Qualidade de vida em pacientes adultos e pediátricos com vitiligo : estudo baseado em questionários de qualidade de vida genéricos e específicos

Introdução: O vitiligo é uma doença da pele frequente que afeta cerca de 1% da população em todo o mundo. Ocorre em pessoas de qualquer idade ou etnia, e mais da metade dos pacientes desenvolvem a doença antes dos 20 anos de idade. O vitiligo pode afetar negativamente a qualidade de vida (QoL) do paciente. Um questionário de qualidade de vida específico para vitiligo foi desenvolvido e validado na língua inglesa: the vitiligo-specific quality-of-life instrument (VitiQoL). Objetivo: O objetivo deste estudo é avaliar a QoL em pacientes adultos com o VitiQoL e o DLQI e pediátricos com o CDLQI em uma amostra de pacientes com vitiligo no sul do Brasil. Métodos: Na primeira etapa, o instrumento foi traduzido, adaptado culturalmente e validado para o português falado no Brasil (VitiQoL-PB). Após, foram incluídos pacientes selecionados no Ambulatório de Dermatologia do Hospital de Clínicas de Porto Alegre e numa clínica privada de Porto Alegre. Foi realizada amostragem de conveniência de casos consecutivos. A qualidade de vida de pacientes pediátricos foi avaliada com o questionário CDLQI. Em pacientes adultos foram aplicados o VitiQoL – PB e o DLQI. Resultados: Observou-se uma forte correlação entre os escores do total de VitiQoL e o DLQI (r = 0,81; p <0,001). O fator que mais contribuiu para a pontuação final do VitiQoL foi estigma. Em nossa amostra, as mulheres apresentaram escores mais elevados do que os homens (p <0,05). Problemas psiquiátricos foram associados com uma pior qualidade de vida. Na população pediátrica, a mediana do escore do CDLQI foi 3 (intervalo interquartil 1,3-7,3). Houve uma correlação estatisticamente significativa entre a idade da criança e o escore no CDLQI (rs = 0,41, p = 0,044). Não houve diferença entre meninos e meninas (p = 0,219). Conclusão: Este estudo confirma não só que o VitiQoL é fácil de administrar, como também, acrescenta informações importantes sobre o impacto do vitiligo na América do Sul. Instrumentos genéricos são úteis e permitem comparações com outras dermatoses, mas não são suficientemente específicos para detectar nuances na maneira como os pacientes lidam com o vitiligo. A autoestima dos pacientes é muito afetada e a estigmatização está bastante presente. Há grupos de pacientes que são mais vulneráveis ao impacto da doença, como as mulheres, os adolescentes e os pacientes com doenças psiquiátricas. / Background: Vitiligo is a common skin disease that has been reported to affect approximately 1 % of the population worldwide. It affects people of any age or ethnicity, more than half of who develop it before the age of 20 years. Vitiligo can negatively affect patient’s quality of life (QoL). A specific questionnaire for vitiligo has been developed and validated in the English language: the vitiligo-specific quality-of-life instrument (VitiQoL). Objective: The aim of this study was to access the QoL in adult patients through VitiQoL and DLQI and pediatric patients through CDLQI in a sample of patients with vitiligo in Southern Brazil. Methods: In the first phase of the study the instrument was translated, cultural adapted and validated into Brazilian Portuguese (VitiQoL-PB). Then patients were selected from a Dermatological Outpatient Clinic from Hospital de Clínicas de Porto Alegre and from a Private Practice in Porto Alegre. In this study, we used convenience sampling of consecutive cases. The QoL of pediatric patients was performed using the CDLQI questionnaire. In adult patients we applied the VitiQoL – PB and the DLQI. Results: A strong correlation between the scores of the total VitiQoL and DLQI was observed (r = 0.81; p <0.001). The factor that most contributed to the final score of VitiQoL was stigma. In our sample, women had higher scores than men (p<0,05). Psychiatric problems were associated with lower QoL. In the pediatric population, the median score of CDLQI was 3 (interquartile range 1.3 to 7.3). There was a statistically significant correlation between the child's age and the score on CDLQI (rs = 0.41, p = 0.044). There was no difference between boys and girls (p = 0.219). Conclusion: This study confirms that VitiQoL is easy to administer and adds important information about the impact of vitiligo on a South American population. Generic instruments are useful and allow comparisons with other dermatoses, but are not specific enough to detect nuances in how patients deal with the overall vitiligo burden. A profound identity: evidence of homogeneity in Gabriel Fauré’s thirteen piano Nocturnes

In this dissertation, I argue that Fauré’s reuse and recombination of certain material musical elements throughout his life deserves recognition as a central aspect of his musical style. The inspiration for my work has primarily been drawn from the work of the musicologist Carlo Caballero, who argues that Fauré would have believed that consistency of style over time, or homogeneity, is a crucial property of any music that is truly original. Yet, few scholars have analyzed the degree to which homogeneity is manifest in Fauré’s music. More often, their focus has been directed at analyzing topics within segments of the three major periods of Fauré’s compositional output and highlighting stylistic contrasts between those periods. In contrast to this more common approach, I build on Caballero’s ideas to present a two-stage argument. First, I delineate certain criteria fundamental to defining a musical style, and observe that a straightforward classification of Fauré’s music is not easily discerned upon applying those criteria to Fauré’s music. Second, I offer support for the more novel perspective that acknowledging homogeneity itself, absent the need to classify the music materially, constitutes an important, informative perspective on Fauré's musical style. The dissertation is structured in two parts. Part I, encompassing the first three chapters, reviews the struggle to gain widespread understanding that Fauré’s music has historically faced. Chapter 1 discusses the criteria of style as they have historically been understood and used; Chapter 2 applies those criteria to Fauré’s music; and Chapter 3 explores Caballero’s assertion that Fauré’s music should in fact display homogeneity. In order to support Caballero’s claim, the following seven chapters of Part II present analytic evidence from the piano Nocturnes. Chapters 4 through 10 identify recurring usages of harmony, tonality, motivic processes, formal events, and textures by Fauré in Nocturnes dating from all three major periods of his compositional life. I conclude that a strong degree of homogeneity has been demonstrated across the piano Nocturnes, and suggest ways in which that perspective can contribute to more widespread understanding and dissemination of Fauré’s music.

A relação de Georg PhilippTelemann (1681-1767) com o gosto francês remonta ao início das atividades musicais do compositor, com ponto alto em sua viagem a Paris (1737), se estendendo até o final de sua vida. Esta pesquisa investiga as categorias de quartetos setecentistas a partir das definições de Scheibe (1740) e Quantz (1752), traçando o percurso dos gêneros e estilos nos quatuors de Telemann - com ênfase nos Nouveaux Quatuors (Paris, 1738). Este trabalho dá especial atenção ao estilo de conversação, ou diálogo - mencionado por Brossard (1708) e Mattheson (1739) - em contraste com os quartetos sonata em estilo contrapontístico anteriores. No que se refere ao estilo, caráter e função (delectare), o gênero de conversação encontra eco nos textos franceses setecentistas que tratam da arte da conversação galante em voga nos salões franceses no séc. XVIII. Propõe-se estudar mais profundamente os gêneros e estilos nos quartetos de Telemann, abordando desde seus primeiros, não publicados, a suas últimas coleções (publicadas em Hamburgo e em Paris), a fim de investigar como o compositor foi gradativamente desenvolvendo novas maneiras de compor a 4. / The relationship of Georg Philipp Telemann (1681-1767) with French taste goes back to the beginning of the composer\'s musical activities, with highlight on his trip to Paris (1737), to the end of his life. This research investigates the categories of eighteenth-century quartets starting from Scheibe (1740) and Quantz (1752) definitions, tracing the route of genres and styles in Telemann\'s quatuors - with emphasis to Nouveaux Quatuors (Paris, 1738). Special attention is given to the conversational style, or dialogue - mentioned by Brossard (1708) and Mattheson (1739) - in contrast to the previous quartet sonatas in contrapuntal style. With regard to the style, character and function (delectare), the conversation genre is echoed in eighteenth-century French texts dealing with art of gallant conversation in vogue in the French salons in XVIII century. It is proposed to study more deeply the genres and styles in Telemann\'s quatuors, analyzing from his first, unpublished, to the latest collections (published in Hamburg and Paris) in order to investigate how the composer was gradually developing new ways of composing for 4 voices.

The exploration of maternal perception of compliance in adolescents

Objective: To explore 'Maternal Perception of Compliance in Adolescents Model.' The purpose of the model is to explore interrelated factors outside adolescents' agency, but which contribute to maternal perception of severity of compliance in adolescents. This model was originated from reflections on the practice of psychology. Methodology: This inquiry began with pilot studies consisting of a focus group with four parents and an interview. The pilot assisted the fine-tuning the instrument, addressing ethical concerns, forming analytical strategies and gathering a priori themes for the analysis of data from the thesis. The thesis is a qualitative inquiry, data was collected through interviews and utilised a 'maximum variation sampling' of ethnicity (Asian, White, and Black) with socioeconomic status (SES) (low, medium, and high), to recruit six mothers who had adolescent children. Analysis: There was a hybrid approach to the thematic analysis: a) in 'confirmatory analysis,' a priori themes were deductively confirmed and b) in 'exploratory analysis,' new emergent themes were inductively identified. In the first part of the analysis each participant's data was individually analysed, using the hybrid method, to explore the factors that contributed to the model. The second part of the analysis was a 'cross-case analysis,' in which factors identified as contributing to the model, in the first part of the analysis were consolidated to further explore the model. Results: Various types of knowledge (self-reflective, observational, academic, transgenerational, maternal), notions of an ideal child, expectations, style of parenting, style of attribution, maternal anxiety, SES, and single-motherhood were identified as having an effect on the model. In terms of exploration of the model: a) The expectations were largely affected by maternal ideals and standards about adolescents. b) These ideals and standards were derived from their previously acquired knowledge. c) Mothers used the resources at their disposal based on their SES and authoritative parenting style to nurture the child toward their notion of an ideal child. d) The maternal anxiety/single mother/permissive parenting style and some types of knowledge affected the expectation of degree of compliance. e) Mothers often rationalised or justified children's noncompliance through a system called the 'mediating mechanism.' f) Some types of knowledge, mixed parenting style, and external attribution were involved in the 'mediating mechanism.' g) Maternal perception of the extent of adolescents' compliance was based on their degree of expectation of compliance, i.e. the more divergence between the expectations and perception of compliance, the more severe or problematic the perception of compliance. h) Maternal views on compliance varied between mothers, based on their parenting styles. Conclusions: Maternal perception of severity of compliance is not entirely based on children's actual behaviour; rather it is influenced by number of factors that are outside the children's agency. The model, which illustrates how maternal perception of severity of compliance is formed, could have a significant impact on diagnosis and treatment options for compliance related disorders. The subjective diagnostic criteria for evaluating compliance in the DSM-5, augment this complexity in diagnosis and treatment. A larger scale replication of the study is warranted to elicit generalisation.

Happiness at work in the UAE : the role of leadership style and human resource management

There is a wealth of literature on job satisfaction from both Western and Eastern experiences and perspectives, but the literature is limited when it comes to happiness at work, especially in the Arab region. This study is an attempt to encourage research studies in the field of happiness at work in the Middle East and specifically in the United Arab Emirates, the context of the present research, to influence planning in government organisations in implementing a strategy for long-term impacts on happiness levels. This research uses a mixed methods approach of both questionnaires and interviews to collect data. The analysis is conducted by using quantitative and qualitative tools to recognise the gaps and the factors that influence happiness at work and suggests recommendations in order to raise happiness across organisations over the long run. The findings of this study emphasise the necessity of correlating HRM practices, rules and systems with strategic planning that focuses on employee happiness as the greatest purpose of an organisation. The findings emphasise the power of three factors related to employees' feelings towards work which are: Leadership style, organisational culture and organisation structure and how reflecting them in an organisation will positively influence satisfaction and build a strong, positive and happy workplace. This research also discusses one of the controversial inquiries found in the literature about the relationship between job satisfaction and happiness. A conceptual framework is suggested to illustrate the connection between job satisfaction and happiness from a new perspective and clarify how these two concepts positively interact. It clarifies that organisations that indicate a high-level of happiness at work usually indicate a high-level of job satisfaction, and when an organisation achieves a high-level of job satisfaction, it can be deduced that people in the organisation are happy, even where happiness per se is not measured. The findings also show that the role of religion in raising happiness in general is clearer than its role in raising happiness at work and that employees from the sample group, who emphasise the importance of practicing religious rituals, still indicate low feelings towards work regardless of all the facilities that are provided to them to practice their religion.

Independentemente de tais distúrbios, esses estudantes, como quaisquer outros indivíduos, possuem um repertório de cognições, atitudes e habilidades que pode ou não ser coerente com a competência necessária para a atuação profissional. As grades curriculares dos cursos de graduação em nutrição tem predomínio de disciplinas da área biológica, sendo a abordagem psicossocial do comportamento alimentar pouco desenvolvida. O presente estudo propôs-se a caracterizar o comportamento alimentar, o estado nutricional e a imagem corporal de estudantes de Nutrição quando de seu ingresso e ao final do último ano da graduação, discutindo-o à luz da abordagem psicossocial. A população foi formada por 167 estudantes do sexo feminino, ingressantes (n=64) e concluintes (n=103), de três instituições de ensino do interior do Estado de São Paulo, com média de idade de 21,6 anos (ingressantes) e de 24,5 anos (concluintes). A metodologia adotada foi a quantiqualitativa, mediante levantamento de dados antropométricos e recordatório alimentar de 24 horas, identificação do estilo alimentar (pelo Questionário Holandês do Comportamento Alimentar), das atitudes relativas à imagem corporal (Escala de Silhuetas) e realização de grupos focais, para questionamento das atitudes, percepções e sentimentos relativos à alimentação e imagem corporal, ao início e quando da conclusão do curso. Os resultados mostram 10,9% de excesso de peso entre as ingressantes... / Eating behavior and body image of nutrition students has been the subject of increasing interest and research, given the occurrence of eating disorders and restrictive feeding in this population. Regardless of such disorders, these students, like any other individual, have a repertoire of cognitions, attitudes and skills that may or may not be consistent with the skills necessary for professional practice. The curriculum of undergraduate nutrition courses has a predominance of disciplines of biological and psychosocial approach of eating behavior is poorly developed. This study aimed to characterize the eating behavior, nutritional status and body image of nutrition students at the beginning and in the last year of undergraduate course, according to the psychosocial approach. Participants for this study were 167 female students (64 freshmen and 103 graduates) from three institutions of Sao Paulo State, aged between 21.6 (freshmen) and 24.5 (graduates). The methodology was quantiqualitative by anthropometric profile, the 24 h recall, eating style identification (Dutch Eating Behaviour Questionnaire), attitudes toward body image (Silhouette scale) and focal groups to investigate the attitudes, perceptions and feelings related to eating. V této práci se zabývám firemní komunikací. Pro její plánování a rozvoj je třeba udržovat aktuální komunikační a informační strategii. Pro zjednodušení jsem systém firemní komunikace rozdělil na 4 subsystémy: Kultura, informační systém, komerční komunikace a firemní styl, z nichž každým se podrobně zabývám. Společným působením těchto čtyř disciplín vzniká unikátní identita organizace a toto jsou její základní kameny. Jedním ze způsobů, jak docílit silné identity, je budování promyšleného a stabilního firemního stylu. Firemní styl je úžeji vymezen jako vizuální tvář podniku, tzv. Corporate Design. Jeho základními elementy jsou: logotyp, barevnost a typografie. Přestože firmy si mohou být v těchto základních elementech podobné, odlišují se jejich aplikacemi na komunikační aktivity. Každá vizuální identita se stává unikátním otiskem složitého firemního organismu. Účelem aplikací není pouze marketingový úspěch či vizuální dokonalost; ten spočívá v jejich funkčnosti. Proto je při jejich tvorbě nutné respektovat daleko více kritérií.
