Mifepristonbehandling vid myom : Effekt- och säkerhetsaspekter

Bakgrund: Myom är den vanligaste gynekologiska benigna tumören hos kvinnor i fertil ålder. Det finns idag läkemedelsbehandlingar, invasiva ingrepp och icke-invasiva ingrepp vid behandling av myom. Utvecklingen av läkemedel mot myom är lågprioriterat, eftersom de främst är benigna och snarare leder till sjuklighet än dödlighet. De läkemedel som idag används vid behandling av myom är ulipristal, GnRH-agonister, NSAID, tranexamsyra, p-piller eller hormonspiral. Ulipristal och GnRH-agonister har utöver förbättring av symtom som alla de senast nämnda, en effekt på reducering av myomstorlek. Icke-invasiva ingrepp innefattar idag myolys, embolisering av arteria uterina och fokuserad ultraljudskirurgi under MRI-vägledning, framtagna för att slippa operativa ingrepp. Myomektomi och hysterektomi är två operativa ingrepp som genomförs om inga andra behandlingar fungerar, eftersom de medför en längre återhämtningstid och en större risk för komplikationer. Mifepriston är en antiprogesteron, vilket innebär att den hämmar progesterons effekt. Progesteron i sin tur spelar en stor roll i utvecklingen av ett myom. Behandling med läkemedlet har i många olika studier visat goda resultat både i avseende på myomstorlek, symtom, livskvalité och biverkningar. Syfte: Syftet med examensarbetet var att undersöka om mifepriston är ett säkert och effektivt läkemedel vid behandling av myom hos kvinnor. Metod: Examensarbetet är en litteraturstudie och baseras på 6 olika randomiserade kontrollerade vetenskapliga studier som undersökte mifepristons effekt och säkerhet vid behandling av myom. Studierna hämtades från PubMed. Resultat: Samtliga studier visade en reducering i myomvolym, en förbättring av symtom och milda biverkningar med olika doseringar av mifepriston vid behandling av myom. I 2 av studierna undersöktes livskvalitén vilket ökade hos de personer som behandlades med mifepriston. Slutsats: Det är svårt att dra generella slutsatser vilken dos och behandlingstid som är optimala på grund av de olika behandlingstiderna, doseringarna och studieuppläggen som användes i samtliga studier. Dock har mifepriston en god effekt avseende på reducering i myomstorlek, symtom, biverkningar och livskvalité. Det krävs dock fler studier för att säkerställa dosering och behandlingstid samt fler jämförelser med andra behandlingsalternativ som idag finns för behandling av myom.

The Effect of the Space Flight Environment on Mucin Production in the Mouse Uterine Tube

Numerous studies have indicated that the microgravity environment of space has harmful effects on several tissues throughout the body. Although this phenomenon is well documented, research in this area is still in its relative infancy. This study investigates the effects of space flight on mucin production of the uterine tubes of mice. This study examined the epithelium of the uterine tubes from female mice that were flown on the space shuttle Endeavour for 13 days in August, 2007 and their concomitant controls. The tissue was qualitatively analyzed for the type of mucin produced, i.e.; acidic, neutral, acidic/neutral mixture. Further, the tissue was quantitatively analyzed for the amounts of mucins produced by measuring the thickness of the mucin layer for each region of the uterine tube: isthmus, ampulla, and infundibulum. One way ANOVA tests were used to compare mucin thickness between all three sets of animals. Results indicate similar but not identical results between the three regions of the uterine tube. The Baseline tissue had the thickest mucin layer regardless of treatment group. In the ampulla the mucin layer was the thinnest in the Flight tissue, followed by the Ground Control, with the Baseline being the thickest. Analysis of the mucin layer of the infundibulum of the three treatment groups indicated no difference in its thickness between the three regions of the uterine tube. These results indicate a trend toward thinning of the mucin layer of the uterine tube in space flight, but also indicate an influence by the housing environment.

Uterine and Eggshell Structure and Histochemistry in a Lizard With Prolonged Uterine Egg Retention (Lacertilia, Scincidae, Saiphos)

The eggshell of lizards is a complex structure composed of organic and inorganic molecules secreted by the oviduct, which protects the embryo by providing a barrier to the external environment and also allows the exchange of respiratory gases and water for life support. Calcium deposited on the surface of the eggshell provides an important nutrient source for the embryo. Variation in physical conditions encountered by eggs results in a tradeoff among these functions and influences eggshell structure. Evolution of prolonged uterine egg retention results in a significant change in the incubation environment, notably reduction in efficiency of gas exchange, and selection should favor a concomitant reduction in eggshell thickness. This model is supported by studies that demonstrate an inverse correlation between eggshell thickness and length of uterine egg retention. One mechanism leading to thinning of the eggshell is reduction in size of uterine shell glands. Saiphos equalis is an Australian scincid lizard with an unusual pattern of geographic variation in reproductive mode. All populations retain eggs in the uterus beyond the embryonic stage at oviposition typical for lizards, and some are viviparous. We compared structure and histochemistry of the uterus and eggshell of two populations of S. equalis, prolonged egg retention, and viviparous to test the hypotheses: 1) eggshell thickness is inversely correlated with length of egg retention and 2) eggshell thickness is positively correlated with size of shell glands. We found support for the first hypothesis but also found that eggshells of both populations are surprisingly thick compared with other lizards. Our histochemical data support prior conclusions that uterine shell glands are the source of protein fiber matrix of the eggshell, but we did not find a correlation between size of shell glands and eggshell thickness. Eggshell thickness is likely determined by density of uterine shell glands in this species.

Does Spaceflight Increase the Chance of Female Astronauts Developing Uterine Cancer?

One of the main questions put forth by NASA and the European Space Agency (ESA) is whether or not an organism, especially mankind, can complete an entire life cycle in space. With this in mind, it is essential to study the effect of spaceflight on reproductive tissues. Using simulated microgravity techniques and whole-body radiation we sought to determine if females subjected to a simulated spaceflight environment have increased incidences of uterine cancer. Uterine tissue from mice subjected to simulated spaceflight was analyzed using immunohistochemical staining and western blot analysis. Two pathways commonly activated in cancer were investigated. Additionally, the uterine tissue was evaluated for gross morphological changes using standard histological staining. The findings of this study indicate that none of the treatment parameters used to simulate the spaceflight environment were found to induce uterine cancer.

Evaluation of the Effects of Heifer Development Method on Subsequent Reproductive Performance and Progeny Growth

The objective of the present study was to evaluate the effects of low input development protocols on Southeastern replacement beef females and their offspring. Fall and spring calving crossbred beef heifers managed through one of two heifer development programs (low input, LOW or conventionally developed, CON) were evaluated for metrics of uterine artery hemodynamics, patterns of heifer and offspring growth, postpartum reproductive performance, and economic efficiency. There were significant treatment × season (P = 0.0001) and season × day (P = 0.003) interactions on heifer body weight (BW) during gestation. There was a season × day interaction on total blood flow (TBF; P = 0.05) whereby TBF increased as gestation progressed and spring calving heifers displayed increased TBF. When adjusted for BW, an additional main effect of treatment was observed (P = 0.0007) in which LOW heifers had increased TBF compared to CON heifers. There were no significant main effects of treatment or season or interactions thereof on the proportion of females cycling at the start of the breeding season (P > 0.05). Additionally, there were main effects of treatment (P = 0.0095; P = 0.02) and season (P = 0.012; P = 0.007) on calf ADG and weaning weights, respectively whereby calves born to CON females grew faster than calves born to LOW heifers, and SPRING calves gained more rapidly than FALL calves. Partial budgeting demonstrated that LOW bred heifers were $250/hd less expensive to purchase than CON heifers due to lighter BW. Calves born to LOW females yielded $6.38 less per calf compared to CON calves. The discrepancy in reproductive efficiency coupled with the failure of LOW females to achieve BW similar to CON females resulted in a difference of $403/hd in average cull value between LOW and CON females in which CON females were worth more. Thus, CON females were $159.18 more profitable than LOW females when purchased as bred heifers and sold at the weaning of their first calf. In conclusion, though uterine artery BF was not negatively affected, low input heifer development resulted in sub-optimal heifer growth, poor first calf performance, and inadequate reproductive efficiency.
