Combined bioactive approach over atherosclerosis risk biomarkers / Abordagem combinada de compostos bioativos sobre biomarcadores de risco para aterosclerose.

Atherosclerosis, one major cause of morbidity and mortality worldwide, is a complex and multifactorial disease that involves three mainly conditions: chronic inflammation, dyslipidemia and oxidative stress. Although statins are the first-line therapy for LDL cholesterol (LDL-C) lowering, the efficacy of cardiovascular events prevention is limited to 30-40%. This residual risk brought attention to the need of new therapies and clinical targets beyond LDL-C, such as inflammation and oxidative stress. Importantly, suboptimal treatment and/or statin discontinuation due to adverse effects have also been a very challenging clinical problem. Complementary diet therapy can be an effective and safe approach to support pharmacological treatment, especially when drugs alone are insufficient to attenuate risk factors and/or the recommended dose is not well tolerated. The aim of this study was to evaluate the effects of three bioactive components, namely omega-3 fatty acids, plant sterols and polyphenols, on markers of dyslipidemia, inflammation and oxidative stress in patients treated with statins. A randomized, crossover clinical study was carried out, with the participation of 53 subjects. At each intervention period, study participants received a packaged for the functional or control treatment. Functional treatment consisted of fish oil (1.7 g of EPA+DHA/day), chocolate containing plant sterols (2.2 g/day) and green tea (two tea sachets/day). Control treatment consisted of soy oil softgels, regular chocolate and anise tea. After 6 weeks of intervention, functional treatment reduced plasma LDL-C (-13.7% ± 3.7, p=0.002) and C-reactive protein (-35.5% ± 5.9, p=0.027). Plasma triacylglycerol (-15.68% ± 5.94, p=0.02) and MDA (-40.98% ± 6.74, p=0.04) were reduced in subgroups of patients (n=23) with baseline values above the median (93 mg/dL and 2.23 umol/L, respectively). Analysis of lathosterol and campesterol in plasma suggested that intensity of LDL-C reduction was influenced by cholesterol absorption rate rather than its endogenous synthesis. After multivariate analysis, patients identified as \"good responders\" to supplementation (n=10) were recruited for a pilot protocol of statin dose reduction with complementary diet therapy. Responders received the functional treatment for 12 weeks: standard statin therapy was kept during the first 6 weeks and reduced by 50% from weeks 6 to 12. No difference was observed for plasma lipids and inflammation biomarkers, cholesterol efflux capacity or HDL particle number after statin dose reduction when compared to standard therapy. Although limited by the small sample size, our study demonstrates the potential for a new therapeutic approach combining lower statin dose and specific dietary compounds. This may be particularly helpful for the many patients with, and at risk for, CVD who cannot tolerate high-dose statin therapy. / A aterosclerose, uma importante causa mundial de morbidade e mortalidade, é uma doença complexa e multifatorial que envolve três principais condições: inflamação crônica, dislipidemia e estresse oxidativo. Embora as estatinas sejam fármacos de primeira linha para redução de LDL colesterol (LDL-C), sua eficácia na prevenção de eventos cardiovasculares é limitadada a 30-40%. Este risco cardiovascular residual evidencia a necessidade de novas terapias e marcadores clínicos que vão além do LDL-C, como inflamação e estresse oxidativo. Não obstante, tratamento subótimo e/ou interrupção do uso de estatinas devido à ocorrencia de efeitos adversos também é um grave obstáculo na clínica médica. Neste contexto, a terapia dietética complementar representa uma abordagem efetiva e segura para o suporte do tratamento farmacológico, especialmente quando as drogas são insuficientes para atenuar fatores de risco e/ou quando a dose recomendada não é bem tolerada. O objetivo do presente estudo foi avaliar o efeito de três compostos bioativos - ácidos graxos ômega 3, fitosteróis e polifenóis - sobre marcadores de inflamação, lipemia e estresse oxidativo em indivíduos tradados com estatinas. Foi realizado um estudo clínico randomizado, de delineamento crossover, com a participação de 53 voluntários. A cada período de intervenção, os participantes receberam um tratamento funcional ou controle. O tratamento funcional foi composto por cápsulas de óleo de peixe (1.7 g/dia de EPA+DHA), chocolate contendo fitosteróis (2.2 g/dia) e chá verde (dois sachês/dia). O tratamento controle foi composto por cápsulas de óleo de soja, chocolate sem adição de fitosteróis e chá de anis. Após 6 semanas de intervenção, o tratamento funcional reduziu a concentração plasmática de LDL-C (-13.7% ± 3.7, p=0.002) e proteína C-reativa (-35.5% ± 5.9, p=0.027). Triglicerídeos (- 15.68% ± 5.94, p=0.02) e malondialdeído (-40.98% ± 6.74, p=0.04) foram reduzidas apenas em subgrupos de indivíduos que apresentavam valores basais acima da mediana (93 mg/dL e 2.23 umol/L, respectivamente). A análise de latosterol e campesterol no plasma sugeriu que a intensidade da redução de LDL-C não foi influenciada pela síntese endógena de colesterol, mas sim pela taxa de absorção. Após análise multivariada dos resultados, pacientes identificados como \"good responders\" à suplementação (n=10) foram recrutados para um estudo piloto de redução da dosagem da estatina, aliado à terapia dietética complementar. Estes pacientes receberam o tratamento funcional por 12 semanas: durante as 6 primeiras semanas mantevese a dosagem de estatina, que em seguida foi reduzida em 50% das semanas 6 a 12. Não foram observadas diferenças para os marcadores plasmáticos de lipídeos, inflamação, capacidade de efluxo de colesterol ou número de partículas de HDL após a redução da dose de estatina, quando comparada à terapia convencional. Embora limitado pelo reduzido número de pacientes, o estudo demonstra o potencial para uma nova abordagem terapêutica, combinando reduzida dose de estatina com específicos compostos bioativos. Esta pode ser uma importante alternativa para muitos pacientes em risco cardiovascular e que são intolerantes à terapia com altas doses de estatina.

O presente trabalho pr etende avaliar a composição matéria orgânica e os processos micr obiológicos na Lagoa de Araruama (RJ) e relacionar às condições hipertróficas at uais. Neste sentido, foram considerados eventos distintos, tais como período chuv oso, seco e marés (vazante e enchente). Os locais de amostragem da co luna d’água e sedimento foram selecionados para caracterizar o aporte fluvial – estação Ar aruama (AR), a ciclagem interna – estação São Pedro da Aldeia (SP) e as trocas com o oceano – estação Cabo Frio (CF). A elevada produção primária encontrada (concentração média de clorofila: 450 μ g.L -1 ) e as baixas concentrações das formas nitrogenadas na coluna d’água (NO 2 , NO 3 e NH 4 ), indicam consumo pelo fitoplâncton. Fo ram encontradas elevadas concentrações de carbono orgânico dissolvido (média de 17,5mgC.L -1 ), refletindo numa elevada taxa respiratória (média de 2,08 μ MO 2 .L -1 .h -1 ). As concentrações de biopolímeros foram elevadas no sedimento, principalmente li pídios e carboidratos (2,99 e 2,57mg.g -1 , respectivamente), apresentando a relação [lipídios]>[carboidratos]>[proteínas], diferindo ao descrito pela li teratura para o hemisfério norte. O carbono orgânico estocado nos sedimentos da lagoa apresentou valores médios de 5,5%, C/N de 10,4 e δ 13 C (‰) e δ 15 N (‰) de -14,8 e 6,21, respectivam ente. Os valores em geral apontam para uma mistura de fontes ao carbono depositado nos se dimentos. A proporção relativa entre os esteróis pesqui sados demonstrou uma predominância de estigmasterol na maioria das amostras, que poder ia ter como fonte o fitoplâncton uma vez que sua relação com as plantas vascula res não é válida para um sistema com poucos rios. O coprostanol apresentou elevadas concentrações em AR (média de 263 μ g.g -1 ), sendo encontrado em menor escala em Cabo Frio e São Pedro, e se apresentou como melhor indicador de es goto em ambiente hipersalino que os coliformes termotolerantes. A biomassa e atividades bacterianas no sedimento (metabólica e enzimática) apresentara m-se elevadas, evidenciando a intensa ciclagem da M.O. neste co mpartimento e indicando que a lagoa poderá retornar à melhores condições em curto ou médio praz o, caso sejam adotadas medidas efetivas para sua recuperação / ABSTRACT The hypersaline lagoon of Araruama - RJ is suffering, in the last decades, disordered occupation of its drainage basin and consequent contribution of M.O. originating from domestic sewage, altering drastically your trophic characteristics, from oligo to hypertrophic. The present work intends to evaluate the organic matter composition and the microbial processes in the Araruama Lagoon (R J) and to relate to the current hypertrophic conditions. Thus, di fferent events were considered, such as rainy, dry and tide (low and high) periods. The sites for water column and sediment sampling were selected to characterize the fluvial contri bution in Araruama stat ion (AR), the intern cycling - São Pedro da Aldeia station (SP) and the exchanges with the ocean - Cabo Frio station (CF). The high prim ary production found (chlorophyll: 450 μ g.L -1 ) and the reduced concentrations of the nitrogen forms in the water column (NO 2 , NO 3 and NH 4 ) indicate phytoplankton consumption. High c oncentrations of dissolved organic carbon were observed (average of 17.5mgC.L -1 ), resulting in a high re spiratory rate (average of 2.08 μ MO 2 .L -1 .h -1 ). The biopolymer concentrations were high in the sediment, mostly lipids and carbohydrates (2.9 and 2.57mg.g -1 , respectively), presenting the relationship lipids>carbohydrates>proteins, which differs fr om what is described by the literature for the Northern Hemisphere. The organic carbon stocked in the lagoon’s sediments presented medium values of 5,5%, 10,4 for C/N a nd -14.8‰ and 6.21‰ for δ 13 C and δ 15 N, respectively. Generally, those results indicate a mixture of sources for the deposited carbon in the sedi ments. The relative propor tion among the researched sterols demonstrated a predominanc e of stigmasterol in most of the samples, probably coming from phytoplankton. Despite of this compound are commonly related to vascular plants the absence of a high amount of rivers in the drainage basin turn this source without importance. Coprostanol wa s present in high concentrations at AR station (average of 263 μ g.g -1 ), but it was also found in sm all scale at Cabo Frio and less at São Pedro, and it seems to be a better sewage indicator at the hypersaline environment than the termotolerant coliforms. The bacterial biomass and activities in the sediment (metabolic and enzymatic) were high, evidencing the intense cycling of organic matter in this compartment and indica ting that the lagoon can return to better conditions in short or medium period, if e ffective programs for its recovery are adopted

Hygiene Aspects of Greywater and Greywater Reuse

Greywater is domestic household wastewater without inputfrom the toilet, i.e. wastewater from sinks, the shower,washing machine and dishwasher in a home. Source separation ofgreywater can be a strategy to enhance recirculation of plantnutrients and/or improve water use. The risk for transmissionof disease when reusing greywater is largely dependent on thecross-contamination by faeces. High levels of faecalindicators, mainly thermotolerant coliform bacteria, have beenreported in greywater, indicating substantial faecal pollution.However, growth of indicator bacteria within the system leadsto an overestimation of thefaecal input and thus the hygienerisk. The faecal input of the greywater in Vibyåsen,Sollentuna, North of Stockholm, was estimated to be 0.04 ±0.02 g faeces person-1 day-1 from the quantification of thefaecal sterol coprostanol, compared to 65 g, 5.2 g and 0.22 gp-1 d-1 using E. coli, enterococci and cholesterolrespectively. Prevalence of pathogens in the population and the faecalload based on coprostanol concentrations were used to form thebasis of a screening-level quantitative microbial riskassessment (QMRA) that was undertaken for rotavirus, Salmonellatyphimurium, Campylobacter jejuni, Giardia intestinalis andCryptosporidium parvum, looking at the treatment required to bebelow an acceptable level of risk (10-3) for reuse or dischargeof the greywater. The different exposure scenarios simulated–groundwater recharge, direct contact, irrigation andrecreational water–showed that a reduction of 0.7–3.7 log was needed for rotavirus, with the measured level offaecal load in Vibyåsen. The other pathogen of concern wasCampylobacter, where a 2.2 log reduction was needed forgroundwater recharge. The infectious dose of Salmonella is highand the excretion numbers of Giardia cysts and Cryptosporidiumoocysts low, resulting in no treatment requirements for theseorganisms under these circumstances. Pathogen input fromcontaminated food via the kitchen sink had a minor effect onthe microbiological quality of the greywater. Studies on virusoccurrence in greywater as well as validation of the faecalload of greywater at another site would give valuable input forfuture QMRAs. Greywater treatment efficiency studies, especially on virusremoval, are scarce and more investigations are warranted.Active sludge may not be a suitable technique for greywater dueto the low carbon content in this flow. Chemical precipitationhas the advantage of removing phosphorus as well as virusesefficiently and it is suggested as one possible method fortreating greywater. Otherwise the most common practice forgreywater treatment in Sweden is soil infiltration. Untersuchung ausgewählter Inhaltsstoffe in Kaffee und Kaffeezubereitungen

Kaffee ist seit vielen Jahren das beliebteste Getränk der Deutschen. Im Handel wird Kaffee auf mannigfaltige Art angeboten z.B. als Röstkaffee, Instantkaffee oder auch als Bestandteil von anderen Getränken. Eingesetzt werden fast ausschließlich die Kaffee Arten Coffea arabica und Coffea canephora var. Robusta, dabei wird Arabica Kaffee fast doppelt so teuer gehandelt wie Robusta-Kaffee. Werden Kaffees als „100 % Arabica“ ausgelobt, so sollten auch solche Bohnen zu 100 % enthalten sein. Mit dieser Dissertation ist es gelungen, erstmalig mit einer Schnellmethode Robustazumischungen zu Arabica-Kaffee über die 16 OMC Ester zu bestimmen. Außerdem konnte die Analytik der schwierigen Matrix Instantkaffee wesentlich verbessert werden. Weiterhin konnte gezeigt werden, dass durch die Veränderungen der Einstellungen an einem Kaffeeautomaten die Cafestolgehalte im Espresso drastisch reduziert werden können und so das Risiko deutlich minimiert wird, an koronaren Herzerkrankungen zu erkranken. Weiterhin wurde die Sterolfraktion von Roh und Röstkaffees untersucht, dabei wurden erstmalig Sterolglucoside für Kaffee beschrieben. In einem weiteren Abschnitt wurden Milchmischgetränke mit Kaffeezusatz untersucht, um den wertgebenden Inhaltsstoff Kaffee zu quantifizieren.

Novel non phospholipid liposomes with high sterol content : development and characterization

Les liposomes sont des nanovecteurs polyvalents et prometteurs quant à leur utilisation dans plusieurs domaines. Il y a une décennie, un nouveau type de liposome constitué d’amphiphiles monoalkylés et de stérols est né fortuitement dans notre groupe. Ils sont nommés Stérosomes puisqu’ils contiennent une grande proportion de stérols, entre 50 et 70 mol %. Les objectifs de cette thèse sont de développer de nouvelles formulations de Stérosomes ayant des caractéristiques spécifiques et d’acquérir une compréhension plus profonde des règles physicochimiques qui dictent leur comportement de phase. Nous avons spécifiquement examiné le rôle de motifs moléculaires des stérols, de la charge interfaciale et de la capacité à former des liaisons H dans les interactions intermoléculaires menant à l’autoassemblage. Le comportement de phase a été caractérisé par calorimétrie différentielle à balayage (DSC), par spectroscopie infrarouge (IR) et par spectroscopie de résonance magnétique nucléaire du deutérium (²H NMR). Premièrement, nous avons établi certaines corrélations entre la structure des stérols, leur tendance à former des bicouches fluides en présence d'amphiphile monoalkylé et la perméabilité des grandes vésicules unilamellaires (LUV) formées. La nature des stérols module les propriétés de mélange avec de l’acide palmitique (PA). Les stérols portant une chaîne volumineuse en position C17 sont moins aptes à induire des bicouches fluides que ceux qui ont une chaîne plus simple, comme celle du cholestérol. Un grand ordre de la chaîne alkyle de PA est un effet commun à tous les stérols investigués. Il a été démontré que la perméabilité des LUV peut être contrôlée en utilisant des stérols différents. Cependant, ces stérols n’ont aucun impact significatif sur la sensibilité des Stérosomes au pH. Afin de créer des liposomes qui sont sensibles au pH et qui ont une charge positive à la surface, des Stérosomes composés de stéarylamine et de cholestérol (Chol) ont été conçus et caractérisés. Il a été conclu que l’état de protonation de l’amine, dans ce travail, ou du groupe carboxylique, dans un travail précédent, confère une sensibilité au pH et détermine la charge à la surface du liposome. Les premiers Stérosomes complètement neutres ont été fabriqués en utilisant un réseau de fortes liaisons H intermoléculaires. Le groupe sulfoxyde est capable de former de fortes liaisons H avec le cholestérol et les molécules d’eau. Une bicouche fluide métastable a été obtenue, à la température de la pièce, à partir d'un mélange équimolaire d’octadécyl méthyl sulfoxyde (OMSO) et de Chol. Ce comportement distinct a permis d’extruder le mélange pour former des LUV à la température de la pièce. Après 30 h, le temps de vie de la phase métastable, des Stérosomes stables et imperméables existaient toujours sous une forme solide. Un diagramme de température-composition a été proposé afin de résumer le comportement de phase des mélanges d’OMSO/Chol. Finalement, nous avons élaboré des Stérosomes furtifs en incorporant du polyéthylène glycol (PEG) avec une ancre de cholestérol (PEG-Chol) à l’interface de Stérosomes de PA/Chol. Jusqu’à 20 mol % de PEG-Chol peut être introduit sans perturber la structure de la bicouche. La présence du PEG-Chol n’a aucun impact significatif sur la perméabilité de la LUV. L'encapsulation active de la doxorubicine, un médicament contre le cancer, a été réalisée malgré la faible perméabilité de ces LUV et la présence du PEG à l’interface. L’inclusion de PEG a modifié considérablement les propriétés de l’interface et a diminué la libération induite par la variation de pH observée avec des LUV nues de PA/Chol. Cette formulation inédite est potentiellement utile pour l’administration intraveineuse de médicaments. / Liposomes are promising and versatile nanocarriers suitable for potential applications in many fields. A decade ago, a new type of liposomes formed from monoalkylated amphiphiles and sterols was born somehow fortuitously in our group. They are referred to as Sterosomes, because they contain a large proportion of sterols, between 50 and 70 mol %. The objectives of the present thesis are to develop novel Sterosome formulations with specific features, and to gain a deeper understanding of the physicochemical rules that dictate their phase behavior. We have specifically examined the role of the molecular features of sterols, of the interfacial charges and of the H-bond capacity in the intermolecular interactions leading to the self-assembly. The phase behavior was characterized by differential scanning calorimetry (DSC), infrared spectroscopy (IR), and nuclear magnetic resonance spectroscopy of deuterium (2H NMR). First, we have established some correlations between the structure of the sterols, the propensity to form fluid bilayers, and the permeability of the resulting large unilamellar vesicles (LUVs). The nature of the sterol modulates the properties of the mixture with palmitic acid (PA). Sterols bearing a bulky tail chain at C17 are less capable to induce fluid bilayers than those with a non-bulky tail chain, like that of cholesterol. A large ordering of the alkyl chain of PA is an effect exhibited by all of the investigated sterols. It is shown that the permeability of the LUVs can be controlled using different sterols. However, these sterols have no significant impact on the pH-sensitivity of Sterosomes. In order to create liposomes that are pH-sensitive and that have a positive surface charge, Sterosomes composed of stearylamine and cholesterol (Chol) were designed and characterized. It is concluded that the protonation/deprotonation state of the amine (in this work) and carboxylic acid (in previous work) groups confers the pH-sensitivity and determines the surface charge of the liposomes. The first completely neutral Sterosomes were crafted based on the creation of strong intermolecular hydrogen bond networks. The sulfoxide group was capable of forming strong hydrogen bonds with cholesterol and water molecules. In an equimolar octadecyl methyl sulfoxide (OMSO)/Chol mixture, a metastable fluid bilayer was obtained at room temperature. This distinct phase behavior allowed extruding the mixtures to form LUVs at room temperature. After 30 h, the life-time of the metastable phase, stable and impermeable Sterosomes still existed in the solid form. A temperature–composition diagram was proposed to summarize the phase behavior of OMSO/Chol mixtures. Finally, a further step was made to prepare “stealth” Sterosomes by incorporating polyethylene glycol (PEG) with a cholesterol anchor (PEG-Chol) at the interface of PA/Chol Sterosomes. Up to 20 mol % PEG-Chol can be introduced without disturbing the bilayer structure. The presence of PEG-Chol had no significant impact on the permeability of the resulting LUVs. Active-loading of an anti-cancer drug, doxorubicin, can be achieved despite the low permeability of these LUVs and the presence of the PEG at the interface. The inclusion of PEG modified considerably the interface properties and decreased significantly the pH-triggered release observed with naked PA/Chol LUVs. This novel formulation is potentially useful for the application of intravenous administration in the drug delivery field.

Les lagunes côtières sont des écosystèmes semi-fermés à l'interface de la terre et des océans et sous l'influence de ces deux compartiments. De par ces caractéristiques, elles sont soumises à de fortes variations spatiales et saisonnières de leurs ressources trophiques. De par leur nature fermée et peu profonde, elles vont amplifier la force des processus qui conditionnent la disponibilité des ressources alimentaires et par conséquent l'écophysiologie des organismes benthiques qui composent les assemblages associés. Ce travail de thèse se propose de caractériser l'écophysiologie trophique de deux espèces de bivalves filtreurs (Nodipecten subnodosus et Spondylus crassisquama) au sein d'une lagune du Pacifique Nord-est (Basse Californie, Mexique). Une approche mufti-tissus et multi-traceurs (isotopes stables, acides gras, stérols et analyse isotopique sur acides gras) a été adoptée afin de pouvoir caractériser l'écologie trophique des espèces, mais aussi leurs régulations physiologiques face aux variations environnementales. Deux facteurs clés se sont révélés largement explicatifs de la variabilité spatio-temporelle observée dans le régime alimentaire : la période (hiver vs été) et la localisation dans la lagune par rapport à l'embouchure. L'importance des apports de phytoplancton océanique a été démontrée dans l'ensemble de la lagune, avec une contribution plus importante de diatomées océaniques notamment en période estivale pour les individus à proximité de l'embouchure. Par ailleurs, les bivalves en fond de lagune semblaient davantage dépendre de la matière organique locale, en particulier issue du recyclage bactérien, voire de la production des herbiers de phanérogames. Ces contrastes spatiaux se reflétaient également dans la composition des membranes cellulaires, mettant en évidence les interactions entre apports trophiques et régulations métaboliques des compositions en acides gras chez ces deux bivalves. Cette thèse présente une approche intégrée dédiée à la compréhension des variations spatiales et saisonnières des ressources trophiques d'un écosystème lagunaire, ainsi que leurs répercutions trophiques et physiologiques sur les assemblages benthiques. / Coastal lagoons are semi-enclosed ecosystems at the interface between land and ocean and under the influence of these two compartments. Owing to these characteristics these ecosystems are submitted to spatial and seasonal variations in their trophic resources. Because of their enclosed and shallow nature, lagoons amplify the intensity of processes conditioning the availability of food resources and consequently the ecophysiology of the benthic organisms which compose associated assemblages. This thesis aims to characterize the trophic ecophysiology of two species of suspension feeding bivalves (Nodipecten subnodosus and Spondylus crussisquomo) within a lagoon of Northeast Pacific (Baja California, Mexico). A mufti-tissue and multi-tracer approach (stable isotopes, fatty acids, sterols, and isotope analysis on fatty acids) was used in order to characterise the trophic ecology of both species, as well as their physiological regulations against environmental variations. Two key factors have been found to explain the observed spatio-temporal variability: the period (winter vs summer), and the location in the lagoon with respect to the distance from the mouth. The importance of oceanic phytoplankton inputs has been highlighted throughout the lagoon, with a greater contribution of oceanic diatoms particularly during the summer period for individuals near the mouth. Bivalves from the inner lagoon seemed more dependent of local organic matter, in particular resulting from bacterial recycling, or even from the production of eelgrass. These spatial contrasts were also reflected in the composition of cell membranes, highlighting the interactions between trophic contributions and metabolic regulation of fatty acid composition in both bivalves. This thesis presents an integrated approach to understand spatial and seasonal variations of the trophic resources of a lagoon ecosystem, as well as their trophic and physiological repercussions on benthic assemblages.

Phytosterols have been shown to be effective in reducing serum cholesterol levels in numerous human clinical studies and regular consumption is recommended as part of therapeutic lifestyle changes aimed at reducing low density lipoprotein (LDL-C) in the treatment of hyperlipidaemia, a risk factor for cardiovascular disease. Fat based spreads have been shown to be a very successful vehicle for delivery of plant sterols, readily accepted by consumers and efficacious in reducing cholesterol levels. Alfa One™ Rice Bran Oil (RBO) spread is a new product entering into the market place. It is derived from rice bran oil and contains high levels of unsaponifiable material rich in phytosterols, triterpene alcohols, ferulic acid esters ([gamma]-oryzanol) and vitamin E isomers. As such it may have the potential to lower serum cholesterol levels when consumed on a daily basis. In order to establish the effectiveness of Alfa One™ Rice Bran Oil (RBO) spread compared with Flora pro-activ® margarine, a well established brand of plant sterol margarine already proven to lower cholesterol, a randomised double blind cross-over human clinical trial over 12 weeks was conducted. The study was divided into two treatment arms. The first arm of the study was to determine whether Alfa One™ RBO spread (containing 1.5% plant sterols) could lower total and LDL cholesterol levels to a greater extent than standard Flora margarine (containing no plant sterols) or Flora Pro-activ® margarine (containing 8% plant sterols). The second study arm tested the proposition that daily consumption of Alfa One™ Rice Bran Oil (RBO) spread in conjunction with rice bran oil (containing 0.5% plant sterols) would lower total and LDL cholesterol to a greater extent than Alfa One™ RBO spread in isolation and more than Flora margarine in conjunction with sunflower oil. Eighty mildly hypercholesterolaemic individuals (total cholesterol [greater than or equal to] 5 mmol/L and [less than or equal to] 7.5 mmol/L) were recruited and randomised into two groups of forty. Participants were asked to continue with their normal dietary pattern but to replace any margarine/butter/fat consumption with the trial products. One group of 40 were then assigned to the first treatment arm of the study (margarine-only group) and were randomised to consume 20 g (4 teaspoons) Alfa One™ RBO spread daily for 4 weeks, or 20 g Flora margarine daily for 4 weeks, or 20 Flora pro-activ® daily for 4 weeks. Phytosterol levels delivered in these amounts were: RBO margarine: 118mg phytosterol and 14 mg [gamma]-oryzanol; Flora proactiv® 1600 mg phytosterol; Flora margarine 0mg phytosterol. The second group of 40 were allocated to the second arm of the trial (margarine and oil group) and consumed 20 g Alfa One™ RBO spread and 30 ml rice bran oil (RBO) daily for 4 weeks, or 20 g Flora margarine and 30 ml sunflower oil daily for 4 weeks, or 20 g Alfa One™ RBO spread daily for 4 weeks, changing treatment at the end of each 4-week period. Each participant consumed all three treatments in a random order over a 12 week period. At baseline and following each 4 week intervention period, measurements were made of weight and blood pressure. Venous blood samples were collected for analysis of total cholesterol, low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C), total cholesterol: HDL-C, triglycerides and plasma phytosterols. Three-day diet records from each individual were also collected for analysis of normal dietary intake. Results showed that compared to a standard Flora margarine, Alfa One™ RBO spread significantly reduced total cholesterol by 2.2% (P=0.045), total cholesterol:HDL by 4.1% (P=0.005) and LDL-C by 3.5% (P=0.016), but was not as effective overall as Flora Pro-activ® which reduced total cholesterol by 4.4% (P=0.001), total cholesterol:HDL by 3.4% (P=0.014) and LDL-C by 5.6% (P=0.001). Consumption of Flora margarine alone produced no significant decrease from baseline figures in any of the cholesterol parameters measured. Surprisingly, in group two, the addition of rice bran oil to the Alfa One™ RBO spread produced no differences in cholesterol levels. The reason for this unexpected result is being explored further. These results confirm that Alfa One™ RBO spread is effective in lowering serum cholesterol levels when consumed as part of a normal diet. Studies have shown that a 1% reduction in LDL-C can equate to a 2% decrease in coronary heart disease (CHD) risk thus suggesting that the 3.5% reduction demonstrated by Alfa One™ RBO spread in this study could be effective in reducing CHD risk as much as 6% in a mildly hypercholesterolaemic population.
