Teaching doctors : the relationship between physicians' clinical and educational practice

This thesis explores the relationship between physicians’ clinical and educational roles in the context of UK General Practice (GP) education by investigating the experiences of seven GP trainers through an ethnographic approach employing Activity Theory (AT). The Introduction considers the philosophy and structures of GP education and outlines the author’s professional biography to provide context. The Literature Review focusses on the development of medical education as a discrete field and identity formation in medical educators, concluding that: specialist medical educators are a relatively new group; and there is a paucity of knowledge regarding the impact on physicians of occupying dual clinical and educational roles. The thesis then focusses on three Research Questions (RQs), namely: 1. What is the impact of GP trainers’ clinical practice upon their educational work? 2. How does GP trainers’ educational practice influence their clinical work? 3. What are the social contexts for GP trainers’ clinical and educational practice? These questions are addressed within a pragmatic theoretical framework to build up an ethnographic description of the participants’ experiences. Data collection is through semi-structured interviews and observation of video-recorded teaching. Ethical issues associated with the study are discussed in detail, in particular the challenges of “insider” research. Four approaches are used for data analysis: global impressions; word cloud analysis; thematic analysis; and analysis shaped by AT. In answer to RQs 1 and 2, the study finds that GP trainers experience their dual roles as intimately linked, intuitively transferring their skills between their clinical and educational practice. The study also finds that GP trainers reconstruct their professional identities through teaching. With regard to RQ 3, engaging in teaching can lead to internal conflict for GP trainers and tensions with their colleagues, trainees and regulators. These findings are discussed in relation to medical education research methodology and the impact the study on the researcher is explored. The thesis closes by considering the conflicted position the participants occupy, concluding that teaching offers physicians the opportunity to reconstruct their professional identities so they can approach tensions in their practice with a sense of agency and optimism.

The Analysis Of The Perceptions Of English Language Instructors At Tobb University Of Economics And Technology Regarding Inset Content

The purpose of this study was to analyze the extent of instructors at TOBB University of Economics and Technology, Department of Foreign Languages regard in-service training programs crucial for their professional development. Another aspect of the study was to identify the professional needs of the instructors teaching preparatory classes as well as to determine the preferred instructional methods of an INSET program. The final aim of the study was to determine whether there are statistically significant differences between novice and experienced instructors regarding their needs and expectation of an in-service training program.The study was conducted among thirty-nine instructors who are teaching preparatory classes at TOBB ETU, DFL. The chairperson of the DFL also took part in the study. Data from the 39 instructors were collected through questionnaires which were developed to uncover the opinions of instructors in terms of the characteristics of effective in-service training programs, to identify the needs of the instructors and determine the instructional methods preferred in-service training program. The questionnaires included both Likert scale and open-ended items. Moreover, 50-minute lessons of 10 instructors were video recorded. Video recordings aimed to identify the needs of the instructors that should be addressed through an in-service training program. Finally, interviews were conducted with 10 instructors whose classes were video recorded. The aim of the interviews was to enable the instructors to reflect on the findings of the questionnaires and the video recordings. Another interview was conducted with the chairperson of the DFL to uncover to what extent his perceptions about in-service training activities correlate with those of the instructors. Data collected through the instruments were analyzed both quantitatively and qualitatively. Close-ended items were analyzed via the SPSS program and mean scores for each item were calculated besides an independent sample t-test. The qualitative data were analyzed through content analysis. The results of the study revealed that the instructors believe INSET is crucial for their professional development, they would like to keep on track with new developments in the field of ELT and participate in seminars and conferences as part of INSET programs. In terms of the professional needs, it was found that instrcutors need to be trained on teaching speaking skills and pronunciation, written and oral correction, promoting student autonomy, dealing with classroom management problems, teaching mixed-ability groups, raising language awareness of students and reflecting on their own experience. The t-test results demonstrated that there was a statistical significance between the answers of novice and experienced instructors concerning being trained in &lsquo / teaching vocabulary,&rsquo / &lsquo / teaching grammar&rsquo / and &lsquo / improving in written and oral correction strategies.&rsquo / It was found that experienced teachers are particularly interested in training sessions on teaching grammar, teaching vocabulary, and improving written and oral correction strategies. As a result of the findings, an ongoing INSET program model was proposed for the Department of Foreign Languages at TOBB ETU.

An Evaluation Of The In-service Teacher Training Program For The Newly Hired Instructors In The School Of Foreign Languages At Middle East Technical University

The purpose of this study was to evaluate the effectiveness of the in-service teacher training program implemented for the newly hired teachers in the School of Foreign Languages (SFL) at Middle East Technical University. The study was conducted with the help of three groups of participants who were the trainees who participated in the training program in the 2004-2005 academic year, the experienced teachers who were current members of the SFL and the teacher trainers. Data from the 12 trainees were collected through two questionnaires which were developed to identify the needs of the trainees at the start of the program and their degree of satisfaction at the end. Moreover, an interview was conducted with the trainees. Data from the 4 teacher trainers were collected through a questionnaire and it aimed to identify their expectations from and satisfaction with the program. 45 experienced teachers, on the other hand, were involved in the study to understand whether they need in-service training. The questionnaires included both Likert scale and open-ended items. Data collected through the instruments were analyzed both quantitatively and qualitatively. Closed-ended items were analyzed via the SPSS program and mean scores for each item were calculated. To identify if there is any statistically significant difference between the mean scores, a paired-sample t-test was calculated. The qualitative data were analyzed through content analysis. The results of the study revealed that the trainees especially need to improve their practical teaching skills which are immediately necessary in the new setting. Their degree of satisfaction was also found to be high. The results about the experienced teachers showed that they did not feel the need to enroll in an in-service training program. The findings about the teacher trainers revealed that they are generally happy with the program although they listed some suggestions for improvement.

A co-operative inquiry into counselling and psychotherapy trainers' inter- and intra-personal concerns and challenges in a higher education context

Key Aim: The purpose of this study was to examine complex concerns and challenges encountered by counselling and psychotherapy trainers, and support them to deliver a consistent, relationship-centred learning approach within Higher Education (HE). Background: Counselling and psychotherapy training is central to regulating practice, however, studies conceptualising trainers’ concerns and challenges in the United Kingdom (UK) are sparse. Literature generally evaluates trainer challenges from a professional competence and/or gatekeeping perspective. Little evidence exists identifying problems connected with ‘professionalisation’. Aims and Objectives: The aim was to evaluate trainers’ multidimensional unease that can hinder working relationships. The intention was to: explore difficult patterns of behaviour and group dynamics in the ‘training alliance’; explore trainers’ perceptions and experiences when confronted with gatekeeping issues; collaboratively develop strategies to enhance trainers’ learning experience; examine the processes needed to sustain these strategies; and identify the lessons learnt to inform practice, education, and research. Approach and Methods: A qualitative, co-operative inquiry approach enabled trainers to question their situated and propositional knowledge, reconcile professional challenges, allay concerns about individual fitness to practice, and provide alternative responses to students, peers, and managerial hierarchies in HE and professional bodies. This approach has a political and social element, according with personal desire to make change. Thematic analysis uncovered new insights, expanded or modified principles and re-examine accepted interpretations during 8 inquiry sessions with 5 experienced trainers, and 3 associated workshops. A primarily iterative and inductive process of immersion, involved reflexive engagement, and sharing of data with trainer/practitioners. Findings: 6 overarching themes were identified: Trying to Make Sense of Significant Events; Negotiating Conflict and Incongruity in Training Groups; Navigating Inherent Challenges within Counsellor Training Teams; Teaching as a Never-Ending Challenge; Organisational Constraints and Challenges; and Contemplating Individual Connection in a Collaborative Context. Discussion and Conclusion: Findings supported previous research suggesting trainers require training, and that trainers’ concerns and challenges are interlinked; beginning with interpersonal challenges that subsequently impact on trainers’ professional and intra-personal sense of identity. Co-operative inquiry can benefit programme teams in terms of the co-construction of trainers’ realities and dynamic negotiation of meaning. Co-researchers’ knowledge and confidence in responding to potential conflict in training was enhanced. To achieve the best outcome, this knowledge needs implementing in practice; programme team involvement is a prerequisite, and support is required by professional bodies and HE to ensure ethical training practice in the face of student disgruntlement, management demands in HE and from professional accrediting bodies.

Student Peer-Group Focusing in Psychology Training: A Phenomemological Study

The present study is an empirical phenomenological investigation of the influence of peer group Focusing practice (Gendlin, 1981) on doctoral psychology students' senses of their developing clinical expertise. Focusing, a therapeutic bodily awareness and symbolization practice, was proposed as a method that would support the development of student self-reflection, self-assessment, and self-care. The present study investigates the experiences of three female doctoral students who participated in a peer-initiated and peer-run Focusing group for five semesters. The methodological procedures for a reflective empirical phenomenological study as articulated by Giorgi and Giorgi (2003), Robbins (2006), and Wertz (1984) were followed. Procedures adapted from Walsh (1995) to ensure phenomenological researcher reflexivity and to explicate the researcher's approach to the phenomenon were also used. All participants provided data via audiotaped individual interviews, read provisional interpretations and provided written and verbal feedback to the researcher. The interpretive analyses of these texts indicated that all participants found their participation in the peer Focusing group to enhance some aspects of their clinical expertise. The findings support the idea that peer group Focusing is a helpful method for directly training psychology graduate students in self-reflection, self-assessment, and self-care. Relationships between these findings and research on the use of mindfulness meditation in graduate psychology training are discussed. A resposta a esta problemática de pesquisa teve como objetivo geral verificar quantos e quais cursos de Biblioteconomia em IES públicas brasileiras incluem a disciplina empreendedorismo nas suas matrizes curriculares, desdobrando-se como objetivos específicos a identificação da modalidade da oferta da disciplina na estrutura curricular; a identificação da frequência e da distribuição regional da oferta da disciplina de empreendedorismo no Brasil e a verificação da motivação dos cursos para inclusão da disciplina na estrutura curricular. Para realizar a pesquisa optou-se pela combinação dos métodos do levantamento (survey) para identificar os Cursos de Biblioteconomia em IES públicas brasileiras que apresentam nas matrizes curriculares e ofertam a disciplina de empreendedorismo, associado ao método documental para realizar a análise de projetos pedagógicos e/ou planos de disciplina, juntamente com a utilização do estudo de casos para verificar como vem sendo realizada a oferta dessa disciplina nos Cursos de Biblioteconomia. Para a obtenção das informações optou-se pela adoção da técnica da aplicação de questionários junto aos coordenadores e/ou docentes dos Cursos de Biblioteconomia e da realização de entrevistas com os professores que ministram a disciplina de empreendedorismo. A análise dos dados se realizou a partir da integração das abordagens quantitativas e qualitativas. Os resultados identificaram que as experiências com a oferta da disciplina de empreendedorismo nos Cursos de Biblioteconomia em IES públicas brasileiras ainda são em número limitado, porém essas experiências já oferecem elementos qualitativos que podem contribuir para o avanço da capacitação empreendedora na formação do bibliotecário brasileiro e ratificam a importância do desenvolvimento de novos estudos sobre o ensino do empreendedorismo. / ABSTRACT The great potential of entrepreneuship teaching for the formation of innovative Professional librarian with the right skills to act in one unstable and competitive job market was the theme of this dissertation. It has had as its purposed to verify whether entrepreneurship has been worked as a curriculum content in the formation of the Brazilian librarian by the public institutions of higher education. The answer to this research question it was raised the goal like to verify how many and what courses of Librarianship in Brazilian public universities that include entrepreneurship as a matter of their educational curriculum. As specific objectives it was established: to identify the mode of discipline offered in the curriculum; to identify the frequency and regional distribution of the offering of the entrepreneurship discipline in Brazil; to ascertain the motivation of the librarianship courses for including entrepreneurship discipline in the educational curriculum. To carry out the research it was chosen the combination of the (survey) methods in order to identify Librarianship courses in Brazilian public universities the offers on their curricular the entrepreneurship discipline. In addition it was used together the documentary method to analyse of the educational projects and / or plans of discipline, along with the use of case studies to see how the offer is being conducted in the entrepreneurship discipline in the library science courses. To obtain these information it was adopt the technique of questionnaires together applied to the coordinators and /or professors of library science courses and also it was carried out interviews with teachers who teach the entrepreneurship discipline. Data analysis was carried out based on the integration of quantitative and qualitative approaches. The results showed that experiences with the offer of the entrepreneurship discipline of librarianship courses in in Brazilian public uiversities are still limited in number, but these experiments provide qualitative elements that can contribute to the advance of entrepreneurial training in the formation of the Brazilian librarian and ratify the importance of developing of new studies on entrepreneurship education.

Cette thèse étudie le rôle et l’impact du « stage en entreprise » sur la construction du projet professionnel de personnes de « bas niveau de qualification » engagées dans une formation en alternance. Notre objectif principal vise, d’une part, à mettre en évidence leurs profils, leurs besoins ainsi que les représentations qu’elles ont de la formation, du travail et du projet professionnel, et, d’autre part, à identifier et caractériser leurs attitudes et les problèmes spécifiques qu’elles affrontent lorsqu’elles sont en interaction avec les acteurs et le milieu de l’entreprise. Nous partons de l’hypothèse que l’efficacité du stage et du dispositif de formation dépend de la « dynamique biographique » de chaque stagiaire et des difficultés qu’il exprime dans le contexte de l’alternance. Après une phase d'enquête sur terrain et la réalisation d’études de cas à partir d’une série de questionnaires et entretiens, nous pensons que les problèmes rencontrés par ces stagiaires ne sont pas de l’ordre de la construction proprement dite du projet professionnel, mais des doutes, sinon des craintes, sur leurs capacités à pouvoir le construire. Le blocage semble en amont même de la construction du projet : ils peinent à surmonter des obstacles biographiques, personnels, psychologiques et contextuels. L’efficacité recherchée pourrait être obtenue à deux conditions : une réorganisation institutionnelle et pédagogique des dispositifs de formation et une amélioration de l’accompagnement psychologique et social des stagiaires. / This thesis analyses the role and impact of “a company internship” onto the development of professional scheme with “low skilled individuals” involved in alternative training. Our main target is, on the one hand, to highlight their profiles, their needs, as well as the representations they have of their trainings, their jobs, their professional schemes and, on the other hand, to identify and characterize their attitudes and the particular problems they are confronted with, when interacting with the different actors and with their business environment. We take for granted that the efficiency of the training system is connected with the biographical dynamics of each trainee together with the types of difficulties experienced in the context of alternative training. The research highlights that the problem for this group of the professional scheme as such but rather to doubts, even fears about their actual capacity to carry it out. The blocage seems even prior to the development of the scheme: they are having difficulties to overcome the biographical, personal, psychological and contextual obstacles. The expected efficiency could be obtained on two conditions: an institutional and pedagogical reorganization of the training system together with a better psychological and social accompaniment of the trainees.
