The growing American health penalty: International trends in the employment of older workers with poor health

Many countries have reduced the generosity of disability benefits while making them more activating - yet few studies have examined how employment rates have subsequently changed. We present estimates of how the employment rates of older workers with poor health in 13 high-income countries changed between 2004-7 and 2012-15 using HRS/SHARE/ELSA data. We find that those in poor health in the USA have experienced a unique deterioration: they have not only seen a widening gap to the employment rates of those with good health, but their employment rates fell per se. We find only for Sweden (and possibly England) signs that the health employment gap shrank. We then examine possible explanations for the development in the USA: we find no evidence it links to labour market trends, but possible links to the USA's lack of disability benefit reform - which should be considered alongside the wider challenges of our findings for policymakers. / Series: Department of Economics Working Paper Series

The development of a tool to support the work of the Rehabilitation care worker in documenting information about rehabilitation and health needs among persons with disability in home and community setting

The purpose of the study is to contribute to the development of a tool that can support the rehabilitation care worker towards intervention planning and the monitoring of their clients. The National Health Insurance and the Framework and Strategy for Disability and Rehabilitation of the South African Department of Health are strategies to work towards accessible, affordable, equitable and quality health care, which includes health promotion, disease prevention, curative, rehabilitation and palliative services for all South Africans. Both strategies emphasise the use of community health workers and mid-level workers as a key component of primary health care. In the Western Cape provincial Department of Health a new cadre, namely the rehabilitation care worker has been introduced as a member of the rehabilitation team. The introduction of the rehabilitation care worker is still in the pilot phase. The rehabilitation care workers face many barriers to providing effective care. One such challenge is the lack of a contextually relevant resource tool to collect information on the rehabilitation and health needs of persons with disabilities. The aim of the study was to develop a contextually relevant resource tool that would support the rehabilitation care worker in understanding and documenting how the rehabilitation and related health needs of persons with disabilities are met in home- and community-based settings. Three specific objectives were defined: i) to develop the content and domains of the rehabilitation and health information tool; ii) to establish the validity (face and content) of the rehabilitation and health information tool; and, iii) to test the application of the rehabilitation and health information tool on a sample of persons with disabilities. This study was an exploratory descriptive study adopting a sequential mixed methods design. There were two phases in this study. Phase 1 involved qualitative research methods in the development of the rehabilitation and health information tool through the use of document review and a focus group discussion with experts. Phase 2 of the study involved quantitative research methods in the field testing of the rehabilitation and health information tool by the rehabilitation care workers on a sample of persons with disabilities. The results of phase 1 included the development of the rehabilitation and health information tool, which was deemed by the experts to be a comprehensive, contextually relevant tool with face and content validity and could be easily administered by the rehabilitation care worker. The conceptual framework of the International Classification of Functioning, Disability and Health provided domains that could comprehensively document the multidimensional needs of persons with disabilities. The result was a draft rehabilitation and health information tool with 17 questions ranging across the domains of activities of daily living, sexual health, health behaviours, barriers and facilitators to good health, finance and understanding of disability. Changes were proposed to the wording, layout and flow of the tool and the persons with disabilities’ goals were included as an element . The inclusion of the end users as experts in the development resulted in a richer understanding needed for the shaping of this tool. The results of phase 2 highlighted that the rehabilitation and health information tool was able to describe the rehabilitation and health needs of persons with disabilities. Additionally the tool was able to document the specific goals of the persons with disabilities which is useful to plan and monitor intervention. The rehabilitation care workers reported the tool to be useful, easy to use, and provided a structured manner to collect information. They also reported that it was useful in stimulating conversations on sensitive topics. However, it was indicated that it took too long to complete and there were components that were incomplete. The rehabilitation and health information tool requires further refinement, validation and further follow-up testing before it can be formally adopted and implemented as part of the rehabilitation care worker’s standard practice.

The health and wellbeing of female street sex workers

Previous research on female street sex workers (FSSWs) has primarily concentrated on the stigmatisation of women's involvement in the sex industry particularly with reference to the spread of HIV/AIDS. The response of the criminal justice system to the regulation of the illegal aspects of women's engagement in street sex work has also been criticised. However, the impact of street sex work on the health and wellbeing of these women requires further research. The aim of this study was to explore the perceptions and needs of female street sex workers in relation to their own health and wellbeing. The study used a qualitative mixed methods approach that included analysis of three sets of data: visual data, secondary data and primary data. There were 10 FSSWs recruited for the primary data sample. The epistemological position underpinning this study is social constructivism and a feminist paradigm has informed the conduct of the research process and data analysis. The theoretical application of Bourdieu's framework of habitus, capital and field has provided the lens through which to explore the socially constructed experiences of FSSWs health and wellbeing. Findings from this study revealed that FSSWs experienced poor physical, mental and social health and wellbeing. They faced limited life choices and often felt discriminated against by the agencies and institutions that should have offered support. The women spoke of their personal histories especially traumatic life events in childhood consisting of sexual abuse, neglect, loss, rejection as well as intimate partner violence in adult life. The loss of their children to social services, housing difficulties and addiction to alcohol and crack cocaine were also significant in contributing to social exclusion and their multiple positions of vulnerability. This study contributes to the body of work on women's health and wellbeing. In particular, it adds to our understanding of the lived experiences of women involved in street sex work. A key public health priority should be the development of policies and systems to provide quality services to support the health, safety and wellbeing of FSSWs.

Despite all the technological development achieved nowadays, such events continue to be present in the quotidian of workers, generating many repercussions in their lives. The present dissertation, belonging to the field of Work Social Psychology, had the objective of analyzing reports of workers who suffered accidents typical of the industrial environment to understand their career paths and how they experience the repercussions of the accident on their lives. To do so, we conducted open in-depth interviews with workers from different industrial segments and localities, where they were presented to the researcher through their contact?s network. The analysis of the information obtained with the interviews was organized according to the themes that most stood out in their reports regarding their experiences after the occurrence of the work accident. The themes refer to access and non-fulfillment of constitutionally guaranteed rights, changes in the performance of daily work activities and the loss of recognition among co-workers, situations of violence and moral harassment, to the fallacy of business speech about worker?s appreciation, expressions of mental illness related to work and the psychic consequences coming from the accident. In the end, it was considered the relevance in discussing the repercussions of work accidents and the continuity of research on this subject, in order to subsidize actions that aim to change situations that cause suffering and illness due to work accidents. / Os acidentes de trabalho fazem parte de relatos sobre as atividades laborais ao longo da hist?ria da humanidade. Apesar de todo o desenvolvimento tecnol?gico alcan?ado nos dias atuais, tais eventos continuam presentes no cotidiano dos trabalhadores, gerando diversas repercuss?es em suas vidas. A presente disserta??o, situada no campo da Psicologia Social do Trabalho, teve como objetivo analisar relatos de trabalhadores que sofreram acidentes de trabalho t?picos em ind?strias, a fim de compreender seus percursos laborais e como vivenciam as repercuss?es do acidente em suas vidas. Para tanto, foram realizadas entrevistas abertas em profundidade com os trabalhadores de diferentes segmentos industriais e localidades, os quais foram apresentados ? pesquisadora por meio de sua rede de contatos. A an?lise das hist?rias acessadas por meio das entrevistas foi organizada de acordo com os temas que mais se destacaram nas falas dos trabalhadores a respeito das viv?ncias ap?s a ocorr?ncia do acidente de trabalho. Agricultural migrant workers navigating the health system: Access, continuity of care and the role of community health workers in De Doorns, Western Cape

In addition, Statistics South Africa (2014) notes very significant internal migration. This mobile population is affected by chronic communicable and non-communicable diseases such as TB, HIV, and diabetes, although it has a Constitutional right to health and healthcare. Their quality of healthcare and disease control also affects the general population and the burden on the health system can be increased by inadequately managed chronic conditions as well as acute health care needs. Access to healthcare and continuity of care reflect both patient agency and the health system. Community Health Workers (CHWs) play an important role in linking communities and patients to health services and vice versa. The aim of this study was to understand how agricultural migrants in the Cape Winelands District of Western Cape Province of South Africa navigated the healthcare system to access healthcare services including securing continuity of care, and in particular the role of CHWs in this process, in order to inform policy and practice.
