Specifická ošetřovatelská péče u dětí s rozštěpovými vadami obličeje / Specific nursing care of children with facial cleft defects

The thesis deals with specific nursing care for children with facial cleft anomalies. Facial clefts belong to frequent congenital anomalies with incidence of 1 : 500 in lively born children, which is why adequate care has to be provided to these children to honour the founder of Czech plastic surgery Prof. František Burian. Two goals were set in this thesis. The first goal was to elaborate a proposal of a nursing procedure for children with facial cleft. The other goal was to elaborate an educational programme for nurses and parents of children with facial cleft anomalies. Both the goals have been achieved. Two hypotheses were set in this thesis: Hypothesis No. 1 ?Nurses working at a specialized department are sufficiently informed on the care about children with facial cleft anomalies?. Hypothesis No. 2 ?Nurses working at maternity wards are sufficiently informed on the care about children with facial cleft anomalies?. Two research questions were also set before the research itself: Research question No. 1: How do nurses provide specific nursing care to children with facial cleft anomalies? Research question No. 2: What principles do nurses consider important for the care about children with facial cleft anomalies? The questionnaire, interview and observation methods were used for data collection. The questionnaire was based on information obtained from literature and was designed for ward nurses. The interview was prepared upon own experience from observation of nurses in the Královské Vinohrady University Hospital in the Centre for Cleft Anomalies Treatment and upon information from literature. 60 questionnaires were distributed in the České Budějovice Hospital in March 2011. There were 47 completed questionnaires suitable for processing. 11 questionnaires were distributed in the Královské Vinohrady University Hospital in March 2011. 9 completed questionnaires were used for processing. The research sample for the interview consisted of 11 nurses working in the Královské Vinohrady University Hospital, the Centre for Cleft Anomalies Treatment, the Paediatric ICU and the Paediatric ward. The results show that the nurses working at maternity wards are sufficiently informed. The results also show how nurses provide specific nursing care and what principles they consider important. We have elaborated educational programmes for parents and nurses within the set goals and upon the interviews with nurses, which may help improve the nursing care and particularly teach parents to care about their children with facial cleft properly.

The thesis explores the problematics of cyber environment for the ICS/SCADA systems. First, shorter section is mainly focused on general introduction into the ICS/SCADA systems and their inner workings. Communication model of a general SCADA system and its foundational elements are explained. It is mainly theoretical passage and it serves as an introduction. It is necessary for understanding the second part which is mainly practical. The appropriate system is chosen as a first thing in the practical part of the thesis for the implementation of the whole project. There are defined criteria on which the system itself is implemented. Following that the system itself is implemented under a framework called openMUC and it is tested with help of the simulators according to the objective of the thesis.

Digital secondary substations with auto-configuration of station monitoring through IEC 61850 and CIM

This thesis explore the possibility to automate a process for configuration of secondary substations monitoring and control. By using a network information system (NIS), information of secondary substations can be extracted, such as feeder naming, primary equipment type, rating and model. From this information an automated process of configuring the secondary substation is possible, which open up the possibility to cost-efficiently digitalise the distribution grid. In the project, the standard IEC 61850 for configuration of communications of intelligent electrical devices was used to automate and standardize the process. The process starts with a extracted IEC 61970 CIM file from the NIS. The IEC 61970 CIM file is converted into a IEC 61850 SCL file through an system engineering tool. The configuration is based of information from the NIS, where the models and types of the equipments decides what type of functionality that is needed for the secondary substation. With help of the created SCL file hardware and human-machine interface (HMI) was configured, creating a full functional system for the secondary substation monitoring and control equipment. The usage of 400V capable input module together with bus couplers, configured in IEC 61850, lowers the configuration needed for the hardware. The usage of SCL files also helps automate the creation of HMI for the secondary substation through IEC 61850 based tools in SCADA software. Creating views of both single-line diagrams as well as digital representation of the secondary substation outgoing feeders with measured values on display. The result of the project helps show NIS information is sufficient and standards mature enough to allow an almost fully automated system. Lowering the total time spent on each stations configuration to around two hours. Leading the way for future development of automating software for configurations of the secondary substations.

Cleft lip is one of the most common human birth deffects. Its etiopathogenesis is multifactorial and many aspects of its occurrence remain unknown in the fields of both genetics and teratology. One of the set of known negative external factors causing cleft lip is chemical hydrocortisone. Its effect on cell proliferation is highly heterogeneous and depends on attributes of a specific cell population. In this work we studied the cleft beak origin after the hydrocortisone treatment on the basis of Chick Embryotoxicity Screening Test (CHEST). Our main aim was to detect cell cycle changes in the chick frontonasal process after hydrocortisone injection via flow cytometry analysis. Hydrocortisone caused S phase arrest within a minor subpopulation of highly granular cells with specific cell cycle. This sensitive subpopulation was localized in the areas of previously defined proliferative centers within the frontonasal process using immunohistochemistry of frozen sections. Quantitative analysis of cells in these areas revealed significant decrease of M phase portion in the hydrocortisone treated samples in comparison with the control samples. In the power industry, electrical grids are undergoing a modernization intosmart grids. The new smart grids integrate the electrical grid and informationand communication technologies, such as software, automation, and informationprocessing. While enabling remote communication with devices on thegrid, putting the grid online also introduces some major problems, such as therisk of being the target of cyber attacks.In this thesis, a security analysis of the Siemens SICAM CMIC CP-8000 remoteterminal unit, used for remote monitoring and automation of electricalgrids is done. Threat modeling was done to identify vulnerabilities in the system,followed by a penetration test of the web interface, used to configure thedevice, as well as a couple of network attacks. During the penetration test, twocross-site scripting vulnerabilities were discovered, one of which could allowan unauthorized attacker to execute Javascript code in the victim’s browser. Itwas also discovered that a user’s login credentials are leaked in the browserconsole in cleartext when logging in. / I kraftindustrin genomgår elnät en modernisering till smarta elnät. Det upptäcktesäven att en användares inloggningsuppgifter läcktes i webbläsarens konsol iklartext när användaren loggar in.

The topic of this diploma thesis is an analysis of interdisciplinary approach to children with orofacial clefts. The first chapter deals with theoretical base and defines terminology - communication, communication disabilites. It also defines anatomy and physiology of nasal cavity and oral cavity. The second chapter relates to issues of orofacial clefts. It describes classification, etiology, symptomatology, prevention, aestetic and psychosocial impacts and orofacial cleft children care. The third chapter deals with occurence of communication disabilites of children with orofacial clefts. It focuses on the definition of palatholaly, velopharyngeal inadequacy and it describes various linguistic levels and speech therapy and intervention. The fourth chapter is empiric and it deals with analysis of interdisciplinary approach to children with orofacial clefts. In research processes there were used scientific literature analysis, anamnestic data analysis and available specialized materials analysis. Based on all of these materials six case studies were created. Main goal of this empiric chapter was to analyze interdisciplinary approach to children with orofacial clefts. Hodnocení morfologie patra u BCLP pacientů s palatoláliemi / Evaluation of the palate morphology in bilatelar cleft lip and palate clefts with palatolaly

The diploma work was based on the evaluation of dental casts of patients with bilateral cleft lip and palate (BCLP) with a mean age of 10. Patients consist of two groups, patients without defect in speech and with speech impairment (palatolaly). Palatolalies in the literature are primarily associated with velopharyngeal insufficiency. The study tested the working hypothesis that in the failure of speech is involved a different, specific in some way, palatal shape. Dental casts were scanned using a laser scanner and analyzed by 3-D geometric morphometry and multivariate statistics: principal component analysis (PCA), linear regression analysis and finite element analysis (FESA). Using linear regression it was found that the shape of the palate is affected in younger individuals by age, and so had to be 5 patients excluded for further analysis. Patients with palatolaly have lower variability the palatal shape than patients without palatolalie, so their palates are similar to each other and have a specific shape. Palates are wider and lower than in individuals without speech disorder and they have a characteristic deepening behind the anterior part of the palate.
