Avaliação da função autonômica cardíaca e sua relação com a capacidade funcional em pacientes com DPOC

However, 1) if the degree of lung impairment negatively impacts HRV responses and 2) whether the injury of the autonomic control may be associated with reduced exercise capacity in patients with COPD remain unclear. Thus, two studies were conducted in order to verify if functional status at rest and during exercise would be related to autonomic impairment in COPD patients. In the first study, entitled "Relationship between linear and nonlinear dynamics of heart rate and impairment of lung function in COPD patients," we investigated whether the impairment static lung volumes and lung diffusion capacity (DL) would be related to HRV indices in moderate-to-severe COPD. Sixteen patients with COPD underwent pulmonary function tests (spirometry, plethysmography and lung diffusion capacity for carbon monoxide - DLCO). The RR interval was registered in the supine, standing and seated positions and during a respiratory sinus arrhythmia maneuver (M-RSA). Our results suggest that responses of HRV indices were more prominent during M-RSA in moderate-to-severe COPD. Moreover, greater lung function impairment was related to poorer heart rate dynamics. Finally, impaired DLCO is related to an altered parasympathetic response in these patients. The second study, entitled "Are linear and nonlinear heart rate dynamics in submaximal exercise related to cardiorespiratory responses during maximal exercise in patients with COPD?", we inquired whether there is a relationship between HRV responses and exercise capacity in patients with COPD. Fifteen patients underwent incremental cardiopulmonary exercise testing and six-minute walk test (6MWT). The RR interval was registered at rest (standing position) and during 6MWT. Our results showed that HRV responses at rest and during simple field tests may reflect functional impairment of COPD patients, providing important information about both ventilatory and hemodynamic inefficiency in these patients. / Em pacientes com doença pulmonar obstrutiva crônica (DPOC), as alterações funcionais e estruturais do pulmão podem impactar negativamente na variabilidade da frequência cardíaca (VFC). Além disso, a reduzida capacidade de exercício se traduz como um marcador prognóstico nesses pacientes. No entanto, ainda não estão esclarecidos 1) se o grau de comprometimento pulmonar tem impacto negativo sobre as respostas da VFC, frente a diferentes estímulos autonômicos, e 2) se o prejuízo no controle autonômico pode estar relacionado à reduzida capacidade de exercício nos pacientes com DPOC. Sendo assim, dois estudos foram realizados com o intuito de relacionar as alterações funcionais, no repouso e no exercício, com o grau de prejuízo autonômico em pacientes com DPOC. O primeiro estudo, cujo título é Correlação entre as dinâmicas linear e não linear da frequência cardíaca e o comprometimento da função pulmonar em pacientes com DPOC , teve como objetivo investigar se o comprometimento nos volumes pulmonares estáticos e na difusão pulmonar estaria relacionado aos índices da VFC em repouso e em resposta às mudanças posturais. Dezesseis pacientes com diagnóstico de DPOC foram submetidos à prova de função pulmonar (espirometria, pletismografia e capacidade de difusão pulmonar ao monóxido de carbono DCO) e à coleta da VFC nas posturas supino, ortostatismo e sentado e durante a manobra de arritmia sinusal respiratória (M-ASR). Nossos resultados sugerem que as respostas da VFC frente a um estímulo vagal (M-ASR) são mais evidentes. Ainda, quanto maior o comprometimento da função pulmonar pior a dinâmica da frequência cardíaca. Por fim, a redução da DCO está relacionada à alterada resposta vagal nos pacientes com DPOC. O segundo estudo, intitulado Os índices da dinâmica linear e não linear na frequência cardíaca no exercício submáximo estão relacionados com as respostas cardiorrespiratórias ao exercício máximo em pacientes com DPOC? teve por objetivo avaliar se existe relação entre as respostas da VFC e a capacidade de exercício em pacientes com DPOC. Quinze pacientes foram submetidos aos testes de exercício cardiopulmonar incremental e de caminhada de seis minutos (TC6). A coleta da VFC feita em repouso (ortostatismo) e durante o TC6. Os resultados mostraram que as respostas da VFC no repouso e em testes simples de campo podem inferir o prejuízo funcional de pacientes com DPOC, fornecendo informações importantes acerca das limitações ventilatória e hemodinâmica destes pacientes.

La décompensation cardiaque se manifeste par une symptomatologie de dyspnée et de fatigue, et par une diminution de l’aptitude aérobie. La décompensation cardiaque peut être globale ou gauche (DCG), ou droite comme dans le cas de l’hypertension artérielle pulmonaire (HTAP). Ein präventivmedizinisches Konzept zur Untersuchung der kardiovaskulären Gesundheit

Kardiovaskuläre Erkrankungen spielen eine immer dominanter werdende Rolle als Ursachen von Morbidität und Mortalität. Endotheliale Dysfunktion, Arteriosklerose und ischämische Herzerkrankungen sind unangefochten die häufigsten Todesursachen in hochentwickelten Ländern. Die Krankheitslast kardiovaskulärer Erkrankungen wird durch die globale Zunahme von Risikofaktoren wie Übergewicht, metabolischem Syndrom und Diabetes mellitus Typ 2 in Zukunft wohl weiter steigen. Keines der in der heutigen Form existierenden Gesundheitssysteme ist den gesundheitlichen Folgen dieser bedenklichen epidemiologischen Entwicklung gewachsen, ohne dafür beträchtliche Einschnitte in Qualität oder Quantität hinnehmen zu müssen. Präventivmedizinische Strategien haben im Vergleich zur Akutmedizin bisher eine wesentlich geringere Aufmerksamkeit erfahren. Aufgrund der weiterhin steigenden Krankheitslast werden gerade präventivmedizinische Strategien immer wichtiger. Die Ermittlung der individuellen kardiovaskulären Gesundheit beinhalten neben einer Untersuchung wichtiger physiologsicher Kernsysteme, einen besonderen Fokus auf die Stärkung der gesundheitlichen Eigenverantwortung durch Vermittlung entsprechender medizinischer Zusammenhänge. Dies kann durch geeignete und prognostisch wertvolle Untersuchungsmethoden und laienverständliche Interpretationen der komplexen kardiovaskulären Zusammenhänge realisiert werden. Mithilfe eines besseren Verständnisses für die eigene kardiovaskuläre Gesundheit und gezielter konkreter Hilfestellungen von ärztlicher Seite, können grundlegende Strukturen für einen achtsameren Umgang mit der eigenen Gesundheit geschaffen werden. Alltägliche körperliche Aktivität, Körperzusammensetzung, Gefäßsteifigkeit und die maximale Leistungsfähigkeit bilden aktuellen Studien zufolge vier relativ einfach zu erhebend Hauptdeterminanten kardiovaskulärer Gesundheit mit großer prädiktiver Vorhersagekraft für kardiovaskulärer Ereignisse. Die Einordnung der Untersuchungsbefunde unter Berücksichtigung altersentsprechender Referenzdaten bietet zusätzlich die Möglichkeit, chronologisches und biologisches Alter zu differenzieren. Darüber hinaus könnte das hier vorgestellte Untersuchungskonzept ebenso dafür eingesetzt werden, die Effektivität von zuvor eingeleiteten therapeutischen Strategien zu überprüfen und eine Übermedikation zu vermeiden. Ein stärkerer Fokus auf nachhaltige kardiovaskuläre präventivmedizinische Strategien birgt das Potential Morbidität und Mortalität zu Reduzieren und die Lebensqualität und Selbstbestimmung der Klienten zu steigern. / Nowadays endothelial dysfunction, arteriosclerosis and ischaemic heart disease depict major issues in most developed countries. Chronic diseases cause an increasing number of deaths worldwide. Responsible for approximately 30% of all deaths, this number is shocking despite the knowledge that these lives could be saved in a realistic point of view. However, disturbing global tendencies and the increase in epidemic overweight, obesity, glucose intolerance and diabetes mellitus type 2 seem to amplify cardiovascular diseases and deaths. No existing medical care system could ever sufficiently match these emergent burdens of global chronic diseases if these tendencies will continue to grow. Due to fatal epidemiological changes general practitioners will not only have to treat more diseases of elderly people, but also be the first contact person for enquiries about their health concerning the cardiovascular systems of younger and older generations. Strengthening prevention concerning cardiovascular diseases is probably the most efficient and only way to escape the vicious circle of pathophysiological processes. To implement cardiovascular prevention it is essential to focus on cardiovascular health rather than on cardiovascular disease. In order to master this challenge sufficiently a deliberate concept of preventive examinations for cardiovascular health is required to quantify cardiovascular wellness and sharpen the awareness of one’s health. Therefore, it is to establish precise, predictive and cost-efficient examination methods to quantify cardiovascular health and subclinical changes at early stages of a disease. Broader acceptance of the preventive potential and the use of modern technological advances could therefore be a powerful instrument to solve the growing global problem of chronic diseases. In this context the routine evaluation of daily physical activity, body composition, arterial stiffness and the maximum physical performance could be a starting point for a preventive diagnostic approach. They can serve as future parameters reflecting health, detecting subclinical diseases and encouraging people’s responsibility for their own health. Detailed information about the major cardiovascular parameters opens specific and efficient course of action to create multiple strategies for gaining or rehabilitating cardiovascular wellbeing

L’impact de la prématurité sur l’oxygénation musculaire à long terme : une étude pilote

La naissance prématurée (<37 semaines de gestation) touche près de 10% de la population canadienne chaque année et induirait des altérations permanentes dans la structure et la fonction des organes. Nous avons démontré que de jeunes adultes nés ≤29 semaines de gestation présentent une capacité aérobie à l'effort réduite, un facteur prédictif de la qualité de vie et de la mortalité. Cette capacité dépend de l'intégrité des systèmes impliqués dans le transport et la consommation de l'oxygène, comme le système musculosquelettique. Cependant, l’oxygénation musculaire a été très peu étudiée chez les individus nés prématurément. Ainsi, nous postulons que les jeunes adultes nés très prématurément (PT) démontrent une oxygénation musculaire squelettique réduite et altérée par rapport aux jeunes adultes nés à terme (T). Cette étude pilote porte sur 16 jeunes adultes PT et 35 témoins T ayant entre 18 et 29 ans. L'oxygénation musculaire a été mesurée en continu par spectroscopie proche infrarouge (Artinis, Oxymon III, Leiden, Pays- Bas) sur le vastus lateralis et le gastrocnémien lors d’un test d’épreuve à l’effort. La comparaison entre les groupes a été fait par test-U Mann-Whitney pour données non paramétriques. Au niveau des mesures d’oxygénation musculaire, nous n’avons pas détecté de différences dans la variation relative d’hémoglobine oxygénée, désoxygénée et totale dans les deux muscles. Bien que la naissance prématurée pourrait affecter l’intégrité du muscle squelettique, nous n’avons pas décelé de différence dans l’oxygénation à l’effort chez les PT comparés aux T. / Premature birth (<37 weeks of gestation) affects about 10% of the Canadian population each year and induces permanent alterations in the structure and function of organ systems. We have shown that young adults born ≤29 weeks of gestation have a reduced aerobic exercise capacity, a strong predictor of quality of life and mortality. Aerobic capacity depends on the integrity of the systems involved in the transport and consumption of oxygen, such as skeletal muscles. However, muscle oxygenation has not yet been studied in individuals born prematrurely. Thus, we postulate that young adults born very prematurely (PT) have reduced and impaired skeletal muscle oxygenation compared to young adults born at term (T). This pilot study examined 16 young adults PT and 35 controls T between 18 and 29 years of age. Muscle oxygenation was measured continuously by near-infrared spectroscopy (Artinis, Oxymon III, Leiden, Pays-Bas) on the vastus lateralis and gastrocnemius during a cardiopulmonary exercise test. Comparison between both groups was accomplished by the Mann-Whitney U-test for non-parametric data. In terms of muscle oxygenation, we did not detect any differences in the relative variation of oxygenated, deoxygenated and total hemoglobin change in both muscles. Although premature birth would appear to affect the integrity of skeletal muscle, we did not find a difference in its oxygenation during exercise in PT compared with T young adults.
