Analýza potřeb uživatelek návykových látek v Praze / The Needs Analysis of women who use drugs in Prague

The main subject of the dissertation are women - users of addictive substances. The dissertation charts previous studies that are dealing with this specific group, defines specifics of using illegal addictive substances by women, deals with gender issues and presents "drug programs" for women in Czech Republic or Prague. There are four categories in the dissertation: women as users, pregnant users, mothers as users. The major purpose of the dissertation is to find out, which needs belong to female users of addictive substances, how offered services corespond to these needs and what is missing within care system. As a result the dissertation suggests appropriate services for female users so that their needs would be satisfied, which services should we offer so that female users would be more in contact with care facility, how should we eliminate unwanted pregnancy or how could we help female users to have children in their own care. Material for the study were gathered through field work by semistructured interview with female users of illegal addictive substances in Prague - clients of drug services. Another information was acquired through an interview with three employees of drug services interested in the same issue.

Background: Intravenous way of drug application is the most common between problem drug users at Prague. Due Mravčík et al. (2015), the estimate is, that there was a 15 700 intravenous drug users at Prague in 2014 (7 500 pervitin users, 2 500 heroin users and 5 700 buprenorphine users). And that they spend about 15,5 millions of syringes every year (Švůgerová, 2015). But the data in Annual report on drug situation 2014 says, there was only 2, 6 millions of syringes distributed (Mravčík et al., 2015). This means, that Prague intravenous drug users spend many more syringes, that are distributed by low-treshold services and pharmacies. That is a reason of charting of the behaviour of drug users in connection with application of addictive substance, especially how the consuption of injecting material is connected with single type of drug user. Aims: The main aim of the thesis is to find out the frequency of drug application in intravenous drug users and to estimate the total consumption of injecting materialial depended on single of users typology (used drugs, frequncy of contact with harm reduction services…) Research sample: The sample consists of 155 clients of harm reduction services at Prague (3 contact centers and 3 streetwork programs). The subject of this thesis is the issue of criminological aspects of drug-related crime. This topic is considerably wide, therefore this thesis focuses on two specific aspects of this particular crime: the relationship between drug use and crime and the criminal careers of drug-addicted offenders. Since these two aspects are still being overlooked in the Czech Republic, the aim of this thesis is to introduce this issue using foreign literature and to complement it with my own research carried out within the Capital City of Prague. The first part of the thesis (Chapters 1 and 2) represents an introduction into the issue of drug-related crime. First, it explains basic notions that are essential to know for easy orientation within the thesis. Then, it discusses the history of drug abuse in the Czech Republic from 1918 until nowadays, and also complements current situation with an actual drug policy strategy. The second part of this thesis (Chapters 3 and 4) focuses on acquainting with the issue of the relationship between drug use and crime and the criminal careers. Diplomová práce měla za cíl odhalit specifika použití TAT u problémových a závislých uživatelů drog a na základě srovnání s výsledky, kterých v testu dosáhla kontrolní skupina, přinést evidenci pro zhodnocení diagnostického potenciálu TAT u uživatelů drog. Výzkumné šetření bylo provedeno v Kolíně na klientech nízkoprahového kontaktního centra, které poskytuje služby harm reduction skryté populaci uživatelů nealkoholových drog. Těžiště výzkumu bezprostředně souvisí s metodou analýzy příběhů, jejichž vyprávění bylo u probandů vyvoláno předložením tabulí TAT. K analýze příběhů byl použit psychometricky zakotvený skórovací systém SCORS (Social Cognition - Object Relation Scale) současného amerického autora D. Westena. Použití TAT u uživatelů drog tak bylo zaměřeno na zhodnocení úrovně jejich fungování v sociálních vztazích. Metodologický princip diplomové práce tkví v induktivním usuzování a statistickém zpracování dat. Způsob zpracování dat, který je až na několik oblastí výhradně v režii kvantitativních metod, spolu s dostatečně velkým souborem zkoumaných osob umožňuje zobecnění důležitých rozdílů ve výsledcích TAT u výzkumné a kontrolní skupiny. Možnosti a meze postpenitenciární péče v ambulantních službách pro uživatele drog / Opportunities and limits of post-penitenciary care in out-patient services for drug users

In my thesis I deal with the postpenitenciary care in ambulatory services for drug users. The aim of this study was to explore why clients seek and visit the institute, where my study took place, what services they used and which services did they miss there. Then I investigated reasons of their early abortion of these services. For these purposes, I chose a qualitative study - six case studies of clients who have used postpenitenciary services. My research took place in one organization - the ambulatory center for drug users. I was in contact with the clients throughout their whole attendance at the service. For the purpose of qualitative research I used data folders of the clients - records of consultations made with clients and all documents that were relevant for the study. All respondents were sent to the institution by workers of Probation and Mediation Service for Czech Republic. Four of the six clients completed the program properly. Two clients aborted the service earlier. Abroad is the method of peer work dealing with the involvement of drug users (active and retired) extended far more than in the Czech Republic. Information and data are available mainly from foreign sources, where peer work has been continuing a tradition. In the Czech Republic, this method is still not so much used, as well as is the lack of Czech professional literature on the subject. Method of peer work in the Czech Republic has not been comprehensively mapped, described and evaluated. They are organizations dealing with the involvement and employment of drug users, which usually draw infromation from foreign sources and work with their data. There is no comprehensive overview, providing information and data in this area in the Czech Republic. The target of this work is to map the use of peer work in the Czech Republic in the area of drug services. Within the sub-targets are the structure and functioning of each peer programs studying. The practical part is realized by an empirical research (research orientation) in order to get a general overview of the analysis of the problem in the form of quantitative research. Consumption rooms for drug users are implemented as a tool of tertiary prevention by a number of European countries. These services are based on harm reduction philosophy. The objectives of these programs include minimizing health risks, overdose prevention, motivating users to change their lifestyle. Consumption room has also effect in reduction of negative effects associated with open drug scenes such as the drug consumption in public places. The thesis is about a need analysis of a theoretical application of a consumption room in the capital city Prague. After a theoretical introduction, which introduces the background of the project, the project objectives and examples of foreign practice presented in a literature, are presented the results of qualitative research. The aim of the research, conducted by the technique of semi-structured interview, is to answer the basic questions of needs and expectations of potential users, experts in the treatment and prevention of addiction and other persons who may be considered as involved from the perspective of discussion about consumption. The thesis deals with economic theory and research of the drug markets. Analysis of the literature compares individual approaches to theories by present authors, including their conclusions on data for the U.S. Established hypotheses based on the conclusions of analyzed literature are tested mainly using regression analysis on the available Czech data, where data on marihuana markets have high interdependency. The effects of selected variables on the price of drugs, which is a key characteristic of drug markets, are tested on data for the EU-27. The work describes trends in European drug markets and provides evidence on the influence of the variables on the price of drugs.

Kvalita života u drogové závislých klientů před a po ústavní léčbě v Praze / The Quality of life for drug dependent clients before and after the institutional treatment in Prague

BACKGROUND: Quality of life is defined by WHO as an individual perception of one's own position in life, taking into account cultural aspects of the society and a system of values in which the individual lives, as well as his/her goals, standard of living and interests. Because quality of life measure is strongly a subjective measure, it can only be assessed by the individual himself/herself. AIMS: The aim of this thesis was to compare of the quality of life of drug users before and after institutional treatment. SAMPLE AND METHODS: WHOQOL- BREF questionnaire was employed to collect the data. A research sample consisted of all clients (both men and women) in the institutional treatment at the department of treatment of dependencies at the Clinic of Addictology, The clients filled the questionnaire in before treatment entry or within the first week after treatment entry, and at the release from the treatment. RESULTS: None of the clients was very satisfied with his/her quality of life before treatment. After treatment, the quality of life was perceived as positive rather by women. After treatment, 44% of women found their quality of life good, whereas it was assessed as very good by 13% of women.
