This research investigated the relationships between clothing interest and self-perception among scoliosis patients and their non-handicapped peers. The objectives were (1) to investigate the differences in selected clothing interest factors and self-perception domains between female adolescents with scoliosis and non-handicapped female adolescents and (2) to investigate relationships between clothing interests and self-perception among female adolescents with scoliosis and non-handicapped female adolescents.
Data were obtained from 70 female adolescents of which 35 had scoliosis and 35 were non-handicapped. A clothing inventory, developed by Creekmore in 1963 and revised by Borsari in 1978, titled "Dimensions of Clothing Interest," was used to measure clothing dimensions of interest, conformity, modesty, psychological awareness, and self concept.
A second instrument, the "Self-Perception Profile for Adolescents", developed by Harter in 1988, was used to measure five domains of self-perception which were: social acceptance, athletic competence, romantic appeal, physical attractiveness, and global self-worth. / Master of Science

Title: Current approaches in conservative treatment of scoliosis - literature review Objective: The goal of this diploma thesis is evaluate the most efective current within a combination of a bracing with exercises therapy. Next goal is to summarize matters of adolescent idiopathic scoliosis disease, especially outline of clinical picture, causes and predisposition for origin these deformity and last but not least options in conservative treatment. Methodics: This diploma thesis is processed as literature review from available literary resources. First part of thesis contains all of theoretical bases and knowledge about adolescent idiopathic scoliosis. Second part has a decriptive-analytical charakter and contains analysis of studies, which was used a bracing supplemented by exercises treatment. Results: Established criteria after finish of review fulfill only nine studies, which interest about efectivness of treatment by bracing in pacient with adolescent idiopathic scoliosis. Because the way of studies and their very low number of studies, is not possible to evaluate results of studies and conclusively interpret, which one is the most effective access. Nevertheless in all of this studies was demonstrated higher efficancy by therapy with probands undergoing a bracing along with active exercise...

Despite its high prevalence and clinical impact, etiology of AIS remains largely unknown. Among the number of proposed hypothesis and observations on the etiopathogenesis of AIS, low bone mineral density (BMD) is one of the most reported factor (Cheng et al. 1999; Hung et al. 2005; Cheung et al. 2006; Hui et al. 2011). However, the underlying mechanism of low BMD in AIS has not been sufficiently studied scientifically and its link to the etiopathogenesis is still not clear. From a previous pilot study, our group has reported the histological features of reduced osteoblastic activity in bone biopsy specimens obtained from AIS subjects intraoperatively, thus providing the early evidence of abnormal bone mineral acquisition and mineralization (Cheng et al. 2001). / Osteopontin (OPN) has been recognized as one the major non-collagen extracellular matrix proteins in bone and plays an important role in bone mineralization. Recent report suggested that AIS patients have higher OPN level than normal controls (Moreau et al. 2009). It was hypothesized that the low BMD in AIS is associated with abnormal bone matrix mineralization which may be related to abnormal expression of OPN in the plasma and at tissue level. / In this series of studies, the first part aimed to investigate the differential cortical and trabecular bone mineral density of AIS Vs normal controls. The non-dominant proximal femur areal BMD (aBMD) (femoral neck, Ward’s triangle and greater trochanter) of the subjects were measured with dual-energy x-ray absorptiometry (DXA). The volumetric bone mineral density (vBMD) in non-dominant distal radius was measured with peripheral quantitative computed tomography (pQCT) that allows accurate three dimensional assessment of the cortical and trabecular bone mineral density and other parameters of bone quality. AIS was found to have lower aBMDs, trabecular BMD (TBMD) and cortical BMD (CBMD) in different age groups and year since menarche (YSM) groups. Furthermore, the percentage difference of CBMD between AIS and controls was increased with age while a decreasing trend was observed in the TBMD. / The second part of the study investigated the bone mineralization and bone micro-architecture with micro-computed tomography (micro-CT) and histomorphometry study of bone biopsies obtained from AIS and normal controls. Three-dimensional structural parameters including material bone mineral density (mBMD) and bone architecture were evaluated by micro-CT. Bone histomorphometry was assessed by undecalcified sectioning with Goldner’s trichrome staining. mBMD of trabecular bone in AIS was found to be significantly lower than the normal control while no difference could be demonstrated in BV/TV, Tb.N, Tb.Th and SMI measurement between the two groups. It was also shown that the percentage of low-mineralized bone in AIS was significantly higher than that in normal controls. / The third part aimed to study the plasma OPN level and its association with the BMD in AIS Vs normal controls. Plasma OPN level in AIS and age-matched controls was measured by ELISA. With multivariate regression analysis, the plasma OPN level was found to be negatively correlated with Age and YSM in both AIS and normal controls. In addition, the plasma OPN level in AIS was significantly higher and correlated with the low trabecular BMD. / The fourth part of the study investigated the OPN expression in bone tissues level and its association with histomorphometric bone mineralization and bone micro-architectural parameters in AIS Vs normal controls. OPN expression in bone biopsy was semi-quantified by immunohistochemistry. It was found that the bone tissue OPN level was significantly higher in AIS and also positively correlated with plasma OPN level. In addition, in this pilot study, we found the trend that OPN expression in trabecular bone was negatively associated with mBMD, and positively with the percentage of low-mineralized bone. / The present study showed that AIS had lower bone mineralization than normal controls. The low cortical and trabecular BMD found in AIS is likely to be resulting from abnormal regulation of bone mineralization. The association of OPN with abnormal BMD and bone mineralization further suggested that abnormal OPN level might play an important role in affecting the bone mineral acquisition in AIS. All of these findings strongly supported the hypothesis that the low BMD in AIS is associated with abnormal bone matrix mineralization which could be related to abnormal expression of OPN. Abnormal skeletal growth and bone remodeling in adolescent idiopathic scoliosis: a morphological and genetic study. / CUHK electronic theses & dissertations collectionJanuary 2006 (has links)
Adolescent idiopathic scoliosis (AIS) is a complex three-dimensional structural spine deformity with lateral curve and vertebral rotation occurring predominantly in adolescent girls during the peri-pubertal period. The prevalence of AIS is nearly 4% in Hong Kong and 2-3% worldwide. AIS without treatment or with improper treatment may deteriorate progressively and lead to significant cosmetic problems and functional disabilities. In severe cases, increased mortality rate can result from the associated early onset of cardiopulmonary failure. Up to now, the treatment of the AIS is basically passive through bracing and corrective spinal surgery. The current protocol of treatment is not totally satisfactory since the curve may continue to progress with brace treatment and corrective surgery is associated with major surgery and permanent fusion of parts of the spine. This is due to the fact that one is still uncertain about the etiology of AIS and therefore cannot directly treat the AIS. Among the different proposed etiology of AIS, the role of abnormal skeletal growth and development during peri-pubertal period has been one of the main focuses in addition to genetic predisposition in the development of AIS. / It has been well established that girls with idiopathic scoliosis have a tendency to be taller and more slender than their peers. Recently, it has been shown that the trabecular bone mineral density at the spine, hip, and peripheral bones of AIS girls was lower than their healthy peers. Studies from our center have also demonstrated growth discrepancy between anterior and posterior vertebral column using magnetic resonance imaging (MRI) technique. The vertebral bodies were shown to be slender in AIS patients than that in normal controls. The observation pointed to a disproportionate growth of the anterior and posterior spinal column resulting from imbalance in endochondral and membranous ossification. The present study hypothesizes that the abnormality of skeletal growth could be a systemic problem affected by both endochondral ossification and membranous ossification. The degree of abnormal growth could vary with different curve severities. The concurrent finding of abnormal skeletal growth and osteopenia could be related to certain underlying abnormal genetic factors affecting the etiopathogenesis of AIS. The hypothesis leads to the following objectives: (1) To study the anthropometric measurements and the related changes in AIS girls with different curve severity; (2) To document the presence of abnormal systemic growth through endochondral ossification; (3) To document the presence of abnormal membranous ossification through studies of the morphology and bone mineral density of the midshaft of the appendicular skeleton and the skull; (4) To study the association of occurrence of AIS and its related phenotypes with the genes associated with growth and osteopenia. A biomechanical study of top screw pullout in anterior scoliosis correction constructsMayo, Andrew January 2007 (has links)
Top screw pullout is a significant problem in anterior scoliosis correction, with rates of 5-15% reported in the literature. The Mater Misericordiae Hospital in Brisbane currently has a series of 125 patients with scoliosis treated by thoracoscopic anterior fusion, instrumentation and correction between April 2000 and August 2007. In this series 11 top screws are known to have pulled out (a rate of 8.8%), with six occurring in the first week, and all within 6 weeks, suggesting that the problem is one of excessive static force rather than fatigue.
This thesis describes a biomechanical investigation into the mechanics of vertebral body screw pullout in anterior scoliosis surgical constructs. Previous biomechanical studies of vertebral body screws have evaluated their resistance to either straight pullout or cephalo-caudad compression forces, however the aim of this study was to assess screw resistance to more realistic loading conditions, namely pullout of initially angled screws, and pullout where the motion path is an arc rather than a straight axial pullout, as would be expected in a single rod anterior construct.
The first series of experiments involved straight and angled pullout tests using synthetic bone. In the angled tests, both locked and free-to-pivot configurations were tested. The second series of experiments tested the effect of cephalo-caudad pre-compression (the actual deformity correction step performed during surgery) on subsequent axial pullout strength. A third series of experiments performed arc pullouts using synthetic bone, and the final series of experiments tested the pullout resistance of a newly proposed screw position configuration against the standard screw positioning using ovine lumbar vertebrae.
Synthetic bone testing revealed that for initially angled pullout, resistance is greatest as the screw angle approaches 0 (ie a direct axial pullout). Cephalo-caudad pre-compression reduced subsequent pullout strength for cases where a staple was not used under the screw head, but if a staple was used the pre-compression did not decrease pullout force significantly. Arc pullout resistance was greatest when the screw was angled at 10 cephalad, and the mean pullout strength for the proposed screw configuration using ovine lumbar vertebrae (1864N) was almost double that of the standard screw positioning (993N).
The clinical implication of this study is that top screw pullout resistance can be maximised by placing the top screw as close as possible to the top endplate and the bottom screw as close as possible to the bottom endplate, although this will have detrimental effects on the pullout of the second screw should the top screw pull out. Screw angulation is a less important factor but any angulation should be in a cephalad direction and around 10º in magnitude. The experimental results also suggest that the use of a staple may play a role in preventing cephalo-caudad pre-compression forces from reducing screw resistance to subsequent pullout forces.

<p>The notochord plays critical structural and signaling roles during vertebrate development. At the center of the vertebrate notochord is a large fluid-filled organelle, the notochord vacuole. While these highly conserved intracellular structures have been described for decades, little is known about the molecular mechanisms involved in their biogenesis and maintenance. Here we show that zebrafish notochord vacuoles are specialized lysosome-related organelles whose formation and maintenance requires late endosomal trafficking regulated by the vacuole-specific Rab32a, and H+-ATPase-dependent acidification. We establish that notochord vacuoles are required for body axis elongation during embryonic development and identify a novel role for notochord vacuoles in spine morphogenesis. Thus, the vertebrate notochord plays important structural roles beyond early development.</p> / Dissertation

Descriptions of obesity, cerebral palsy, and scoliosis were rank-ordered (from one to twenty) and rated on a five-point scale by 75 college students. The descriptions were of two types: with the appearance of an individual on five levels of severity of a disorder and with the appearance and level of functioning of an individual on five levels of severity of a disorder. The ranking data indicated males rank-ordered descriptions of obese individuals different from cerebral palsy (p < 0.01) and scoliosis (p < 0.05) . The ratings of the descriptions were analyzed in two studies with 2 x 2 x 2 x 5 ANOVA's. Three-way interactions of the level of severity, complexity of description, and type of disorder rated were discussed in terms of likeability of handicapped individuals .

Profilage Moléculaire de la Scoliose Idiopathique de l’AdolescentYuan, Qing 01 1900 (has links)
La scoliose idiopathique de l’adolescent (SIA) est la déformation la plus fréquente en orthopédie pédiatrique. C'est une maladie qui génère des déformations complexes du rachis, du thorax et du bassin affectant plus sévèrement et majoritairement les filles à la puberté. Malgré de nombreuses années de recherches approfondies dans le domaine de la SIA, la cause n'a toujours pas été résolue.
OBJECTIFS : Il a été démontré qu’il existe une dysfonction de la signalisation de la mélatonine par les protéines G inhibitrices (Gi) dans les ostéoblastes isolés de patients atteints de SIA. Ceci a conduit à une stratification des patients en trois groupes fonctionnels. Le but de ce projet de maîtrise est d’établir les profils d’expression moléculaire correspondant à chacun des groupes fonctionnels. Les objectifs spécifiques sont d’identifier les gènes responsables de l’initiation de la SIA et du défaut différentiel observé dans la signalisation des récepteurs couplés aux protéines Gi.
MÉTHODES : Des ARNs ont été préparés à partir d’ostéoblastes isolés de patients atteints de SIA sélectionnés pour chaque groupe fonctionnel et comparés aux ARNs obtenus d’ostéoblastes de sujets sains. En plus, des cellules sanguines (PBMCs) isolées d’une paire de jumelles monozygotes discordantes pour la scoliose ont été étudiées pour comparer leur profil d’expression génétique. Nous avons utilisé des biopuces à ADN contenant un ensemble de 54,000 sondes permettant l’analyse du niveau d’expression de plus de 47,000 transcrits représentant 30,000 gènes humains bien caractérisés (Affymetrix, GeneChip® Human gene 1.0 ST array). Les gènes retenus ont par la suite été validés par qPCR sur un plus grand nombre de patients afin de tester la spécificité des profils d’expression pour chaque groupe de patients à partir des cellules osseuses dérivées lors de la chirurgie.
RÉSULTATS: Le profilage moléculaire proposé permettra d’établir les fondements moléculaires de la SIA selon le test fonctionnel développé par le Dr Moreau et son équipe et d’identifier de nouveaux gènes associés à la SIA. Ce projet a permis de mettre en évidence des gènes possiblement impliqués dans le défaut de signalisation associé aux protéines Gi communs aux trois groupes, ainsi que des gènes spécifiques à certains groupes, pouvant contribuer au développement de certaines co-morbidités et/ou au risque d’aggravation des déformations rachidiennes. L’étude préliminaire des jumelles monozygotes discordantes pour la scoliose a mis en évidence un nombre limité de gènes possiblement associés au défaut de signalisation observé chez la jumelle scoliotique, gènes dont l’expression pourrait être influencée par des modifications d’ordre épigénétique liées à l’environnement.
CONCLUSION: Les données obtenues par des approches de transcriptomiques dans le cadre de ce projet soutiennent notre méthode de stratification fonctionnelle des patients SIA et ont conduit à l’identification de nouveaux gènes associés à la SIA. Ces résultats jettent un éclairage nouveau sur la SIA et contribuent à une meilleure compréhension des mécanismes et facteurs impliqués dans l’étiologie de la SIA. À cet égard, nos résultats permettront éventuellement d’identifier des cibles thérapeutiques potentielles et des traitements personnalisés compte tenu des profils moléculaires distincts observés pour chaque groupe de patients. / The adolescent idiopathic scoliosis (AIS) is the most common deformity in paediatric orthopaedics. It is a disease that generates complex deformations of the spine, thorax and pelvis and severely affecting mainly girls at puberty. Despite many years extensive research into the field of AIS, the cause of this disorder has still not been resolved.
OBJECTIFS: We have demonstrated a differential melatonin signaling dysfunction through G inhibitory (Gi) proteins in osteoblasts isolated from AIS patients. This led to a stratification of AIS patients into three distinct functional groups. The goal of this project is to establish molecular expression profiles corresponding to each functional group. The specific objective is to identify genes responsible for the initiation of the AIS and the differential dysfunction in the signaling of Gi protein-coupled receptors.
METHODES: Osteoblasts isolated from AIS patients of each functional subgroup were selected and compared to osteoblasts obtained from healthy controls. In addition, blood cells (PBMCs) isolated from a pair of monozygotic discordant twins of scoliosis was studied to compare their gene expression profiles. We used DNA chips with 54,000 probe sets that allow analysis of expression level of over 47,000 transcripts and variants from over 30,000 well-characterized human genes (Affymetrix, GeneChip® Human gene 1.0 ST array). The identified genes were subsequently validated by qPCR in a larger number of patients in order to test the specificity of expression profiles for each group of patients using bone cells derived during surgery.
RESULTS: The molecular profiling will establish the molecular basis of AIS according to the functional test developed by Moreau et al., and identify new genes associate with AIS. This project may allow us to identify genes involved in the dysfunction of Gi protein signaling which present in all three groups of patients. Also genes associated specifically with a particular group, can contribute to the development of certain AIS co-morbidities and/or to a risk of aggravation
of spine deformities. The preliminary study of monozygotic discordant twins of scoliosis revealed a limited number of genes potentially associated with the signaling defect observed in the scoliotic twin, the gene expression could be influenced by epigenetic changes related to the environment.
CONCLUSION: The data obtained by transcriptomic approaches further support our functional method of AIS patients’ stratification, also allowed the identification of novel genes associated with AIS. Such results would provide us a better understanding of the mechanisms and factors involved in the etiology of AIS. In this regard, our results would help to identify potential and specific therapeutic targets and personalized treatments based on the distinct molecular profiles observed in each patient group.

Title: Evaluation of the effect of conservative approach in the treatment of scoliosis Objective : The purpose of this thesis is to summarize scoliosis issue, natural history, causes, consequences and effectiveness of conservative therapy. Based on the critical literature review there was an intention to evaluate effects of physiotherapy intervention in the treatment of scoliosis and compare individual physical therapy approaches. Methods: This thesis is a descriptive analysis based on a literature review. Results: Based on the research paper only twelve articles met our criteria that address the effect of exercise to influence scoliosis and its consequences. The most commonly used conservative approaches included: Schroth concept, FITS, SEAS, SIR, Klapp-Cotrell concept, Vojta method, program of active self- correction and task- oriented exercise, forward head corrective exercise program, MedX Rotary Torso Machine and LTX 3000. The resulting data are compared with national anthropological research of 2001. The measured values are further statistically analyzed and confronted with the data in scientific literature.
