FunÃÃo do assoalho pÃlvico e qualidade de vida em mulheres na pós-menopausa com e sem disfunÃÃo do assoalho pÃlvico / Pelvic floor function and quality of life in women in post menopause and pelvic floor dysfunction

Objetivo: Este estudo teve como objetivo comparar a funÃÃo dos mÃsculos do assoalho pÃlvico (MAP) em mulheres na pós-menopausa com disfunÃÃo do assoalho pÃlvico (DAP) versus mulheres sem DAP e a relaÃÃo entre a funÃÃo dos MAP e a qualidade de vida (QV). MÃtodo: Estudo caso-controle com duzentos e dezesseis mulheres na pós-menopausa. Foram avaliadas e classificadas com (n: 126) e sem DAP (n: 90). AvaliaÃÃo da funÃÃo MAP foi realizada pela palpaÃÃo digital vaginal (escala PERFECT) e incluiu: contraÃÃo voluntÃria, a forÃa (Escala Oxford 0-5), resistÃncia e repetiÃÃo dos MAP. Escala de Oxford foi comparada com a QV especÃfica utilizando King Health Questionnaire (KHQ) para mulheres com IU e Prolapse Quality of Llife (P-QoL) para mulheres com POP. Analisamos as mulheres com DAP em duas categorias de acordo com o valor obtido no Power (escala PERFECT): as mulheres com forÃa ≤2 ou ≥ 3 usando o teste Qui quadrado. A QV geral avaliada pelo SF-36 foi utilizada para comparar as mulheres com e sem DAP com o Teste t Student, (valor p ≤ 0,05 foi considerado para significÃncia). Resultados: As mulheres tinham 58,0  9,0 anos de idade, paridade 3,7 + 3,5. As mulheres com DAP tinham incontinÃncia urinÃria (IU), n: 44; prolapso de ÃrgÃos pÃlvicos (POP), n: 21; UI + POP, n: 61. A escala Oxford teve um valor mÃdio de 2 (0-5) em todas as mulheres estudadas, sem diferenÃa estatÃstica entre os grupos com ou sem DAP. A maioria das mulheres estudadas tinha forÃa insuficiente, resistÃncia (p = 0,428) e repetiÃÃo reduzidas (p = 0,721) nos dois grupos. AlÃm disso, foi analisada a consciÃncia perineal de acordo com a ausÃncia (Power = 0) ou na presenÃa (Power > 1) de contraÃÃo perceptÃvel. Mais uma vez, nÃo foram observadas diferenÃas entre os grupos caso e controle. Quando analisamos a QV geral (SF-36), encontramos que mulheres com DAP apresentam pior escores de QV em todos os domÃnios em comparaÃÃo a mulheres sem DAP (p≤0,05). Foi encontrada diferenÃa estatisticamente significativa apenas para domÃnio percepÃÃo de estado geral de saÃde do KHQ (p = 0,007). NÃo foi encontrada associaÃÃo entre a funÃÃo do assoalho pÃlvico e domÃnios PQoL. ConclusÃes: A forÃa dos MAP foi semelhante em mulheres com e sem DAP o que sugere que hà outros fatores relacionados ao desenvolvimento DAP. A QV geral à pior em mulheres com DAP. Contudo, a funÃÃo MAP nÃo foi relacionada com a QV especÃfica, avaliada pela KHQ e P-QoL em mulheres com IU e POP, respectivamente. / Objective: this study aims to compare pelvic floor muscle (PFM) function in post-menopause women with pelvic floor dysfunction (PFD) versus women without PFD and the relationship between PFM function and quality of life (QoL). Design: a case-control study. Methods: Two hundred sixteen post-menopause women with (n: 126) and without PFD (n: 90) were enrolled. Measurement of PFM function was performed by digital vaginal palpation (PERFECT scale) included: voluntary PFM contraction, PFM strength (Modified Oxford Grading Scale 0-5), PFM endurance and repetition. Oxford scale were compared on specific QoL using Kingâ Health Questionnaire (KHQ) for women with UI and prolapse quality-of-life (PQoL) for women with POP. We analyzed women with PFD into two categories according to the value obtained for the Power (from PERFECT scale): women with Power ≤2 or ≥ 3 using a Qui Square test. A general QoL using SF- 36 was used to compare women with and without PFD with a T-test, (p value ≤ 0. 05 was considered for significance). Results: The subjects were 58.0  9.0 years of age, parity 3.7+3.5. Women with PFD had urinary stress incontinence (UI), n: 44; pelvic organ prolapse (POP), n: 21; UI+POP, n: 61. Modified Oxford Grading Scale had a median value 2 (0-5) in all women studied without statistical difference between groups with or without PFD. Most of the women studied had insufficient strength, reduced endurance (p=0.428) and repetition (p= 0.721) in both groups. Further, we analyzed the perineal awareness according to the absence (Power =0) or presence (Power> 1) of perceptible contraction. Again, no differences were observed between the case and control groups. When we analyzed the general QoL in all women using SF-36, we found statistically significant difference between women with and without PFD in all domains (p≤0.05). We found statistically significant difference only for perception of general health domain of KHQ (p= 0.007). No association was found between pelvic floor function and P-QoL domains. Conclusions: PFM strength was similar in women with and without PFD which suggests that there are other factors related to PFD development. General QoL is worse in women with PFD. However PFM function was not related to specific QoL as assessed by KHQ and P-QoL in women with UI and POP, respectively.

By living in an inclusive society, the disabled teenager can harness their physical, sensoryperceptive, cognitive and psychosocial skills towards educational, professional, cultural, sporting and spiritual activities that comprise the occupational performance areas. Thus, it is supported that the development of a Community-Based Rehabilitation (CBR) should enhance in the sense that there must be equality of opportunities for teenagers with some sort of disability. The goal of this research was to evaluate the strategy of CBR as social inclusion tool in the mediation of the occupational performance of impoverished disabled adolescents. It is an interventionist research with emphasis on the qualitative dimension of the phenomenon being studied. Data collection was done through participating observations associated with the administration of a "social support equipment" questionnaire and an occupational performance self-assessment" form. Besides, a focal group was carried out. The organization of data sought support in the SPSS 15.0 software for Windows, as well as in the collective subject speech technique. Seven disabled teenagers, seven family members, ten health community agents, one educator and one community leader participated in the study, all residents linked to Secretaria Executiva Regional VI (6th Regional Executive Office), in the Municipality of Fortaleza-CE, and whoever were involved in the implementation of the strategy of CBR from March to October, 2009. The results brought the understanding of the real determinant conditions of autonomy in disabled adolescents occupational performance. Furthermore, through collective speech, it was known that the participants involved in the dynamics of the CBR could experience a sound learning that generates personal and collective change with effects on the routine of people with disabilities (PWD). It is concluded that, with the development of this research there were still many gaps. However, in the whole of the health practice, it was considered that the CBR project deployed in such community stands out as a strategy enabling the autonomy of the occupational performance in adolescents with disabilities and in poverty, thus promoting their social inclusion. / Adolescer implica fazer escolhas e assumir responsabilidades. Vivendo em uma sociedade inclusiva, o adolescente com deficiência pode direcionar suas habilidades físicas, sensório-perceptivas, cognitivas e psicossociais para as atividades educacionais, profissionais, culturais, esportivas e espirituais, que compõem as áreas do desempenho ocupacional. Destarte, defende-se a posição de que o desenvolvimento de um projeto de Reabilitação Baseada na Comunidade (RBC) deva ser incrementado no sentido da igualdade de oportunidades dos adolescentes com alguma deficiência. O objetivo desta pesquisa foi avaliar a estratégia de RBC como ferramenta de inclusão social na mediação no desempenho ocupacional de adolescentes com deficiência e em situação de pobreza. Trata-se de pesquisa intervencionista, com ênfase na dimensão qualitativa do fenômeno estudado. A coleta de dados foi realizada por meio de observações participantes, associadas à aplicação de questionário sobre equipamentos de apoio social e formulário de auto-avaliação do desempenho ocupacional . Ainda, realizou-se grupo focal. A organização dos dados buscou apoio no software SPSS 15.0 for Windows, bem como na técnica do Discurso do Sujeito Coletivo. Participaram do estudo sete adolescentes com deficiência, sete membros familiares, dez agentes comunitários de saúde, uma educadora e uma líder comunitária, todos residentes em comunidades vinculadas à Secretaria Executiva Regional VI, no Município de Fortaleza-CE, e que estiveram envolvidos na implementação da estratégia de RBC, no período de março a outubro de 2009. Os resultados trouxeram a compreensão das reais condições determinantes da autonomia no desempenho ocupacional do adolescente com deficiência. Ainda, no discurso coletivo, foi conhecido o fato de que os participantes implicados na dinâmica da RBC podem experimentar uma aprendizagem considerável, que enseja mudança pessoal e coletiva com efeitos no cotidiano da pessoa com deficiência (PCD). Conclui-se que, com o desenvolvimento desta pesquisa, ainda muitos hiatos ficaram. No conjunto da práxis da saúde, todavia, considerou-se que o projeto de RBC implantado na aludida comunidade desponta como estratégia facilitadora da autonomia no desempenho ocupacional do adolescente com deficiência e em situação de pobreza, promovendo a sua inclusão social

No. of bitstreams: 0 Previous issue date: 2011-12-16 / This dissertation aims to relate the daily aspects of early childhood apprehended through the clinic of Occupational Therapy and psychic constitution in the light of psychoanalytic theory. To respond our objectives, we established conditions for understanding the concept of everyday life, first studied by the social sciences and later incorporated into the occupational therapy clinic. Subsequently, we discourse about the Occupational Therapy and then about early childhood and psychic constitution. Through the study of three cases, we casted aspects of daily life of children, in line with the theoretical propositions of the study, that made possible the emergence of categories as time, space, play and childcare. Based on these ones, we composed the joints with the theories of the psychic constitution in the light of psychoanalysis, suggesting that these are elements of convergence that relate to the subject and culture and thus we may think the routine previously described by the Occupational Therapy also in its psychic dimension. Psychic constitution. / Esta dissertação objetiva relacionar os aspectos do cotidiano da primeira infância apreendidos através da clínica da Terapia Ocupacional e a constituição psíquica à luz da teoria psicanalítica. A fim de atender nossos objetivos, estabelecemos condições de compreensão acerca do conceito de cotidiano, primeiramente estudado pelas ciências sociais e, posteriormente, incorporado à clinica pela Terapia Ocupacional. Em seguida, discorremos acerca da Terapia Ocupacional e,em seguida, sobre a Primeira infância e a constituição psíquica. Através do estudode três casos clínicos, elencamos aspectos do cotidiano das crianças que, em consonância com as proposições teóricas do estudo, possibilitaram emergir as categorias tempo, espaço, brincar e cuidados infantis.A partir destas, compusemos articulações com as teorias da constituição psíquica à luz do referencial psicanalítico, propondo que estes sejam elementos de convergência que dizem respeito à cultura e ao sujeito e, assim, possamos pensar o cotidiano já descrito pela Terapia Ocupacional também em sua dimensão psíquica. In this context, there are many feelings generated by their experiences in teaching. Some of them may be considered positive for the profession, others, however, impose contradictions and barriers to their development. Considering this problem, we carried out this research in order to observe the teachers' feelings which were generated in their teaching practice, what factors contribute to the emergence of these feelings and how they are experienced and represented by the teachers. A bibliographic research foundation with the studies of Freire and Codo, with a qualitative nature, using the interview method in a public school in the city of Água Branca, Piauí, with five teachers. The data were analyzed using the collective subject discourse analysis method, enabling the examination of the dynamics and nature of teaching practices and the knowledge of the feelings experienced by teachers in their working quotidian in the school researched. Among the feelings, there were clear signs of disappointment with the profession, lack of credibility towards the teacher's work and dissatisfaction with their performance resulting from a variety of problems faced by the teachers and present at school, such as professional devaluation, bad remuneration, excessive working hours, besides other factors related to the students such as indiscipline, disinterest, which may generate feelings of discomfort, stress, malaise, insatisfaction and even psychic suffering, caused by tiredness due to their long work hours in search of better economic conditions. All these feelings affect their performance in the teaching practice, getting to compromise the quality of their teaching practice and, perhaps, taking them to experience a crisis of professional identity. Keywords: Teaching practice, Feelings, Teachers. / A ocupação, ainda, é utilizada como eixo principal de atuação da Terapia Ocupacional. Nos últimos 10 anos, surge a cinesioatividade. Prática baseada em teorias cinesioterápicas, associadas a relação sujeito-objeto, traduzida em uma tarefa-exercício. O termo aglutina as palavras atividade e cinesioterapia . Para estudar sua relevância terapêutica, no resgate da capacidade relacional com o meio, foram isoladas e analisadas 3 atividades, prescritas para hemiplégicos. Posteriormente, foram comparados 2 sujeitos (A e B), com hemiplegia E, para verificar o nível funcional, dos quais apenas A vivenciou a cinesioatividade. Os sujeitos foram submetidos a uma bateria de atitudes cotidianas, tendo destaque o sujeito A. Finalmente, sob entrevista aberta, 15 sujeitos hemiplégicos usuários da cinesioatividade, relataram melhoria da atividade diária. O resultado levou a concluir a validade da abordagem, junto adultos com disfunções decorrentes da hemiplegia.

Crianças hospitalizadas por acidentes e as histórias infantis como estratégia promotora de saúde em terapia ocupacional

No. of bitstreams: 0 Previous issue date: 2005-12-20 / Injuries occurred during childhood and their subsequent hospitalizations consist in great challenges for health promotion in children, due to the problems that could be caused to the infant development, especially in the interruption of occupational roles during childhood, raising repercussions in the life cycle. In this way, the study aimed the understanding of the sense of infant stories applied as health promoting strategy in Occupational Therapy, in the hospitalized child s perception. The methodological pathway was delineated and based upon the qualitative approach, characterizing itself as a descriptive study. The investigation was held from July to October, 2005, at the Paediatric Unit of Instituto Dr. Jose Frota, an emergency hospital, reference to the north and north-eastern of Brasil for polytraumatized patients, located in Fortaleza Ceará. Ten children, between 6 to 10 years old, of both genders, victims of non-intentional injuries participated on the study. Due to the nature of the study, data were obtained by means of documental research, interview, story telling, notes in a field diary, drawing and participant observation. The results were analysed enlighten by this theme s theorists, as well as by the historical and cultural theory s concepts of mediation and internalization and pointed out to the categories: diversity and the injuries ecology, the stories sense, reading practice during hospitalization and the plurality of the care. These categories converged towards the theme: childhood stories as a mediating instrument for heath promotion in Occupational Therapy. It is concluded that the use of infant stories in the hospital environment a result of the occupational therapist s practice is a health promoting strategy as it favours the children s physical, mental and social well-being, and to develop the self-construction of the knowledge concerning injuries prevention. It is also an instrument adaptable to the environment, it is in consonance with the necessities and possibilities of hospitalized children, it considers the individual, social and cultural contexts and it favours the articulation of the health promotion guidelines. / As injúrias ocorridas na infância e as conseqüentes hospitalizações se constituem grandes desafios para promoção da saúde em crianças, em razão dos problemas que podem acarretar ao desenvolvimento infantil, principalmente na interrupção dos papéis ocupacionais durante a infância, trazendo repercussões em seu ciclo de vida. Nessa perspectiva, o estudo se propôs a compreender o sentido das histórias infantis utilizadas como estratégia promotora de saúde em Terapia Ocupacional, na percepção da criança hospitalizada. O caminho metodológico foi delineado e sustentado na abordagem qualitativa, caracterizando-se como estudo descritivo. A investigação foi realizada nos meses de julho a outubro de 2005, na Unidade Pediátrica, no Instituto Dr. José Frota, hospital de emergência com referência Norte-Nordeste do Brasil, em atendimento ao politraumatizado, situado em Fortaleza, Ceará. Participaram do estudo 10 crianças entre 6 e 10 anos, de ambos os sexos, vítimas de injúrias não intencionais. Em virtude da natureza do estudo, os dados foram obtidos em pesquisa documental, entrevista, leitura de história, anotações no diário de campo, desenho e observação participante. Os resultados foram analisados à luz dos teóricos, bem como pelos conceitos de mediação e internalização da teoria histórico-cultural e apontaram para as categorias: diversidade e ecologia da injúria, sentido das histórias, prática da leitura na hospitalização e pluralidade de cuidados. Essas categorias convergiram para o tema: histórias infantis como instrumento mediador na Promoção da Saúde em Terapia Ocupacional. Conclui-se que a utilização das histórias infantis no ambiente hospitalar, fruto da prática do terapeuta ocupacional, é estratégia promotora de saúde, por favorecer o bem-estar físico, mental e social das crianças, e desenvolver a autoconstrução de conhecimentos sobre a prevenção de injúrias. Acrescenta-se que é um instrumento adaptável ao meio, está em consonância com as necessidades e possibilidades das crianças hospitalizadas, considera os contextos pessoais, sociais e culturais e favorece a articulação com as diretrizes da promoção da saúde.

nÃo hà / Wake-up stroke (WUS) define um Acidente Vascular Cerebral (AVC) detectado quando o indivÃduo desperta com sintomas de AVC. Cerca de 25% dos casos de AVC sÃo WUS. O papel que as alteraÃÃes do sono e comorbidades desempenham nesses casos permanece por ser estabelecido. O objetivo deste estudo foi avaliar as caracterÃsticas clÃnicas, as alteraÃÃes do ritmo vigÃlia-sono e os fatores associados a evoluÃÃo e gravidade dos sintomas em casos de AVC com e sem WUS. Trata-se de estudo de coorte prospectivo de carÃter observacional. Pacientes com AVC isquÃmico (N=102), dos quais 28,4% com WUS, foram avaliados no evento vascular inicial e apÃs 3 e 12 meses. Os pacientes (30-83 anos) eram provenientes da unidade de terapia de AVC do Hospital Geral de Fortaleza e foram recrutados entre 2013-2015. Foram incluÃdos aqueles que apresentavam AVC isquÃmico nos Ãltimos 15 dias, NIHSS>25, quadro clÃnico estÃvel, e condiÃÃes cognitivas suficientes para realizar as avaliaÃÃes. Foram estudadas as caracterÃsticas clÃnico-demogrÃficas e as comorbidades associadas. Um registro do sono foi feito com polÃgrafo nivel III (Stardust Phillips Respironics) para investigar a apneia do sono. Um Ãndice de apneia e hipopneia (IAH)>20 foi considerado o ponto de corte para definir a presenÃa da apneia obstrutiva do sono (AOS). QuestionÃrios sobre desempenho funcional (Escala de Rankin Modificada â ERM), habilidades motoras (Escala de Barthel Modificada âEBM) e sonolÃncia (Escala de sonolÃncia de Epworth-ESE) foram aplicados prospectivamente, realizando-se uma avaliaÃÃo inicial, uma segunda avaliaÃÃo por telefone 3 meses depois e uma terceira avaliaÃÃo por telefone vez apÃs 12 meses à avaliaÃÃo inicial. SonolÃncia Excessiva Diurna (SED) foi definida como um ESE>10. A amostra estudada (N=102, 64% masculino) apresentava hipertensÃo (73%), diabetes (29,4%), cardiopatia (16,7%), sedentarismo (69,6%), tabagismo (32,4%) e etilismo (17,6%). Na amostra total, observou-se variaÃÃo do IAH (1,20-57,3), da SpO2 mÃdia (82-97%) e do Ãndice de dessaturaÃÃo (5-94). Foram registrados IAH>20 (35,3%). NÃo houve diferenÃas clÃnicas e poligrÃficas entre os casos com e sem WUS. Diabetes tipo 2 associou-se com WUS (OR=0,37 CI: 0,15-0,92; p=0,03). Os pacientes com AOS apresentavam maiores Ãndices cintura-quadril (p=0,01). Os pacientes com SED eram mais jovens (p=0,009). A avaliaÃÃo longitudinal nÃo mostrou diferenÃa na gravidade dos sintomas nos casos com e sem WUS. Prospectivamente, os casos com AOS evoluÃram com pior desempenho funcional (ERM, p=0,02). / Wake-up stroke (WUS) defines a cerebrovascular accident (CVA) detected when the individual awakens with symptoms of stroke. About 25% of cases of stroke are WUS. The role that sleep disorders and comorbidities play in these cases is yet to be established. The aim of this study is to evaluate clinical characteristics, changes in sleep-wake cycle and factors that influence the development and severity of symptoms in cases of stroke with and without WUS. This is a longitudinal study of observational nature. Patients with ischemic stroke (N=102), 28.4% with WUS, were evaluated at baseline and after 3 and 12 months. Patients (30-85 years old) were recruited from the stroke care unit of the General Hospital of Fortaleza from 2013 to 2015. We included those that had ischemic stroke in the last 15 days, NIHSS<15, stable clinical condition, and that were able to answer behavior questionnaires. The clinical and demographic characteristics and associated comorbidities were studied. A sleep recording was done with level III polygraph (Stardust Phillips Respironics) to investigate sleep apnea. An apnea-hypopnea index (AHI)> 20 was considered the cutoff point to define the presence of obstructive sleep apnea (OSA). Questionnaires on functional performance (modified Rankin Scale â MRS), motor skills (modified Barthel Scale âMBS) and sleepiness (Epworth Sleepiness Scale â ESS) were applied. Excessive Daytime Sleepiness (EDS) was defined as an ESS> 10. Prospectiveley, pacients are evaluated (ESS, MRS, MBS) after 3 months and 12 months after the initial evaluation by telephone. In the sample studied (N = 102, 64% male), hypertension (73%), diabetes (29.4), heart disease (16.7%), physical inactivity (69.6%), smoking (32.4%) and alcohol consumption (17.6) were found. In the total sample, AHI (1.20 to 57.3), SpO2 (82-97%) and the desaturation index (5-94). We recorded AHI> 5 (92.9%), AHI> 15 (44.7%), AHI> 20 (35.3%) and AHI> 30 (11.8%) were variable. Cases with and without WUS did not differ regarding clinical and polygraphic characteristics. Type 2 diabetes was associated with WUS (OR = 0.37 CI: 0.15 to 0.92; p = 0.03). Individuals with OSA had higher waist-hip ratios (p = 0.01). Patients with EDS were younger (p = 0.009). The longitudinal evaluation showed no difference in the severity of symptoms in cases with and without WUS. Prospectiveley, cases with OSA evolved with worse functional performance (ERM, p = 0.02). Conclusion: Among patients with ischemic stroke, WUS manifests itself in about 1/3 of cases and it associates with type 2 diabetes. Those with stroke and OSA (AHI> 20) evolve with worse functional performance after one year.

The sample consisted of 11 sedentary volunteers (05 men and 06 women between 30 - 45 years). We conducted a clinical study, crossover, randomized, double-blind. The protocol was divided into two stages. The protocols used participant (0.5 g / kg) of L-glutamine or calcium caseinate added to a milk drink Nescau Light (200ml) for seven consecutive days, with an interval of one week of completion of a protocol and early other. The volunteers were evaluated for cardiopulmonary and metabolic response during cardiopulmonary exercise testing on a treadmill. Were determined in each individual oxygen consumption (VO2 Max), respiratory quotient (RQ), the workload performed, heart rate (HR), systolic and diastolic blood pressure (SBP / DBP) in phases: pre - effort, the anaerobic threshold, maximal effort and during the recovery effort. Were also evaluated serum concentrations of glucose and lactate in four stages: fast (at least 8 hours), immediately before the effort at the end of exercise and 30 minutes after exercise. The prior offer of L-glutamine (0.5 g / kg in sedentary individuals, orally for seven days did not cause hemodynamic changes during physical exertion. On the other hand modified the metabolic response by reducing the lactacemia before the physical activity (3, 68 Â 0.8 versus 2.2 Â 0.9, p = 0.01) in the presence of maximal (12.4 Â 5.1 versus 10.3 Â 3.2, p = 0.04), and at the end of the exercise stress test (6.9 Â 3.6 versus 5.02 Â 1.77, p = 0.01). InfluÃncia da mecÃnica e padrÃo respiratÃrios e do tipo de ventilador pulmonar mecÃnico na assincronia paciente-ventilador durante a ventilaÃÃo nÃo invasiva / Influence of mechanical and respiratory pattern and the type of mechanical ventilator asynchrony in patient-ventilator during noninvasive ventilation

Contextualization: Ideal patient-ventilator synchrony can be very difficult to achieve, especially during NIV, due presence of. Intensive care unit (ICU) ventilators were designed to function without the presence of leaks and are likely to suffer interference in their presence during NIV, but the latest generation of ventilators incorporated NIV algorithms (âNIV modesâ) to compensate e deal better with leaks. Auto-Trak consists in a technology capable to automatically adjust, cycle by cycle, triggering and cycling mechanisms during PSV. Objectives: Determine the influence of the type of pulmonary ventilator, respiratory mechanics and breathing pattern on patient-ventilator asynchrony during NIV, with and without the use of NIV algorithms, and with and without an automatic triggering and cycling system. Methods: Experimental bench study using the mechanic lung model, ASL 5000TM. Three profiles of respiratory mechanics were studied: normal, obstructive and restrictive, with neural inspiratory time of 0.5, 1.0, 1.5 and 2.0 seconds and maximum intensity of muscle effort (Pmus) fixed in -7.5 cmH2O. We simulated NIV in five ICU ventilators and in four noninvasive ventilators. Auto-TrakTM was studied when available in the ventilator. Primary outcomes were: respiratory asynchronies, inspiratory delay time and cycling asynchrony time identifying, in the second case, two possible types, late or premature cycling. Results: Inspiratory delay time was shorter on dedicated NIV ventilators in most of situations. A short neural time was associated with late cycling and a long neural time with premature cycling. Dedicated NIV ventilators cycled later than the ICU ventilators, when the neural time was 0,5s and mostly in the obstructive pattern, but was associated with a shorter cycling asynchrony time when the neural time was longer (> 1,0s). NIV algorithms and Auto-TrakTM had little impact on triggering and cycling, however remained slightly more stable the effective PEEP. Conclusion: Respiratory mechanics and breathing pattern influence the degree of patient-ventilator asynchrony during NIV. Neural time is a determinant factor of triggering and cycling asynchronies, associated to late cycling when short and to premature cycling when long. The type of mechanical ventilator influence, on varying intensity, the degree of asynchrony. NIV algoritms and Auto-TrakTM have little impact on triggering and cycling. Objetivo: Determinar a influÃncia do tipo de ventilador pulmonar, da mecÃnica e do padrÃo respiratÃrios sobre a assincronia paciente-ventilador durante a VNI, com e sem o uso de algoritmos para VNI e com e sem o sistema de disparo e ciclagem automÃticos. MÃtodo: Trata-se de estudo experimental, de bancada utilizando o simulador pulmonar mecÃnico, ASL 5000Â. Estudaram-se trÃs perfis de mecÃnica respiratÃria: normal, obstrutivo e restritivo, com tempo inspiratÃrio neural de 0,5, 1,0, 1,5 e 2,0 segundos e intensidade mÃxima do esforÃo muscular (Pmus) fixada em -7.5 cmH2O. Simulou-se VNI em cinco ventiladores de UTI e em quatro ventiladores especÃficos de VNI. O Auto-Trak foi estudado quando disponÃvel no ventilador. Os desfechos primÃrios foram: assincronias respiratÃrias, tempo de retardo inspiratÃrio e tempo de assincronia de ciclagem identificando, neste segundo caso, dois tipos possÃveis, ciclagem tardia ou precoce. Resultados: O tempo de retardo inspiratÃrio foi menor nos ventiladores especÃficos de VNI na maioria das situaÃÃes. O tempo neural curto se associou à ciclagem tardia e o tempo neural longo à ciclagem precoce. Os ventiladores especÃficos de VNI ciclaram mais tardiamente que os de UTI no tempo neural de 0,5s e sobretudo no padrÃo obstrutivo, mas associaram-se a um tempo de assincronia de ciclagem menor quando o tempo neural foi mais longo (> 1,0s). Os algoritmos de VNI e o Auto-Trak mantiveram um pouco mais estÃvel a PEEP efetiva. ConclusÃo: A mecÃnica e o padrÃo respiratÃrios influenciam o grau de assincronia paciente-ventilador durante a VNI. Tempo neural à fator determinante de assincronias de disparo e ciclagem. O tipo de ventilador mecÃnico influencia, em intensidade variÃvel, o grau de assincronia. Background: The COPD assessment test (CAT) can provide a simple, efficient and reliable tool for assessing and monitoring COPD patients. Itâs important to perform the validation study of the Brazilian version of the instrument and therefore may become available for studies with the Brazilian population. The literature indicates the existence of different modes of questionnaires administration (selfadministered, interview, telephone), however there are many ways to influence the responses obtained from these instruments, culminating in the appearance of bias, especially in countries with low educational levels. Objectives: to perform the validation and reproducibility of the Brazilian version of CAT (group 1) and to compare psychometric properties of two forms of the questionnaire application: face to face interview and telephone. Methods: Group 1- There were selected patients with stable COPD from two hospitals in Fortaleza-CearÃ. CAT was administered in 50 patients on two occasions with an interval of 30 minutes, and a third time after one week. CAT has eight components: coughing, mucus production, chest tightness, capacity for exercise and activities, confidence, sleep quality and energy levels. Also the following tests were performed: Saint George questionnaire for respiratory diseases (SGRQ), pulmonary function tests, six-minute walk distance test (6MWD), modified index of dyspnea (MMRC) and hospital anxiety and depression scale (HADS). Group 2 â 100 patients were randomly assigned to either the face-to-face interviewer administered CAT (CAT-FFIA) or telephone interviewer administered CAT (CAT-TIA) group to asses internal consistency reliability, cross-sectional validity and test-retest reliability of the questionnaires. The patients answered the questionnaires twice, one week apart. In the same occasion, the patients answered the following questionnaires: SGRQ, HADS and MMRC. Results: Group 1 â CAT demonstrated excellent reproducibility, with an inter-rater ICC of 0.96 (95% CI: 0.93-0.97; p< 0.001) and an intra-rater ICC coefficient of 0.98 (95% CI: 0.96-0.98; p< 0.001). Bland-Altman plots showed good test-retest agreement for the CAT. The total score presented a significant correlation with SGRQ, espirometric values, 6MWDT, MRC and depression scores of HADS (p< 0.05). Group 2: For both administration formats, we found good internal consistency reliability (Crohnbach's alpha between 0,89 and 0.74 for the CAT-FFIA and CAT-TIA, respectively). In general, moderate-to-high correlations of CAT with SGRQ were observed, independent of the format of administration. For the test-retest reliability, the intraclass correlation coefficients were very similar for both CAT-FFIA and CAT-TIA group : 0,96 (95%CI 0,93-0,97) vs 0,98 (95%CI 0,96-0,98) respectively. In relation to discriminative properties of the CAT, it was observed that the questionnaire is able to assess the severity of COPD. The results showed significant differences in the scores of CAT in patients with moderate to severe COPD in both CAT-FFI and CAT-TIA. Conclusions: the Brazilian Portuguese version of CAT is a valid, reproducible and reliable instrument to evaluate the impact of COPD on patientâs lives. Objetivos: realizar a validaÃÃo e a reprodutibilidade da versÃo brasileira do CAT (grupo 1) e comparar as propriedades psicomÃtricas de duas formas de aplicaÃÃo do questionÃrio: entrevista face-aface e telefone. (grupo 2). MÃtodos: Grupo 1 - foram selecionados pacientes com DPOC estÃveis em dois hospitais de ensino na cidade de Fortaleza-CearÃ. O CAT foi aplicado em 50 pacientes em dois momentos, com intervalo de 30 minutos, e em um terceiro momento, apÃs uma semana. O CAT à composto de 8 itens: Tosse, Catarro, Aperto no peito, Falta de ar, LimitaÃÃes nas atividades domiciliares, ConfianÃa em sair de casa, Sono e Energia. Adicionalmente foi avaliado: questionÃrio Saint George para doenÃas respiratÃrias (SGRQ), teste de funÃÃo pulmonar, Teste da caminha de seis minutos (TC6), Ãndice modificado de dispneia (MMRC) e a escala hospitalar de ansiedade e depressÃo (HADS). Grupo 2 - 100 pacientes foram randomizados para receber o CAT no formato entrevista (CAT-EFF) e no formato telefone (CAT-ET), sendo avaliado a consistÃncia interna, validade e reprodutibilidade do questionÃrio. Os pacientes responderam ao questionÃrio duas vezes, com uma semana de intervalo. Na mesma ocasiÃo os pacientes responderam aos questionÃrios: SGRQ, HADS e MMRC. Resultados: Grupo 1 - O CAT evidenciou excelente reprodutibilidade tanto inter-examinador quanto intra-examinador, com um CCI de 0,98 (IC95% 0,96-0,98; p<0,001) e 0,96 (IC95% 0,93-0,97; p< 0,001)), respectivamente. Os grÃficos de Bland-Altman demonstraram boa concordÃncia no teste re-teste com o CAT. A pontuaÃÃo total do CAT apresentou correlaÃÃo significativa com o SGRQ, espirometria, TC6, MMRC e escores de depressÃo do HADS. Grupo 2 â Em ambos os formatos de aplicaÃÃo do CAT evidenciou-se boa consistÃncia interna (Coeficiente Alfa de Cronbach entre 0,89 e 0,74 para o CAT-EFF e CAT-ET, respectivamente). Houve correlaÃÃo de moderada a alta do CAT com o SGRQ em ambos os grupos. Em relaÃÃo a reprodutibilidade, o CCI de ambos os grupos foram similares: 0,96 (IC95% 0,93-0,97) no CAT-EFF e 0,98 (IC95% 0,96-0,98) no CAT-ET. Em relaÃÃo as propriedades discriminativas do CAT, observou-se que o questionÃrio à capaz de avaliar a gravidade da DPOC. Os resultados apontaram diferenÃa significativa da pontuaÃÃo do CAT nos pacientes com DPOC moderado e grave tanto no grupo CAT-EFF como no CAT-ET. ConclusÃo: A versÃo brasileira do CAT à vÃlida, reprodutÃvel e confiÃvel no tocante a avaliaÃÃo do impacto da DPOC na vida dos pacientes. InteraÃÃo da ventilaÃÃo nÃo invasiva e mudanÃa de decÃbito sobre a ventilaÃÃo pulmonar regional: anÃlise por tomografia de impedÃncia elÃtrica / Interaction of noninvasive ventilation and changing positions on regional lung ventilation: analysis by electrical impedance tomography

A distribuiÃÃo da ventilaÃÃo pulmonar regional (DVPR) Ã heterogÃnea e pode ser influenciada por alteraÃÃes posturais e pela aplicaÃÃo de ventilaÃÃo nÃo invasiva (VNI), sendo a tomografia de impedÃncia elÃtrica (TIE) capaz de detectar essas alteraÃÃes. Objetivo: Avaliar os efeitos agudos da VNI, de mudanÃas de decÃbitos, e a combinaÃÃo dos dois, sobre a distribuiÃÃo da ventilaÃÃo pulmonar regional (DVPR), em voluntÃrios adultos jovens sadios, atravÃs de anÃlise de imagens obtidas por TIE. MÃtodos: Estudo aplicado, experimental, quantitativo e transversal, realizado em dez voluntÃrios saudÃveis, registrando-se o padrÃo ventilatÃrio nos decÃbitos dorsal (DD), ventral (DV) e laterais direito (DLD) e esquerdo (DLE), em respiraÃÃo espontÃnea (RE), e com aplicaÃÃo de CPAP (10cmH2O) e BiPAP (15cmH2O de IPAP e 5cmH2O de EPAP), por dez minutos, em sequÃncia randomizada. Resultados: A distribuiÃÃo da ventilaÃÃo foi direcionada para as regiÃes pulmonares dependentes nos DD, DLD e DLE. No DV a DVPR foi maior na regiÃo nÃo dependente (zona dorsal). A mudanÃa na DVPR foi maior nos decÃbitos laterais em comparaÃÃo ao dorsal, com um maior incremento de ventilaÃÃo para o pulmÃo dependente no DLD do que no DLE. A aplicaÃÃo de VNI (CPAP e BiPAP) nÃo alterou a DVPR, apesar do aumento significativo (p < 0,05) do volume corrente em DD no modo BiPAP, ao ser comparado com a RE e ao modo CPAP. ConclusÃes: A anÃlise das imagens do tÃrax obtidas pela TIE permitiu avaliar os efeitos da variaÃÃo de decÃbitos e da aplicaÃÃo de VNI sobre a DVPR. A DVPR foi significativamente influenciada pelas mudanÃas de decÃbitos em respiraÃÃo espontÃnea. Em DV, posiÃÃo prona, a DVPR foi maior nas partes posteriores do pulmÃo (regiÃo nÃo dependente). Durante a VNI, a distribuiÃÃo proporcional da ventilaÃÃo pulmonar nÃo foi alterada, porÃm no modo BiPAP, uma fraÃÃo maior do volume corrente foi direcionado para a regiÃo dependente, sobretudo quando adotados decÃbitos laterais, em comparaÃÃo a RE e ao modo CPAP. / The distribution of regional lung ventilation (DVPR) is heterogeneous and may be influenced by postural changes and the application of non-invasive ventilation (NIV). The electrical impedance tomography (EIT) is probably able to detect and measure these changes. Objective: To evaluate the acute effects of NIV and changes in decubitus, and the combination of both interventions, on the DVPR in young adult healthy volunteers, through analysis of images obtained by EIT. Methods: This is an applied, experimental, quantitative and cross-sectional study in ten healthy volunteers, analyzing the ventilatory patterns in dorsal decubitus (DD), ventral (DV), right lateral (RLD) and left lateral (LLD), breathing spontaneously (BS), with CPAP (10 cmH2O) and BiPAP (IPAP of 15 cmH2O and EPAP of 5 cmH2O) for a ten minutes period. Results: The distribution of ventilation was directed to the dependent regions of the lungs on DD, LLD and RLD. During DV, the DVPR was higher in the non-dependent regions (dorsal pulmonary zones). The change in DVPR was higher in the lateral decubitus position compared to the DD, with a greater increase in ventilation in the dependent lung while in the RLD as compared to the LLD. The application of NIV (CPAP and BiPAP) did not change the spatial DVPR, despite the significant increase (p < 0.05) in tidal volume in DD in the BiPAP mode, when compared to the BS and the CPAP mode. Conclusions: The analysis of the images of the thorax obtained by EIT allowed to evaluate the effects of changes in body positioning and application of NIV on the DVPR. The DVPR was significantly influenced by changing decubitus during spontaneous breathing. In the DV (prone positioning) the DVPR was higher in the dorsal parts of the lung (non-dependent regions). During NIV, the ventilation distribution was not changed, but a greater fraction of the tidal volume was directed to the non-dependent regions in the BiPAP mode as compared comparison to the BS and the CPAP mode and, especially when in both lateral decubitus.
