Incidência de fraturas em próteses fixas: estudo retrospectivo. Análise da sobrevivência de próteses metalocerâmicas após um período mínimo de quatro anos em função / Fractures Incidence on fixed prostheses: a retrospective study

As próteses metalocerâmicas ainda são apontadas como o tratamento de escolha em reabilitação oral por apresentarem altas taxas de sobrevivência, mesmo que o fator estético possa ser comprometido devido às propriedades óticas relacionadas à infraestrutura metálica. Além disso, há pouca informação na literatura científica sobre os fatores que levam às fraturas das próteses fixas metalocerâmicas. Com isso, o presente estudo teve como objetivo avaliar se a quantidade de elementos que compõe uma prótese, se a quantidade de elementos por retentor, se o tipo de prótese, se o tipo de pilar, se a localização da prótese, se o bruxismo e se a utilização de placa miorrelaxante influenciam na incidência de fraturas em próteses fixas metalocerâmicas. Para este estudo, foram selecionados 16 pacientes, totalizando 74 próteses metalocerâmicas, instaladas entre 2000 e 2010, que tiveram um acompanhamento mínimo de 4 anos. Foram coletados dados, como: gênero, idade, data de instalação das próteses e sistema cerâmico utilizado. Foi realizada a avaliação clínica considerando integridade da prótese, características do elemento antagonista e coleta da história odontológica em caso de fratura, além de um questionário para identificar provável ou possível bruxismo. Foram determinadas as taxas de sucesso, insucesso e sobrevivência da mesma. Os resultados mostraram que a taxa de sucesso das próteses metalocerâmicas instaladas foi de 87,8% e a taxa de sobrevivência foi de 89,1%. Além disso, a taxa de sucesso não foi influenciada pela idade (p=0,903), tempo de instalação (p=0,830), número de próteses na boca (p=0,872), número de elementos (p=0,937) e número de pilares (p=0,064). Para as variáveis qualitativas também não houve diferença estatística significante (p>0,05) entre as proporções das taxas de sucesso e insucesso. Contudo, os resultados mostraram que pacientes que não usavam placa miorrelaxante tiveram taxa maior de sucesso que os pacientes usuários de placa (p=0,004). Assim, pode-se concluir que as próteses metalocerâmicas apresentam altas taxas de sucesso e sobrevida garantindo a longevidade desse tipo de reabilitação. / Metal-ceramic prosthesis are still the treatment choice in oral rehabilitation because they present high survival rates, even if the aesthetic can be compromised due to optical properties related to metallic infrastructure. In addition, there is little information on scientific literature about factors that lead to fractures of fixed metalceramic prosthesis. The purpose of this study was to evaluate if prosthesis elements number, number of elements per prosthetic abutment, type of prosthesis, type of prosthetic abutment, the prosthesis location, bruxism and myorelaxant plaque influences on incidence of fractures in fixed metal-ceramic prosthesis. For this study, 16 patients were selected totaling 74 metal-ceramic dentures installed between 2000 and 2010 with a follow-up of at least 4 years. Data were collected, such as: gender, age, prosthesis installation date and the ceramic system used. Besides a questionnaire to identify probable or possible bruxism, a clinical evaluation was performed to evaluate prosthesis integrity, antagonist element characteristics and dental history in case of fracture. Success, failure and survival rates were determined. The results showed that the success rate of metal-ceramic prosthesis was 87.8% and the survival rate was 89.1%. In addition, the success rate was independent of age (p=0.903), installation time (p=0.830), number of prosthesis (p=0.872), elements (p=0.937) and abutments (p=0.064). The qualitative variables also did not show significant statistical results (p> 0.05) between success and failure rates. However, the results showed that patients who did not use myorelaxant plaque had a higher success rate than plaque users (p=0.004). Thus, it can be concluded that the metal-ceramic prosthesis has high success and survival rates, ensuring longevity of this type of rehabilitation.

Deformações geradas no intermediário e região peri-implantar após aplicação de cargas inclinadas em coroas unitárias

A reabilitação protética de pacientes desdentados é uma área da Odontologia amplamente estudada por pesquisadores do mundo todo, e com o desenvolvimento dos implantes osseointegrados surgiram muitas perguntas sobre o desempenho das próteses instaladas sobre estes. Conhecer o comportamento mecânico do conjunto coroa unitária, intermediário, implante e tecido de suporte é importante para entender o processo de transmissão de forças e suas consequências, processos estes que ainda não estão totalmente compreendidos. O objetivo deste trabalho foi verificar a deformação gerada no intermediário e no osso peri-implantar após aplicação de uma carga inclinada sobre coroas unitárias metalocerâmicas. Foi utilizado um modelo mestre de poliuretano, simulando o osso mandibular, com um implante hexágono externo de 3,75mm x13 mm de comprimento, no qual foi fixado um intermediário multi-unit. O trabalho foi realizado com 3 grupos de 4 coroas cada, divididos de acordo com o tipo de cilindro e liga de fundição (cilindros de ouro sobrefundidos com liga de PdAg, cilindro de acrílico fundido com liga de NiCoCr e cilindros de CoCr usinado sobrefundidos com liga de NiCoCr). Cada corpo de prova foi submetido cinco vezes à aplicação de carga oblíqua de 15 e 30 graus no centro da oclusal de cada coroa com uma força de 300N em uma máquina universal de ensaios. Foram realizadas leituras das deformações geradas no intermediário e no poliuretano, com o uso de extensômetros lineares elétricos (strain gauges) os quais foram colados em três faces do intermediário (Disto lingual, Mesio lingual, Vestibular) e nas superfícies mesial, vestibular, distal e lingual do osso simulado ao redor do implante. Os dados obtidos foram submetidos à análise de variância a 3 critérios (ANOVA) e o teste de Tukey, utilizando um nível de significância (p<0,05). Os resultados mostraram que entre as duas angulações utilizadas existiram diferenças estatísticas significantes. Os valores médios de deformação se mantiveram dentro dos padrões de normalidade, na janela fisiológica descrita por Frost entre 50 &#x3BC;&#x3B5; e 1500 &#x3BC;&#x3B5; independente do tipo de cilindro utilizado, com exceção dos valores de compressão obtidos na carga oblíqua de 30 graus onde nos três tipos de cilindro ultrapassaram a janela de sobrecarga patológica com mais de 4000 &#x3BC;&#x3B5. / The prosthetic rehabilitation of edentulous patients is a dental area widely studied by worldwide researchers, and through the development of dental implants, appear many questions about the performance of prostheses installed on them. Knowledge of the mechanical behavior of the integration between single crown, abutment, implant and support tissue is very important to understand the forces transmission process and its consequences but these processes are not fully understood. The objective of the present study was to evaluate the deformation generated in the abutment and simulated peri-implant bone after application of an oblique load on metal-ceramic crowns. A polyurethane master model was used to simulate the jawbone with an implant external hexagon 3.75 mm x13 mm length, which was fixed in a multi-unit abutment. The study was conducted with twelve metal ceramic crowns divided according to type of cylinder and casting alloy, Group 1: Gold cylinder cast with PdAg alloy; Group 2: Acrylic cylinder cast with NiCoCr alloy; Group 3: CoCr cylinder cast with NiCoCr alloy, thus forming three groups of four sample each. Each specimen was subjected five times the application of oblique load of 15 and 30 degrees in the center of the occlusal of each crown with a 300N force in a universal testing machine. Readings were taken of the deformations generated in the abutment and bone simulated by polyurethane, with the use of electric linear strain gauges which have been positioned on three sides equidistant to each other to measure microstrains at abutment (disto-lingual, meso-lingual, Vestibular) and the simulated bone around each implant received four strain gauges, positioned on the mesial, distal, buccal and lingual aspects. The data were submitted to analysis of variance to 3 criteria (ANOVA) and Tukey test, using a significance level (p <0.05). The results showed that between the two angles tested there were significant statistical differences. The deformation average values remained within normal limits, at the adapted state described by Frost between 50 &#x3BC;&#x3B5 and 1500 &#x3BC;&#x3B5 independent of the type of cylinder used, with the exception of compression values in oblique load of 30 degrees where the three types of cylinder surpassed the pathological overhead state with over 4,000 &#x3BC;&#x3B5.

Vanished comforts : locating roles of domestic furnishings in Scotland, 1500-1650

Household inventories record objects that can be compared with surviving artefacts contributing to the study of material culture and social history. However, this thesis shows how heterogeneous inventories found in early modern Scottish sources resist quantification and aggregation. Instead, qualitative use of inventory evidence is advocated. Inventories can contribute information on the locations of activities in the home. These activities may be preferred to the object as evidence of historical change and as units of international comparison. Furnishing a house was cultural activity, and a construction of culture. In this study, objects are regarded as participants in cultural activities, strategies, and the construction of values. Sixteenth-century inventories are often impersonal and tend to show similarities in content, encouraging mechanistic interpretations of domestic life. The seventeenth century saw a proliferation of household equipment and furnishing for elites throughout Europe due to changes in production and consumerism. Some of this new furnishing was bought in London, some in France. While national difference was apparently maintained in architecture, new furnishings may have effaced distinctions within elite rooms. Scottish and English culture was merged by aristocratic intermarriage. This new culture is seen in the inventories of Mary, dowager Countess of Home. She maintained houses in England and Scotland. Some of her furnishings represented the style of an inner circle at court. Her inventories are also significant because they detailed equipment for a range of activities. She personally prepared medicines and sweetmeats, and had a number of scientific instruments. Pursuits reconstructed from the detail of later inventories can illuminate other domestic situations where clues are more subtle or absent. The level of autonomy Lady Home and her daughters exercised over their homes is a reminder of the agency exercised by women over furnishings, gardens, architecture, and estate policy.
