Submaximal Exercise Capacity is Associated with Moderate-to-Vigorous Physical Activity in Children with Complex Congenital Heart Disease

Background: Children with complex congenital heart disease (CHD) are exposed to cyanosis from birth until their surgical repair and are often not expected to participate in physical activities to the same extent as healthy peers because of a limited maximal exercise capacity (V̇O2max). Despite limitations in V̇O2max, these children may still have the capacity to perform most daily physical activity because it requires only a submaximal effort. The purpose of this research was to examine the relationships between submaximal exercise capacity, daily physical activity and cyanosis exposure, in children with complex CHD. Methods: Children with a single functioning ventricle (Fontan), tetralogy of Fallot or transposition of the great arteries, 10 to 17 years old were deemed eligible. The Bruce treadmill protocol with breath-by-breath analysis of oxygen consumption was used to assess submaximal exercise capacity. Five measures of submaximal exercise capacity were evaluated: energy consumption (V̇O2) at the ventilatory threshold, V̇O2 at a heart rate of 130 beats per minute (bpm), metabolic equivalents (METs) at ventilatory threshold, METs at 130 bpm and heart rate at stage 1 of the Bruce protocol. Moderate-to vigorous physical activity (MVPA) was measured (Actical accelerometer with 15 second epochs) for 7 consecutive days. Exposure to cyanosis was calculated by subtracting the child’s date of birth from the date of surgical repair. Results: Participants were children with a Fontan single ventricle (n=5), tetralogy of Fallot (n=4) or transposition of the great arteries (n=7). Daily physical activity was positively associated with V̇O2 at ventilatory threshold (r = 0.78, n = 16, p = < 0.01) and V̇O2 at a heart rate of 130 bpm (r = 0.61, n = 16, p = 0.01). Children who did more than 60 minutes of physical activity per day (n=4) achieved significantly higher energy expenditure before reaching ventilatory threshold, (95% CI of the difference [8.23, 24.85], t(14) = 4.27, p = < 0.01) and at a heart rate of 130 bpm (95% CI of the difference [1.61, 14.33], t(14) = 2.69, p = 0.02). Lastly, V̇O2 at ventilatory threshold was negatively associated with days spent in cyanosis (r = .55, n = 16, p = 0.03), Conclusion: Higher V̇O2 at ventilatory threshold and V̇O2 at a heart rate of 130 bpm was associated with more daily minutes spent in moderate-to-vigorous physical activity. These results suggest that children who meet the recommended 60 minutes of MVPA would have a higher submaximal exercise capacity (V̇O2 at ventilatory threshold or a heart rate of 130 bpm), than children who did not meet the MVPA guidelines. Lastly, children who were exposed to cyanosis for a longer period of time had a lower submaximal V̇O2 at ventilatory threshold, than children who were exposed to cyanosis for a shorter period of time.

A Hemoglobina humana constitui-se de dois pares de cadeias polipeptídicas (globinas), cada qual envolvendo um grupo prostético heme, que se liga reversivelmente ao O2. Mais de 1000 variantes estruturais da Hb humana já foram descritas, parte delas relacionada a manifestações clinicas importantes. Algumas variantes apresentam alteração funcional, que se traduz em afinidade modificada pelo O2. Quando elevada, há uma produção compensatoriamente maior de hemácias, levando a policitemia; quando diminuída, resulta em cianose. O Laboratório de Hemoglobinopatias do Departamento de Patologia Clinica da FCM/UNICAMP e um dos laboratórios de referencia no pais, tendo identificado, ate o momento, 12 hemoglobinas novas e 51 variantes raras. Destas, as variantes novas Hb Hb Boa Esperanca (?16 Lys?Thr), Hb Itapira (?30 Glu?Val), Hb Bom Jesus da Lapa (?30 Glu?Ala), Hb Olinda [ß22 (b4) -25 (b7)], Hb Caruaru (ß 122 Phe?Ser) e Hb S-Sao Paulo (ß6 Glu?Val ; ß65 Lys?Glu) foram aqui caracterizadas funcionalmente, assim como as variantes raras Hb Iwata (a87 His??Arg), Hb Sunshine Seth (?94 Asp?His), Hb Deer Lodge (ß2 His?Arg), Hb Deer Lodge (ß2 His??Arg) + Hb S (ß6 Glu??Val), Hb G-Siriraj (ß7 Glu??Lys), Hb G-Siriraj (ß7 Glu??Lys) e Hb C (ß6 Glu??Lys) em associacao, Hb M-Saskatoon (ß63 His??Tyr), Hb Redondo (ß92 His? Asn), Hb Koln (ß98 Val??Met), Hb Coimbra (ß99 Asp??Glu) e Hb Dhonburi (ß126 Val?Gli)+ Btal. O metodo analitico empregado foi descrito por Rossi-Fanelli & Antonini (1958), e analisa espectrofotometricamente o comportamento da Hb frente a conhecidas pressões parciais de oxigênio (pO2), possibilitando a medida de afinidade (p50), da constante de Hill (n), que verifica o fenômeno de cooperatividade entre as cadeias globinicas para ligação do O2, o efeito Bohr (que indica facilidade na ligação Hb-O2 conforme e elevado o pH do meio), bem como a interação com o Inositol Hexafosfato (IHP), um fosfato polianion que dificulta a ligação entre a Hb e o O2. A exceção das Hbs Itapira e Bom Jesus da Lapa, em concentrações reduzidas nos hemolisados, todas as demais apresentam afinidade alterada pelo oxigênio. Como complementação ao estudo funcional, as Hbs Caruaru e São Paulo também foram submetidas a analise por dinâmica molecular, através dos programas VMD (Visual Molecular Dynamics), com dados inseridos nos softwares CHARMM (Chemistry at Harvard Molecular Mechanics) e NAMD (Nanoscale Molecular Dynamics). As demais variantes tiveram sua analise estrutural individualizada feita através da visualização de estruturas nativas depositadas no Protein Data Bank, para melhor compreensão da relação entre o aminoácido substituído e a função protéica alterada. Deste modo, foi possível estabelecer correlações entre a estrutura e a função e inferir sobre as manifestações clinicas resultantes da presença destas heme proteínas anômalas, ilustrando-se assim a importância desses estudos para a completa caracterização das variantes. / Abstract: The human hemoglobin (Hb) is the respiratory pigment of human organisms, found in high concentrations in the erythrocytes and is responsible for transporting O2 from the lungs to the peripheral tissues. This molecule is composed by two pairs of polypeptide chains (globin), each one involving a prosthetic group heme, which bind reversibly to the O2. More than 1000 structural variants had been described already; part of them related to important clinical manifestations. Some variants present functional alteration, with modified affinity for the O2. When it is increased, it has a higher compensatory production of the erythrocytes, causing policitemia; when decreased, it results on cyanosis. The Laboratory of Hemoglobinopaties of the Department of Clinical Pathology of the FCM/UNICAMP is one of the references laboratories in the country, and identified, until this moment, 12 new hemoglobins and 51 rare variants. The new variants Hb Hb Boa Esperanca (?16 Lys?Thr), Hb Itapira (?30 Glu?Val), Hb Bom Jesus da Lapa (?30 Glu?Ala), Hb Olinda [ß22 (b4) -25 (b7)], Hb Caruaru (ß 122 Phe?Ser) e Hb S-Sao Paulo (ß6 Glu?Val ; ß65 Lys?Glu) were, in the present study, functionally characterized, as well as the rare variants Hb Iwata (a87 His??Arg), Hb Sunshine Seth (?94 Asp?His), Hb Deer Lodge (ß2 His?Arg), Hb Deer Lodge (ß2 His??Arg) + Hb S (ß6 Glu??Val), Hb G-Siriraj (ß7 Glu??Lys), Hb G-Siriraj (ß7 Glu??Lys) e Hb C (ß6 Glu??Lys) em associacao, Hb M-Saskatoon (ß63 His??Tyr), Hb Redondo (ß92 His? The analytical method used was described for Rossi-Fanelli & Antonini (1958), which analyses, by spectrophotometer, the behavior of the Hb against known partial pressures of oxygen (pO2), making possible the measure of affinity (p50), the constant of Hill (n), that verifies the cooperativity, the Bohr effect (that it indicates the relation between Hb-O2 binding according to the pH), as well as the interaction with Inositol Hexaphosphoric acid (IHP), that difficult linkage between the Hb and the O2. Except from Hb Itapira and Hb Bom Jesus of the Lapa, in reduced concentrations at the total hemolysate, all the others presented modified affinity to the oxygen. As a complement of the functional study, the Hb Caruaru and Hb S-Sao Paulo were also submitted to previous molecular dynamics simulation. All the other variants were structurally analyzed by the study of the native structure deposited at the Protein Data Bank, to understand better the relation between the substituted residue and the modified one at the protein function. Embora a história natural dos ALTEs seja benigna, existe risco de mortalidade subseqüente. Objetivos: Avaliar as características clínicas, epidemiológicas e os fatores associados ao óbito de lactentes atendidos na Unidade de Emergência Referenciada (UER) Pediátrica do Hospital de Clínicas (HC) da Universidade Estadual de Campinas (UNICAMP) com história de ALTE. Casuística e Métodos: Estudo transversal retrospectivo descritivo e analítico. Foram incluídos lactentes menores de 12 meses que apresentaram evento súbito de cianose, palidez, hipotonia e/ou apnéia e que foram atendidos na UER Pediátrica. Os dados foram coletados a partir dos Boletins de Atendimento de Urgência (BAUs), através de uma ficha previamente elaborada. Os dados obtidos foram processados com o software SPSS, versão 16.0, utilizando-se o teste de Mann-Whitney para comparação das idades. Para avaliar a associação do óbito com as variáveis independentes, determinou-se os valores de odds ratio bruto por regressão logística. Foram selecionadas para inclusão no modelo logístico multivariado não condicional aquelas que apresentavam associação na análise bivariada. Adotou-se o nível de significância de 5%. Resultados e Conclusão: O ALTE foi mais freqüente na faixa etária entre 9 e 15 semanas de vida, afetando igualmente ambos os gêneros. A manifestação mais comum de ALTE foi cianose, acompanhada ou não de outros sinais e sintomas. Os eventos únicos e de curta duração foram mais freqüentes, a maioria com melhora espontânea. Metade dos pacientes permaneceu internada por mais de 72 horas, em observação clínica e investigação etiológica do evento. O diagnóstico da doença que provocou o ALTE não foi esclarecido em 35,2% dos lactentes. Os demais foram elucidados e a maioria relacionada a doenças do trato respiratório. Durante a internação, a mortalidade dos lactentes com ALTE foi de 7,6%. O maior risco de óbito esteve presente na faixa etária acima dos 6 meses e quando os episódios não tiveram resolução rápida e espontânea, principalmente quando ocorreram como primeira manifestação de doenças do sistema cardiocirculatório / Abstract: Introduction: ALTEs (apparent life-threatening events) are sudden episodes of color change (paleness or cyanosis), apnea and marked change in the muscle tone (limpness), which may be the first sign of an underlying disease (congenital or acquired). Although the ALTE?s natural course is usually benign, there is a risk of subsequent death. Objectives: The aim of this study was to evaluate clinical and epidemiological characteristics, as well as the risk factors associated with mortality of infants who had presented an ALTE and were taken to the Emergency Pediatric Unit from the Clinical Hospital of the State University of Campinas (HC UNICAMP). Casuistic and Methods: Retrospective, transversal, descriptive and analytical study, There were included every infant younger than 12 months old, who had been taken to the Emergency Pediatric Unit after experienced a sudden episode of cyanosis, paleness, limpness and/or apnea. The data was collected from the Emergency care bulletin of each infant, using a previously elaborated card. The data obtained was processed with the software SPSS 16.0. To compare the ages, Mann-Whitney test was used. To evaluate the association among death with the independent variables, logistic regression was used to establish the odds ratio. The multivariate analysis included those variables with positive association in the bivariate analysis. The significance value used was 5%. Results and Conclusion: ALTE occurred more frequently around the ages of 9 and 15 weeks, achieving equally male and female infants. The most common sign observed during the event was cyanosis, with or without other signs and symptoms. The isolated events, with short duration were more frequent, mostly with spontaneous recovery. About 50% of the infants were hospitalized for more than 72 hours in observation and investigation of the underlying disease that caused the ALTE. The diagnosis couldn?t be established in 35.2% of the patients, and in the others, respiratory diseases were the most frequent ones. During the hospitalized investigation, 7.6% of the infants died. The risk factors associated with subsequent mortality were the age greater than 6 months and episodes with slow recovery, especially if the underlying disease affected the cardio-circulatory system / Mestrado / Pediatria / Mestre em Ciências

INTRODUÇÃO: Crianças portadoras de cardiopatia congênita cianogênica com hipofluxo pulmonar (CCCHP) necessitam de tratamento odontológico com frequência. Não encontramos estudos na literatura sobre o impacto na oxigenação dessas crianças durante os procedimentos odontológicos. A hipótese inicial era de que essas crianças submetidas a tratamento odontológico em consultório, poderiam apresentar alterações de saturação periférica de oxigênio (SpO2), pressão arterial sistêmica (PA) e frequência cardíaca (FC). OBJETIVOS: O objetivo principal da pesquisa foi avaliar a ocorrência e magnitude de variações na SpO2, FC e PA em crianças portadoras CCCHP durante exodontia de dente decíduo. Os objetivos secundários foram avaliar o risco potencial de submeter a criança portadora de CCCHP ao tratamento odontológico em consultório e instituir protocolo de atendimento odontológico à criança portadora de cardiopatia congênita do tipo cianogênica. MÉTODOS: Foram estudados 44 pacientes, sendo 20 do grupo de estudo (GE) e 24 do grupo controle (GC), com idade entre 6 e 12 anos submetidos à exodontia de dente decíduo superior. O estudo foi dividido em três períodos: lúdico (PL); anamnese, exame clínico e radiográfico (PAER) e período de procedimento odontológico (PPO), sendo esse último dividido em quatro subperíodos: pré-procedimento (PPR), anestesia (PAN); procedimento (PP) e pós-procedimento (PPP). Durante os períodos foram realizados registros da SpO2, da FC e da PA e aplicada escala de ansiedade facial image scale para avaliação do estresse. RESULTADOS: Os valores da SpO2 , FC e PA sistólica, no GE, durante o protocolo de tratamento variaram, discreta mas significativamente (p<0.05), de 80,5 (±7,6)% a 82,8 (±7,8)%, de 98,7 (±11,0) bpm a 101,3 (±9,8) bpm e de 93,6 a 108,5 mmHg, respectivamente. O GC também apresentou variação significativa durante o procedimento. DISCUSSÃO e CONCLUSÃO: Embora tenham sido observadas alterações significantes nos marcadores estudados durante os períodos, eles foram considerados pelos autores discretos e sem relevância clínica, sendo desnecessários a interrupção do tratamento odontológico ou suporte terapêutico às crianças contrariando a hipótese inicial com base na literatura, pois, ocorreu aumento de SpO2 durante o PPO quando comparado aos períodos de menor estresse. O tratamento odontológico de crianças com cardiopatia cianogênica, com uso de um protocolo padronizado, envolvendo preparo das crianças revelou ser um procedimento seguro, o que permite sua implantação em serviços, sem suporte de centro cirúrgico, facilitando assim a abordagem destas crianças em serviços descentralizados. Os resultados encontrados sugerem que o protocolo estabelecido para o atendimento a crianças portadoras de CCCHP foi adequado / INTRODUCTION: Children with cyanotic congenital heart disease with decreased pulmonary blood flow (CCHHP) often require dental treatment. We didnt find studies about the impact of the oxygenation in those children during dental procedures. The initial hypothesis was that these children undergoing dental treatment in the clinic could present a peripheral oxygen saturation (SpO2), systolic blood pressure (BP) and heart rate (HR). OBJECTIVES: The main objective of this research was to evaluate the occurrence and magnitude of changes in SpO2, HR and BP in children with CCHHP during extraction of primary deciduous. Secondary objectives were to evaluate the potential risk in subjecting the child with CCHHP to dental treatment in the office and establish protocol for dental care to children with congenital cyanotic type. METHODS: We studied 44 patients, 20 out of the study group (SG) and 24 from control group (CG), aged between 6 and 12 years who underwent upper primary tooth extraction. The study was divided into three periods: leisure time (LT), anamnesis, clinical and radiographic examination period (ACER) and period of dental procedure (PDP), the latter being divided into four sub-periods: pre-procedure (PPR), anesthesia (PAN); procedure (PP) and post-procedure (PPP). During periods of SpO2 records were made of HR and BP and applied an anxiety facial image scale to evaluate the stress. RESULTS: The values of SpO2, HR and systolic BP at SG during the treatment protocol varied, quite but significantly (p <0.05), 80.5 (± 7.6) to 82.8% (± 7, 8)%, 98.7 (± 11.0) to 101.3 bpm (± 9.8) bpm and 93.6 to 108.5 mmHg, respectively. The CG also varied significantly during the procedure. DISCUSSION and CONCLUSION: Although significant changes were observed during the periods studied, they were considered, by the authors, discrete and without clinical relevance, and unnecessary interruption of dental treatment or therapeutic support to children contradicts the initial hypothesis based on the literature because of an increase of SpO2 during the PPO compared to periods of lower stress. The dental treatment of children with cyanotic heart disease using a standardized protocol involving the preparation of children proved to be a safe procedure, which allows its implementation in services supported by surgical centers, thus facilitating the approach of those children in local services. The results suggest that the established protocol for the care of children with CCHHP was adequate

METHODS: We studied 44 patients, 20 out of the study group (SG) and 24 from control group (CG), aged between 6 and 12 years who underwent upper primary tooth extraction. The study was divided into three periods: leisure time (LT), anamnesis, clinical and radiographic examination period (ACER) and period of dental procedure (PDP), the latter being divided into four sub-periods: pre-procedure (PPR), anesthesia (PAN); procedure (PP) and post-procedure (PPP). During periods of SpO2 records were made of HR and BP and applied an anxiety facial image scale to evaluate the stress. RESULTS: The values of SpO2, HR and systolic BP at SG during the treatment protocol varied, quite but significantly (p <0.05), 80.5 (± 7.6) to 82.8% (± 7, 8)%, 98.7 (± 11.0) to 101.3 bpm (± 9.8) bpm and 93.6 to 108.5 mmHg, respectively. The CG also varied significantly during the procedure. DISCUSSION and CONCLUSION: Although significant changes were observed during the periods studied, they were considered, by the authors, discrete and without clinical relevance, and unnecessary interruption of dental treatment or therapeutic support to children contradicts the initial hypothesis based on the literature because of an increase of SpO2 during the PPO compared to periods of lower stress. The dental treatment of children with cyanotic heart disease using a standardized protocol involving the preparation of children proved to be a safe procedure, which allows its implementation in services supported by surgical centers, thus facilitating the approach of those children in local services. The results suggest that the established protocol for the care of children with CCHHP was adequate

Développement des fonctions exécutives et des théories de l’esprit chez l’enfant ayant une cardiopathie congénitale cyanogène opérée en période néonatale / Executive functions and theory of mind development in children with cyanotic congenital heart disease after neonatal open-heart surgery

L’objectif général de cette thèse est d’explorer le développement des fonctions exécutives (FE) et des théories de l’esprit (TdE) chez l’enfant ayant une cardiopathie congénitale cyanogène (CCC) et ceci à une période clé dans la construction de ces capacités de « haut niveau » et dans leurs relations fonctionnelles réciproques. L’étude des enfants ayant une CCC et présentant un risque d’hypoxie circonscrit à la période néonatale permet d’explorer la vulnérabilité précoce des capacités neurocognitives à maturation tardive comme les FE et les TdE. En adoptant une approche longitudinale et transversale, nos objectifs visent à (1) déterminer la trajectoire développementale des FE (inhibition, mémoire de travail et flexibilité cognitive) et spécifier la nature et le degré de leurs dysfonctionnements (2) caractériser le développement des capacités méta-représentationnelles et évaluer le rôle prédictif des différentes composantes exécutives dans les TdE de premier et deuxième ordre, (3) explorer la généralisation des dysfonctionnements aux TdE avec un contenu mental affectif, (4) déterminer les facteurs de risque neurologique néonataux associés aux déficits exécutifs et en TdE, (5) évaluer la contribution des dysfonctionnements exécutifs, des variables médicales et démographiques sur la prévalence de prise en charge de ces enfants. Une cohorte de 45 enfants nés avec une CCC opérés à coeur ouvert en période néonatale et un groupe d’enfants sains ont été évalués à un âge moyen de 5 ans (T1) puis ont été à nouveau testés pendant deux années consécutives (T2 et T3). Nos résultats ont mis en évidence un profil de dysfonctionnement hétérogène selon l’âge et le domaine cognitif. Ainsi, naitre avec une CCC altère la progression développementale des FE et des TdE avec toutefois une normalisation pour les composantes ayant une émergence plus précoce. Les liens prédictifs observés entre les niveaux initiaux des FE et le développement ultérieur des TdE reflètent des mécanismes de construction typiques mais néanmoins décalés dans le temps. Enfin, le diagnostic prénatal de la CCC, qui a un rôle neuroprotecteur face au risque de cyanose néonatale, est également associé à un meilleur pronostic des FE et des TdE. Ce travail de thèse offre des résultats pionniers dans la compréhension de l’impact des vulnérabilités neurologiques néonatales sur la dynamique développementale de fonctions neurocognitives complexes. / The general aim of this doctoral dissertation is to explore the development of executive functions (EF) and theory of mind (ToM) in school-aged children with a cyanotic congenital heart disease (CHD) at a key developmental period of progression and functional interactions between these “higher order functions”. The study of children born with a cyanotic CHD and exposed to a hypoxic risk limited to the neonatal period allows to explore the early vulnerability of late developing neurocognitive functions such as EF and ToM. We used a longitudinal and cross-sectional approach in order to: (1) determine the developmental trajectory of EF (inhibition, working memory and cognitive flexibility) and specify their nature and degree of impairment, (2) characterize the development of meta-representational abilities as well as EFs predictive roles in first- and second-order ToM, (3) evaluate the potential generalization of dysfunction to ToM with an affective component, (4) determine neonatal neurological risk factors associated to EF and ToM impairments, (5) evaluate the impact of executive impairments and medical and demographic variables on the early use of remediation services in these children. A cohort of 45 children born with a cyanotic CHD who underwent neonatal open-heart surgery and a group of comparison children were evaluated at a mean age of 5 years (T1) and then followed-up for two consecutive years (T2 and T3). Our results showed a heterogeneous pattern of dysfunction depending on age and specific domains. Cyanotic CHD affects the dynamics of development for these functions even though the earliest developing abilities tend to normalize. Predictive relations observed between early EF and later ToM suggest typical developmental mechanisms though chronologically delayed. Finally, prenatal diagnosis of the cyanotic CHD, known to play a neuroprotective role for cyanosis management is also associated to a better prognosis for EF and ToM outcomes. This work offers pioneer results on the comprehension of the impact of neonatal neurological vulnerabilities on the developmental dynamics of complex neurocognitive abilities.
