The thesis focuses on one of the possible methods of the opioid addiction treatment, on the substitution therapy. It also describes specific form of substitution therapy in Ústí nad Labem, part of the particular low-threshold facility for drug users. The paper also briefly discusses opioid addiction and the options of its substitutional therapy in Ústí nad Labem (e.g. methadone therapy, buprenorfine therapy). The aim of this thesis is to evaluate life quality of substitutional therapy's clients in the low-threshold facility in Ústí nad Labem and compare those results with life quality of clients who are using buprenorfine obtained on the black market. Another goal is to use these findings to improve and expand services for this target group. The theoretical part briefly describes the opioid addiction and possible methods of its treatment, mainly substitutional therapy. The research evaluates the life quality of clients who are using buprenorfine and identifies problematic parts of their lives. 3456 ABSTRACT: Background: VHC is a widespread serious chronic infectious disease. Most of the patients affected by the disease consist of IDUs. Treatment of VHC is now perceived as an important preventive element for the spread of the disease in IDUs population. Adherence to treatment is absolutely crucial for the successful completion of treatment. Adherence to treatment is often questioned by clinicians who treat VHC in patients using addictive substances. Objectives: To compare the efficacy of treatment of VHC in patients with a history of drug in patients without a history of drug, to compare treatment adherence among patients without a drug history and patients with drug history, to map out the factors affecting adherence to treatment and to find a place in the therapeutic team for expertise of addictologist. Purpose: The purpose of the study was to map the sexuality of older (between the ages of 30 and 50) heterosexual regular marihuana users, which up to now remains little explored. We tried to find out whether there is a difference between the three groups of marihuana users: 1. users, who use only marihuana, 2. users who use marihuana and alcohol, and 3. users, who use marihuana, alcohol and party substances (LSD, ecstasy, magic mushrooms, cocaine). More over we compared our findings with some characteristics of the same-age sample of the Czech population. Methods: The research was of quantitative nature. The data were collected by using an original anonymous questionnaire, which was separately answered by 417 respondents 202 respondents from them satisfied our criterions (age, frequence and length of marihuana use). The data were processed by SPSS. Results: 61,7 % of respondents marihuana users are satisfied with their sexual life. Some kind of change as a result of marihuana abuse noticed 53,9 % of all respondents (48,5% of them noticed change for the better 51,2% of them noticed higher quality of orgasm). Title: Analysis of need of recreational drug users in Melnik Region Abstract Prevalence of drug using among those dance events visitors in the country increases steadily throughout life (Petroš Zábranský, 2003 Mravčík, Škařupová, Orlíková, 2008). Increase is also the acceptability of the use of some drugs, especially cannabis (Blahut, 2014). The research which was designed to determinate the prevalence of drug using among members of different musical subcultures in the district of Melnik, that number of recreational drug users in the Mělník region relatively high. 98 % of respondents (194 respondents) said that at least once in their life used drugs recreationally (Přádova, 2013). A group of recreational users compared to other types of drug users is however greatly neglected. Interventions are insufficient for a large percentage of recreational users, unavailable or about it users do not even know (Mravčík et al, 2008). People tend to enjoy the addictive substances in a fun environment. There is however missing feedback from these users which would helpful to develop appropriate and effective interventions. Bakgrund: Patienter med drog- och alkoholmissbruk riskerar många komplikationer till följd av missbruket. Missbruket leder till många dödsfall varje år och är en stor kostnad för samhället. Patienter med missbruksproblematik upplever att de behandlas orättvist i omvårdnaden. Sjuksköterskor anser att omvårdnaden av den här patientgruppen är komplex. Syfte: Syftet är att sammanställa forskning om sjuksköterskors inställning och upplevelser av att vårda patienter med drog- och alkoholmissbruk inom både somatisk och psykiatrisk vård. Metod: En litteraturstudie, med 16 vetenskapliga artiklar, 12 med kvalitativ metod och fyra med kvantitativ metod. Resultat: Resultatet presenterades i olika kategorier som var: ”Att vara osäker”, ”Att känna sig otillräcklig”, ”Att känna sig inkompetent i sin roll”, ”Att kommunicera med patienten” samt ”Att vara ensam eller i team”. Sjuksköterskorna upplevde svårigheter i omvårdnaden med den här patientgruppen. Kunskapsbrist beskrevs i samband med omvårdnaden samt känslor som ångest och stress. En del relationer mellan sjuksköterska och patient baserades på misstro och några sjuksköterskor uttryckte en vilja att förbättra omvårdnaden med den här patientgruppen. Slutsats: Utifrån denna litteraturstudie kan slutsatsen dras att sjuksköterskor kan uppleva brister i omvårdnaden av patienter med drog- och alkoholmissbruk. Bristerna visar sig genom osäkerhet, otillräcklighet, känsla av inkompetens, i kommunikationen med patienten och samverkan i teamet. Utbildning, kunskap, samverkan och engagemang visar sig vara viktiga faktorer som kan påverka kvaliteten på omvårdnaden av den här patientgruppen. Det innebär således att det finns ytterligare behov av att utveckla både kunskapsläget och rutinerna när det kommer till denna typ av vård. / Background: Patients with drug- and alcohol abuse risk various complications related to the misuse. Drug- and alcohol abuse leads to a high yearly rate of death and an important social cost. Patients with drug- and alcohol abuse experience that they are treated unfairly in the care. Nurses consider the nursing care of this patient group to be complex. Aim: The aim of this study was to describe nurses’ experiences in the care for patients with drug- and alcohol abuse, both in somatic and psychiatric wards. Method: A literature review including 16 scientific articles, 12 with qualitative method, 4 with quantitative method. Results: The result is presented in different themes: “To be unsure”, “To feel inadequate”, “To feel incompetent”, “To communicate with the patient” and “To work alone vs in a team”. The nurses experienced difficulties in the care. A lack of knowledge was described and also emotions like anxiety and stress. Many relationships were based on distrust. Some of the nurses expressed a will to ameliorate the care for this patient group. Conclusion: The conclusion of this literature review is that nurses can experience deficits in the care of patients with drug- and alcohol abuse. The deficits manifest through insecurity, insufficiency, a sense of incapacity, in the communication with the patient and in the interplay inside the team. Education, knowledge, teamwork and commitment show to be important factors that can affect the quality of the care for this patient group. This implies that there are additional needs to evolve the knowledge and routines concerning this type of care.
