exercício físico, a furosemida parece ter papel importante na atenuação destes fatores, o que suporta a sua utilização nestes animais. Foram avaliados 21 equinos da raça Puro Sangue Inglês, atletas de distintas modalidades (corrida e polo), com e sem HPIE em dois momentos, antes (T0) e 4 horas após a administração da furosemida (T1) na dose de 0,5 mg/kg. Os animais foram submetidos a exame ecocardiográfico nos dois momentos, e também a colheita de sangue para avaliação hematológica e bioquímica. Ao exame ecocardiográfico foram consideradas as seguintes variáveis: FC, DC, VEj, FEj, VSFVE, VDFVE, FS, SIV, PLVE, DVE em sístole e diástole, AEs, Ao, AE:Ao, TEVE, E-S, e pico de velocidade do fluxo na artéria pulmonar. Para os exames laboratoriais foram consideradas as seguintes variáveis: He, Hb, Ht, VCM, HCM, CHCM, Leu, Neu, Linf, Mon, Plaq, AST, GGT, Alb, Pt, BT e BD. O uso da furosemida exerceu influência sobre os índices ecocardiográficos estudados de animais sem HPIE alterando a velocidade do fluxo da artéria pulmonar e o TEVE. Os demais parâmetros ecocardiográficos estudados bem como os parâmetros hemáticos não apresentaram alteração significativa, exceção feita aos valores de ureia e creatinina que aumentaram em T1, em função da hemoconcentração, sendo significantemente maiores no grupo sem HPIE. / Many racehorses at endoscopic examination after race present evidence of Exercise Induced Pulmonary Hemorrhage (HPIE). HPIE is a bleeding that occurs from pulmonary origin in horses during intense physical activity, often associated, by the high prevalence, to the racehorses, being able to present itself in many other equines, since undergoing intense physical exercise. Considering, based on previous studies, that the main mechanism involved in the development of HPIE involves the increase of the transmural pressure, resulting from the increase of the pulmonary pressure caused by the physical exercise, furosemide seems to play an important role in the attenuation of these factors, which supports their use in these animals. Twenty-one Thoroughbred horses, athletes, with and without HPIE were evaluated at two moments, before (T0) and 4 hours after furosemide administration (T1) at a dose of 0.5 mg / kg. All equines were submitted to echocardiographic examination at both moments, as well as blood sampling for hematological and biochemical evaluation. Echocardiographic examination showed the following variables: FC, DC, VEj, FEj, VSFVE, VDFVE, FS, SIV, PLVE, DVE, AEs, Ao, AE:Ao, TEVE, E-S, and flow in the pulmonary artery. For the laboratory tests, the following variables were considered: He, Hb, Ht, VCM, HCM, CHCM, Leu, Neu, Linf, Mon, Plaq, AST, GGT, Alb, Pt, BT and BD. Furosemide influenced some echocardiographic indexes of non-HPIE animals, altering the velocity of pulmonary artery flow and TEVE. All others echocardiographic and hematological parameters did not present any influence of furosemide administration, although urea and creatinine values have increased in T1 due to the hemoconcentration in non-HPIE group.

Atualmente, muitos animais encaminhados para necropsia apresentam histórico de óbito ocorrido durante ou após procedimentos como banho e tosa, adestramento, traslados, permanência em hotéis e outras situações similares. Segundo relato de médicos veterinários e auxiliares presentes no local do óbito, grande parte desses animais ao serem manipulados apresentavam agressividade, ansiedade e agitação, e muitos vinham a óbito ainda nas mãos do próprio tosador, enquanto outros, minutos após a chegada em casa. Em um levantamento de exames necroscópicos contendo 1391 animais, realizados pelo Pet Legal Centro de Diagnósticos e Perícias na cidade de São Paulo, no período de 2004 a 2009, 95 cães e 16 gatos de ambos os sexos, e de diferentes raças e idades vieram a óbito durante os procedimentos descritos anteriormente. Em 28% dos casos, os animais vieram a óbito em conseqüência de traumatismos de origem mecânica contundente, tendo uma maior incidência de traumas em região de cabeça, caracterizados por fraturas e lesões de tecido nervoso, ocasionados por queda ou golpe, muitas vezes apresentando lesões de golpe e contragolpe. Em 72% dos casos, os animais vieram a óbito em conseqüência de um colapso respiratório, tendo como alterações principais a congestão, edema e hemorragias pulmonares, sendo esta última, determinante para o óbito. Nos casos de óbito por colapso respiratório, os animais apresentavam alterações em seu comportamento e na freqüência cardiorrespiratória, fato observado na ação de luta ou fuga e que faz parte da fase de alarme da síndrome geral de adaptação. Este é o primeiro trabalho em âmbito nacional que relata as causas de óbitos durante os procedimentos em petshops e similares, fornecendo as informações necessárias aos médicos veterinários, proprietários e principalmente às partes jurídicas, conferindo a esta última, respaldo forense no processo legal. / Nowadays, many animals sent for necropsy have a history of deaths that occurred during or after procedures such as bathing and grooming, training, transfers, staying in hotels and other similar situations. Avaliação da utilização de Furosemida em eqüinos Puro Sangue de Corrida e sua correlação com a Hemorragia Pulmonar Induzida por Exercício / Evaluation of the use of furosemide in thoroughbred horses and its correlation to exercise-induced pulmonary hemorrhage

A hemorragia pulmonar induzida por exercício (HPIE) ocorre com freqüência em cavalos atletas acarretando prejuízo econômico. O presente estudo foi compreendido em duas etapas, tendo como objetivo na primeira, avaliar a performance dos cavalos levando em consideração a colocação nos páreos e administração da furosemida, no período de setembro de 2005 a julho de 2006, sendo analisados os resultados de 824 eqüinos no Jockey Clube de Porto Alegre. Dos eqüinos que participaram do primeiro estudo, 389 não foram medicados e 435 foram submetidos à medicação prévia com furosemida. Foi observado que nos animais tratados previamente o medicamento influencia positivamente o desempenho do animal, com uma redução de 2,9% no tempo final. A segunda etapa foi desenvolvida durante a campanha de 2007, onde foram examinados 146 eqüinos, 15 minutos após o final da prova. Os animais foram submetidos a um exame endoscópico em que foram observadas as possíveis alterações do trato respiratório anterior do animal, sendo a principal delas a hemorragia pulmonar induzida por exercício. Estes achados foram relacionados com sua performance e com a administração de furosemida. Foi observado que a furosemida não interfere no sangramento. Não se observou influência do sexo na incidência de HPIE, entretanto observou-se aumento de hemorragia com o aumento da idade. De um modo geral, não se observou correlação positiva entre a HPIE e as alterações das vias aéreas superiores e traquéia. Conclui-se que a aplicação de furosemide diminuiu o tempo final empregado em corrida, mas não preveniu a incidência e severidade da HPIE. A incidência de HPIE aumenta com a idade. / Exercise-induced pulmonary hemorrhage (EIPH) commonly occurs in athletic horses causing economic losses. The present study was set in two trials. The first of them aimed to evaluate the performance of horses taking into account their ranking and furosemide administration, in the period of September 2005 to July 2006, being analysed the results from 824 horses, were analysed at the Porto Alegre Jockey Club. From the equines these, 389 were not treated and 435 were submitted to previous furosemide treatment. On treated animals, it was observed that the medicine affects animal performance, and also a reduction of 2.9% at the final time. The second trial was developed during 2007, where 146 equines were examined, 15 minutes after the race. The animals were submitted to an endoscopic exam in which there were observed possible abnormalities of the upper respiratory tract, being the main one the exerciseinduced pulmonary hemorrhage. These findings were related to their performance and furosemide administration. It was observed that the furosemide does not interfere on EIPH incidence. Gender did not influence the HPIE incidence; however it was observed an increase in the pulmonary bleeding with the increase of age. . Generally, it was not observed a positive correlation between EIPH and the upper respiratory tract and windpipe abnormalities, and only 15.25% of the cases related to the presence of EIPH with dorsal dislocation of the soft palate, being not statistically significant. I was concluded that the use of Furosemide enhancest the performance time, but did not prevent the incidence and the degree from EIPH. The incidence of EIPH increases with age.

Avaliação da utilização de Furosemida em eqüinos Puro Sangue de Corrida e sua correlação com a Hemorragia Pulmonar Induzida por Exercício / Evaluation of the use of furosemide in thoroughbred horses and its correlation to exercise-induced pulmonary hemorrhage

A hemorragia pulmonar induzida por exercício (HPIE) ocorre com freqüência em cavalos atletas acarretando prejuízo econômico. O presente estudo foi compreendido em duas etapas, tendo como objetivo na primeira, avaliar a performance dos cavalos levando em consideração a colocação nos páreos e administração da furosemida, no período de setembro de 2005 a julho de 2006, sendo analisados os resultados de 824 eqüinos no Jockey Clube de Porto Alegre. Dos eqüinos que participaram do primeiro estudo, 389 não foram medicados e 435 foram submetidos à medicação prévia com furosemida. Foi observado que nos animais tratados previamente o medicamento influencia positivamente o desempenho do animal, com uma redução de 2,9% no tempo final. A segunda etapa foi desenvolvida durante a campanha de 2007, onde foram examinados 146 eqüinos, 15 minutos após o final da prova. Os animais foram submetidos a um exame endoscópico em que foram observadas as possíveis alterações do trato respiratório anterior do animal, sendo a principal delas a hemorragia pulmonar induzida por exercício. Estes achados foram relacionados com sua performance e com a administração de furosemida. Foi observado que a furosemida não interfere no sangramento. Não se observou influência do sexo na incidência de HPIE, entretanto observou-se aumento de hemorragia com o aumento da idade. De um modo geral, não se observou correlação positiva entre a HPIE e as alterações das vias aéreas superiores e traquéia. Conclui-se que a aplicação de furosemide diminuiu o tempo final empregado em corrida, mas não preveniu a incidência e severidade da HPIE. A incidência de HPIE aumenta com a idade. / Exercise-induced pulmonary hemorrhage (EIPH) commonly occurs in athletic horses causing economic losses. The present study was set in two trials. The first of them aimed to evaluate the performance of horses taking into account their ranking and furosemide administration, in the period of September 2005 to July 2006, being analysed the results from 824 horses, were analysed at the Porto Alegre Jockey Club. From the equines these, 389 were not treated and 435 were submitted to previous furosemide treatment. On treated animals, it was observed that the medicine affects animal performance, and also a reduction of 2.9% at the final time. The second trial was developed during 2007, where 146 equines were examined, 15 minutes after the race. Avaliação de índices ecocardiográficos de equinos sob efeito de furosemida / Echocardiographic evaluation of horses under furosemide effect

Muitos equinos participantes de provas de corrida, ao exame endoscópico, apresentam evidências de ocorrência de Hemorragia Pulmonar Induzida pelo Exercício (HPIE). A HPIE trata-se de um sangramento de origem pulmonar que ocorre em equinos durante atividade física intensa e pela alta prevalência, muitas vezes associada aos equinos de corrida. Considerando, baseado nos estudos realizados, que o principal mecanismo descrito no desenvolvimento dos quadros de HPIE envolve o aumento da pressão transmural, resultante do aumento da pressão pulmonar causada pelo exercício físico, a furosemida parece ter papel importante na atenuação destes fatores, o que suporta a sua utilização nestes animais. Foram avaliados 21 equinos da raça Puro Sangue Inglês, atletas de distintas modalidades (corrida e polo), com e sem HPIE em dois momentos, antes (T0) e 4 horas após a administração da furosemida (T1) na dose de 0,5 mg/kg. Os animais foram submetidos a exame ecocardiográfico nos dois momentos, e também a colheita de sangue para avaliação hematológica e bioquímica. Ao exame ecocardiográfico foram consideradas as seguintes variáveis: FC, DC, VEj, FEj, VSFVE, VDFVE, FS, SIV, PLVE, DVE em sístole e diástole, AEs, Ao, AE:Ao, TEVE, E-S, e pico de velocidade do fluxo na artéria pulmonar. Para os exames laboratoriais foram consideradas as seguintes variáveis: He, Hb, Ht, VCM, HCM, CHCM, Leu, Neu, Linf, Mon, Plaq, AST, GGT, Alb, Pt, BT e BD. O uso da furosemida exerceu influência sobre os índices ecocardiográficos estudados de animais sem HPIE alterando a velocidade do fluxo da artéria pulmonar e o TEVE. Os demais parâmetros ecocardiográficos estudados bem como os parâmetros hemáticos não apresentaram alteração significativa, exceção feita aos valores de ureia e creatinina que aumentaram em T1, em função da hemoconcentração, sendo significantemente maiores no grupo sem HPIE. / Many racehorses at endoscopic examination after race present evidence of Exercise Induced Pulmonary Hemorrhage (HPIE). HPIE is a bleeding that occurs from pulmonary origin in horses during intense physical activity, often associated, by the high prevalence, to the racehorses, being able to present itself in many other equines, since undergoing intense physical exercise. Considering, based on previous studies, that the main mechanism involved in the development of HPIE involves the increase of the transmural pressure, resulting from the increase of the pulmonary pressure caused by the physical exercise, furosemide seems to play an important role in the attenuation of these factors, which supports their use in these animals. Twenty-one Thoroughbred horses, athletes, with and without HPIE were evaluated at two moments, before (T0) and 4 hours after furosemide administration (T1) at a dose of 0.5 mg / kg. All equines were submitted to echocardiographic examination at both moments, as well as blood sampling for hematological and biochemical evaluation. Echocardiographic examination showed the following variables: FC, DC, VEj, FEj, VSFVE, VDFVE, FS, SIV, PLVE, DVE, AEs, Ao, AE:Ao, TEVE, E-S, and flow in the pulmonary artery. For the laboratory tests, the following variables were considered: He, Hb, Ht, VCM, HCM, CHCM, Leu, Neu, Linf, Mon, Plaq, AST, GGT, Alb, Pt, BT and BD. Furosemide influenced some echocardiographic indexes of non-HPIE animals, altering the velocity of pulmonary artery flow and TEVE. All others echocardiographic and hematological parameters did not present any influence of furosemide administration, although urea and creatinine values have increased in T1 due to the hemoconcentration in non-HPIE group.

Exercise-induced pu[l]monary hemorrhage: determination of mechanisms and potential treatments

Doctor of Philosophy / Department of Anatomy and Physiology / David C. Poole / Exercise-induced pulmonary hemorrhage (EIPH) or epistaxis has been recognized in racehorses since the 16th century. Since this time, great strides have been made in terms of identifying the lungs as the source of the hemorrhage via the endoscope, utilization of bronchoalveolar lavage to quantify the hemorrhage, and the discovery of successful treatments such as furosemide and the nasal strip that ameliorate, but do not abolish EIPH. It has been determined that, in addition to extremely high pulmonary arterial pressures and the negative intrapleural pressures being the major physiologic forces causing pulmonary capillary stress failure, other factors have the potential for influencing the severity of EIPH including locomotory impact trauma, inflammatory airway disease (IAD), upper airway obstruction, coagulation anomalies, and high blood viscosity. It has been hypothesized that EIPH is detrimental to performance and this was recently confirmed by Hinchcliff et al. in 2004. EIPH is a complex multi-factorial condition with much still unknown about the etiology, best method for diagnosis, and most effective form of treatment. Chapter one of this dissertation determined the effectiveness of a novel treatment, concentrated equine serum, in ameliorating EIPH via reduction of IAD. Chapter two refuted the hypothesis that herbal formulations commonly used in the field with anecdotal success would decrease EIPH by correcting coagulation deficits during exercise, as scientific efficacy was not evident, at least at the dose and duration used in our investigation. Chapter three addressed the dogma that EIPH only occurs during maximal intensity exercise, and in demonstrating significant EIPH during sub-maximal exercise, emphasized the role that the airways play in contributing to the initiation and severity of EIPH. Chapter four examined the occurrence and severity of EIPH in the horse’s canine counterpart, the racing Greyhound. The demonstrated presence of mild EIPH in the Greyhound, a physiologically similar yet different athlete in comparison to the horse sheds new light on the etiology of this condition in both species. The results of these investigations have advanced the frontiers of our knowledge concerning EIPH. Specifically, they have generated novel information on the mechanistic bases of EIPH and have provided evidence supporting additional treatment options for reducing the severity of EIPH in horses.
