Primary health care nursing students' perceptions of the case-based learning approach employed at a selected nursing education institution in Durban : an exploratory-descriptive study.

Background: Case-based learning (CBL) is one approach which is gaining popularity. The selected School of Nursing adopted this approach to learning in 2000. The school reviewed all the old case studies and introduced new ones in 2010. These new case studies were used for the first time with the 2011 Decentralised PHC programme. Hence, the need for a study that explored students perceptions towards case-based learning. Research Methodology: A quantitative approach and descriptive exploratory design were adopted in this study. A total number of 101 students were from the three Decentralised Primary health care (PHC) programme participated in this study. Data was collected from three decentralised sites used to offer PHC programme to students; Durban, Port Shepstone and Pietermaritzburg. Data was collected by means of a questionnaire – a self-reporting instrument which included 62 structured questions. Data was analysed statistically for frequency distributions and the relationship between the results from three sites were explored using a Chi Square Pearson Test, with a p value set at .005. Results: The results of this study show that, overall, students view case-based learning in a positive light. Although the majority reported that cases were distributed in case study booklet form (n=66) 65.3%, other modes such as cases presented to students in each class session using transparencies (n=52) 51.5% as well as through emails was reported. Regarding benefits about (n=90) 90.1% of the respondents stated that cases presented added a lot of realism. The researcher also explored to see if there were any variations of results across the three delivery centres. The findings of this study demonstrated that there was a significant difference for seven items, some of these items included teachers feedback and advise after class was relevant for students to cope on their own p<.000; case studies preparing student for working in PHC institutions in South Africa p<.042; preferred lectures more than CBL p<.003 and CBL was too demanding in terms of preparation and the content p<.004. Discussion: Students enjoyed the experience but also identified their own limitations and mistakes. They were able to plan their own learning needs. The students experienced personal and professional growth using cases which represented the real-life challenges. They had the opportunity to apply the theory, their experiences and their newly developed skills from the use of CBL in there clinical practice. The end product was visible and of benefit to the clinical settings as students was now competent in analyzing cases and solving practical problems systematically. Providing a real-life challenge to students in CBL instead of teaching a predominantly theoretical course proved to be beneficial. Recommendations: The researcher felt that a follow-up study, taking into account all six sites where students are placed could result in a difference in the students’ perceptions of CBL as a teaching methodology, as the students in the other three sites are in the more rural areas which limits their resources. There was some inconsistency with presentation and distribution of cases also feedback to students, staff may require development with teaching skills to co-ordinate all centres in a similar way. Port Shepstone and Pietermaritzburg struggle with CBL, maybe they are not incline with Self directed learning (SDL), therefore a follow up study in these centres would assess in students are inclined with SDL. / Thesis (M.N.)-University of KwaZulu-Natal, Durban, 2013.

Vocation-specific isiZulu language teaching and learning for medical students at the University of KwaZulu-Natal.

Introduction Being an effective communicator is a core competency required of all health care practitioners. Some undergraduate medical students at the Nelson R. Mandela School of Medicine (NRMSM), University of KwaZulu-Natal, Durban, are unable to communicate in isiZulu, the mother tongue of nearly 80% of the 10.2 million people in the province, and the most common home language spoken in South Africa. A one-year isiZulu course to teach communicative competence in the language, and assessed in the first three years of the 6-year MBChB programme, is currently offered at the first year level to medical students. Aim This study was conducted in 2012 to assess how isiZulu clinical communication was perceived by a third year cohort of medical students, and whether current teaching in the period 2010-2012 had prepared them to communicate with their patients. Methods An observational, cross-sectional study design was used to assess the knowledge, attitude and practice of the study group through a self-completed questionnaire. Their knowledge of isiZulu was assessed in a written test, and the students’ marks were compared with their marks in 2010. Oral competence was assessed in an isiZulu history-taking station in the objective structured clinical examination. Students’ comments on their experiences and their recommendations were recorded. Ethics approval was obtained to conduct the study, and informed consent was obtained from participants. Results & Discussion Medical students’ competence in isiZulu had improved. They were largely positive about learning the language of their patients but seldom used the language in the clinical setting. Many students indicated that the current teaching of isiZulu in the programme was inadequate for their needs. Conclusion & Recommendations Although isiZulu is taught to non-isiZulu students, this does not convert into an ability to use the language in the clinical setting. In order for medical students to acquire a satisfactory and safe level of communicative competence in isiZulu, and in line with the UKZN Language Plan to emphasize language training specific to various vocational groups, it is recommended that the teaching, learning and assessment of isiZulu language and culture be integrated into all levels of the undergraduate medical programme in the form of vocation-specific isiZulu for medical students. / Thesis (M.Med.Sc.)-University of KwaZulu-Natal, Durban, 2013.

A process evaluation of the clinical skills laboratory in a private nursing school (KZN)

Background: The Private Nursing School was founded in 1999 in Durban when the first nursing students started the Nursing program. In 2006 there was an upgrade to the Private Nursing School when a clinical skills laboratory was introduced to enhance the students’ clinical skills. The clinical skills laboratory was furnished with the equipment needed for demonstrations, role-plays and lectures to take place, and a need arose to evaluate the laboratory. Purpose: The purpose of this study was to conduct a process evaluation to evaluate the clinical skills laboratory at the Private Nursing School with regard to the quality of equipment, satisfaction of the students and the efficiency of clinical facilitators. Methodology: A process evaluation was conducted of the clinical laboratory, including a quantitative survey to evaluate the students’ satisfaction (n=97), a quantitative audit of the quality of the equipment, a review of the utilisation of the clinical skills laboratory and semi-structured interviews with the four clinical facilitators. Students from two years were purposively selected for the study as they made the most use of the clinical skills laboratory. Survey tools were developed by the researcher. Findings from the surveys and audits were described and Chi-square test and Mann-Whitney to compare satisfaction and beliefs of the students. Information from the clinical facilitator interviews were analysed for common themes on usage and satisfaction. Results: The students and facilitators reported overall satisfaction with the running of the clinical skills laboratory. The findings of the evaluation showed that the clinical laboratory was reaching the target groups with all students making use of the clinical skills laboratory for the purpose of viewing demonstrations, subjecting skills to assessment or practising skills. The students felt that learning did take place in the clinical skills laboratory with most of the students (94, 96.2%) reporting that knowledge was gained from demonstrations and practice. Almost two thirds of the students (59.8%) felt that the support from the clinical facilitators was beneficial to them. Some quality issues in terms of equipment were identified where damaged equipment could not be used by the support staff and facilitators stating that there was a need to improve the simulation experience for the students so that they can gain the much-needed practical and theoretical knowledge required for their stipulated course. Conclusion: The clinical skills laboratory is functioning at a level that is satisfactory to the nursing students and the clinical facilitators. Exploring promotion of primary health care philosophy in a community-based nursing education programme at a selected higher education institution in KwaZulu-Natal : the students' perspective.

The study sought to explore six women from Ilembe District who are managing in different levels within their organisations. The study focused on Principals, Deputy Principals and HOD‟s from different schools. The aim of the study was to find out whether Professional self-advancement has embraced women in Education Leadership with skills that would help them withstand the challenges mostly faced by women leaders that are imposed by the patriarchal gendered background. This qualitative study was set in the interpretive paradigm. It used semi structured interviews and documents analysis, as its method of data collection. The findings revealed that women were reluctant at some point to improve themselves professionally because there is lesser advancement of women into leadership positions, as compared to their male counterparts. It transpired that some women believed that the employment of feminine characteristics of managing embraced women with a sense of worth and responsibility in their organisations. On the other hand some women were pre-conditioned that in order to be recognised as a good leader one had to employ masculine character traits to be acceptable. To most participants professional self-advancement has empowered them to be confident, knowledgeable and be resilient to withstand all negative misconceptions towards them as women leaders. In large majority, women believed that their exposure to management courses has helped them deal with conflicts and improve inter-relations within their organisations. They see themselves as approachable, nurturing, supportive and relational to all their subordinates and this bring meaning to them as women that they have a major role to play to help shape our education. Grogan (2010) hinted that studies pursued by Shakeshaft and Grogan on US women leaders have noted five different characteristics portrayed by women managers which are; leadership for learning, leadership for social justice, relational leadership, spiritual leadership and balance leadership. The study recommends that both the victims and perpetrators of gendered situations should work collaboratively to re-socialise both women and men so as to help women find their self worth within the education system. This could be done by assisting women improve themselves through engaging with institutions of higher learning, attending seminars, motivational talks, enhance networking, have mentors and attend departmental programmes that focus on enhancement of leadership skills. Lastly, for outstanding performance, women should be accepted as they are, to regain their self confidence and exhaust all their capabilities, but not to disregard their mothering flair. Lumby and Azaola (2013) in their study on women principals in South Africa said that, their „mothering identity‟ develops skills even in areas where they report there is considerable doubt about their competence, for example in disciplining boys. In their study they picked that as mothers, they are better able than men to discipline boys. Through exposure to management studies, some participants have found that they use both feminine and masculine style of leadership. Coleman (1996) as cited in Pace and Pace (2005) found that female head teachers exhibit both feminine and masculine qualities, and hence could be identified as androgynous leaders. It is therefore, important that women are exposed to skills development workshops; nurture their studies, engage in networking, and advance their knowledge in every way possible to become better leaders in education. / M.N. Use of electronic databases by postgraduate students in the Faculty of Science and Agriculture at the University of KwaZulu-Natal, Pietermaritzburg.

The purpose of this study was to investigate the use of electronic databases by postgraduate students in the Faculty of Science and Agriculture at the University of KwaZulu-Natal, Pietermaritzburg, with a view to proposing a set of recommendations based on the findings that would contribute to a more effective use of these databases. The study focussed on the use students made of electronic databases provided by the University of KwaZulu-Natal, Pietermaritzburg Library and any other electronic databases which provided information and were needed for their studies. The study population consisted of 500 registered postgraduate students. The survey instrument used to elicit data was a self-administered questionnaire that was distributed to a sample population of 100 postgraduate students. The questionnaire sought to establish if postgraduate students used electronic databases, which electronic databases postgraduate students used and the reasons for their choice, to identify problems postgraduate students encountered, how students became aware of the databases and how often they used the available databases. A total of 65 postgraduate students responded, a response rate of 65%. The results were analysed in terms of frequency responses and they are graphically displayed in the form of tables and graphs. The study found that postgraduate students did use the electronic databases, but a few of the databases were not used. A number of problems were experienced when using the databases. Students became aware of the availability of electronic databases from a variety of sources such as friends, library orientation programmes and academic staff. Search engines were identified as a resource that was very popular with almost all the students. There was a need for training on use of the databases and a need for improving access for all campus and off-campus users. The study was motivated by the 2002 Language Policy for Higher Education (LPHE) that was promulgated by the Department of Education (DoE) in response to its concerns over the alarmingly high failure, dropout and retention rates of particularly black South African students. The LPHE has identified English-medium instruction as the possible main factor in denying the majority of black students’ access to and success in higher education. However, the LPHE is yet to be fully implemented in the country partly due to the fact that sociolinguistic studies among black-African-language speakers indicate that there is a strong preference for English over black African languages in all formal sectors of society, including academia. This preference for English is, in part, a result of the lack of development and the under-resourcing of black African languages in education. Also, black South Africans, while they desire quality mother tongue instruction (MTI), strongly wish to improve their English proficiency. Following on these indications, this study developed and piloted dual language instruction (DLI) (isiZulu-English) teaching and learning course material on academic literacy and communication skills. The purpose of the study was to investigate the extent to which participation in the DLI pilot course might contribute towards ‘attitude change’ as regards the use of isiZulu as a teaching and learning resource alongside English in higher education. The investigation, which took place at Mangosuthu University of Technology (MUT) in the province of KwaZulu-Natal in South Africa, was undertaken among isiZulu-speaking students and their lecturers, all of whom were involved in an Academic Literacy and Communication Skills course for engineers at foundation level. The important finding of this study was that the majority of students indicated from the onset that they preferred to use their primary language as a learning resource while they also valued the role of English in education. This was taken as an indication of positive attitudes to bilingual education. As a result, the use of the DLI pilot course contributed to a minimal attitude change in that after its use there were a few students who for the first time acknowledged the positive role of isiZulu in education. The majority of lecturers also approved of the use and/or the role of L1 in education and indicated support for its use in content subjects. However, the longitudinal investigation of attitudes amongst students in their final year of study revealed a shift in attitude in that the majority identified English as the only language of education. The thesis concluded by suggesting that it is attitudes based on personal experience rather than on preconceived ideas that should inform our decisions on language education policy implementation. An investigation into the use of the Internet by students at St. Joseph's Theological Institute, KwaZulu-Natal, South Africa.

The discussion of introducing the Internet at St. Joseph's Theological Institute in KwaZulu-Natal, South Africa took place in 2002 and access to the Internet was introduced in 2003 via 70 computers which were housed in the library. Given that the Internet has emerged as an important learning tool at the tertiary level, the aim of the study was to investigate the use of the Internet by the students at St. Joseph's. The specific objectives of the study were as follows: to find out which students were using the Internet, to find out what students were using the Internet for, to find how often students used the Internet and when they used it, to find out the problems experienced by those students who used the Internet, to find out why certain students do not use the Internet, and lastly the study aimed to make recommendations concerning the use of the Internet by students. The survey research method was used and the data was collected through a self administered questionnaire. No sampling was done as the entire population of 188 students was surveyed. Sixty five students responded - a response rate of 34.6 %. Findings revealed that of the 65 students who completed the questionnaires 34 (52.3%) had used the Internet and 31 (47.6%) had not. Out of the 24 respondents who gave reasons for not having used the Internet, the majority 17 (70.8%) of respondents said that they had not received formal training and did not know how to use the Internet. The major problems facing the Internet users at St. Joseph 's were the shortage of computers mentioned by 13 (38.2%) respondents and computers being slow as indicated by 13 (38.2%) respondents. Six (17.6%) respondents also pointed to not having enough training in the use of Internet facilities. The use of library resources by doctoral students of the University of KwaZulu-Natal, College of Humanities, Pietermaritzburg campus.

The purpose of this study was to examine the use of library resources by doctoral students of the University of KwaZulu-Natal (UKZN), College of Humanities, Pietermaritzburg campus. The study investigated the extent of the use of the library resources by doctoral students in the College of Humanities, the type of library resources used and the factors that influenced such use. The study also looked at the competency of doctoral students to use them. The study population consisted of registered doctoral students in the College of Humanities as at April 2012 when the data was collected. Out of a total of 118 questionnaires distributed, 99 doctoral students responded, which yielded a response rate of 83.9%. The method that the research took was triangulation where both qualitative and quantitative data were collected. The researcher used one-on-one interviews with subject librarians at the UKZNP Library and a questionnaire as data collection instruments. The quantitative data was analyzed using SPSS version 21, while the qualitative data was analyzed using thematic content analysis. The questionnaire was pre-tested before it was used. The findings of the study revealed that a majority of the doctoral students did use the UKZNP Library resources for their research studies. As regards the importance of the library resources, out of 98 respondents who responded to the question, 77 (78.6%) of the respondents indicated that their use of library resources was because of its importance to their research study. Regarding the dependency on library resources, out of 98 respondents who responded to the question, 60 (61.2%) respondents said their extent of use of the library‟s resources was because they were very dependent on the resources of the library for their research study. In terms of usage of the library‟s resources, out of the 99 respondents who responded to the question, 50 (53.8%) said they use the library‟s resources more for their research study. When investigating the types of library resources used by doctoral students in the College of Humanities, 85 (86.7%) respondents used electronic databases, while 77 (78.6%) used print materials (books and journals); 68 (69.4%) used the inter-library loan service and 60 (61.2%) used theses and dissertations amongst other resources. With regards to the influencing factors for such use of library resources by doctoral students in the College of Humanities, the study found that 93 (97.9%) influencing factors for the respondents was to borrow materials, like printed books and journal articles, for their research studies, while for 52 (54.7%) respondents, a factor was to use inter-library loan services for their research studies and for 38 (40.0%) it was to search the databases for information for their research studies. Furthermore, the study also wanted to determine how competent the doctoral students in the College of Humanities were in using the different library resources of the UKZNP Library. Out of 99 respondents, 79 (79.8%) considered themselves to be competent users of the different library resources with particular reference to electronic resources, while 16.2 (16.2%) considered themselves not to be competent and four (4.0%) did not respond to the question. Having seen the extent of use of the library resources by doctoral students in the College of Humanities, the type of resources the doctoral students used, the factors that influenced such use and the competency of use, the study also discovered that there were challenges that these doctoral students encountered while using the library‟s resources. Some of these challenges were: difficulties in locating resources on shelves as a result of the misplaced and disorganized arrangement of material; out of date printed resource material and insufficient time for borrowing books on reserve. With electronic databases, some of the challenges were that important journals and books were not available electronically, remote access was difficult because of passwords and logins, and some electronic databases only offer abstracts rather than full text articles. In addition, electronic databases were often very slow to conclude searches. A final challenge encountered by the doctoral students was that subject librarians were always very busy attending to students and sometimes not available to help. Recommendations based on the findings of the study were also presented in chapter five and six of this study. Perceptions of undergraduate students of University of KwaZulu-Natal regarding HIV counselling and testing in the year 2012.

South Africa has one of the highest prevalence of HIV infections in the world. Sexual transmission is the primary mode of transmission across the country’s population. Misinformation about AIDS, negative attitudes to HIV testing and AIDS’ stigmatizing beliefs represent potential barriers to seeking HIV testing. Youth between 15-24 years have the greatest risk of HIV infection but only 37% of students at the University of KwaZulu-Natal (UKZN) had undertaken HIV counselling and testing (HCT) according to the HEAIDS (2008) report. Prevention of HIV and behaviour change includes knowledge about, and adequacy of, HIV counselling and testing (HCT) resources on campuses. The aim of this study was to investigate students’ perceptions about HCT at UKZN and make recommendations regarding improvements. An observational, analytical, cross-sectional study of UKZN undergraduate students on the five campuses was conducted. Participants completed anonymous self-administered questionnaires. Of 965 surveyed students, 663 (71%) had tested for HIV among whom 545 (58%) were females and 501(52%) were 20-25 years. Fear of positive results, perceived low risk of acquiring the infection and having confidence in his/her partner influenced student HIV testing behaviour (p<0.001). A significant difference was also found in positive attitudes towards HCT on campus between people who had tested (median score 20) and people who had not tested (median score 18) (p < 0.05). Since one third of students at UKZN had not tested for HIV, HCT at UKZN needs to be advertised more effectively so as to increase HIV testing by students on campuses. / Thesis (M.Med.Sc.)-University of KwaZulu-Natal, Durban, 2013.
