La tomodensitométrie (TDM) effectuée dans les centres référents chez les patients ayant une indication de transfert immédiat ainsi que la TDM répétée dans le centre de traumatologie spécialisé sont déconseillés dans les guides de pratiques cliniques. Nous avons mené une étude de cohorte rétrospective multicentrique pour estimer l'incidence de la TDM avant et après le transfert dans l'ensemble et dans les groupes de patients ayant une indication de transfert (traumatismes crâniens modérés ou sévères, blessures médullaires, fractures pelviennes et polytraumatismes). Nous avons également évalué les variations interhospitalières ; identifié les prédicteurs ; et quantifié l'influence de la TDM avant le transfert sur le délai du transfert. Nous avons observé que plus de la moitié et plus du tiers des patients avaient subi respectivement une TDM avant et après le transfert. La variation interhospitalière était modérée pour la TDM avant le transfert et faible pour la TDM répétée après le transfert. L'augmentation de l'âge, le sexe masculin, les chutes et des antécédents médicaux étaient associés à une probabilité élevée de subir une TDM avant le transfert. La TDM répétée après le transfert était associée à la période d'étude, l'âge, la blessure à la tête, la sévérité des blessures, des antécédents médicaux et le niveau de désignation du centre référent. Quel que soit le type de traumatisme, le délai de transfert était deux fois plus long chez les patients ayant subi une TDM avant le transfert. Ces informations peuvent être utilisées afin d'améliorer l'adhésion aux directives de pratique clinique. / Clinical guidelines advise against computed tomography (CT) imaging performed at referral centers in patients with an indication for immediate transfer as well as repeat CT scans at the major trauma center. We conducted a multicenter retrospective cohort study to estimate the incidence of pre- and post-transfer CT overall and in injury groups with an indication for immediate transfer (moderate to severe traumatic brain injuries, spinal cord injuries, severe pelvic fractures, and multi-system trauma). We also assessed interhospital variation; identified predictors; and quantified the influence of pre-transfer CT on delays to transfer. We observed that more than half and more than one-third of patients underwent pre- and post-transfer CT imaging, respectively. Interhospital variation was moderate for pre-transfer CT and lower for repeat post-transfer CT. Increased age, male sex, falls, and certain pre-existing conditions were associated with an increased odds of pre-transfer CT. Repeat post-transfer CT was associated with study year, age, head injury, injury severity, certain pre-existing conditions, and designation level of the referral center. Regardless of trauma types, delays to transfer were doubled in patients who had a pre-transfer CT. This information may be used to drive improvements adherence to clinical practice guidelines.

International comparison of injury care structures, processes, and outcomes between integrated trauma systems in Québec, Canada, and Victoria, Australia

Titre de l'écran-titre (visionné le 1er mai 2023) / Contexte et objectifs : Les comparaisons entre les différents systèmes de traumatologie peuvent révéler des pistes d'amélioration de la qualité des soins. Cette étude vise à comparer les systèmes de traumatologie de la province de Québec, Canada et de l'État de Victoria, Australie, en termes de structures, processus de soins et résultats cliniques des patients. Méthodologie : Les éléments structurels des systèmes de traumatologie recommandés par « The American College of Surgeon » ont été utilisés pour comparer les structures des systèmes. La comparaison des processus de soins et les résultats cliniques des patients ont été basées sur les données des registres des traumatismes du Québec et de Victoria entre 2013 et 2017. Les processus de soins pris en compte comprenaient le délai d'arrivée à l'établissement qui fournit les soins définitifs, le temps de séjour à l'urgence, et le délai avant le premier scan de la tête. Ils ont été comparés à l'aide d'un test-z des délais log-transformés. Les résultats cliniques notamment la mortalité et la durée de séjour à l'hôpital ont été comparés en utilisant une standardisation indirecte. Résultats : Concernant les structures, le système préhospitalier d'urgence à Victoria a un recours aux soins avancés de réanimation sur les lieux de la blessure, alors qu'au Québec le système préhospitalier a un recours aux soins de base. Quant aux processus, les victimes de traumatismes majeurs au Québec sont arrivées plus tôt à l'établissements de soins définitifs que leurs homologues de Victoria (médiane : 1,93 vs. 2,13 h, p = 0,002), mais ont séjourné plus longtemps à l'urgence (médiane : 8,23 vs. 5,15 h, p <0,0001) et ont attendu plus longtemps avant leur premier scan de la tête (médiane : 1,90 vs. 1,52 h, p< 0,0001). La mortalité et la durée du séjour hospitaliers étaient plus élevées au Québec comparativement à Victoria (SMR : 1,15, IC 95 % : 1,09 - 1,20 ; ratio standard de durée de séjour : 1,10, IC 95 % : 1,09 - 1,11). Conclusion : Nous avons observé des différences dans les composantes structurelles et les processus de soins des systèmes de traumatologie du Québec et de Victoria qui pourraient expliquer certaines différences observées dans les résultats cliniques. / Context and objectives: Comparisons across different trauma systems may reveal opportunities for quality improvement. This study aimed to compare the integrated trauma systems in Québec, Canada and Victoria, Australia, regarding their structure, care processes, and patient outcomes. Methods: The trauma system elements recommended by the American College of Surgeons were used to compare trauma systems structures. Comparisons of care processes and patient outcomes were based on data from trauma registries between 2013 and 2017. Care processes included time to reach a definitive care facility, time spent in the emergency department, and time-lapsed before the first head CT scan. These processes were compared using a z-test of log-transformed times. The outcomes of hospital mortality and hospital length of stay were compared using indirect standardization based on multiple logistic and linear regression. Results: Among differences in trauma system structure, paramedics in Victoria use Advanced Trauma Life Support at the scene, whereas basic life support is used in Québec. Patients in Québec arrived at their definitive care hospital earlier than their counterparts in Victoria (Med.: 1.93 vs. 2.13h, p = 0.002), but spent a longer time in the emergency department (Med.: 8.23 vs. 5.15 h, p <0.0001) and waited longer for their first head CT (Med.: 1.90 vs. 1.52 h, p < 0.0001). Mortality and hospital LOS were higher in Québec than in Victoria (SMR: 1.15, 95% CI: 1.09 - 1.20; Standard LOS ratio: 1.10, 95% CI: 1.09 - 1.11). Conclusion: We observed important differences in the structural components and care processes between Québec and Victoria trauma systems, which might explain some of the observed differences in patient outcomes. This study demonstrates the value of international comparisons in trauma care and identifies potential opportunities for quality improvement.

Évaluation des pratiques cliniques à faible valeur dans les soins aigus des traumatismes au Québec

Les organisations de traumatologie reconnues à l'échelle internationale émettent des directives contenant des recommandations sur les pratiques cliniques à éviter car elles sont inefficaces et qualifiées à faible valeur à l'échelle de la santé des populations ; cependant nous manquons de données sur la manière dont ces recommandations sont appliquées dans la pratique. Nous avons identifié les pratiques cliniques à faible valeur conformément aux lignes directrices de ces organisations. Afin de mesurer leurs fréquences et d'évaluer leurs variations entre les hôpitaux, nous avons conduit une étude de cohorte populationnelle, rétrospective, multicentrique avec 117 847 patients, victimes d'un traumatisme, admis dans les 57 centres de traumatologie au Québec entre 2010 et 2017. Neuf algorithmes ont été développés et validés à l'aide des données du registre des traumatismes du Québec. Les trois pratiques cliniques à faible valeur présentant les fréquences absolues et relatives les plus élevées étaient la radiographie du bassin chez les patients ayant une blessure pelvienne contondante stable avec un examen physique négatif (n=370/863) (42,9%); la tomodensitométrie de la tête chez les adultes présentant une blessure mineure à la tête sans aucun critère de décisions cliniques (n=2524/10410) (24,2%) et la radiographie thoracique chez les patients stables avec un examen physique négatif (n=127/1841) (6,9%). Nous avons observé une forte variation inter-hospitalière dans l'indication de la prise en charge chirurgicale des lésions cervicales pénétrantes sans signes de gravité, avec un coefficient de corrélation intra-classe (CCI) à 27%. Les résultats de cette étude font progresser les connaissances sur les pratiques à faible valeur en soins aigus de traumatologie et peuvent être utilisées afin d'améliorer la qualité des soins. / Internationally recognized trauma organizations issue guidelines with recommendations on clinical practices to avoid because they are ineffective and low-value qualified at the population health level; however, we lack data on how these recommendations are applied in practice. We identified low value clinical practices in accordance with the guidelines of these organizations. In order to measure their frequencies and to evaluate their variations between hospitals, we conducted a population-based, retrospective, multicenter cohort study with 117,847 trauma patients admitted to the 57 trauma centers in Quebec between 2010 and 2017. Nine algorithms have been developed and validated using data from the Quebec Trauma Registry. The three lowest-value clinical practices with the highest absolute and relative frequencies were pelvic radiography in patients with stable blunt pelvic injury with a negative physical examination (n=370/863) (42.9%); CT scan of the head in adults with minor head injury with no clinical decision criteria (n = 2524/10 410) (24.2%) and chest X-ray in stable patients with a negative physical examination (n=127/1841) (6.9%). We observed a strong inter-hospital variation in the indication of the surgical management of a penetrating cervical lesion without signs of seriousness, with an intra-class correlation coefficient (ICC) at 27%. The results of this study advance knowledge of low value practices in acute trauma care and can be used to improve the quality of care.

Élaboration et validation de critères de transfert des traumatisés majeurs vers les centres tertiaires de traumatologie

Introduction: L’organisation des réseaux de traumatologie a amélioré la survie des traumatisés majeurs. Toutefois, la littérature actuelle est imprécise quant aux indications de transfert vers les centres tertiaires de traumatologie. Objectifs : 1) Élaborer un consensus de critères de transfert des traumatisés majeurs en centre tertiaire, 2) évaluer l’association entre la présence des critères et la gravité des cas, 3) démontrer la validité prédictive de ces indications de transfert en centre tertiaire concernant les TCC. Méthode : Un consensus a été élaboré à l’aide de la méthode Delphi par un groupe d’experts multidisciplinaire provenant de centres primaires, secondaires et tertiaires. Toutes nos analyses ont été effectuées à l’aide d’un échantillon tiré du Registre des Traumatismes du Québec et comportant les données de 146 630 patients (1998-2008). Résultats : Douze situations cliniques ont été retenues par les experts. Pour la population admise directement en centre tertiaire, le risque ajusté de décès, d’admission à l’unité des soins intensifs et de complications majeures [IC 95 %] pour les patients répondant à l’un ou l’autre des critères du consensus est supérieur à celui des patients ne présentant aucun critère, soit respectivement RR = 6.2 [5.6-6.8], 3.5 [3.4-3.7] et 2.8 [2.7-3.0]. De plus, pour les patients ayant subi un TCC et initialement amenés dans un centre primaire ou secondaire, on a observé que la cote de décès [IC 95 %] des patients transférés est inférieure à celle des non transférés pour les situations cliniques suivantes: 1) score &lt; 9 sur l’échelle de coma de Glasgow, RC = 0.18 [0.06-0.52], 2) fracture ouverte ou enfoncée du crâne, RC = 0.21 [0.06-0.71], 3) altération de l’état de conscience avec tomodensitométrie cérébrale anormale, RC = 0.35 [0.15-0.85], et 4) hématome sous-dural, épidural ou hémorragie intracérébrale, RC = 0.53 [0.36-0.79]. Conclusion : Des critères de transfert en centre tertiaire de traumatologie ont été élaborés et validés. Les victimes d’un TCC répondant à l’un des critères ci-haut mentionnés présentent un meilleur pronostic s’ils sont transférés en centre tertiaire. Une validation prospective et une mesure d’impact pourront être réalisées si ces critères sont implantés au sein du réseau de traumatologie. / Introduction: Trauma care systems have improved the survival of major trauma patients. However, the current literature is unclear about the indications for these patients to be transferred to tertiary trauma centers. Objectives: 1) To develop consensus criteria for the transfer of major trauma, 2) to assess the association between the presence of criteria and severity of cases and 3) to demonstrate the predictive validity of the criteria for patients with traumatic brain injury (TBI). Methods: A Delphi consultation with experts led to a multidisciplinary consensus criteria for transfer to tertiary centers. The panel was composed of emergency physicians, surgeons, intensivists and neurosurgeons from primary, secondary and tertiary trauma center. All analyzes were performed using the Quebec Trauma Registry (QTR), with the data of 146 630 patients (1998-2008). Results: Twelve clinical situations were selected by the panel. For patients admitted directly to a tertiary trauma center, the adjusted risk of death, admission to intensive care unit and major complications [IC 95 %] among patients meeting one or other of the consensus criteria was higher than that of patients with no criteria, respectively RR = 6.2 [5.6-6.8], 3.5 [3.4-3.7] et 2.8 [2.7-3.0]. In addition, in a population of patients sustaining a traumatic brain injury (TBI) and initially transported to a primary or secondary center, it was observed that the adjusted odds of death [IC 95 %] for patients transferred was lower than that of non-transferred patients for the following clinical situations: 1) score &lt; 9 on the Glasgow Coma Scale (GCS), OR = 0.18 [0.06-0.52], 2) open or depressed skull fracture, OR = 0.21 [0.06-0.71], 3) altered state of consciousness with abnormal CT scan, OR = 0.35 [0.15-0.85] and 4) subdural or epidural hematoma, or intracerebral hemorrhage, OR = 0.53 [0.36-0.79]. Conclusion: Criteria for transfer to tertiary trauma center were developed and validated. TBI victims with anyone of the proposed consensus criteria have a better prognosis if they are transferred to a tertiary trauma center. A prospective validation and an impact measurement will be possible if these criteria are implemented in a trauma system.

Hintergrund Die Versorgung Schwerverletzter setzt das zügige Erkennen lebensbedrohlicher Verletzungen und deren Priorisierung voraus. Hierzu verfügt das Universitätsklinikum Würzburg seit 2018 über ein Doppelschockraumkonzept mit Ganzkörper-CT, fahrbarer CT-Gantry und einer mobilen Schutzwand, wodurch zwei Personen nahezu simultan behandelt werden können. Das Ziel der vorliegenden Arbeit war zum Einen mögliche Spezifika von simultan versorgten Patienten und Patientinnen zu identifizieren und zum Anderen die Evaluation möglicher Unterschiede in der Qualität der Versorgung in einem Doppel- und einem Einzelschockraum, insbesondere der Zeit bis zur CT-Bildgebung und bis zum Beginn operativer Maßnahmen. Methodik Im Rahmen einer retrospektiven Untersuchung wurden die Patientendaten aller Schockraumaufnahmen des Universitätsklinikums Würzburg vom 1. Mai 2019 bis zum 29. April 2020 analysiert. Die Datensätze wurden bei einer simultanen Versorgung im Schockraum der Doppelschockraumgruppe (Gruppe 1) und bei einer alleinigen Versorgung der Einzelschockraumgruppe (Gruppe 2) zugeordnet. Ergebnisse 10,9 % aller Schockraumaufnahmen wurden simultan im Schockraum versorgt (46 von 423). Personen aus Gruppe 1 verunglückten häufiger bei PKW-Unfällen (47,8 % vs. 19,6 %; p < 0,05). Keine Unterschiede fanden sich bezüglich Alter, Geschlecht, ASA, ISS und präklinischer Versorgung. Die Klinik bei der Aufnahme unterschied sich nicht bezüglich A-, B-, C- und D-Problemen, allerdings litten Personen aus Gruppe 1 häufiger unter Schmerzen (hier als E-Problem nach ATLS klassifiziert) (45,7 % vs. 29,2 %; p < 0,05). Die Versorgung im Schockraum (Instrumentierung, Medikamentenapplikation, Transfusion) ergab keine relevanten Unterschiede. Insbesondere zeigte sich keine klinisch relevante Verzögerung bei simultan versorgten Patienten und Patientinnen bis zur CT-Bildgebung oder dem Beginn operativer Maßnahmen (tCT: 8 vs. 6 min (Gruppe 1 vs. Gruppe 2), p < 0,05; tOP: 99 vs. 90 min (Gruppe 1 vs. Gruppe 2), p < 0,05). Auch das Outcome in beiden Gruppen war vergleichbar. Diskussion Die simultane Versorgung zweier Schwerverletzter stellt hohe Anforderungen an Personal, Ausstattung und Organisation. Das in dieser Arbeit untersuchte Doppelschockraumkonzept kann auch bei simultan versorgten Patienten und Patientinnen eine bestmögliche Versorgung auf individualmedizinischem Niveau gewährleistet werden. / Background The treatment of seriously injured patients requires a fast diagnosis of life-threatening injuries and their prioritization. Therefore, the university hospital Würzburg has a dual-room trauma suite concept with whole-body CT, a movable CT gantry and a mobile X-ray protection wall since 2018, which allows the simultaneous treatment of two patients. The aim of the present study was to identify the specifics of simultaneously treated patients and to evaluate differences in the quality of treatment between a dual and a single trauma suit treatment, especially in the time until CT diagnosis and the time until the beginning of surgical procedures. Methods The data of all trauma suite administrations between January 5, 2019 and April 29, 2020 was analyzed as an observational study. The data were assigned to a dual trauma suite treatment group (group 1) if two patients were treated simultaneously and to a single trauma suite group (group 2) if patients were treated solo. Results 10.9 % of all trauma room administrations were treated simultaneously (46 out of 423). There were more car accidents in patients in group 1 (47.8 % vs. 19.6 %; p< 0.05). There were no differences in age, sex, ASA, ISS and preclinical treatment. There were no differences in the clinical status at the administration regarding A-, B-, C- and D-problems, but patients in group 1 suffered more often from pain (classified as an E-problem) (45.7 % vs. 29.2 %; p < 0.05). There were no differences in the treatment in the trauma suite (catheters, medication or transfusion). There was no clinically relevant delay until the CT diagnosis and the beginning of surgical procedures. (tCT: 8 vs. 6 min) (group 1 vs. group 2), p < 0.05; tOR: 99 min vs. 90 min (group 1 vs. group 2; p < 0.05). The outcome in both groups was comparable. Discussion The simultaneous treatment of two seriously injured patients makes high demands on personnel, equipment and organization. The examined dual trauma suite concept may ensure the best possible treatment at the highest standards for single-treated patients as well as for simultaneously treated ones.

Un modèle éléments finis de l'épaule : du traumatisme à la réhabilitation / A finite element model of the shoulder : from trauma to rehabilitation

L’épaule est la plus mobile et la plus fragile des articulations. Elle est souvent touchée en traumatologie ou altérée à cause de dégénérescences. Afin d’étudier son fonctionnement et les diverses problématiques associées, un modèle éléments finis multidisciplinaire a été développé. Les différents composants anatomiques ont été reconstruits à partir d’images médicales ; puis un comportement mécanique et des propriétés spécifiques ont été paramétrés. Des propriétés contractiles ont été implémentées pour les principaux muscles. Différentes applications ont été étudiées :- En traumatologie, la chronologie lésionnelle a été expliquée et détaillée dans le cas d’un impact latéral.- En orthopédique, l’enclouage centromédullaire d’un humérus traumatique a permis de comprendre le fonctionnement et les conséquences de ce type d’implant.- En physiologie du mouvement, le concept de la prothèse inversée a été évalué mécaniquement à travers l’influence des muscles de la coiffe des rotateurs. / The shoulder is the most mobile and the most fragile joint. It is often affected by trauma or impaired due to degenerations. In order to study its behaviour and various problems associated, a multidisciplinary finite element model was developed. The anatomical components have been reconstructed from medical data, and then mechanical behaviour and specific properties have been configured. Contractile properties were implemented for the main muscles. Different applications have been studied:- In trauma field, the injury timeline was explained and detailed in the case of a side impact.- In orthopaedic domain, the intramedullary nailing of a traumatic humerus has to understand the function and the consequences of this type of implant.- In the physiologic field, the concept of reverse prosthesis has been evaluated mechanically through the influence of the muscles of the rotator cuff.
