Psychiatric compulsion and long-term social outcomes for patients with psychosis : is there an association?

Compulsory interventions are widely used in general adult psychiatry for the treatment and care of patients with severe mental illness. While involuntary hospitalisation is established in practice around the world, the use of compulsory interventions outside of hospital – so called community treatment orders (CTOs) – is a more recent development. Three randomised controlled trials of CTO effectiveness have been conducted to date finding no benefits for patients in terms of reduced relapse and readmission to hospital. However, these trials have been relatively short (11-12 months) and focused almost exclusively on CTO effects on clinical functioning and service use. Little attention has been given to patients' social outcomes and broader welfare despite their recognised importance. A sub-sample (n = 121) from the Oxford Community Treatment Order Evaluation Trial (OCTET) were interviewed 48-months after randomisation to assess whether the duration of the CTO intervention was associated with more or less favourable social outcomes at follow-up. Social outcomes were assessed using three main concepts: social networks (Study 1), social inclusion (Study 2), and capabilities (Study 3). No significant associations between the duration of the CTO intervention and social outcomes at follow-up were found. The association between involuntary hospitalisation and social outcomes was also tested but no significant associations were found. Because the field of social outcome measurement is relatively undeveloped, a further aim was to contribute to the validation and testing of two recently developed instruments: the Social and Community Opportunities Profile (SCOPE) (Study 4) and the Oxford Capability Questionnaire for Mental Health (OxCAP-MH) (Study 5). The SCOPE performed well overall as a measure of social inclusion but questions were raised about its feasibility (length) and the reliability of one of its sub-scales. The OxCAP-MH demonstrated good psychometric properties (reliability and validity) and represents a promising new multi-dimensional patient-reported outcome measure for use in mental health research.

Governance of Protected Areas in the Serengeti Ecosystem, Tanzania

The purpose of this dissertation is to assess the contribution of protected areas (PAs) to the attainment of both conservation and social outcomes including poverty reduction within the Greater Serengeti Ecosystem in Tanzania. The research focused on the role of governance as a factor influencing the attainment of these goals. This research was carried out in the Serengeti, Ngorongoro and Meatu districts of Tanzania in 2012. Two villages were selected from each district using stratified sampling techniques to obtain villages bordering many PAs including the core PA (i.e. Serengeti National Park). A mixed methods approach was used in this research which comprised of quantitative interviews with 389 households, key informants interviews with 88 stakeholders, 12 focus group discussion and document analysis. The study used a quantitative questionnaire to measure the effectiveness of governance as perceived at the household level. The questionnaire yielded 10 governance factors that accounted for 85% percent of the explained variance using factor analysis. The findings indicated weak governance particularly for Ikorongo-Grumeti Game Reserve. When examined across the ecosystem, weak linkages were evident between the PA actors and other actors such particularly at local community level. There was no difference in governance scoring between community-based PAs and the more traditional top-down government owned PAs. Furthermore, local communities were not adequately represented in PA governance despite being important actors. Findings indicated mixed results in terms of conservation and social outcomes. Results indicate implied relationship between mixed outcomes and weak governance with weak outcomes thought to be related to weak governance. This study recommends adaptations in the Serengeti ecosystem particularly the re-engineering of the ecosystem governance structure to bring on-board more actors in decision-making and management processes and actions through increased linkages between governance actors, governance structures and processes. / Graduate / 0366 / 0768 / 0478 / akisingo@mwekawildlife.org

Asperger syndrome and emotional intelligence

Individuals with Asperger syndrome (AS), an autism spectrum disorder, are characterized by average to superior intelligence while at the same time experiencing severe and pervasive deficits in social interaction. While many individuals with AS report that they keenly desire social relationships, the combination of repeated social failures and intelligence sufficient to appreciate these difficulties increases the risk for developing depression, anxiety, and other mental health concerns (Tantam, 1998; 2000). <p>Emotional intelligence (EI) is a construct that offers potential to understand individual emotional and social characteristics. The broad purpose of the two studies in this project was to examine ability and trait approaches to EI to understand if EI offers enriched understanding of social outcomes in AS. Further, this study explored EI, executive functions (EF), and theory of mind (ToM) to understand whether EI singularly or in combination with other theoretical explanations best accounts for social outcomes in individuals with AS. <p>The participants in this study were 25 young adults (aged 16-21) diagnosed with AS in Alberta and Manitoba. In study 1, trends and differences between AS and normative groups were examined. Further, correlation and multiple regression were employed to explore relationships amongst variables. Results indicated that trait EI was impaired for individuals with AS; however ability EI was intact. Regression analyses revealed that trait and ability EI together predicted 57% the variance for self-reported interpersonal skills and 31% of the variance for parent-reported social skills. Trait EI alone predicted 19% of the variance for self-reported social stress. <p>In study 2, EI, EF, and ToM were explored as predictors of social outcomes. Low correlations between EF and outcome variables precluded further analysis with this particular set of variables. Multiple regression procedures revealed that together ToM and trait EI predicted 33 % of the variance for self-reported Social Stress. The findings suggest that including ToM and EI measures in assessment protocols for individuals with AS provides important information to inform interventions.

Social Outcomes of Students with Language Impairment: An Eight-Year Follow-up Study

This study re-examined the language and social skills of four females with language impairment who were initially studied eight years ago (Brinton, Fujiki, Montague, & Hanton, 2000; Fujiki, Brinton, Isaacson, & Summers, 2001). Language measures included the Clinical Evaluation of Language Fundamentals-Fourth Edition (Semel, Wiig, & Secord, 2003) and a thirty minute language sample. Social measures included the UCLA Loneliness Scale-Version 3 (Russell, 1996), parent, teacher and student forms of the Social Skills Rating System (Gresham & Elliott, 1990), and an interview with each participant's teacher or speech-language pathologist. Results of the current assessment were compared to results from the original assessment. In terms of social ability, the two girls who were classified as having the best social skills initially, Jean and Kristine, still appeared to be the most successful in the current study. Despite their social strengths, Jean's teacher indicated that she was socially immature and had difficulty reading the social cues of teachers and peers. Kristine reported that she prefers isolation. Her teacher reported that Kristine may be at risk for self-harm. Amy was still enrolled in resource and speech-language services. Though she had found acceptance in a cultural peer group, her communication style often appeared rude and disrespectful to adults. Marie was dismissed from speech-language intervention, but was still enrolled in resource and received extra academic support from Sylvan Learning Center to be moderately successful. Socially, she demonstrated a high level of problem behaviors and mood swings. According to the UCLA Loneliness Scale, she experienced the most loneliness and isolation of the four subjects. Similar to what has been observed in group studies of children with language impairment, the results from this study found social deficits in these individuals persisted into young-adulthood.

Academic and Social Functioning of College Students with Attention-Deficit/Hyperactivity Disorder (ADHD)

Attention-deficit/hyperactivity disorder (ADHD) is a complicated psychiatric disorder that is typically first diagnosed in childhood and associated with negative outcomes in adulthood such as poor academic performance and difficulties with social relationships. ADHD can be difficult to accurately diagnose in adulthood, given the absence of clear, agreed upon ADHD symptomology in adults. In the current study, two raters used psychometrically sound instruments and diagnostically valid assessment techniques on an archival dataset to create three distinct groups: ADHD [2/3 with other mental health diagnosis (OMH)], OMH only, and no diagnosis. Findings support the value of comprehensive assessment, combined with a thorough evaluation of the material by a trained clinician, for the accurate diagnosis of ADHD for research purposes. Comparisons were made across groups to infer that college students with ADHD have lower grade point averages and academic self-concept than students without mental health diagnoses. Yet, contrary to much of the current literature, college students with ADHD seem to create as strong, deep, supportive and harmonious relationships with loved ones and close friends as their non-diagnosed peers. Clinicians working with college students with ADHD may use the results of the current study to better inform conceptualization, better recognize the innate resilience college students with ADHD likely have, and inform treatment interventions.

Community Participation And Travel Choice: An Analysis Of Central Florida New Urban And Conventional Suburban Residents

Previous research has demonstrated a relationship between the built environment and social and transportation outcomes when comparing traditional and conventional suburban neighborhoods, but much remains to be learned about whether New Urbanism can produce similar results. Among studies where new urban neighborhoods have been assessed, most have centered on regions with highly-utilized public transit systems and with climates that are amenable to utilitarian physical activity. This research sought to build on the existing research base through direct comparison of new urban and conventional suburban neighborhoods in central Florida, a region with an under-developed transit system and a climate that renders utilitarian physical activity impractical. Further, this research sought to lend greater insights into neighborhood selection factors across neighborhood types. A mixed-methods, single-case design was utilized to evaluate one new urban and one conventional suburban neighborhood in the central Florida region. Regional new urban neighborhoods were subjectively rated for adherence to tenets of the Charter of the New Urbanism, with the neighborhood (Celebration, in Osceola County, FL) found to most closely adhere to these tenets selected as the experimental group for the study. A socio-demographically comparable conventional suburban neighborhood (Sweetwater, in Seminole County, FL) was selected as the control group. Quantitative methods consisted of a household survey issued to 250 randomly- and convenience-sampled addresses in each neighborhood, followed by regression analysis to evaluate study hypotheses. Qualitative methods employed analysis of open-ended survey responses, detailed case studies of selected neighborhoods, and resident interviews. The household survey yielded net response rates of 15.79 percent and 25.50 percent iv for experimental and control neighborhoods, respectively, and a mean cross-neighborhood response rate of 20.64 percent. Twenty resident interviews (10 per neighborhood) were conducted. Quantitative and qualitative findings were compared to collectively address research questions. Regression results indicated no statistically significant difference between neighborhoods in attitudinal and behavioral components of community participation, in vehicle miles driven per week, or utilitarian physical activity frequency. However, results indicated that new urban residents had more positive attitudes toward utilitarian physical activity than conventional suburban residents and that attitudes toward community participation and utilitarian physical activity were positively correlated with associated behaviors. Qualitative findings provided substantial individual- and environmental-level insights to factors impacting evaluated attitudes and behaviors, and supported some quantitative findings while not aligning with others. Neighborhood selection factors were found to be quite different across neighborhoods: Celebration residents identified neighborhood social atmosphere and connection to the Walt Disney Company brand as top contributors to their selection decision, while Sweetwater residents expressed that access to quality schools was the most important factor in their selection decision. Qualitative findings indicated that car culture and climate within the central Florida region diminished both attitudinal and behavioral components of utilitarian physical activity across neighborhood types. This research expanded the understanding of the social and transportation outcomes of New Urbanism, particularly with respect to the central Florida region. While case and quantitative limitations may have impeded the ability of this study to draw decisive conclusions v about research questions, distinctive themes regarding social and transportation outcomes were identified. Findings of this research supported those of some prior studies while contradicting others, indicating that further exploration is needed to establish a firm understanding of the capabilities of new urban development to achieve desired outcomes, and of regional characteristics that may influence these outcomes.
