Use of ‘wearables’ to assess the Up-on-the-toes test

Yes / The mechanical output at the ankle provides key contribution to everyday activities, particularly step/stair ascent and descent. Age-related decline in ankle functioning can lead to an increased risk of falls on steps and stairs. The rising up-on-the-toes (UTT) 30-second test (UTT-30) is used in the clinical assessment of ankle muscle strength/function and endurance; the main outcome being how many repetitive UTT movements are completed. This preliminary study describes how inertial measurement units (IMUs) can be used to assess the UTT-30. Twenty adults (26.2 ± 7.7 years) performed a UTT-30 at a comfortable speed, with IMUs attached to the dorsal aspect of each foot. Use of IMUs’ angular velocity signal to detect the peak plantarflexion angular velocity (p-fAngVelpeak) associated with each repeated UTT movement indicated the number of UTT movements attempted by each participant. Any UTT movements that were performed with a p-fAngVelpeak 2SD below the mean were deemed to have not been completed over a sufficiently ‘full’ range. Findings highlight that use of IMUs can provide valid assessment of the UTT 30-second test. Their use detected the same number of attempted UTT movements as that observed by a researcher (average difference, -0.1 CI, -0.2 – 0.1), and on average 97.6 ± 3.1% of these movements were deemed to have been completed ‘fully’. We discuss the limitations of our approach for identifying the movements not completed fully, and how assessing the consistency in the magnitude of the repeated p-fAngVelpeak could be undertaken and what this would indicate about UTT-30 performance.

An investigation of the dynamic angle of gait and radiographic characteristics of the first metatarsophalangeal joint in subjects with hallux limitus

[Truncated abstract] Limitation of sagittal plane dorsiflexion, or hallux limitus (HL), represents the second most commonly encountered pathomechanical disorder affecting the first metatarsophalangeal joint, after hallux valgus (HV). The kinematic role of the first metatarsophalangeal joint represents an integral component of the gait cycle. It has often been reported that for adequate leverage and propulsion to occur during normal gait, the hallux must be able to dorsiflex approximately 65° on the head of the first metatarsal. Secondary gait compensation mechanisms have often been observed clinically as a result of HL. The effect of HL on gait can be reflected in transverse plane alteration of the foot in relation to the line of progression during gait, defined as the angle of gait (AOG). The first purpose of this study served to investigate potential differences in dynamic angle of gait AOG in subjects with HL compared to a control group. A validated technique using coloured powdered footprints was used to quantify AOG. Furthermore, it was required to establish whether the relative amount of transverse plane motion observed in the AOG was related to factors intrinsic or extrinsic to the foot. Intrinsic factors such as the amount of forefoot to rearfoot abduction was considered, and achieved by measuring the rearfoot to forefoot axis (RFA) angle using a weight bearing composite (COMP) view radiograph. The remaining objectives of the study served to investigate other common aetiological factors associated with HL and their potential influence on AOG in subjects with HL ... Results further indicated that the amount of first metatarsophalangeal joint dorsiflexion did not appear to influence AOG in the two groups, and that AOG did not reflect the iii amount of forefoot to rearfoot abduction in a foot with HL compared to the control group. When comparing foot type, as indicated by CIA, it appeared AOG did not significantly alter between the HL and control groups. Finally, the results indicated AOG did not differ significantly between subjects with unilateral HL. This thesis study indicated that with the current sample population, the wide variability in AOG prevented detection of any subtle differences that may exist in subjects with HL. Results also emphasised the need to incorporate other variables such as symptomology and foot dominance when considering the effects first metatarsophalangeal joint pathology might have on HL, such as AOG.

Analysis of dynamic angle of gait and radiographic features in subjects with hallux valgus

[Truncated abstract] Hallux valgus (HV) is one of the most common foot deformities encountered in clinical practice. This complex deformity primarily affects the first metatarsophalangeal joint (MPJ), leading to altered foot structure and function. By virtue of the lateral displacement of the hallux on the first metatarsal, HV has the potential to influence adjacent joints of the foot. In doing so, function of the foot may be altered, and clinically this may result in abduction of the foot during the stance phase of gait. However the relationship between an abducted angle of gait (AOG) and HV has never been substantially examined. The purpose of this study is to investigate the relationship between HV and AOG, and determine if specific radiographic features are associated with the deformity or with a particular AOG. Such information would assist in understanding aetiological factors and the effects of intervention to treat the deformity ... Length and elevation of the first metatarsal were associated in subjects with HV, implying that length of the metatarsal may be related to whether or not it becomes elevated (R: 0.50, CI: 0.21, 0.71, P< 0.05). Similarly, an association was found between length of the first metatarsal and the amount of first MPJ dorsiflexion, suggesting perhaps length of the metatarsal has implications for first MPJ range of motion (R: -0.37, CI: -0.62, -0.04, P<0.05). However the amount of first MPJ dorsiflexion did not influence the AOG in HV subjects when compared to the control group. First MPJ dorsiflexion was also associated with the first intermetatarsal angle. Interestingly, the HV group alone did not show an association between the hallux abductus angle and the first intermetatarsal angle. The findings of this study are contrary to those suspected in clinical practice and alluded to in the literature. Despite the documented support for the biomechanical causes of HV, an abducted AOG was not significantly different in HV subjects when compared to controls. Possible explanations may have related to limitations of the present study including the size and gender demographics of the sample population, and greater variability in normal AOG ranges than reported in the literature. The present study indicated a possible need to gather information regarding foot dominance and leg length; factors extrinsic to the foot capable of influencing transverse plane orientation of the foot; and, the influence of symptoms and subsequent compensatory mechanisms adopted during gait.

An Automated Approach to Instrumenting the Up-on-the-Toes Test(s)

Yes / Normal ankle function provides a key contribution to everyday activities, particularly step/stair ascent and descent, where many falls occur. The rising to up-on-the-toes (UTT) 30 second test (UTT-30) is used in the clinical assessment of ankle muscle strength/function and endurance and is typically assessed by an observer counting the UTT movement completed. The aims of this study are: (i) to determine whether inertial measurement units (IMUs) provide valid assessment of the UTT-30 by comparing IMU-derived metrics with those from a force-platform (FP), and (ii) to de-scribe how IMUs can be used to provide valid assessment of the movement dynamics/stability when performing a single UTT movement that is held for 5 s (UTT-stand). Twenty adults (26.2 ± 7.7 years) performed a UTT-30 and a UTT-stand on a force-platform with IMUs attached to each foot and the lumbar spine. We evaluate the agreement/association between IMU measures and measures de-termined from the FP. For UTT-30, IMU analysis of peaks in plantarflexion velocity and in FP’s centre of pressure (CoP) velocity was used to identify each repeated UTT movement and provided an objective means to discount any UTT movements that were not completed ‘fully’. UTT movements that were deemed to have not been completed ‘fully’ were those that yielded peak plantarflexion and CoP velocity values during the period of rising to up-on-the-toes that were below 1 SD of each participant’s mean peak rising velocity across their repeated UTT. The number of UTT movements detected by the IMU approach (23.5) agreed with the number determined by the FP (23.6), and each approach determined the same number of ‘fully’ completed movements (IMU, 19.9; FP, 19.7). For UTT-stand, IMU-derived movement dynamics/postural stability were moderately-to-strongly correlated with measures derived from the FP. Our findings highlight that the use of IMUs can provide valid assessment of UTT test(s).

Effects of induced astigmatism on foot placement strategies when stepping onto a raised surface

Yes / Large changes in spectacle prescription can increase falls risk in older people. We investigated the effect of induced astigmatism (a common cause of distorted or blurred vision in older people) on locomotor stepping patterns to determine whether the orientation of astigmatic changes could have differential effects on gait safety when negotiating steps and stairs. Methods 10 older adults (mean age 76.0±6.4 years) walked up to and stepped onto a raised block whilst wearing their spectacle prescription and when blurred with ±3.00D cylinders at axes 45°, 90°, 135° and 180°. Gait measurements included foot placement before the block, toe clearance over the block edge and foot placement on the block. Results Induced astigmatism with axes at 90°, providing magnification in the horizontal meridian only, caused no change in stepping pattern. Induced astigmatism with axes at 180° caused foot placement changes in the anterior or posterior direction according to whether magnification was positive or negative in the vertical meridian (block perceived higher or lower respectively). Induced astigmatism with axes oblique at 45° and 135° (causing the block to be perceived as a parallelogram sloping downwards either to the right or left) caused gait changes in the anterior and posterior, vertical and lateral directions. Changes in lateral foot placement appeared to be an attempt to maintain constant foot clearance levels over the block edge by stepping over the perceived ‘lower’ side of the ‘sloping’ block. Conclusions Astigmatic changes with oblique axes had the greatest effect on gait. Clinicians, including optometrists, physiotherapists, occupational therapists and nurses should counsel older patients about the effects of astigmatism on gait safety. Furthermore, partial prescribing of astigmatic corrections should be considered to reduce the risk of falling.

An investigation of the nature and effects of the learning environment in agricultural science classrooms in Nigeria

This study consolidates a long tradition of research involving the development/adaptation and validation of instruments assessing students' perceptions of psychosocial aspects of their classroom learning environments, and their use in investigating both the effects of classroom environment on student outcomes and determinants of classroom environment. The present study is distinctive, however, in that it is one of the few such studies conducted in Nigeria and the first classroom environment study conduced specificallly in agricultural science classrooms.The sample consisted of 1 175 students in 50 classes in 20 schools in eight states and the Federal Capital Territory. Both the individual student and the class mean were used as units of statistical analysis. The classroom environment instrument assessed negotiation, autonomy, student centredness, investigation and differentiation, and the student outcomes were attitudes, enquiry skills and practical performance.Each classroom environment scale was found to have satisfactory internal consistency reliability and discriminant validity and to differentiate between the perceptions of students in different classrooms. Satistically significant associations were found between classroom environment and the two student outcomes of attitudes and enquiry skills, but not for practical performance. When classroom environment dimensions were used as dependant variables, significant differences were found between schools with different school-level environments and between schools in forest and savanna regions.

Does my step look big in this? A visual illusion leads to safer stepping behaviour

No / BACKGROUND: Tripping is a common factor in falls and a typical safety strategy to avoid tripping on steps or stairs is to increase foot clearance over the step edge. In the present study we asked whether the perceived height of a step could be increased using a visual illusion and whether this would lead to the adoption of a safer stepping strategy, in terms of greater foot clearance over the step edge. The study also addressed the controversial question of whether motor actions are dissociated from visual perception. METHODOLOGY/PRINCIPAL FINDINGS: 21 young, healthy subjects perceived the step to be higher in a configuration of the horizontal-vertical illusion compared to a reverse configuration (p = 0.01). During a simple stepping task, maximum toe elevation changed by an amount corresponding to the size of the visual illusion (p<0.001). Linear regression analyses showed highly significant associations between perceived step height and maximum toe elevation for all conditions. CONCLUSIONS/SIGNIFICANCE: The perceived height of a step can be manipulated using a simple visual illusion, leading to the adoption of a safer stepping strategy in terms of greater foot clearance over a step edge. In addition, the strong link found between perception of a visual illusion and visuomotor action provides additional support to the view that the original, controversial proposal by Goodale and Milner (1992) of two separate and distinct visual streams for perception and visuomotor action should be re-evaluated.

L'étendue des mouvements que les robots humanoïdes peuvent réaliser est fortement limitée par des contraintes dynamiques. Une loi de commande qui ne prend pas en compte ses res- trictions, d'une manière ou autre, ne va pas réussir d'éviter une chute. La Commande Prédictive est capable de considérer les contraintes sur l'état et le contrôle de manière explicite, ce qui la rend particulièrement appropriée pour le contrôle des mouvements des robots marcheurs.Nous commençons par dévoiler la structure spécifique de ces contraintes, démontrant notamment l'importance des appuis au sol. Nous développons ensuite une condition suffisante pour l'évitement d'une chute et nous proposons une loi de commande prédictive qui y réponde. Cette formulation nous sert ensuite pour la conception des contrôleurs pratiques, capables d'un contrôle plus efficace et plus robuste des robots marcheurs humanoïdes. / The range of motions that humanoid robots are able to realize is strongly limited by inherent dynamical constraints so that any control law that does not consider these limitations, in one way or another, will fail to avoid falling. The Model Predictive Control (MPC) technique is capable of handling constraints on the state and the control explicitly, which makes it highly apt for the control of walking robots.We begin by unveiling the specific structure of these constraints, stressing especially the impor- tance of the supports on the ground. We give thereupon a sufficient condition for keeping balance and formulate an MPC law that complies with it. This formulation serves us then for the design of practicable controllers capable of more efficient and more robust control of humanoid robots.
