Novel tools for interventional magnetic resonance imagingRube, Martin January 2014 (has links)
Magnetic Resonance Imaging (MRI) provides unique advantages such as superior soft tissue contrast, true multiplanar imaging, variable contrast mechanisms, measurement of temperature changes, perfusion and diffusion, and no ionizing radiation. Despite considerable research efforts in the field of interventional MRI, numerous challenges remain including restricted access to the patient, high acoustic noise and a shortage of MRI-safe devices. Novel methods and devices are presented in this thesis with the primary objective of enabling effective MRI-guided interventions, particularly abdominal needle and common catheter-based endovascular interventions. Firstly, a set of MRI-safe devices (guidewires, micro guidewires, catheters and micro catheters) were developed with passive or inductively coupling resonant markers for MRI visualisation. Secondly, a method was implemented for wireless tracking and dynamic guidance of instruments. Thirdly, a framework of technologies was developed for in-room display, wireless MRI remote control and multi-user communication along with a dedicated user interface and imaging protocol. These implementations were assessed in regards to MRI-safety, performance and usability and evaluated for MRI-guided liver biopsies, balloon angioplasty procedures and also for mechanical thrombolysis. Flow phantoms, Thiel soft-embalmed human cadavers with partially re-established perfusion and a porcine model were used for in vitro, ex vivo and in vivo validation, respectively. The results demonstrate that these interventions are experimentally feasible and practical when using the presented developments: automated device tracking and equipment designed for MRI-guided interventions streamlined procedural workflow. Specifically, it was shown that fast and accurate needle placements along complex trajectories were feasible using a wireless interactive display and control device with a dedicated user interface for interventions. Moreover, safe and efficacious balloon angioplasties of the iliac artery were practical using the described framework of technologies along with a dedicated MRI protocol. Finally, it was demonstrated that these developments could be adapted and applied to MRI-guided endovascular mechanical thrombolysis of the middle cerebral artery. The technologies, described in this thesis have been shown to overcome many of the present limitations and should therefore be useful for enabling MRI-guided interventions while not further constraining the operating physician in an already complex environment. Nevertheless, it is acknowledged that many crucial issues remain to be solved in the field of iMRI and in the context of the presented research. In particular further device optimisations, improvements of the tracking implementation along with further in vivo evaluations are required before moving towards clinical evaluation. This thesis sets the groundwork for moving ahead with the eventual clinical realisation of optimised MRI-guided interventions.

Os sistemas de informação baseados na tecnologia Web (SIW) possuem características que permitem supor que seu desenvolvimento apresenta diferenças com relação ao de sistemas não Web. O objetivo desta pesquisa foi levantar, através de um estudo exploratório de múltiplos casos, as questões relevantes ao desenvolvimento de sistemas de informação baseados na tecnologia Web que apóiam aplicações de negócios nas organizações. Para tanto, procuramos identificar as principais dificuldades e facilidades, as alterações sucedidas nas tarefas e na estrutura do desenvolvimento, assim como analisar a adoção de técnicas e metodologias. Esperamos ter contribuído para que as organizações possam aprimorar o desenvolvimento de SIW de forma a aproveitar ao máximo as oportunidades criadas pela tecnologia Web. / The Web technology was created to divulge the scientific knowledge, although it has been used as a way to access several types of business information systems as well as to facilitate the communication between them, generating many business opportunities for the organizations. Web-based information systems (WIS) have characteristics that allow us to assume their development is different from non-Web information systems. The objective of this research was to identify, through an exploratory multi-case study, the main questions about business Web-based information systems development. We have tried to identify the main difficulties and easiness, the changes occurred in development tasks and structure, and to analyze techniques and methodologies adoption. We hope we had contributed to allow organizations be able to improve the WIS development process in order to take the maximum advantage of the opportunities generated by Web technology.

Over a few years back, REST APIs were considered standard web APIs, which nowhave a strong competitor. REST APIs provide some excellent features like stateless serversand structured access to resources. However, over time, it doesn’t offer flexibility withthe access of data and client changing requirements. In 2015 GraphQL was introduced byFacebook, which overcomes the problems with the REST and provides more flexibility andefficiency to the client requirements. For example, remove the over and under fetching.To change the existing APIs into GraphQL APIs require considerable time and effort.Therefore, some server implementation tools are developed to reduce the developmentcost and time. A few of these tools generate GraphQL schema and server implementationsautomatically over a legacy database.This master thesis studies tools that automatically generate GraphQL server implementationover legacy databases and evaluate such generated GraphQL server’s performance.First, we find some GraphQL server implementation tools such as Hasura andPostGraphile and then compare the server’s performance using benchmark methodology.Secondly, we run an experiment on a computer system and use the performance metricsfor assessment. The results of our experiment concluded that PostGraphile has higherthroughput and low query execution time as compared to Hasura. In most of the querytemplates from the benchmark, PostGraphile outperforms Hasura.

A challenge for the clinical management of Parkinson's disease (PD) is the large within- and between-patient variability in symptom profiles as well as the emergence of motor complications which represent a significant source of disability in patients. This thesis deals with the development and evaluation of methods and systems for supporting the management of PD by using repeated measures, consisting of subjective assessments of symptoms and objective assessments of motor function through fine motor tests (spirography and tapping), collected by means of a telemetry touch screen device. One aim of the thesis was to develop methods for objective quantification and analysis of the severity of motor impairments being represented in spiral drawings and tapping results. This was accomplished by first quantifying the digitized movement data with time series analysis and then using them in data-driven modelling for automating the process of assessment of symptom severity. The objective measures were then analysed with respect to subjective assessments of motor conditions. Another aim was to develop a method for providing comparable information content as clinical rating scales by combining subjective and objective measures into composite scores, using time series analysis and data-driven methods. The scores represent six symptom dimensions and an overall test score for reflecting the global health condition of the patient. In addition, the thesis presents the development of a web-based system for providing a visual representation of symptoms over time allowing clinicians to remotely monitor the symptom profiles of their patients. The quality of the methods was assessed by reporting different metrics of validity, reliability and sensitivity to treatment interventions and natural PD progression over time. Results from two studies demonstrated that the methods developed for the fine motor tests had good metrics indicating that they are appropriate to quantitatively and objectively assess the severity of motor impairments of PD patients. The fine motor tests captured different symptoms; spiral drawing impairment and tapping accuracy related to dyskinesias (involuntary movements) whereas tapping speed related to bradykinesia (slowness of movements). A longitudinal data analysis indicated that the six symptom dimensions and the overall test score contained important elements of information of the clinical scales and can be used to measure effects of PD treatment interventions and disease progression. A usability evaluation of the web-based system showed that the information presented in the system was comparable to qualitative clinical observations and the system was recognized as a tool that will assist in the management of patients.
