Zur Entscheidungshilfe in der Therapiefindung des Mammakarzinoms haben sich bezüglich der Indikation zur Chemotherapie neben den klinischen und histopathologischen Kriterien in den letzten Jahren vorrangig Multigentests etabliert. In der vorliegenden Arbeit wurden Zusammenhänge zwischen dem Oncotyp DX® und 18 immunhistochemischen Markern aus der Tumorbiologie für 78 Fälle hormonrezeptorpositiver, Her2/neu negativer Mammafrühkarzinome mit niedrigem Lymphknotenstatus untersucht. Es erfolgten immunhistochemische Färbungen an Microtissue-Arrays der Tumorproben. Für die Marker AMACR, Cyclin D1, p53, MDM2 und PDL1 ergab sich eine klare statistisch signifikante Korrelation zum Recurrence-Score®des Oncotyp DX® und mit Einschränkungen auch für CDK4. Die Marker p27, Bcl2 und Glut 1 erreichten ein etwas niedrigeres Signifikanzniveau in der statistischen Analyse. Der immunhistochemische Routinemarker Ki67% zeigte eine hochsignifikante Korrelation mit dem Recurrence-Score®. Hierdurch ergeben sich neue Perspektiven zur Risikostratifizierung des Mammakarzinoms, wie beispielsweise die konsekutive Entwicklung eines immunhistochemischen Scores mit prädiktivem Wert für den Recurrence-Score® mit klinischer Anwendung als Prätest oder als eigenständiges Stratifizierungstool bei Brustkrebs. / In addition to clinical and histopathological criteria, multigene tests have become established in recent years as a decision-making aid in the treatment of breast carcinoma.with regard to the indication for chemotherapy. In the present study, correlations between Oncotype DX® and 18 immunohistochemical markers from tumour biology were investigated for 78 cases of hormone receptor-positive, Her2/neu-negative early breast carcinoma with low lymph node status. IImmunohistochemical staining was carried out on microtissue arrays of the tumor samples.There was a clear, statistically significant correlation for AMACR, Cyclin D1, p53, MDM2 and PDL1, to the Recurrence Score® of the Oncotype DX® and with limitations, also for the marker CDK4. p27, Bcl2 and Glut 1 reached a slightly lower significance level in the statistical analysis. Ki67%, a routine immunohistochemical marker, showed a highly significant correlation with the Recurrence-Score®. This opens up new perspectives for risk stratification of breast cancer, such as the consecutive development of an immunohistochemical score with predictive value for the Recurrence-Score® meant for clinical application as a pre-test or as an independent stratification tool for breast cancer.

Challenges with strategy execution for digital transformation

Background: As the world is moving from an industrial to a digital one an increasingly important factor for companies has become their digital transformation. This, Industry 4.0, has brought forward a new set of challenges but conventional strategy-making has not been able to keep up with them, whilst considering that change, in practice, is difficult to implement. The strategy execution for digital transformation has therefore become increasingly important but the challenges faced has not been researched to a sufficient degree. Research question: What are the main challenges with strategy execution during a digital transformation process and how can they be managed? Purpose: The purpose of this thesis is to find and describe the main challenges with strategy execution for digital transformation that has been experienced and or perceived by practitioners working in companies that are a part of a concern. These findings will be conceptualised out of which guiding principles will be created and presented.    Method: In this study a qualitative method has been used together with an abductive approach to reasoning. Semi-structured interviews have been performed with eight respondents chosen by a strategic selection for their knowledge about the studied field. Theoretical frame of reference: Theories about strategy execution, change-management, what a digital transformation is, and how it evolves as well as the activity-based process model have been put forward. Within digital transformation some earlier seen challenges that has been a basis for this research are discussed. Conclusions: An overall conclusion that can be drawn from the analysis is that the primary areas that have been looked at, i.e. strategy development; strategy execution and digital transformation, in themselves create challenges. Whether they are correlated with each other or would exist no matter the other factors cannot be discerned from the findings of this thesis, resulting in the fact that the results of the analysis while able to give a foundation and interpretive thoughts, are, in their nature, uncertain. This would mean that albeit an answer to the research question can be presented by the conceptualisation and guiding principles they should not be seen as general for any given population but rather as a description and analysis of the studied population.

Anorectal Malformations : Long-term outcome and aspects of secondary treatment

Faecal incontinence (FI) is defined as the inability to control bowel movements. The causes of FI are many and diverse. One of the more uncommon reasons for FI is Anorectal Malformations (ARMs). An ARM is a congenital anomaly that affects somewhere between 1/2500 and 1/5000 live born babies. Many ARM patients have persistent FI. Several different procedures have been utilised to address this issue. This thesis aims to evaluate (1) the long-term outcome in adulthood of ARMs in relation to the modern Krickenbeck classification, and (2) scope for treating FI with transanal injection with dextranomer in non-animal stabilised hyaluronic acid (NASHA/Dx), in patients both with and without ARMs. All patients treated for ARMs in Uppsala up to 1993 were invited to participate in a questionnaire study of quality of life and function. The study included 136 patients and compared them with 136 age- and sex-matched controls. The Krickenbeck classification was found to predict functional outcome, and ARM patients had more problems with incontinence and obstipation, as well as inferior Quality of Life (QoL), compared with controls.  Thirty-six patients with FI, owing to causes other than ARMs, were treated with transanal submucous injection of NASHA/Dx. The patients were monitored for two years after treatment. Significant reductions in both their incontinence score and the number of their incontinence episodes were achieved.  A significant improvement in QoL was observed in patients who had at least a 75% reduction in incontinence episodes. No serious complications occurred. A prospective study of transanal injection of NASHA/Dx was conducted on seven patients with persistent FI after ARMs. After six months a significant reduction in the number of incontinence episodes was obtained. A significant improvement in QoL was also found. No serious complications occurred. In conclusion, adult patients with ARMs have inferior outcome of anorectal function and QoL compared with controls. NASHA/Dx is effective and appears to be safe in treating FI in general. This effect seems to be the same in selected patients with persistent FI after ARMs.

The Synthetic Biology of a Man-Made Protein

abstract: Synthetic biology is constantly evolving as new ideas are incorporated into this increasingly flexible field. It incorporates the engineering of life with standard genetic parts and methods; new organisms with new genomes; expansion of life to include new components, capabilities, and chemistries; and even completely synthetic organisms that mimic life while being composed of non-living matter. We have introduced a new paradigm of synthetic biology that melds the methods of in vitro evolution with the goals and philosophy of synthetic biology. The Family B proteins represent the first de novo evolved natively folded proteins to be developed with increasingly powerful tools of molecular evolution. These proteins are folded and functional, composed of the 20 canonical amino acids, and in many ways resemble natural proteins. However, their evolutionary history is quite different from natural proteins, as it did not involve a cellular environment. In this study, we examine the properties of DX, one of the Family B proteins that have been evolutionarily optimized for folding stability. Described in chapter 2 is an investigation into the primitive catalytic properties of DX, which seems to have evolved a serendipitous ATPase activity in addition to its selected ATP binding activity. In chapters 3 and 4 we express the DX gene in E. coli cells and observe massive changes in cell morphology, biochemistry, and life cycle. Exposure to DX activates several defense systems in E. coli, including filamentation, cytoplasmic segregation, and reversion to a viable but non-culturable state. Metodjämförelse av fenobarbital på Immulite 2000 versus Catalyst dx för hundar med epilepsi

Definitionen av epilepsi är ofrivillig förändring av den neurologiska funktionen i hjärnan. Sjukdomen uppkommer när det blir obalans i kommunikationen mellan hjärnans nervceller. Det är en störning/aktivering av impulsfrisättningen i hjärnan där orsakerna inte är fastställda. Fenobarbital är ett lugnande och sömngivande preparat, som motverkar kramper och begränsar retningens spridning i hjärnan genom att förstärka effekten av den hämmande transmittorsubstansen gammaaminosmörsyra (GABA). Syftet med studien var en metodjämförelse av fenobarbitalnivåer i serum uppmätta på Immulite 2000 versus Catalyst Dx hos hundar med epilepsi. Catalyst dx är ett veterinärmedicinskt instrument som använder torrkemisk analys. Immulite 2000 använder en våtkemisk metod och är ett instrument som fungerar på liknande sätt som sandwich ELISA metoden. Resultatet av de 14 prover som analyserades under april-maj 2017 visar på högre värden för samtliga prover analyserade på instrumentet Catalyst dx jämfört med Immulite 2000. Med ett P-värde på <0,0001 betyder det att nollhypotesen förkastas, dvs det förekommer en signifikant skillnad mellan instrumenten. / The definition of epilepsi is unintended change of neurological function in the brain. The disease occurs when there is imbalance in communication between the brains nerve cells. It is disorder/activation of pulse release in the brain where the causes are not determined. Phenobarbital is an appeasement and soporific preparation. It´s a drug that inhibits cramps and limit the spread of the direction in the brain by enhancing the effect of the inhibitory transmittor substance gamma aminobutyric acid (GABA). The aim of this study examine method comparison of the phenobarbital level measured on Immulite 2000 versus Catalyst Dx in dogs with epilepsy. Catalyst dx (IDEXX) is a veterinary instrument which uses dry chemical analysis. Immulite 2000 (Siemens) uses a wet chemical method and it´s an instrument that works similar to the sandwich ELISA method. The results from the 14 samples analyzed in april-may 2017 shows higher values for all samples on the instrument Catalyst dx comparison to Immulite 2000. With a P-value of <0,0001 it means that the zero hypothesis is rejected. There is a significant difference between the instruments.

É feito um resumo dos principais modelos criados para se explicar as intrigantes propriedades do centro DX e atualizar o problema. O decaimento da fotocondutividade persistente (PPC) é medido em AlxGa1-x As dopado com Si e se discute a validade dos modelos em função da cinética de captura dos elétrons pelos centros DX. Boa concordância com o modelo de Chadi e Chang é encontrada desde que se postule a existência de um nível doador mais raso. O crescimento por MBE assim como todo o processamento de amostras para os experimentos realizados é descrito sinteticamente. É discutido também o problema dos contatos a baixa temperatura e a possível influência dos centros DX nos desvios do comportamento ôhmico observados. Inclui-se também a descoberta da. fotocondutividade persistente em AlxGa1-xAs dopado com Pb, que também é relacionado à existência dos centros DX. / A short discussion about the main models created to explain the striking properties of the DX center is done in order to bring the problem up-to-date. The decay of persistent photoconductivity is measured and it is analyzed as a function of the kinetics of electron trapping by DX centers in Si-doped AlxGa1-xAs, according to these models. Good agreement with Chadi and Chang\'s model is found as long as we postulate the existence of a shallower donor. The M.B.E. growth as well as the whole sample processing is shortly described. It in siso diacussed the problem of low temperature contacts and the possible influence of DX centers in the deviation from ohmic behavior. Persistent Photoconductivity has been found in Pb-doped AlxGa1-xAs and it is also related to the DX center existence.

Luminescence ultraviolette et dynamiques excitoniques dans l’oxyde de zinc massif et nano-structuré / Ultraviolet luminescence and exciton dynamics in massive and nano-structured zinc oxide

Cette thèse présente les travaux effectués au CELIA sur la luminescence ultraviolette et les dynamiques excitoniques dans l’oxyde de zinc (ZnO) sous forme massive et nano-structurée. Les mesures ont été effectuées en conditions expérimentales contrôlées (température, fluence d’excitation), pour différentes énergies de photon excitateur. Nous avons mesuré les spectres d’émission sous excitation à un photon UV (4,66 eV), ainsi que à 3 photons IR (1,55 eV), et proposé un schéma séquentiel de formation des excitons (avec simulations), en particulier pour les excitons DX. Nous avons obtenu une durée de vie nanoseconde de DX dans les deux cas, en désaccord avec la majorité des études publiées dans la littérature. / This thesis presents the work carried out at CELIA about ultraviolet luminescence and exciton dynamics in massive and nano-structured zinc oxide (ZnO). Measurements were carried out under controlled experimental conditions (temperature, excitation fluence), according to different excitation photon energies.We measured emission spectra under UV photon excitation (4.66 eV), and 3 IR photons (1.55 eV), and suggested a sequential exciton formation mechanism (with simulations), especially for DX excitons. We found a nanosecond lifetime for DX in both cases, in disagreement with most of the studies published in the literature. Relaxation dynamics of free and bound excitons are linked by the FX trapping process on donor defect and the DX thermal detrapping.Under VUV excitation (20-50 eV), surface effects and strong local excitation density greatly accelerate the relaxation of excitons. Under X excitation (1 keV), good conditions for the formation of DX seems to be close under excitation at 1.55 and 950 eV.The presence of core 2p band of zinc modifies the relaxation dynamics of excitons by the multiplication of local high density excitations zones and the change of the elementary excitations distribution. We have also conducted measurements on nano-particles. The significant surface effects induced by the small size of these system lead to a sharp acceleration of kinetics, masking the intrinsic exciton relaxation process.
