The design of a service outcome measure for SANCA

M.A. / The treatment of substance abuse remains a challenging process with relapse an ever-present risk. For those offering treatment within this field, such as the South African National Council on Alcoholism and Drug Dependence (SANCA), it is of cardinal importance to evaluate the outcomes of their interventions. Currently, there is no quantifiable and standardised method that SANCA can use to establish and assess how the actual outcomes of their programmes compare to their intended programme goals. Kranz and O’Hare (2006) argue for the evaluation of substance abuse treatment programmes through the use of scales to quantify various aspects of the treatment process which can then serve as a measure of its effectiveness. Effectiveness in this instance refers broadly to a patient’s maintenance of his/her sobriety for a period of 12 months or more, combined with an increase in his/her general functioning. Therefore, this study aims to demonstrate the technically complex process of developing a content valid framework for a scale on behalf of SANCA that adheres to their requirements for programme evaluation. The study is guided by an adapted model of ecometric scale development presented schematically by Faul and Hudson (1999). Through the use of a grounded theory approach, the study shows how to identify the expectations for a scale in the organisation and organise SANCA’s treatment strategy into a framework within which the areas of measurement can be placed. The study then demonstrates the construction of operational assessment areas through empirical data collection that adequately reflects such expectations. It also serves to test for content validity of the assessment areas, through the application of domain sampling theory. The study reveals the process undertaken to convert those assessment areas into constructs by conceptualising and operationalising them into working definitions. Lastly, drawing on those defined constructs, it populates a series of exemplar items designed for illustrative purposes.

The Impact of Race, Income, Drug Abuse and Dependence on Health Insurance Coverage Among Us Adults

Little is known about the impact of drug abuse/dependence on health insurance coverage, especially by race groups and income levels. In this study, we examine the disparities in health insurance predictors and investigate the impact of drug use (alcohol abuse/dependence, nicotine dependence, and illicit drug abuse/dependence) on lack of insurance across different race and income groups. To perform the analysis, we used insurance data (8057 uninsured and 28,590 insured individual adults) from the National Surveys on Drug Use and Health (NSDUH 2011). To analyze the likelihood of being uninsured we performed weighted binomial logistic regression analyses. The results show that the overall prevalence of lacking insurance was 19.6 %. However, race differences in lack of insurance exist, especially for Hispanics who observe the highest probability of being uninsured (38.5 %). Furthermore, we observe that the lowest income level bracket (annual income <$20,000) is associated with the highest likelihood of being uninsured (37.3 %). As the result of this investigation, we observed the following relationship between drug use and lack of insurance: alcohol abuse/dependence and nicotine dependence tend to increase the risk of lack of insurance for African Americans and whites, respectively; illicit drug use increases such risk for whites; alcohol abuse/dependence increases the likelihood of lack of insurance for the group with incomes $20,000–$49,999, whereas nicotine dependence is associated with higher probability of lack of insurance for most income groups. These findings provide some useful insights for policy makers in making decisions regarding unmet health insurance coverage.

Therapy development of group contingency management in methadone treatment: Pilot 1 - the application of a percentile schedule to enhance attendance behaviors and cocaine abstinence

In an effort to adapt a prize-based attendance and abstinence reinforcement program to the group therapy modality typically found in community substance abuse treatment, this study applied a contingency management (CM) procedure to behavior outcomes of the group as a whole. An algorithmic shaping percentile schedule was used to establish the outcome criteria determining group access to prize bowl drawings. Both the shaping schedule and hypothesized positive social support secondary to interdependent group contingencies aimed to initiate cocaine abstinence among cocaine abusing methadone-maintained participants having initial low rates of cocaine abstinence. In a multiple baseline across behaviors with reversals design, the procedure was applied to four target behaviors in sequence: incentive group attendance, urinalysis appointment attendance, opiate and cocaine abstinence. Results demonstrated clear improvement in group attendance during the shaping schedules. Effects on opiate abstinence were promising but not definitive; an increase in magnitude of reinforcement yielded a better response. There were no effects on cocaine abstinence using the percentile schedules tried. When shaping was conducted by an experienced psychologist in conjunction with an average 4-fold increase in reinforcer magnitude, 55% achieved some abstinence (compared to 18% in baseline). Participants were monitored for safety due to concern about coercion; no instances of physical violence occurred and infrequent instances of verbal negativity were managed using typical outpatient procedures. Results indicate that interdependent group CM is efficacious for attendance, and a feasible and acceptable procedure with appropriate monitoring. Future studies are needed to determine effective and appropriate shaping and reinforcement schedules in group CM to increase drug abstinence. / Educational Psychology

<p>Anabolic-androgenic steroids (AAS) have been used in clinics for decades. The misuse of AAS has previously been attributed merely to sport athletes, taking AAS with intentions to increase muscle mass, enhance physical performance and to improve results in competitions. Today, the misuse of AAS has spread to adolescents and young adults not connected to sports. Alarmingly, many reports are pointing at severe psychiatric adverse effects among AAS abusers, which include mood swings, mania, anxiety, depression and aggression. Numerous examples of severe and often unprovoked violence and brutal crimes have been connected to AAS abuse and there is a strong need for a better understanding of the underlying biochemical events that might account for the adverse behaviors induced by AAS. The general aim of this thesis was to study the effect of chronic AAS administration on neuropeptide circuits in the rat brain associated with the regulation of rewarding effects, memory, anxiety, depression and aggression, using nandrolone decanoate as a prototype AAS.</p><p>Results demonstrated that daily administration of AAS to rats in doses comparable to those taken by AAS abusers, in certain brain structures significantly affected, <i>a</i>) the levels of the opioid peptides dynorphin B and Met-enkephalin-Arg<sup>6</sup>Phe<sup>7</sup>, <i>b</i>) the levels of the tachykinin substance P (SP), <i>c</i>) the density of the SP neurokinin 1 (NK1) receptor, <i>d</i>) the level of the SP metabolite SP<sub>1-7 </sub>that frequently exerts opposite effects to SP, <i>e</i>) the SP<sub>1-7 </sub>generating enzyme substance P endopeptidase (SPE) and finally, <i>f</i>) the levels of the neuropeptide calcitonin gene-related peptide (CGRP) often co-localized with SP. The alterations seen in the levels and activities of these neurochemical components are in many aspects compatible with behaviors typified among AAS abusers.</p>

Association Tests of the Opioid Receptor System and Alcohol-Related Traits

The opioid receptors and their endogenous ligands have long been implicated in a variety of traits including addiction, impulsive behaviors and substance dependence. Using phenotypic measurements collected from the IASPSAD, data from a latent class analysis and data from a SNP array and additional genotyping assays, association and regression tests were performed to determine the effects of common SNPs encoded in the genes of the opioid receptors and ligands on various traits relating to alcohol dependence. Although only one SNP can be reported as significant for substance dependence within alcoholics, there were a few results approaching significance that may offer some insight into variation within alcoholism.

Substance P Endopeptidase : Purification and Characterizataion of Enzyme Activity and Evaluation of its Function during Stressful Condition

<p>The purification and biochemical characterization of the substance P (SP) hydrolyzing enzyme, substance P endopeptidase (SPE), have been carried out; with subsequent orientation in neurobiological fundamental processes involved in opioid dependence, withdrawal, and heat-stress.</p><p>SPE was purified from rat spinal cord, human spinal cord and cerebrospinal fluid (CSF), rat ventral tegemental area (VTA), and rat hippocampus. The enzyme activity was found to release the biologically active fragments SP(1-7) and SP(1-8) as major products. The purified enzymes were characterized with regard to their biochemical and kinetic properties. The typical SPE is neither inhibited by phosphoramidon nor captopril nor phenylmethanesulfonylflourid (PMSF). In comparison to other known proteases SPE differed in characteristics regarding substrate specificity, inhibition-profile, cleavage pattern, and other kinetic parameters. The technically very delicate approach of micro purification of SPE from the rat ventral tegemental area (VTA) (this is a very small tissue), turned out to be possible with the ÄKTA™-purifier system. Studies revealed a crucial role of SPE in a series of clinically important neuropathological conditions, such as opioid tolerance, and withdrawal (SPE, increased); and heat-stress (SPE, increased). These findings emerged from assessment of enzyme activity in hypothalamus, nucleus accumbens (NAc) periaqueductal gray (PAG), pituitary, striatum, substantia nigra (SN), VTA, spinal cord. Viewing the role of SPE in morphine tolerance, it was possible to note regional differences with a decrease in PAG, and striatum, whereas an increase was seen in SN, and VTA. After heat-stress treatment, SPE was raised in several regions (cerebral cortex, hippocampus, diencephalon, cerebellum, spinal cord), and the most precise observation of this was located to the hippocampus structure.</p>

Ergebnisse einer breit angelegten Repräsentativerhebung zur Substitutionstherapie in Deutschland an 2694 Patienten zeigen, dass trotz hoher und komplexer Ko- und Multimorbidität mit psychischen Erkrankungen nur selten Psychiater an der Therapie beteiligt sind. Nur 16% der Substitutionsärzte (n=223) sind Psychiater oder Psychotherapeuten und nur 21% der substituierten Drogenabhängigen werden von Psychiatern behandelt. Die meisten substituierenden Psychiater sind in Institutsambulanzen psychiatrischer Kliniken tätig (51,3%), kaum in eigener Praxis (7,2%). Der Großteil der Substitutionsversorgung wird von Allgemein- und Hausärzten geleistet. Die relative Abstinenz psychiatrischer Kollegen bei der Substitution drogenabhängiger Patienten wird kritisch diskutiert. / Results of a nationally representative study in 2,694 patients reveal that most physicians (n=223) involved in substitution treatment of opioid-dependent patients are general practitioners, while only 16% have a psychiatric/psychotherapeutic background and only 21% of the addictive patients are treated by psychiatrists. This contrasts with the remarkably complex pattern of co- and multimorbidity with other mental disorders in such patients. Most psychiatrists engaged in substitution treatment work in specialized outpatient wards (51.3%), and few were operative in their own or private practice (7.2%). Implications of these critical findings are discussed.
