Antimycobacterial activity of synthetic compounds isolated from South African medicinal plants against mycobacterium tuberculosis

The disease spreads easily in overcrowded, badly ventilated places and among people who are undernourished. Trends in the incidence of TB together with the development of multi-drug (MDR-TB) and extensively drug resistant (XDR-TB) strains of TB raises the need to intensify the search for more efficient drugs to combat this disease. Herbal remedies used in traditional medicine provide an interesting and largely unexplored source for the discovery of potentially new drugs for infections such as TB. The aim of the study was to evaluate the in vitro antimycobacterial activity of synthesized compounds from medicinal plants against Mycobacterium tuberculosis (M. tuberculosis). About 40 synthesized compounds isolated from South African medicinal plants were screened against H37RV using microplate alamar blue assay (MABA). Identified active compounds were screened against resistant strains of M. tuberculosis (MDR, XDR and pre-XDR) and sensitive clinical isolates of TB. Cytotoxicity and synergistic drug combination studies were done on active compounds to validate their toxicity and synergy levels. Cytotoxicity was done by sulforhodamine assay (SRB) against the C2C12 cell line. Only six compounds showed activity against M. tuberculosis with minimum inhibitory concentration (MIC) below 10μg/ml. The results obtained indicated that the cytotoxicity effects of the three compounds on C2C12 cells demonstrated marginal toxicity except for MVB 282/61215 which showed a high toxicity at the lowest concentration of 0.156μg/ml with over 100% viable cells at the highest concentration (5μg/ml). MVB 282/61271 had the highest percentage cell viability (65%) at the lowest concentration. Only two compounds had a higher potency evoking a bigger response at low concentrations with treated cells still viable after 3 days of incubation with the compound which was comparable with the treatment of isoniazid (INH). Synergistic activity of the six compounds was less in INH combination as compared to the rifampicin’s (RIF) combination. The results demonstrated that the synergistic interaction between the compounds and RIF could the antituberculosis acitivity. In conclusion the synergistic effects with RIF translate to lower dosing requirements of the compounds and the potential to combat multidrug resistant TB. In deed there is no doubt that natural products, with their range of interesting chemical structures and powerful antimycobacterial effects are certain to remain important participants in the development of new generations of antimycobacterial drugs.

Development of a Prototype GeneXpert Assay for Blood-Based Tuberculosis Case Finding

Ett original set av mRNA biomarkörer har nedselekterats till 9 biomarkörer som utgör mRNA signaturen i 10-plex analysmetoden. Urvalet av biomarkörer baserades på probescreening och optimeringstudier utfört med Cepheids ”in cartridge” qPCR teknologi. Den diagnostiska prestandan av mRNA signaturen utvärderades genom att analysera 380 antigenstimulerade kliniska prover klassificerade som M. Tuberculosis infekterad, aktiv tuberkulos, latent tuberkulos och icke infekterad. mRNA signaturens förmåga korrekt klassificera samt diskriminera aktiv och latent tuberkulos bestämdes genom att implementera ROC-kurvor vilket resulterade i ett optimalt AUC-värde på 0.784 vilket visar på diagnostisk kapacitet att separera aktiv och latent tuberkulos. / A 10-color prototype GeneXpert assay based on distinct mRNA expression profiles in ex-vivo antigen stimulated whole blood has been developed at Cepheid for diagnosis of M. Tuberculosis infection and separation of active and latent tuberculosis. An original larger set of mRNA biomarkers has been downselected to a set of 9 biomarkers constituting the mRNA signature in the 10-color assay. The downselection of biomarkers was based on probe screening and optimization studies performed with Cepheid’s “in cartridge” qPCR technology. The 10-color assay was evaluated for diagnostic performance by analyzing 380 banked antigen stimulated clinical samples classified as M. Tuberculosis infected, active tuberculosis, latent tuberculosis and not infected. The ability of the mRNA signature to correctly classify and separate active and latent tuberculosis was determined by implementing ROC curves using delta Ctvalues as test variables and resulted in an optimal AUC value of 0.784 indicating ability to separate active and latent tuberculosis.

Adjunct n-3 Long-Chain Polyunsaturated Fatty Acid Treatment in Tuberculosis Reduces Inflammation and Improves Anemia of Infection More in C3HeB/FeJ Mice With Low n-3 Fatty Acid Status Than Sufficient n-3 Fatty Acid Status

Populations at risk for tuberculosis (TB) may have a low n-3 polyunsaturated fatty acid (PUFA) status. Our research previously showed that post-infection supplementation of n-3 long-chain PUFA (LCPUFA) in TB without TB medication was beneficial in n-3 PUFA sufficient but not in low-status C3HeB/FeJ mice. In this study, we investigated the effect of n-3 LCPUFA adjunct to TB medication in TB mice with a low compared to a sufficient n-3 PUFA status. Mice were conditioned on an n-3 PUFA-deficient (n- 3FAD) or n-3 PUFA-sufficient (n-3FAS) diet for 6 weeks before TB infection. Postinfection at 2 weeks, both groups were switched to an n-3 LCPUFA [eicosapentaenoic acid (EPA)/docosahexaenoic acid (DHA)] supplemented diet and euthanized at 4- and 14- days post-treatment. Iron and anemia status, bacterial loads, lung pathology, lung cytokines/chemokines, and lung lipid mediators were measured. Following 14 days of treatment, hemoglobin (Hb) was higher in the n-3FAD than the untreated n-3FAS group (p = 0.022), whereas the n-3FAS (drug) treated control and n-3FAS groups were not. Proinflammatory lung cytokines; interleukin-6 (IL-6) (p = 0.011), IL-1a (p = 0.039), MCP1 (p = 0.003), MIP1- a (p = 0.043), and RANTES (p = 0.034); were lower, and the antiinflammatory cytokine IL-4 (p=0.002) and growth factor GMCSF (p=0.007) were higher in the n-3FAD compared with the n-3FAS mice after 14 days. These results suggest that n-3 LCPUFA therapy in TB-infected mice, in combination with TB medication, may improve anemia of infection more in low n-3 fatty acid status than sufficient status mice. Furthermore, the low n-3 fatty acid status TB mice supplemented with n-3 LCPUFA showed comparatively lower cytokine-mediated inflammation despite presenting with lower pro-resolving lipid mediators.

Development of a Fluorescent Drug Screening Platform for Inhibitors of Mycobacterium Tuberculosis Protein-Protein Interactions

Tuberculosis (TB) is a respiratory disease caused by Mycobacterium tuberculosis (Mtb) that kills around 1.3 million people annually. Multi-drug resistant TB (MDR-TB) strains are increasingly encountered, in part resulting from shortcomings of current TB drug regimens that last between six to nine months. Patients may stop taking the antibiotics during their allotted regimen, leading to drug resistant TB strains. Novel drug screening platforms are therefore necessary to find drugs effective against MDR-TB. In order to discover compounds that target under-exploited pathways that may be essential only in vivo, the proposed screening platform will use a novel approach to drug discovery by blocking essential protein-protein interactions (PPI). In Mtb, PPI can be monitored by mycobacterial protein fragment complementation (M-PFC). This project will re-engineer the M-PFC assay to include the red fluorescent mCherry reporter for increased efficiency and sensitivity in high-throughput screening applications. To optimize the mCherry assay, we have developed fluorescent M-PFC reporter strains to monitor distinct PPI required for Mtb virulence: homodimerization of the dormancy regulator DosR. A drug screen will then identify novel compounds that inhibit this essential PPI. The screen will involve positional-scanning combinatorial synthetic libraries, which are made up of chemical compounds with varying side chains. This work will develop novel tools for TB drug discovery that could identify new treatments for the emerging world threat of MDR-TB.

BACKGROUND: The prevalence of tuberculosis (TB) has been rapidly on the ascendency in the recent years globally due to its co-infection with HIV/AIDS. TB case finding is one of the technical pillars of the Directly Observed Treatment Short course (DOTS) TB strategy and there has been advocacy for early TB case detection to be the new focus of TB control efforts. PURPOSE: The purpose of this non-experimental study was to assess the determinants of TB case finding among pulmonary TB patients registered for treatment in Makana Municipality METHOD: A quantitative, non-experimental, cross sectional descriptive study among PTB patients registered for treatment at the primary health care clinics in Makana Municipality was done. Data collection was by self-administered questionnaires while sampling was by systematic sampling of PTB patients at five systematically selected clinics. RESULTS: Patient–related delay contributed more to total delay, in this study sample, than health system-related delay. Health system delay was found to be significantly associated with poorer finances, passive smoking history, seeking care from multiple health providers, initially visiting a non- NTCP health provider, TB stigma , overcrowding in the household and having difficulty with breathing as an initial symptom (p<0.05). CONCLUSION: There was a significant delay in TB case finding among PTB patients in this local municipality and patient related determinants contributed more than health system related determinants to this delay. There is need for the municipal TB programme to embark on an aggressive health education programme to furnish the populace with accurate information about TB, improve their health seeking behaviour and help address the issue of stigma associated with TB. / Health Studies

Apresenta grande incidência no mundo e ocorre em maior proporção em populações de baixas condições socioeconômicas. A tuberculose continua sendo uma das doenças crônicas mais importantes da história da humanidade, sobretudo nas áreas mais carentes do mundo com grandes repercussões sociais, pois acomete, predominantemente, a população adulta e produtiva. As estratégias de vigilância e controle da tuberculose incluem fundamentalmente o diagnóstico extemporâneo da doença, o tratamento e acompanhamento dos casos confirmados. O diagnóstico precoce constitui um dos principais problemas, pois, atualmente não há disponíveis testes rápidos, adequados e confiáveis para o diagnóstico em condições de campo e ambulatoriais. Diante disto, este trabalho realizou uma análise comparativa entre os testes sorológicos ELISA com antígenos brutos de PPD e de BCG e o teste imunocromatográfico Hexagon TB com o antígeno determinado A60 para o diagnóstico da tuberculose. Devido a características imunogênicas dos antígenos e da composição da prevalência da doença, a reprodutibilidade do ELISA-BCG foi baixa, bem como os parametros sorológicos de sensibilidade (33 por cento) e especificidade (61 por cento), comprometendo assim, a identificação entre pacientes clinicamente sadios e doentes. Os resultados também mostraram que o kit comercial Hexagon TB possui elevada acurácia (87,5 por cento) e é o mais indicado ao diagnóstico da tuberculose, pois identificou melhor os indivíduos com tuberculose e os indivíduos sadios. / Tuberculosis is a serious infectious disease caused by Mycobacterium tuberculosis, it can affect all organs, especially the lungs. It shows a higher incidence in the world and occurs more often in populations of low socioeconomic conditions. Tuberculosis remains one of the most important chronic diseases in human history, especially in more deprived areas of the world with great social impact, since it affects predominantly adult and productive population. Strategies for surveillance and control of tuberculosis include primarily the belated diagnosis of the disease, the treatment and monitoring the confirmed cases. Early diagnosis is a major problem, since there are no rapid, suitable or reliable tests available for diagnosis in field conditions and outpatients. Thus, this study conducted a comparative analysis of the serological ELISA with crude antigens PPD and BCG and Hexagon TB immunochromatographic test with antigen A60 specific for the diagnosis of tuberculosis. Due to the characteristics of immunogenic antigens and the composition of the desease's prevalence of the, the reproducibility of the ELISA, was low, as well as serological parameters of sensitivity (33%) and specificity (61%), thus compromising the identification between healthy and sick patients. The results also showed that the commercial kit TB Hexagon has high accuracy (87,5%) and is better suited for the diagnosis of tuberculosis, because it identified better the healthy and the sick individuals.

Although robust, the content transmission in this system, like in any radio frequency propagation, can suffer from external attenuating factors, such as distortion by multipath propagation. One of the techniques used in radio communications for minimizing the effects of this type of distortion is the spatial diversity reception, which uses multiple antennas connected to a single receiver. The signals, received by different antennas, are combined, in a technique known as MRC or Maximal Ratio Combiner, so that the output signal-to-noise ratio is greater than the individual signal-to-noise ratios, allowing the successfully decoding of the received content, even though the individual signal in each antenna does not have sufficient quality to be decoded independently. This study aims to establish a method of spatial diversity in receiving television signals in ISDB-Tb, pondering between the advantages and disadvantages of their use in edge regions of coverage, where the reception of the Brazilian digital TV system is not yet total. / No Brasil, o sistema de radiodifusão para conteúdos televisivos em alta definição é o ISDB-Tb. Apesar de robusto, a transmissão de conteúdos neste sistema, como qualquer propagação em radiofrequência, pode sofrer com fatores externos atenuantes, como por exemplo a distorção por propagação em multi-percurso. Uma das técnicas utilizadas em radiocomunicação para que se minimizem os efeitos deste tipo de distorção é a diversidade espacial na recepção, que utiliza múltiplas antenas conectadas a um mesmo receptor. Os sinais, recebidos pelas diferentes antenas, são trabalhados em uma técnica conhecida como MRC ou Combinação de Máxima Razão, de modo que a relação sinal-ruído de saída seja maior do que as relações sinal-ruído individuais, permitindo a decodificação do conteúdo com sucesso, mesmo que os sinais individuais em cada antena não tenham qualidade suficiente para serem decodificados independentemente. Este estudo tem como objetivo propor um método de diversidade espacial na recepção de sinais televisivos no padrão brasileiro ISDB-Tb, ponderando entre as vantagens e desvantagens de sua utilização em regiões de borda de cobertura, onde a recepção do sistema brasileiro de TV digital ainda não é total.

Structure, Microstructure and Magnetic Properties of Fe-Ga and R-Fe based Magnetostrictive Thin Films

Magnetostrictive materials belong to an important class of smart magnetic materials which have potential applications as ultrasonic transducers, sensors, actuators, delay lines, energy harvesting devices etc. Although, magnetostrictive property is exhibited by almost all ferro and ferrimagnetic materials, the R-Fe type (R represents rare earth elements) intermetallic compounds display maximum promise owing to the large magnetostriction exhibited by them at ambient temperature. Among the several R-Fe type compounds, Tb-Fe and Sm-Fe alloys are found to exhibit maximum room temperature positive and negative magnetostriction respectively. Recently, Fe-Ga based alloys have gained significant interest as newly emerging magnetostrictive material due to a good combination of magnetic and mechanical properties. These magnetostrictive materials in thin film form are of interests for several researchers both from fundamental and applied perspectives. Currently, many researchers are exploring the possibility of using magnetostrictive thin films in micro- and nano-electromechanical systems (MEMS and NEMS). Three material systems viz. Fe-Ga, Tb-Fe and Sm-Fe in thin film form have been chosen for our investigations. DC magnetron sputtering and e-beam evaporation techniques were used for deposition of these thin films on Si (100) substrates. Several aspects such as evolution of microstructure, film surface morphology, structure and change in film composition with different processing conditions were investigated in detail, as the existing literature could not provide a clear insight. Further, detailed magnetic characterizations of these films were carried out and established a process-structure-property correlation. The thesis is divided into seven chapters. The first chapter presents a brief introduction of magnetostrictive phenomena and the physics behind its origin. A brief history of evolution of magnetostrictive materials with superior properties is also brought out. Introduction to the material systems considered for the present study has also been presented. Discussions on various aspects like crystal structures, magnetic properties, and phase diagrams of these material systems are also included in this chapter. Magnetostriction in thin films and its importance in current technological applications are discussed in short. Further, a summary of existing literature on thin films of these materials has been narrated to highlight the perspective of the work done in subsequent chapters. In addition to this, a clear picture of the grey area for further investigations has been provided. Formulation of detailed scope of work for this study is also provided in this chapter. Details of different experimental techniques used in this study for deposition and characterization of these films are given in chapter 2. In the third chapter of the thesis a detailed study on the structural, microstructural and magnetic properties of Fe-Ga films deposited using dc magnetron sputtering technique are presented. The effect of sputtering parameters such as (i) Ar pressure, (ii) sputtering power, (iii) substrate temperature and (iv) deposition time/film thickness on the magnetic properties of the films are discussed in detail. All the films are found to be polycrystalline in nature with A2 type structure as evidenced from grazing incidence X-ray diffraction (GIXRD) and transmission electron microscope (TEM) studies. Surface morphology of the films are found to be affected with processing conditions considerably. Thermomagnetic behaviour of the films studied using a Superconducting Quantum Interference Device (SQUID) magnetometer under zero field cooled (ZFC) and field cooled (FC) conditions are also presented. The sputtering parameters are also found to influence the magnetic properties of the films through modifications in microstructure, surface morphology and film compositions. Irrespective of the sputtering parameters, room temperature (RT) deposited Fe-Ga films are found to exhibit large magnetic coercively and large saturation magnetic field as compared to the bulk alloy of similar compositions which are not desirable for micromagnetic device applications. A significant improvement in the magnetic properties of the films was obtained in the films deposited at higher substrate temperatures and is correlated with modifications in grain size and film surface roughness. These films are also found to exhibit better magnetostriction than the RT deposited films. Further, the magnetic properties of Fe-Ga films as a function of film thickness in the range 2 – 480 nm are also presented. The nature of variation of coercively with film thickness was correlated with grain size effect and explained successfully with the help of random anisotropy model. In the fourth chapter, studies on the microstructural and magnetic properties of Tb-Fe films were presented. It was reported earlier that TbxFe100-x films exhibit in-plane magnetic anisotropy for the films with x > 42 at.% of Tb and out-of-plane anisotropy for the composition 28 < x < 42. Presence of these anisotropies is technologically important for different applications. We have studied the magnetic properties of Tb-Fe films in these two composition range. TbxFe100-x films with 54  x  59 were prepared using dc magnetron sputtering technique under varying Ar pressure and sputtering power and the details about microstructural and magnetic properties are presented in this chapter. All the films are found to be amorphous in nature. While the composition of the film is found to remain constant with sputtering power, the Fe concentration in the film is found to be depleted with increase in Ar pressure. Magnetic properties are found to change from superparamagnetic to ferromagnetic behaviour with increase in sputtering power. Curie temperature of the films are found to be low (below RT) and is explained based on sperimagnetic ordering of magnetic sub-lattices. The perpendicular magnetic anisotropy (PMA) or out-of-plane anisotropy behaviour of Tb-Fe films were not studied in detail as a function of film thickness. We have successfully prepared TbxFe100-x films with 29  x  40 using e-beam evaporation technique using alloy target composition of TbFe in order to study the PMA behaviour as a function of film thickness. The thickness of the films was varied from 50 to 800 nm. All the films are found to be amorphous and columnar growth structure with fine channels of voids are observed from the TEM studies. Detailed magnetization and thermomagnetic measurements were carried out using SQUID magnetometer at different temperatures. The out-of-plane magnetic coercivity of the films was found to increase with film thickness and then decreases with further increase in thickness. Maximum coercivity of ~ 20 kOe has been obtained for the 400 nm thick film. Magnetic domain patterns were studied using magnetic force microscopy (MFM) technique and the observed magnetic properties are correlated with domain pattern and microstructures. Although there are several reports on device applications of Sm-Fe thin films which exhibit negative magnetostriction, a comprehensive study on the effect of different process parameters on the magnetic properties and its correlation with structure and microstructure is still elusive. Hence, Sm-Fe films were deposited on Si (100) substrate using dc magnetron sputtering technique under varying Ar pressure and sputtering power. Effect of these parameters on the microstructural and magnetic properties of the films was studied in detail and is presented in chapter 5. The curie temperature of the films was found to increase with increase in sputtering power and Ar pressure. This was attributed to increase in film thickness and size of islands (atomic clusters). Coercivity as low as 30 Oe has been achieved in the film deposited at 15 mTorr Ar pressure. The Curie temperature for the films deposited at higher Ar pressure (10 and 15 mTorr) are found to be above RT. Maximum saturation magnetostriction of ~ - 390 -strains has been achieved in the film deposited at 15 mTorr Ar pressure. Rapid thermal processing (RTP) experiments were also carried out to increase the magnetic ordering in the films deposited at low Ar pressure (5 mTorr) by imparting structural ordering. Large improvement in magnetization and Curie temperature of the film was observed after RTA. However, this could be attributed to the formation of nano-crystalline Fe phase as evidenced from the TEM studies and thermomagnetic measurements. An overall summary of the experimental results has been presented in chapter 6. The scope of work for further study in future has also been highlighted in chapter 7.
