J o h a n n e s b u r g 1988 / Infections can cause autoantibody production. The purpose of this study was to determine the prevalence of autoantibodies in chronic mycobacterial infections in Johannesburg. Sera from 41 leprosy patients and from 49 untreated and 73 treated tuberculosis patients were tested for rheumatoid factor, antibodies against a panel of nuclear antigens, anticardiolipin antibodies and syphilis serology. The antinuclear antibody was positive in 7.3% of the leprosy group, 6.1% of the untreated TB group and 15% of the treated tuberculosis patients (p=0.0125). Antinuclear antibody positivity correlated with duration of treatment (p=0.025). The antinuclear antibody titres were low and there was no specific pattern. / IT2018

Thesis (M.Dev.) --University of Limpopo (Turfloop Campus), 2005 / The aim of this study was to investigate the socio-economic impact of interrupting TB treatment to the families of the TB sufferers and the reasons for patients interrupting treatment. The study was conducted at Mashishimale Village, Ba- Phalaborwa Municipality, Mopani District, Limpopo Province in South Africa. A sample of 35 tuberculosis patients and their family members was drawn from the Mashishimale population. The sample comprised of 17 (49%) males and 18 (51%) females. Self-administered questionnaires were distributed to the participants to complete. The questionnaire elicited demographic information; knowledge about TB, its causes, signs and symptoms, transmission, the reasons for interrupting treatment, and the patients’ coping and support structures. The findings of the study reported that 50% of TB patients are conversant with the signs, symptoms and mode of the spread of TB. However, 43% of the TB patients reported that there was a perception that TB patients are also HIV positive. A further 29% mentioned that there is stigma attached to TB disease. The lack of a Directly Observed Treatment Supporter, poverty and poor nutrition, side-effects of drugs, loss of disability grants, long clinic queues, and traditional healing were some of the reasons cited for the interruption of TB treatment. The interruption of TB treatment had an impact on the socio-economic situation of the family as they often relied on assistance from social grants, other family members and churches.

La tuberculosis es una enfermedad infectocontagiosa de alta incidencia y prevalencia que produce además de trastornos físicos, serias repercusiones en la esfera psicoemocional, el cual se ve reflejado con mayor frecuencia en su autoestima, en tal sentido el presente estudio tuvo como objetivo determinar el nivel de autoestima de los pacientes del programa de control de tuberculosis del centro de salud Laura Rodríguez Dulanto-Duksil 2004. El método que se utilizó fue el descriptivo exploratorio, de corte transversal. La población estuvo conformada por los pacientes del Programa de Control de Tuberculosis del Centro de Salud Laura Rodríguez Dulanto – Duksil, conformado por 21 pacientes. La técnica que se utilizó fue la encuesta y el instrumento fue un formulario tipo cuestionario y el test de Autoestima de Cirilo Toro Vargas. Las conclusiones fueron; que el 56% de los pacientes tienen un nivel de autoestima alta positiva referido a las dimensiones cognitivas, afectivas y ética y 44% con autoestima baja positiva, autoestima alta negativa y autoestima baja negativa, que esta dado por alteraciones en las dimensiones física y social. En cuanto al nivel de autoestima en la dimensión física, el 67% presentan autoestima baja positiva y 33% una autoestima alta positiva. Respecto a la dimensión cognoscitiva 55% tienen autoestima alta positiva y 45% autoestima baja positiva. Acerca de la dimensión afectiva, 71% tienen autoestima alta positiva y 29% autoestima baja positiva. En relación a la dimensión social, 57% tienen autoestima baja positiva y 43% autoestima alta positiva. Sobre la dimensión ética, 81% tiene autoestima alta positiva y 19% autoestima baja positiva y autoestima alta negativa.

Protein-ligand interactions of arylamine N-acetyltransferase from Mycobacterium smegmatis

Tuberculosis is the world's largest cause of death from an infectious agent. Treatment is by an extended period of combination chemotherapy. Drug resistance is an increasing problem in tuberculosis therapy, particularly to the frontline anti-tubercular drug isoniazid (INH). Recombinant arylamine N-acetyltransferase (NAT) of Mycobacterium tuberculosis N-acetylates INH using the cofactor Acetyl Coenzyme A. NAT from M. tuberculosis is a polymorphic enzyme and also acetylates INH in vivo. Acetylated INH is inactive therapeutically against M. tuberculosis both in vivo and in vitro. The acetylation of isoniazid in the mycobacterial cell may compete with the activation of INH by the catalase-peroxidase, katG, and hence contribute to INH resistance in clinical isolates. Inhibition of NAT in M. tuberculosis may thus increase the efficacy of INH therapy. A novel assay based around the detection of free Coenzyme A released during the acetylation reaction was used to determine the substrate specificity of recombinant NAT from the related Mycobacterium M. smegmatis (MSNAT). A relationship was observed between the lipophilicity of simple arylamine substrates and the rate of acetylation by MSNAT. Several MSNAT substrates possess antibacterial activity. The assay could also be used to screen compound libraries for MSNAT inhibitors. Synthesis of seventeen thiazolidinedione sultams in collaboration with Dr.Vickers (Dyson Perrins), identified as weak inhibitors of MSNAT, gave a minimum competitive inhibitory constant of 14μM. Screening a library of 5,074 drug-like compounds for inhibition of MSNAT identified thirteen compounds with semi-maximal inhibition constants (IC<sub>50</sub>) of below 10μM. Based on this, fifteen maleimides were synthesised and were irreversible inhibitors of MSNAT with submicromolar potency. Similarly, ninety-six aminothiazoles were synthesised by Dr. Vickers and were uncompetitive inhibitors of MSNAT with a minimum IC<sub>50</sub> of 1.5μM. The most potent aminothiazole showed no effect on the growth of M. smegmatis or M. bovis BCG or the sensitivity of the bacteria to isoniazid. However the aminothiazoles were shown not to penetrate the cells.
