Problematika duševního zdraví populace České republiky / The issue of mental health in the population of the Czech Republic

The aim of the thesis in the theoretical section was to describe the mental health problems in the Czech Republic. The main goal of the research work was to draw attention to these problems and to explore the psychiatric treatment from the outpatients' perspective. Mental health is a vast topic which I elaborated using many relevant sources mentioned in analytical, statistical and other materials as well as the official Czech and international documents. The findings show that in spite of undisputable pluses of the Czech system, the accessibility of the psychiatric care is unsatisfactory and the promotion of the mental health services has been seriously neglected and delayed in the Czech Republic compared with most EU countries. A fundamental positive change has been expected with the implementation of two key agendas - the Psychiatric Care Reform Strategy and the Mental Health Action Plan. For the thesis an original, non-standardized written questionnaire was used to evaluate depressed and anxious patients' point of view of their treatment. The quantitative research was later evaluated using statistical methods. The quantitative research was evaluated with the SPSS statistical software. Hypotheses were tested using the chi-square. The results suggest that the fear of stigmatisation by psychiatric care as well as the time to a psychiatrist visit are influenced by diagnosis and specific health problems. The results also suggest that the personality type and family milieu are more important than socio-demographic factors. Gender differences do not influence the compliance either. The complex outcomes of the patients' attitude research can be later used in designing better mental health care.

Background: The use of addictive substances during pregnancy is a serious health and social problem. Pregnancy is, in itself, one of the most important and most critical periods in a woman's life, and addictive substances in this case are significantly complicated. Pregnant drug addicts need complex care, paradoxically, however, they often do not have access or use it to a small extent. Objectives: The aim of this work is to map the health and social care provided to pregnant women using addictive substance. In the Czech Republic programs are being implemented for mothers with children or pregnant women using addictive substance. Programs include individual counseling, psychotherapy, relapse prevention, family and partner therapy, parent groups, mothers and social work. The aim of the thesis is to focus on the health and social care provided to this client. Methods: Research is a series of case studies of clients focusing on personal and social history, on their somatic and psychological condition, retrospective view of the course of pregnancy, care of women during pregnancy. Introduction: Bariatric metabolic surgery is recognized as a very effective treatment for obesity, performed when the preceding conventional treatment failed or when significant obesity-related complications occured. Diploma thesis is focused on a current practice of dietitians providing nutritional care in weight-loss surgery (WLS), as they play an important role in a multidisciplinary medical team by performing a clinical nutritional evaluation, education and long-term nutritional follow-up. Objective: The aim of the thesis is to present eating habits in patients of the 3rd Medical Department of the 1st Faculty of Medicine Charles University and General Faculty Hospital in Prague, in 6 and 12 months after the surgery and to evaluate its accordance to existing nutritional strategies and evidence-base guidelines. The institutional care problematic is a topical subject, as many mentally retarded people in the Czech Republic are dependent on such care. However, the institutional care has many liabilities, which can have negative influence on the personality development of these individuals. That{\crq}s why it{\crq}s essential to focus on humanization of the current inward facilities, while applying modern approaches to mentally affected people. In my pursuit, I analyzed the services provided by individual institutions for disordered people (daily or weekly care homes in some cases) in the South Bohemia region. I focused especially on the range of available current methods and forms, which develop personalities of the disordered people. This relates to the possibility of applying work and other specific therapies, but also involves other fields, such as the quality of living, education, worthwhile spending of leisure time and respecting bio-psycho-social needs of the clients. Assertion of these principles leads to personality development of the clients, increases the quality of their lives and generally contributes to the humanization of the institutional care. In order to find the corresponding information, we have used the method of questioning, the technique of controlled interview and the technique of questionnaire. The target file for proving the hypothesis were - when using the controlled interview - chosen leaders of institutes of social care (mostly directors of such establishments), for questionnaires home{\crq}s employees (mostly instructors and persons employed in social services), who enforced some of their actual methods of work and forms of personality development of mentally affected persons. The results of the research have shown that the most commonly used method of personality development besides education is ergotherapy and operational therapy. The facilities also use plenty of forms of rehabilitation including hipotherapy. Often used methods also are artetherapy, music therapy and also very common are canistherapy and logopedics. Dramatherapy, basal stimulation, aromatherapy and dance therapy belong to the group of methods, which are present in lower cases. Moreover, it was found that there are vast differences in the broad offer of these methods between particular establishments. The establishments offer at least three next to the most of ten types of therapies.

This diploma paper deals with the problems of domestic violence. The work itself is divided into two separate consequential parts. Theoretical part is pointed at general description of particular topics concerning domestic violence. These topics are also systematically classified. Theoretical findings lead to the main aim of the theory, which is the chapter violent person. The practical part sets two goals. The first one was to prove that the attitude of the ordinary people to the sufficient work with violent person is negative, which has been proven in my survey. Second goal was to make up a hypothesis in terms of work with the violent person.

The objective of this thesis was the description of the work of the Child and Adolescent Detoxification Centre in Prague, its importance in the treatment drug addiction among children and adolescents. Another objective was to find out about the interrelationship of the work of regional coordinators for the prevention of risk behaviour and forms of follow-up therapy according to individual needs and nature of Child and Adolescent Detoxification Centre in Prague. The research conducted in the period from January to March 2010. Mapping the variants of follow-up therapy of the clients using habit-forming drugs in different regions of the Czech Republic and the forms of prevention are other objectives. The theoretical part of the thesis contains the description of the establishment of the Hospital of Sisters of Mercy of St. Charles Borromeo, in the premises of which the Child and Adolescent Detoxification Centre (CaADC) in Prague is located, as well as the description of the CaADC, and its medical and other personnel. The thesis describes the process of hospitalization, tests which preceded it and the necessary procedures to ensure successful completion of the hospitalization. Part of the theoretical block is dedicated to the care provided by a social worker, psychologist and therapists. Related legislation that forms the basis of prevention methods of individual regional drug coordinators and the centre itself is an integral part of the work. Prevention is a critical form of struggle against the abuse of psychotropic substances by children and adolescents. It is described in detail here, as well as the factors involved in the development of addiction. Other related terms only complement the mosaic of the studied issue. Within quantitative research, a content analysis of the data from the statistics of the Child and Adolescent Detoxification Centre in Prague was used to verify set hypotheses. The responses of regional drug coordinators to a set of open and semi-open questions, which were formulated for this purpose, were also used. Another source was the results of standardized interviews with three doctors at the Child and Adolescent Detoxification Centre in Prague. Hypotheses 1, 3 and 5 have not been confirmed. On the contrary, hypotheses 2 and 4 have been confirmed. The set objectives have been met. Unfortunately, our country still lacks a sufficient number of facilities that provide follow-up care of child drug addicts, as is clear from the research.
