Additionally, according to the literature, no effective treatment protocol for the painful shoulder has been found. Mötta förväntningar och skattad livskvalitet : En korrelativ studie av mål och utfall efter axeloperation

Sambandet mellan patienters förväntningar inför axeloperation och i vilken utsträckning dessa påverkar den upplevda nöjdheten är inte entydigt. Positiva förväntningar är viktiga samtidigt som orealistiskt höga förväntningar kan ge lägre patientnöjdhet. Uppföljning utifrån individens upplevelse är viktig för att följa resultat och öka patientnöjdheten i en personcentrerad vård. Syftet med denna studie var att redogöra för en grupp axelopererade patienters mål inför operation, måluppfyllelse efter ett år och undersöka eventuella samband mellan olika förväntningar och grad av måluppfyllelse. Syftet var också att beskriva upplevd livskvalitet före och ett år efter operation, korrelera skattad livskvalitet och måluppfyllelse efter ett år samt undersöka eventuella skillnader i skattade värden beroende på bakgrundsfaktorer. Metoden var en  korrelerande prospektiv registerbaserad studie. Huvudresultatet visade hög grad av patientnöjdhet både för skattad livskvalitet och uppfyllelse av personligt mål, sambandet mellan dessa värden var starkt. Smärtfrihet var den vanligaste förhoppningen. Signifikant samband mellan olika förväntningar och uppnått mål sågs i grupperna ¨återgå till arbete¨ och ¨smärtfri nattetid¨. Kvinnor skattade signifikant lägre värden än män före operation men ingen signifikant skillnad kan ses vid ettårsuppföljningen. Slutsatsen var att undersökningsgruppen inför operation hade höga förväntningar och att dessa i stor utsträckning uppnåtts. Likaså sågs tydlig ökning i skattad livskvalitet. Minskad smärta/smärtfrihet var tydligt den viktigaste förväntan inför operation. Denna utvärdering av patientens upplevelse av måluppfyllelse och livskvalitet kan ge återkoppling i arbetet för att stärka och utveckla en personcentrerad vård. / The correlation between the expectations before shoulder surgery and to which extent they affect patient satisfaction is not unambiguous. Positive expectations are important whilst unrealistic expectations could lead to reduced patient satisfaction. Follow up based on the experiences of the individual is important to follow results and increase patient satisfaction in a person centred care. The aim of this study was to narrate the preoperative goals and the fulfilment of these goals after one year for a group of patients who had undergone shoulder surgery and to investigate the eventual relationship between different expectations and degree of goal fulfilment. The aim was also to describe the perceived life quality before and one year after surgery, to correlate perceived life quality and goal fulfilment after one year and to examine eventual differences in assessed values due to demographic factors. The method was a correlative prospective register based study. The main result showed high degree of patient satisfaction for both assessed life quality and fulfilment of personal expectations, the correlation between these values where strong. Relief of pain was the most common expectation. Significant correlation between expectations and goal assessment where seen in the groups ¨return to work¨ and ¨relief of nocturnal pain¨. Women assessed significant lower values than men before operation but no significant differences where seen at the follow-up after one year. The conclusion was that the study population had high preoperative expectations and that these where fulfilled to a great extent. An explicit increase in perceived life quality was also seen. Relief or reduction of pain was clearly the most important expectation before surgery. This evaluation of patients’ sense of goal fulfilment and perceived life quality might give feed back in the work to strengthen and generate a person centred approach.

Interpretation of multi-component induction and sonic measurements acquired in high-angle wells and joint 1D radial inversion of resistivity and sonic logs

Multi-component induction resistivity and sonic measurements acquired in high-angle wells can be strongly influenced by shoulder-bed effects, anisotropy resulting from sand-shale laminations, and presence of mud-filtrate invasion. Understanding the corresponding biasing effects aids in the interpretation of resistivity and sonic measurements and subsequently leads to more accurate and reliable formation evaluation. This dissertation describes numerical simulation studies examining the effects on multi-component induction and sonic measurements in a variety of complex formation models. Subsequently, a joint inversion scheme is presented that combines resistivity and sonic measurements to estimate in situ petrophysical and elastic properties in the presence of mud-filtrate invasion. To facilitate the simulation study of multi-component induction logs, I develop a new finite-difference algorithm for the numerical simulation of frequency-domain electromagnetic borehole measurements. The algorithm~uses a coupled scalar-vector potential formulation for arbitrary three-dimensional inhomogeneous and electrically anisotropic media. Simulations show that shoulder-bed anisotropy: enhances shoulder-bed effects across sand layers; and impacts invasion sensitivities to significantly alter the assessment of invasion in terms of invaded- and virgin-zone resistivities, radial length, and front shape. For the simulation study of sonic logs, I develop a three-dimensional, finite-difference time-domain algorithm that models elastic wave propagation in a fluid-filled borehole. Simulations show that presence of anisotropy not only alters the degree of dispersion observed in flexural and Stoneley waves, but also alters their responses to invasion. In addition, presence of a dipping shoulder bed can significantly distort flexural dispersion, making it difficult to identify the low frequency asymptote corresponding to formation shear wave velocity. Lastly, I consider a radial one-dimensional model in the development of a joint resistivity and sonic inversion algorithm. This scheme simultaneously inverts array-induction apparent conductivities and sonic flexural and Stoneley dispersions for the rock's elastic moduli and water saturation in the presence of mud-filtrate invasion. Inversions are performed on numerically simulated data for a variety of models reflecting soft and hard rock formations with presence of water- and oil-based mud-filtrate invasion. Results show the estimated invasion profiles display excellent agreement with the true models, and the elastic moduli are estimated to within a few percent of the true values. / text

Maximal Versus Non Maximal Muscular Exertions: A Study of Valid Measures Using Isokinetic Dynamometry

Muscle strength capabilities are a determinant in the ability to successfully accomplish everyday tasks. As such, the quantification of this aspect of human performance is of interest in many settings. Currently, the validity of muscle strength test results is reliant on the notion that during testing, the participant exerted an effort that is sincere, and that consisted of maximal voluntary contractions. Therefore, the ability to differentiate between maximal and non maximal muscular exertions is of importance. The purpose of this dissertation was to develop and validate probability-based decision rules for differentiating between maximal and non-maximal voluntary exertions of the knee and shoulder joint musculature during isokinetic dynamometry-based testing. For development of the decision rules, healthy participants performed a series of maximal and non-maximal exertions at different testing velocities through a prescribed range of motion. Two different theory-based approaches were subsequently used for decision rule development: the first approach was based on expected better consistency in strength waveform shapes and relative magnitudes during performance of maximal efforts in comparison to non-maximal efforts. The second approach was based on the known force-velocity dependency in skeletal muscles. In terms of discriminatory performance, several of the decision rules pertaining to the knee joint markedly improve upon those previously reported. In addition, a separate investigation demonstrated that the decision rules offer excellent discriminatory performance when applied to test results of participants that have undergone surgical reconstruction of their anterior cruciate ligament. As such, clinicians and researchers may be able to ascertain voluntary maximal effort production during isokinetic testing of the knee joint musculature with a high degree of confidence, and thus be able to rely on such scores for decision-making purposes With regards to the shoulder musculature decision rules, several methodological issues related to test positioning and signal processing need to be addressed prior to consideration of their use in the clinical domain. / Thesis (Ph.D, Kinesiology & Health Studies) -- Queen's University, 2013-06-19 01:12:53.454

The role of glutamate in rotator cuff tendinopathy : glutamate in rotator cuff tendinopathy

Summary answers: • The glutaminergic system is altered in rotator cuff tendinopathy • Changes within this system are seen after common treatments • Specific glutaminergic changes are associated with the resolution of pain following shoulder surgery but do not predict the severity of pain symptoms • Glutamate has significant effects on tendon derived cells. What is known: It is known that extracellular glutamate concentrations are increased in both Achilles and patellar tendinopathy. It has also been previously shown that the glutamate receptors NMDAR1 and mGluR5 are upregulated in patellar tendinopathy. What this thesis adds: This thesis has shown for the first time that glutamate and NMDAR1 are increased in rotator cuff tendinopathy. Increases in cell proliferation, vascularity and HIF1α are seen after surgical rotator cuff repair and these features are not seen after glucocorticoid injection. There are significant differences between painful and pain-free rotator cuff tendons in terms of glutamate receptor expression (KA1, mGluR7 and mGluR2) and inflammatory cell numbers (CD45 and CD206). Exposure to 1.875mM glutamate for 72 hours results in reduced cell viability, decreased collagen (COL1A1 and COL3A1) and increased aggrecan gene expression; NMDAR antagonism with MK-801 attenuates the deleterious effect on cell viability but had no effect on the changes in matrix gene expression. Bias, confounding and other reasons for caution: The observational histological work was limited by the control tissue. Some control tissue was not age matched, while some of the pain-free control tendons were post-surgical intervention. Confounding factors include tendon structure, length of symptoms and previous treatments. Caution must be applied when discussing the in vivo implications of the in vitro work.

Title Use of thermal imaging camera in evaluation of kinezio taping impact on shoulder girdle for female volleyball players Objectives Aim of this diploma thesis is to find out how kinezio taping affects the temperature of shoulder girdle during the physical load at female volleyball players' smash Methods The thesis is devided into theoretical part which serves as a basic for research in the second empirical part. The complex will consist of a total of 16 female volleyball players. During the measurement we will examine the effect of kinezio taping on the temperature change of shoulder girdle during the physical activity using the thermal camera before and after a physical activity. And at the remaining eight players we eill record the values before and after physical activity without apllied kinezio tape. Results Using the thermal imaging camera we found out that after application of kinezio tape the temperature in the shoulder girdle at rest raises gradually. Title of the thesis Diagnosis of the overload the anterior part of the shoulder joint in elite swimmers and the possibility of compensation Thesis aim The aim of this thesis is to look into a group of elite swimmers with a specific pain in the anterior part of the shoulder joint, to determine the cause of the pain and then to create a plan compensation. Methods The thesis was scientific - research quantitative study, which had the character of an experiment. That was attended by 14 probands between 15-30 years of a different swimming styles of the sport sector ,,swimming". The principle was to determine the causes of pain in the anterior part of the shoulder joint using aspection and palpation examination, examination of joint play and special tests for instability of the shoulder joint, tests on the tendon of the long head of the m. biceps brachii, tests on the rotator cuff and tests of the m. pectoralis major. Finally was built a compensation program, which will prevent the shoulder pain. Results Hypotheses that i chose at the beginning of the research, were confirmed in two cases. The cause of the pain in the anterior part of the shoulder joint in swimmers can not be accurately determined with regard to the swimming style. Title: Effects of PNF to change the motoric stereotype of the abduction in the shoulder joint Objectives: The aim of this thesis, which is conceived as a pilot experimental study is to verify the changes in the timing of selected muscles during stereotype abduction of the shoulder after performing repetitive cyclic motion by therapeutic method PNF if a specific number of repetitions of the movement is performed. Methods: This is a pilot experimental study, which was done on seven probands, 4 men and 3 women with an average age of 26.6 years. The surface EMG muscle activity was scanned on selected muscles during the motoric testing of stereotype of the abduction of the shoulder joint before and after the PNF techniques. The EMG signal was then analyzed and we evaluated the timing of muscles before and after the PNF techniques. The results were then compared in terms of both intra-individually and inter-individually comparison. Results: Results provided contradictory answers to the fundamental questions relating to changes in the timing of selected muscles after PNF techniques. In terms of the change in intra-individually timing after PNF hypothesis was proved. But from the perspective of inter-individually comparison the hypothesis was not proved.
