Perceptions and experiences of early-adopting registered dietitians in integrating nutrigenomics into practice

yes / Purpose - This research explores the perceptions and experiences of early adopters of the technology. Design/Method/Approach - Registered Dietitians (RD´s) (N=14) were recruited from the UK, Canada, South-Africa, Australia, Mexico and Israel. Six qualitative interviews and two focus groups were conducted online using a conference calling platform. Data were recorded, transcribed and thematically analysed. Findings - Early adopters of Nutrigenomics (NGx) were experienced, self-efficacious RD’s who actively sought knowledge of NGx through communication with one another and the broader scientific community. They considered NGx an extension of current practice and believed RD’s had the skills to deliver it. Perceived barriers to widening the application of NGx were linked to skepticism among the wider dietetics community. Proliferation of unregulated websites offering tests and diets was considered ‘pseudoscience’ and detrimental to dietetics fully embracing NGx. The lack of a sustainable public health model for the delivery of NGx was also perceived to hinder progress. Results are discussed with reference to ‘diffusion of innovation theory’. Originality/Value - The views of RD’s who practice NGx have not been previously studied. These data highlight requirements for future dietetic training provision and more inclusive service delivery models. Regulation of NGx services and formal recognition by professional bodies is needed to address the research/practice translation gap.

An Interpretive Description of Registered Nurses’ Experiences as Preceptors for Newly Hired Registered Nurses in Acute Pediatric Clinical Settings

Background: In clinical practice, the responsibilities of frontline registered nurses (RNs) include providing patient care as well as precepting students and new staff nurses. The role has been acknowledged as demanding and challenging in acute healthcare settings and the importance of the preceptor role for the ease of the new nurses’ transition to clinical settings has been discussed in the literature. However, less is known about the nurses’ experiences as preceptors in the pediatric acute clinical setting. The purpose of this research was to explore and describe the experiences of RNs as preceptors when they precept newly hired registered nurses in pediatrics settings. Method: An interpretive descriptive design was used to guide the research process. Semi-structured one-on-one interviews, both in-person and virtual, were carried out with five preceptors who are registered nurses in an Ontario hospital. The real-life experiences of these participants are reported using their rich descriptions of events. Data collection and analysis were conducted concurrently and informed by the Braun and Clark (2006) framework. Results: Findings were grouped under the following three themes: the duality role of pediatric nurses; experiences of preceptor pediatrics nurses; and pediatric nurse preceptors’ needs. Implications: This study demonstrates that the role of the registered nurse as a preceptor is diverse and demanding, and that preceptors need to be supported by various stakeholders to ensure that they can carry out their role effectively. Recommendations are made for future practice, education, and research. The goal of this study was to gain an understanding of the registered nurses’ experiences as preceptors for newly hired registered nurses in pediatric healthcare settings. This study showed that preceptors were aware of their dual role, shared experiences of feeling awarded, and discussed challenges they encountered when precepting. Finally, they identified some of their needs that they believed may assist them when being involved in the preceptor-preceptee relationship. It is important to acknowledge the challenges that preceptors encounter when precepting new nurses into the clinical area. Furthermore, it is imperative to prepare nurses for the preceptor role to ensure effective preceptor-preceptee relationships and improve preceptees retention and competence in their role.

The aim of this master thesis is to compare the status of marriage and registered partnerships across branches of law. The purpose of such comparison was to find out whether it is equal or unequal status and indicate any shortcomings of the legislation in this regard. The thesis is divided into five chapters, each dedicated to a different area of comparison or outlining future legal developments regarding this issue. The first chapter describes the history of marriage and registered partnerships, including the development of homosexuality, the conclusion and termination of these institutes and then analyzes their differences in the described area. The second chapter of this thesis deals with comparison of the rights and obligations of spouses and partners in private law. Attention is focused on civil, family and labour law, but the thesis does not omit the private international law. The most important topics in this area are upbringing of child and property arrangements of spouses and partners. Following the third chapter compares rights and obligations of spouses and partners in public law. This thesis analyzes Czech legislation regulating relationships of same-sex couples and their options regarding family founding provided by Czech legal regulation. The aim of the thesis is to analyze and evaluate current Czech legislation and put it into international context. To attain this aim, this thesis uses three different approaches. Firstly it provides conclusions of conterporary social sciences in the field of parenthood of same-sex couples and impacts of homosexual parenting onto various areas of psychological and social adjustment od children. Secondly, the comparative study of czech legislation and legislations of Norway, United Kingdom, Germany and Slovakia is included. Thirdly, relevant cases decided by European Court of Human Rights are analyzed. Upon the findings provided by these analysis, the thesis evaluate suitability of czech legal regulation and propose possible future development in this area.

Perceptions of registered nurses regarding their capability to care for soldiers experiencing mental illness at a military hospital in South Africa

Magister Curationis - MCur / Background: Psychiatric nursing in a military setting is aimed at sustaining and facilitating a healthy lifestyle and performance in the achievement of mental health among soldiers. A Registered Nurse (RN) in the military plays a vital role in patient care from time of admission up to discharge. At military hospitals psychiatric nursing personnel are expected to nurse and manage mental ill patients without the help of security guards. This leaves nurses exposed to violence, abuse and psychological distress resulting in poor communication and interaction between RNs and mentally ill patients. Aim and objectives: The aim of this study was to investigate the perceptions of RNs regarding their capability to care for soldiers experiencing mental illness at a military hospital in South Africa. The objectives of the study were to determine the therapeutic commitment, role support and role competency of RNs towards mentally ill patients. Method: A quantitative-descriptive, cross-sectional study design was used in this study. The population is (N=154) RNs of the identified military hospital in South Africa. All the RNs were accessible at 2 Military Hospital and therefore a total enumeration sample was used with (n=117). Statistical Package for the Social Sciences, (SPSS) version 24 was used to analyse data. Findings: The response rate of the study revealed that there is a correlation to gender in relation to therapeutic commitment and role competency. There was a correlation to RNs that had basic psychiatry in relation to therapeutic commitment and role support. The study also indicated there is a correlation to therapeutic commitment, role support and role competency to the period that military RNs worked in Mental Health (MH). There were no trained advanced psychiatric nurses at the military hospital. There were also positive correlations in therapeutic commitment and role competency in the rank of military RNs.

A COMPARISON OF DEFECTS IN HOUSES CONSTRUCTED BY OWNERS AND REGISTERED BUILDERS IN VICTORIA 1988-1996

The lack of attention to quality control by house builders in the Australian State of Victoria has been a contentious issue for more than two decades. Ina an attempt to improve the quality of housing, various mechanisms such as voluntary and compulsory registration schemes have been adopted and discarded by industry-based organisations and government. While builders are encouraged to improve construction quality, little is known and published about the quality of housing produced by owner builders specifically during the seven year warranty period after construction is completed. With this in mind, this thesis presents research findings that compare the latent defects in houses built by owner builders with those of registered builders. Using inspection reports provided by Archicentre <a subsidiary of the Royal Australian Institute of Architects> a sample of 1772 houses, of which 1002 were owner builders <HO> and 770 were registered builders <HR> was used to determine the severity, the incidence, and location of defects within each house type. Houses less than a year old were found to contain a siginificant proportion of defects for both types of builder. In addition, it was found that HO builders had a mean of 2.74 defects per house and HR builders mean of 2.30 defects per house for the seven-year warranty period. To determine whether there was a significant difference between the quality of housing produced by HO and HR the statistical technique of Chi-squared analysis was undertaken at a 5% level of significance. The analysis revealed that there was a significant difference between the quality of housing procured by owner <HO> and registered <HR> builders. In particular, it was found that the important category of workmanship for HO builders had significantly less defects that HR builders, which suggests that HR builders need to improve their managerial practices and the quality of on-site supervision. In essence, this thesis has provided a series of benchmark metrics for latent defects against which current and future legislative programs con be compared for new housing in the State of Victoria. It is recommended that future research focus on the methods for improving the role of the on-site supervisor as they are considered to be the important link in the quality chain.

The effectiveness of an educational program to teach a group of A.C.T. nurses comprehensive physical assessment skills

A sample of 22 registered nurses participated in an educational program to learn comprehensive physical assessment skills. This study investigated the effects of that program on their nursing practice. Administration of tests and questionnaires provided a measure of the: (a) use of physical assessment skills; (b) knowledge of physical assessment; (c) changes in nursing practice since learning the skills; (d) barriers to the use of the skills. The model used for the program's curriculum development was adult learning theory. The curriculum focused on this theory's key aspects. Namely, adult students build new skills on their past knowledge and already developed skills. Additionally, they must see the relevance of learning the skills and be able to apply them in their current life situation. The findings suggest this model was appropriate. The nurses in the study used the majority of learned skills after they completed the program. Their knowledge concerning these skills also showed dramatic improvement. The major findings regarding the increase in knowledge and use of physical assessment skills, positive changes in nursing practice and lack of barriers to the use of skills indicate that an educational program in physical assessment can be very beneficial to nurses. The results support those of previous studies in that nurses will use the physical assessment skills they have learned in continuing education programs. The implications for nursing practice and education were discussed. Suggestions were then made for future research in this area.
