Bridging the boundaries? : collaboration and community care, Sunderland 1990-1994

The independence of the health and social care agencies makes the coordinated delivery of inter-related and inter-dependent services very difficult. Collaboration in health and social care has been a goal of policy makers for many decades, but it has not been achieved to the degree or to the extent of the aspiration. This thesis examines collaboration in the context of the NHS and Community Care Act 1990, which marked a new stage in the development of community care policy and in collaborative working between health and social services. The thesis takes the form of a case study set in Sunderland during 1990-1994, from the passing of the Act to the first anniversary of the implementation of its community care elements. It considers firstly, collaboration at a strategic planning level between Sunderland Health Authority and the Local Authority Social Services Department in the development and implementation of community care policy; secondly, the evaluation of a collaborative project at an operational level, in the attachment of a social worker to a general medical practice; and thirdly, the evaluation of a project which tried to strengthen collaborative working within the health service, among district nurses, health visitors and general practitioners. The thesis sets these three pieces of work in a number of contexts: the political setting of the NHS and Community Care Act and the changes it introduced; the literature of collaboration; and a description of Sunderland and its need for health and social care. The case study showed how difficult it is for organisations to work together. Relationships between individuals tended to be more collaborative than relationships between corporate bodies, but it is important to see the relationship between those individuals in the context of relationships between organisations. The study also found that for the success of joint projects to be sustainable and generalisable, collaboration needs to be present at all levels of the organisations. The thesis also showed that there is as much need for collaboration within the health service as between the health and social services. The thesis used as a measure a framework of factors which promote collaboration, and found that many elements were lacking in Sunderland. However, in the real world it is necessary to settle for a notion of "pragmatic collaboration" in which joint working is possible even when full collaboration is absent.

Trafficking in facts : talk, text and identity in professional practice

In this thesis by published work nine works are presented, prefaced by a Critical Summary and Review which discusses the genesis of the work and its theoretical presuppositions, and evaluates their contribution to knowledge. The work includes both sole-authored and collaborative writing. This published work adopts a social constructionist approach to knowledge in health and welfare. The first work explores critical approaches to child development and their relevance to professional practice. Subsequent work adopts a post-Wittgensteinian approach to language as practical activity, exploring how practitioners such as social workers and nurses do 'case work1 , making knowledge about people, events and situations in their talk and writing and, in doing so, enact the institutional order. An exploration of the ways in which practitioners construct their practice in reflective writing is a significant focus within several pieces of work. Attention is paid to what social actors (patients/service users and professionals) do in their interactions and communicative practices. Thus, talk and text are not treated as simple vehicles for conveying literal, factual descriptions but as the means by which moral adequacy is portrayed and authentic versions of events are established. These analyses draw inspiration from a variety of sources including micro sociology, discursive psychology and narrative analysis, emphasizing the practical-moral aspects of health and welfare practice in which the production of identity, for example as a caring practitioner, plays a key part. The published work has a strong practice orientation and the implications for professional education are highlighted throughout. 'Reflexive awareness' is promoted as a means by which health and welfare VIprofessionals may challenges tendencies to take practice for granted. By engaging in the processes of making the familiar strange, it is argued that better understandings of practice can be achieved and a stance of 'respectful uncertainty' deployed.

Preventing burnout among clinicians

The purpose of this study is to identify factors that protect social workers and other clinicians from experiencing burnout. Many things contribute to job burnout among clinicians. Some examples of contributing factors include a shortage of government funding, heavy caseloads, and a lack of autonomy. However, this study will look at workplace factors that are more amendable to change in the agency's environment.

Method: This is a cross-sectional and descriptive study, integrant of a multicenter research ?Working conditions, employment conditions and health workers health in Bahia?, realized in Medium complexity services and in the cities of Feira de Santana, Santo Antonio de Jesus and Itabuna, in 2011 to 2012.. The study population was composed by 1,290 health workers. The instrument used was elaborated through literature revision and studies focused on health conditions and health workers job and tested by a pilot study. The dependent variable of this study is the type of employment, obtained by the question: ?Is your employment, nowadays:? categorized in formal and informal. The data was analyzed by means of descriptive statistics, applying the absolute and relative frequencies of the descriptors variables according to the types of bonds. The analysis was realized on the program Statistics Package for Social Sciences (SPSS) for Windows version 20.0. Results: The analysis showed that the socio-demographic variables, life habits, occupational, work conditions and domestic work when calculated by employment, informal workers are in more precarious situations presenting vulnerability situations, what is presented by the absence of the labor rights and precarious job conditions Final considerations: The job precariousness situation contributes to the worker falling ill, although they tell that have quality of live. This transitory relation seems to attack not only the workers? health, but the work process itself by the discontinuing of care generated by the high staff turnover, or interdependency of the precarious bonds. / Objetivo: Descrever o v?nculo de trabalho entre trabalhadores da sa?de da m?dia complexidade. M?todo: Trata-se de um estudo de corte transversal, descritivo, integrante da pesquisa multic?ntrica ?Condi??es de trabalho, condi??es de emprego e sa?de dos trabalhadores da sa?de na Bahia?, realizados nos servi?os de M?dia Complexidade e nos munic?pios de Feira de Santana, Santo Ant?nio de Jesus e Itabuna, nos anos 2011 e 2012.
