The aim of this thesis is a diagnosis of ventricular tachycardias, fibrillations and flutters from electrocardiogram. These disturbances of heart rate are ranked among the life threatening arrhytmias. This work presents own method of the automatic detection, which is created for the ECG holter monitoring system. The proposed algorithm is based on the detection in the spectral domain, which is supported by the detection in the time domain. The results show the discrimination of arrhytmias from the normal sinus rhythm and the discrimination from the noise. The method is tested with ECG records from the The AHA Database (American Heart Association) and from The MIT-BIH Malignant Ventricular Arrhythmia Database.

This work deals with the classification of cardiac cycles, which uses a method of dynamic time warping and cluster analysis. Method of dynamic time warping is among the elderly, but for its simplicity compared to others is still very much used, and also achieved good results in practice. Cluster analysis is used in many fields such as marketing or just for biological signals. The aim of this work is a general introduction to the ECG signal and the method and implementation of dynamic time warping algorithm. Subsequently, cluster analysis and finally the creation of the user interface for the algorithms.

This master thesis named “Measuring of ECG signals” is focused on detailed description of resting signals of ECG, which are derived from known positions of the start and the end of the wave P, complex QRS, and wave T. For this purpose we used classic twelve lead in ECG signals from CSE database, which consisted of 250 measurement records. Outcome of this work will be an algorithm capable of classifying chosen heat anomalies, as well as study which will evaluate quality of this algorithm from statistical standpoint. This study will also carry out statistical calculations concerning most common heart anomalies. These materials could be used as a software support for cardiologists.

The Cardiovascular diseases may evocated the high percentual risk of sudden cardiac death in whole world. In several western countries is the number of death higher then number of cancer death. In this time is used a lot of methods for prediction of sudden cardiac death with focus on ECG T-wave alternance. The aim of the theses was to do stronger relation and cooperation with Internal Cardiac Clinic of Faculty Hospital Brno Bohunice on the risk analysis of sudden cardiac death. Secondly, we met the methods used for detection and quantification of simulated TWA. Last but not least was necessary to find TWA detection methods improvement and process the data on real signals obtained from Faculty Hospital Brno Bohunice. First part of the Thesis is focused on summary of pathologic artifacts in ECG signal, which are important for sudden cardiac risk stratification. There are described further known detection and quantification methods for TWA analysis. An interesting part for clinical practice is analysis of TWA trend in time and looking for the best method, which is able to catch and track the short TWA trend changes. Second part describes the new methods improvements, which were tested with interesting outputs. Further, there was developed method for TWA presence statement probability evaluation.

Metody detekce a klasifikace v analýze EKG signálů / METHODS FOR DETECTION AND CLASSIFICATION IN ECG ANALYSIS

První část práce je zaměřena na měření QT intervalu. QT interval může být použit k hodnocení kardiovaskulárního zdraví pacientů a detekovat potenciální abnormality. QT interval je měřen od začátku QRS komplexu až po konec T vlny. Nicméně, měření konce T vlny je často vysoce subjektivní a jeho verifikace je obtížná. Představujeme dvě metody měření QT intervalu - vlnkovou a šablonovou metodu. Metody byly porovnány mezi sebou a testovány na QT databázi. Druhá část práce je zaměřena na modelování arytmických signálů McSharryho modelem následována klasifikací s použitím umělých neuronových sítí. Metoda používá předzpracování signálů lineární aproximací a shlukování lineárních segmentů pro stanovení počátečních parametrů McSharryho modelu. Byly použity EKG signály standardní MIT/BIH Arrhythmia Databáze. Modelování bylo testováno na celé databázi a svodu MLII (modifikovaný svod II). Všechny signály mohou být modelovány 10 Gaussovými funkcemi bez významného zkreslení. Třetí část práce představuje klasifikaci EKG do dvou tříd. Předčasné komorové kontrakce (PVC) mají vysoký význam při hodnocení a predikci život ohrožujících ventrikulárních arytmií. Představujeme algoritmus pro detekci předčasných komorových kontrakcí s použitím McSharryho modelu a neuronových sítí. Signály modelované 30 Gaussovými parametry byly předloženy na vstup umělé neuronové sítě. Použitý vícevrstvý perceptron dosáhl klasifikační úspěšnosti 93,10% pro předčasné komorové kontrakce (PVC) a 96,43% pro normální stahy.

The practical part is designed in measurement algorithm for ECG signal that has been tested on basic CSE database. The results are discussed in the conclusion.

MUDr. Tomáš Soukup, PhD.2 1 Department of Social and Clinical Pharmacy, Faculty of Pharmacy in Hradec Králové, Charles University 2 2nd Department of Internal Medicine - Gastroenterology, University Hospital in Hradec Králové Title of the master thesis: Effect of pulse glucocorticoid therapy on ECG Glucocorticoid pulse therapy (PT) is used to terminate acute exacerbations of immunologically mediated diseases. The aim of this thesis was to analyze the influence of methylprednisolone (MP) PT on ECG changes, mainly on QT interval, as prolonged QT interval may induce ventricular arrhythmias and to identify other risk factors (RF) for QT prolongation. Data were gained retrospectively from medical records of patients hospitalized at II. Internal Gastroenterology Clinic of University Hospital Hradec Králové. QT was corrected to QTc using Bazzet (QTcB) and Fridericia (QTcF) formula, QTc >450 ms (men) and QTc >460 ms (women) was considered prolonged. QTc changes before and after pulse therapy were determined as ∆QTc. 325 pulses of MP were administered to 277 patients (66,0 % women), ECG was available in 94,2 % (n=306 from 325). After PT there was significant ∆QTcF 14,6 ms (p <0,001) and ∆QTcB 5,6 ms (p <0,001)....
