Evaluation of the outpatient therapeutic programme for management of severe acute malnutrition in three districts of eastern province , Zambia

Magister Public Health - MPH / The Outpatient Therapeutic Programme (OTP) is an integrated public health innovation for treating severe acute malnutrition without medical complications in children 6 to 59 months of age as outpatients within their communities using Ready to Use Therapeutic Food with the aim of reducing case fatality rates. The OTP approach is implemented in the three districts in Eastern Province of Zambia namely; Chipata, Katete and Petauke. Since inception of the OTP in the province, an evaluation of the OTP has not been conducted. The study is aimed at assessing the effectiveness of the implementation of the OTP for management of severe acute malnutrition in the three districts of Eastern Province of Zambia.

Text in Afrikaans / The purpose of this study was to determine the nature, origin and negative consequences of perfectionism and to set guidelines for the therapeutic handling of perfectionism. Two literature studies were done to investigate the phenomenon and therapeutic techniques with regards to perfectionsim. A questionnaire was developed as aid to the therapist to identify negative perfectionism and associated problem areas. An empirical study was done to investigate the effectivity of the questionnaire and to compose a program and guidelines for the therapist and perfectionist. Results of the study indicated that negative perfectionism resulted in affective, cognitive, interpersonal and behavioural consequences for the client. It further showed that it is necessary to find the origin of the client's perfectionism and to give them insight in their problem to enable the therapist to succesfully apply cognitive behavioral therapy. / Educational Studies / M. Integrated community-based management of severe acute child malnutrition : Studies from rural Southern Ethiopia

Background: The World Health Organization (WHO) recommends the community-based Outpatient Therapeutic Program (OTP) as a standard treatment protocol for the management of uncomplicated Severe Acute Malnutrition (SAM) at the community level. OTP has been scaled up and integrated into the existing grassroots level government health systems in several developing countries. The aim of this thesis was to assess the implementation and outcome of a scaled-up and integrated OTP service provided at community level. Methods: One qualitative study and three quantitative studies were conducted in southern Ethiopia. Children admitted to 94 integrated OTPs, their caregivers and health extension workers providing primary health care services in the nearby health posts were included in this study. The quantitative studies were based on data generated from observation of a cohort of 1,048 children admitted to the integrated OTPs. Result: On admission 78.8% of the children had SAM. The majority of these children 60.2% exited the program neither achieving program recovery criteria nor being transferred to inpatient care. Fourteen weeks after admission to OTP, 34.6% were severely malnourished and 34.4% were moderately malnourished, thus 69.0% were still acutely malnourished. Ready-to-use Therapeutic Foods (RUTFs) provided for SAM children were commonly shared with other children in the household and sold as a commodity for the collective benefit of the family thus admitted children received only a portion of the provided amount. Further, the program suffered a severe shortage of RUTFs, where only  46.6% of admitted children were given the recommended amount of RUTFs by providers on admission and only 34.9% of these had uninterrupted provision during the follow-up. Conclusion: The integrated OTPs we studied provide a constrained service and the use of RUTFs by families is not as intended by the program. The majority of admitted children remained acutely malnourished after participating in the program for the recommended duration. For integrated OTPs to be effective in chronically food-insecure contexts, interventions that also address the economic and food needs of the entire household are essential. This may require a shift to view SAM as a symptom of broader problems affecting a family rather than as a disease of an individual child. In addition, further research is needed to understand the health system context regarding RUTFs and medication supply and service utilization of integrated OTPs.

The thesis deals with special treatment of convicts, in particular a special programme of treatment in a special department. The thesis has two parts the theoretical one and the empirical one. The theoretical part defines basic notions connected with education, family, free time, pathological phenomena in society, crime. They are presented on the basis of scientific resources. Next, special treatment of convicts in a special department is described. The convicts suffer from a behaviour disorder caused by abuse of psychotropic drugs. The main goal of the empirical research was to describe, compare and analyse the opinions of the convicts on the relation between crime and family life, free time activities and also the therapeutic programme of the special department.

Chess as a therapeutic medium in a substance abuse rehabilitation centre : a narrative study

This research project is a narrative study on the use of chess as a therapeutic medium in a substance abuse rehabilitation centre. Stabilis Treatment Centre served as the rehabilitation centre where the data was collected and the therapeutic sessions conducted. In this research report I introduced Stabilis Treatment Centre by exploring and providing an overview of their existing programmes. I have stated the research problem and the subsequent research question and goals. A literature review is included where information with regards to drug abuse, exercise psychology and the use of chess in various contexts is provided. The research methodology is examined and narrative practice is introduced and explored. The three participants in this research project are introduced and their experiences of playing chess are explored and extended to their experiences of life and relevant situations. This process is documented in letter format where I as researcher wrote letters to each participant reflecting on their experiences of using chess to narrate their personal narratives. A therapeutic programme for the rehabilitation of youth offenders

The judicial system is moving away from a retributive justice system to a more rehabilitative, restorative justice system. Diverting youth offenders from the justice system by alternative sentencing options such as diversion programmes is one way of doing this. Jt was found that a need exists for the development of a therapeutic programme for the rehabilitation of youth offenders in South Africa. The purpose of the study was to detennine the content of therapeutic rehabilitation programme and subsequently to develop a provisional therapeutic programme for youth offenders. The first part of the literature study summarized a number of theories used to explain the incidence of youth offence. The second part of the literature study discussed various risk factors that could lead to youth offence. Thirdly, the juvenile justice system in South Africa was discussed. Finally, the characteristics of effective rehabilitation programmes were discussed and a number of international and South African rehabilitation programmes were evaluated in tenns ofthese characteristics. In the literature study certain aspects for inclusion in a rehabilitation programme were identified. A list of topics that should be addressed by a rehabilitation programme was drawn up. A provisional therapeutic programme was compiled using the information gained from the literature study and the needs for a programme as identified by staff members at the facility where the programme was to be conducted. The programme was based on experiential learning. lt included a number of activities that addressed the topics identified by the literature study. The provisional programme was evaluated in tenns of its short-term effectiveness and adjustments to the programme were made. The adjusted programme was conducted at two other facilities and evaluated in tenns of its short-term effectiveness by the researcher, the staff at these facilities and the youth who took part in the programme. Staff at the two facilities where the empirical study took place were asked to complete questionnaires to identify their needs with regards to a rehabilitation programme as well as background questionnaires on the youth who were taking part in the programme.
