Geographic Access to Breast Reconstruction and the Influence of Plastic Surgeon Availability

Background: We evaluated geographic patterns for immediate and delayed breast reconstruction (IBR, DBR) in Ontario. The influence of plastic surgeon availability on rates and service provision was determined. Methods: We examined IBR and DBR from 2002 through 2011 across Ontario counties. Regional availability of plastic surgeons was described. Geographic patterns were examined using funnel plots, random-effects models and migration indices. Results: Over ½ Ontario counties have no plastic surgeons. IBR ranged from 0 to 21.5% across counties and differences in plastic surgeon availability explained 41% of variation (p < 0.0001). For DBR there was less variation. 5/45 counties performed ¾ of BR, however rates among local residents were not highest. Interpretation: Nearly 1/3 of the population has limited access to plastic surgeons, contributing to low rates of BR. Geographic access is a major determinant of IBR but is less important for DBR, however service provision for both was highly regionalized.

Validation using 3D CT of the new interpretation of Gerasimov’s nasal projection method for forensic facial approximation

Approximating the facial features for forensic facial approximation is challenging, especially the nose. Numerous methods have been published to position the tip of the nose in profile with variable results. Gerasimov’s two-tangent method is the most commonly used. However, a recent article published by Ullrich and Stephan (2011) states that the method was not properly performed and provides new guidelines. This research used a sample of CT scans from a Denmark population (N=66) to determined which of Gerasimov’s literal translation or Ullrich and Stephan’s (2011) new version of the two-tangent method is the most accurate. A combination of the two methods was also evaluated to determine the effect of each tangent independently, and the effect of intraobserver error. It was determine that the new guidelines result in smaller mean difference but no method can accurately position the tip of the nose due to the lack of experience from the practitioner.

Radial moments for invariant image analysis: computational and statistical aspects

Zernike moments are sets of mathematical quantities that uniquely characterize an image. It is known that they are invariant under rotation and reflection and robust to noise. In this thesis several other algorithms have been used to calculate these moments. The intent of this thesis is: 1. to use the classical method and the algorithms to reconstruct an image using Zernike moments and study their accuracy and 2. to examine if the invariance and noise insensitivity property of the calculated Zernike moments are upheld by these procedures. It is found that the constructed images using these algorithms do not resemble the original image. This prevents us from carrying out further study of these algorithms. The classical method has been successfully used to reconstruct an image when the height and width are equal. The classical method is also shown to be invariant under rotation and reflection and robust to Poisson noise. xxxvii

Ray Traversal for Incremental Voxel Colouring

Image based scene reconstruction from multiple views is an interesting challenge, with many ambiguities and sources of noise. One approach to scene reconstruction is Voxel Colouring, Seitz and Dyer [26], which uses colour information in images and handles the problem of occlusion. Culbertson and Malzbender [11], introduced Generalised Voxel Colouring (GVC) which uses projection and rasterization to establish global scene visibility. Our work has involved investigating the use of ray traversal as an efficient alternative. We have developed two main approaches along this line, Ray Images and Ray Buckets. Comparisons between implementations of our algorithms and variations of GVC are presented, as well as applications to areas of optimisation colour consistency and level of detail. Ray traversal seems a promising approach to scene visibility, but requires more work to be of practical use. Our methods show some advantages over existing approaches in time use. However we have not been as succesful as an- ticipated in reconstruction quality shown by implementation of optimisation colour consistency.

DIVISIONS BETWEEN ARKANSANS IN THE BROOKS-BAXTER WAR

Many historians have failed to consider seriously the role of the Brooks-Baxter War of 1874 in ending Reconstruction in Arkansas. Of those who have, they have not examined participants in the conflict nor attempted a robust study to determine who fought in the conflict. This thesis examines the soldiers and officers of the rival armies of Joseph Brooks and Elisha Baxter. It surveys the participants' class, race, professions, places of birth, and especially places of residence at the time of the conflict. This analysis of the Brooks-Baxter War reaffirms other historians' work on the fall of Reconstruction, while finding unique characteristics to Arkansas's redemption, like substantial support from white Arkansans for upholding Reconstruction and instances of black Arkansans supporting the redeemer army of Elisha Baxter. It concludes that Arkansas redemption was typical of other redemptions in the South in the mid-1870s, insofar as the powerful role that the state Democratic Party and Democratic elites played in ending Reconstruction in the state. The Brooks-Baxter War shows, however, that redemption in Arkansas had a more moderate face in that explicit, naked white supremacist rhetoric was not as apparent in the overthrow of Reconstruction there as in some other Deep Southern states.
