Identifying Attitudes Toward and Acceptance of Osteopathic Graduates in Surgical Residency Programs in the Era of Single Accreditation: Results of the American College of Osteopathic Surgeons Medical Student Section Questionnaire of Program Directors

Purpose The purpose of this study was to quantify the number of surgical programs currently training osteopathic residents and to solicit advice for current osteopathic medical students who are interested in pursuing a surgical residency. Methods A questionnaire was sent to all listed Electronic Residency Application Service® (ERAS®) email contacts for the following specialties: General Surgery, Neurological Surgery, Orthopedic Surgery, Otolaryngology, Urology, Integrated Vascular Surgery, Integrated Plastic Surgery, and Integrated Thoracic Surgery. The questionnaire was sent a total of three times. Results Two hundred sixty-four of the 1,040 surgical residency programs responded to the questionnaire. Of these responses, 19% were formerly American Osteopathic Association (AOA) accredited programs. About 47.3% of responding programs indicated they are not currently training an osteopathic physician. One hundred thirteen programs provided additional comments on how osteopathic medical students may improve the competitiveness of their residency applications. These comments included increasing volumes of research activities, performing well on the United States Medical Licensing Exam (USMLE), and completing a sub-internship in the desired field or at a specific institution. Conclusion Osteopathic students still face many barriers to matching into surgical residencies. This study provides concrete steps students may take to increase the competitiveness of their application.

Intra-abdominal Hypertension and Colonic Hypoperfusion after Abdominal Aortic Aneurysm Repair

Colonic ischaemia (CI), Intra-abdominal hypertension (IAH) and abdominal compartment syndrome (ACS) are devastating complications after abdominal aortic aneurysm (AAA) surgery. The aims of this thesis were to study the incidence and clinical consequences of IAH/ACS and the association between CI and intra-abdominal pressure (IAP) among patients undergoing OR for ruptured AAA (rAAA), to compare extraluminal pHi monitoring, with standard intra-luminal monitoring among patients operated on for AAA, and to study the frequency and clinical consequences of IAH/ACS after endovascular repair (EVAR) for rAAA. The incidence of ACS was 26% in a retrospective study of 27 patients undergoing OR for rAAA. Consensus definitions on IAH/ACS were appropriate for patients after OR for rAAA: 78% (7/9) of patients with IAH grade III or IV developed organ failure and all patients who developed CI had some degree of IAH. Active fluid resuscitation treating hypovolaemia to avoid CI may partly cause IAH. The association between CI and IAP was investigated in a prospective study on 29 patients operated on for rAAA, 86% (25/29) were treated for hypovolaemia and ten (34%) had both IAH and CI. Since monitoring colonic perfusion is very important and there is no ideal method, a new technique, extraluminal colonic tonometry to detect colonic perfusion was compared with standard intraluminal tonometry. Although, this new method was not able to determine the severity of ischaemia it may serve as a screening test. EVAR of rAAA is feasible and patients may benefit from this less invasive procedure. Of 29 patients treated with this technique, 10% developed ACS, and all patients except one with preoperative shock developed some degree of IAH. In conclusion, IAP/ACS is common after both OR and EVAR for rAAA, and is associated with adverse outcome. Monitoring IAP and colonic perfusion with timely intervention may improve outcome.

Nous avons développé un modèle numérique du traitement des anévrismes de l'aorte abdominale (AAA) en utilisant l'analyse par éléments finis (FEA). L'objectif est de simuler les différentes étapes de la procédure endovasculaire dans la phase préopératoire. Pour cela, dans un premier temps, on a fait des études expérimentales Macro et Nano de rupture, indentation, traction, frottement des outils métalliques et des biomatériaux. Ensuite, on a développé une nouvelle approche de la conception des structures anatomiques très angulées et hétérogènes, et enfin, on a crée une maquette numérique de l’interaction outils métalliques et biomatériaux. Dans ce modèle, nous avons pris en compte la géométrie réelle reconstituée à partir des scans, une caractérisation locale des propriétés mécaniques guide / cathéter, une cartographie des propriétés des matériaux composites en fonction de la qualité pariétale, et une projection de l'environnement de l’artère dans l’outil de simulation. Nos résultats ont été validés grâce à un recalage entre les données cliniques et notre simulation pour un groupe donné de patients, dont les artères sont très tortueuses et calcifiées. Cet outil d’aide à l’acte chirurgical permet de contrôler avec précision la navigation endovasculaire en péropératoire, prédire la faisabilité de la chirurgie avec une fiabilité ainsi que d’optimiser le nombre d’outils métalliques pour chaque patient, en tenant compte du risque de rupture des zones avec fortes tortuosités et hétérogénéités. / We have developed a numerical model of the treatment of abdominal aortic aneurysms (AAA) using finite element analysis (FEA) to simulate the endovascular procedure in the preoperative phase. The goal is to simulate different stages of the endovascular procedure. In the model, we took into account the geometry of the biological region reconstructed from scans, a local characterization of the guidewire/catheter mechanical properties, a mapping of material properties depending on the degree of calcification, and a projection of the aorta environment . Our results were validated based on a tuning between clinical data and our simulation for a given group of patients with very tortuous and calcified arteries. It allows to control with accuracy the delivery system rise during surgery, predict the feasibility of the surgery with reliability as well as choose the best guide for each patient, taking into account the risk of rupture of calcified areas in the case of high angulations.
