Zkušenosti ergoterapeutů s výsledky terapie (a dopad ergoterapie na školní výsledky žáků na 1. stupni ZŠ) / The therapists´ experiences with the results of the occupational therapy (and its impact on school results in children in Primary School)

In the theoretical part of this thesis the term occupational therapy is explained, as is the work of an occupational therapist, the possibilities of his application and the influence of occupational therapy on results of children in Primary School. The goal of this thesis is to collect the findings of occupational therapists and explain how occupational therapy impacts school results. The theoretical part acquaints laymen with the particulars of working as an occupational therapist and suggests the importance of occupational therapy. Dialogue and direct unstructured observation are used as research methods. This is a qualitative survey. Interviews focused mainly on the results of occupational therapy were conducted with professional occupational therapists as a part of the research. Four case studies oriented on younger children were fashioned next. Those studies combined occupational therapy and its influence with school results in children in Primary School. Interviews with occupational therapists, pedagogues and mothers were also part of the case studies. Based on our findings, occupational therapy can be acknowledged as very advantageous therapy with significant impact on school results in children in Primary School. Progress is especially obvious in fine motor skills, which are practised during occupational therapy, and its regular exercising improves the proficiency in children, especially while writing, drawing, painting or doing cut outs. Occupational therapy helps clients improve skills they find difficult to master and it should become an integral part of caring for mentally or physically handicapped children and contribute to achieving better school results.

The aim of this master thesis is to elaborate the issue of the PEDI-CAT test and to evaluate its contribution in the proces of occupational therapy in patients with medulloblastoma. The theoretical part deals with the test itself, its origin, process of standardization and comparison with the older version of the test. The theoretical part further informs about the possibility of using the test in different areas and in different diagnoses. The last part of the theory analyzes the area of occupational therapy in pediatric patients with diagnosis of medulloblastoma. The practical part contains three case reports of pediatric patients diagnosed with medulloblastoma, which make it described the benefits of practical use of the PEDI-CAT test in occupational therapy in this diagnosis. In the appendices of the thesis is a brief guide to the test, translated by the author of the thesis, which can serve as a guide to facilitate the work with the test, for those who will take the test in practice. The contribution of this thesis is to introduce the PEDI-CAT to the professional public in the Czech Republic. Efekt terapie kognitivních funkcí u pacientů s demencí / Impact of a cognitive function therapy on people with dementia

This thesis occupies with a cognitive training efficiency with a focus on memory and attention of dementia diagnosed people, especially people with vascular dementia, mixed dementia or Alzheimer's disease. The theoretical part deals with dementia issues and current knowledge on the field of a cognitive and occupational therapy. The main empirical part goal is a determination of the cognitive training by dementia diagnosed people. The next goal is to find out, how much clients are satisfied with the cognitive function therapy. The cognitive function training is made within a period of 8 weeks in social institutions in Jirkov and Chomutov. The research sample is consisted of 24 clients (at the age of 65 and more) with diagnosed Alzheimer's disease, vascular or mixed dementia, they were divided into an experimental and comparative group. Clients are evaluated before the intervention and after the intervention using Mini-Mental Examination test (MMSE), Trail Making test, part A (TMT:A and Swwartz scale of the therapy evaluation (SOS-10). Statistically significant results of set significance rate (p < 0,05) were found by the test MMSE (0,01 < 0,05) and scale SOS-10 (0,02 < 0,05), but the test TMT:A (0,09 > 0,05) hasn't confirmed therapy efficiency from the reason of the small research sample. As a result of rapid development in Emergency Medicine and Intensive Care the patients with brain damage have better chances of surviving than they had in the past. However the survivors need to deal with severe loss of function and consequences resulting from that. An excellent primary care for patients who suffer from strok is provided in Czech Republic, the rehabilitation on the other hand is insufficient. The main problem is an absence of continuity of care which leads to aneffective outcomes and increase of total costs of treatment The purpose of this diploma theses was to evaluate the possibilities of occupational therapist of interprofessional collaboration in community based rehabilitation - in client's home environment. There are several categories of brain damage including acquired brain injury, genetic brain disorders and brain damage acquired during labor. The goal was to detect basic deficient fields. Results of this thesis based on knowledge from foreign literature as well as own experiencis in the team. Results might be used as basis for designed guidelines to improve situation. Motoric system diseases belong to the most frequent ones in the present days. All age categories are affected. Congenital defects are included and also injuries, inflammations, degenerative illnesses and others. In the Czech republic are many centers gathering patients with the same diseases. However the centers for orthopedic patients are missing. The dissertation is divided into a theoretic and empiric parts. The theoretic one is focused generally on problems with joint replacements, rehabilitation and relation between a medic and a patient. The dissertation was targeted on: 1. To verify if the informing about the subsequent postop nursing care provided in the center for patients undergoing aloplasty comes up to a patient?s expectations and fulfils their needs. 2. To find out if hospital nurses perceive the difference in the patient engagement in the nursing care according to if he had visited the center before or not. 3. To reveal potential problems in the center running from the patient, nurse and doctor point of view. The research part of the dissertation was implemented with a combination of both qualitative and quantitative survey. For quantitative survey was chosen a method of questioning with a form technique for patients (after a hip or a knee aloplasty) in the Czech republic and Austria and also for nurses working in the orthopedic department and intensive care unit in the Pelhřimov hospital. Three hypotheses were established. 1. The given nursing care informations provided in the center for patients undergoing aloplasties fulfil the patient?s needs. 2. The hospital nurses assess a significant difference in the patient?s collaboration between those who had visited the center before and who had not. 3. In the center for patients waiting for aloplasty daily running there exist problems from the nurse?s point of view. As for the qualitative part of survey a method of questioning with the technique of a semistructured dialog with open questions was used. The research file was created by patients after principal joints aloplasties. In this part we have tried to answer three main questions. 1. What is the relationship between both hospital and the center nurse activities and the patient?s contentment in the sphere of his needs? 2. What sorts of informations for the patients in the center are missing? 3. The patients evaluate the informations provided in the center as orientational or as significant for easier postop care managing? The results of the research survey were analysed and taken in schemes. Patients suffer from disturbing their both physical and psychological needs and so the goal of collaboration is a satisfied and fully informed patient. From the results it is apparent that a nurse in the center closely cooperates with a hospital nurse. The survey revealed that the patients in the center are missing no informations. The patients are prepared for the operation, orient themselves in the related problems know what comes with the rehabilitation as an important part of the postop care. To be concluded: patients are prepared for easier postop care managing. On the basis of analysis of respondent?s answers it was found that the patients undergoing a principal joint aloplasty are sufficiently informed. Although hospital nurses and doctors sense the difference between a patient who had visited the center before and who had not the stimulus for subsequent survey could be how the patients perceive the differentness of info provided within the center and out of it. The survey results could be a stimulus for establishing new centers not only within Vysočina but also in Austria. In the relation with the dissertation results and on the basis of info provided in the center when the patients assessed the info as sufficient for them an informing prospectus was created. The dissertation could serve as an informational material for students not only in the nursing profession.

Home assessment is an important part of occupational therapy intervention. It can help improve patients' quality of life in many ways: lower the risk of falls, improve their self-dependence, increase participation etc. Despite that, there is little information about home assessment in Czech literature. There is no overview of home assessment tools nor any study summarizing information about current home assessment practice. The main goal of this thesis was to explore home assessment and home visit practice in the Czech republic using a questionaire-based survey among Czech occupational therapists. The results were to be compared to data gathered from foreign respondents. Theoretical part discusses the topic of the thesis iand presents information on home assessment tools. Chosen ones are described in detail (Cougar, WeHSA, Housing Enabler, SEMAFOR home). Practical part summarizes the results of the questionaire using tables, graphs and statistical analysis. Data from 144 Czech and 135 foreign respondents was gathered and analysed. Background: Bilateral training is an intervention used in patients with hemiparesis. This training is based on the movement of both (bi) sides (lateral) of the body simultaneously. There exist a lot of articles focused on specific methods of bilateral training. However, the used terminology is inconsistent. The aim of this thesis is to describe the neurophysiological background of bimanual and bilateral methods, review and clear out the used terminology, and describe the different techniques included in bilateral training. Methods: Relevant literature was searched in PubMed, ScienceDirect and OVID Evidence Based Reviews. The inclusion criteria were the availability of fulltext, publication date after 1980, English language and a connection with medicine. The searched keywords were bimanual or bilateral training, metod and therapy. Results: From 1021 articles, only 132 articles meet the inclusion criteria. Five main rehabilitation interventions are used in practice: a) cooperative activity of both arms, b) synchronous movement of both arms c) Mirror therapy, d) an activity, where the impaired hand assists the paretic hand and d) robotic therapy or therapy with mechanical devices. Thesis title: Evaluation of effectiveness of using myoelectric prosthesis by people with congenital or acquired amputation of the upper limb Aims: The aim of my thesis is to evaluate the effectiveness of myoelectric prosthesis using by people with upper limb amputation. Find out how effectively users are able to manage individual tasks with myoprosthesis and basic ADL activities. Methodology: The research group consisted of 6 people with upper limb amputation. In this thesis, there were used three methods of data collection. The first method that was applied was a survey, which was used to obtain anamnestic data by the research group. The second method of data collection was the observation of probands during the performance of individual tasks. The third chosen method was measurement by using a special test aimed at evaluation of the effectiveness while using and the involvement of myorpothesis in the locomotor chain against a healthy upper limb. Results: Based on observations and measurements, it was performed that all probands are able to functionally and effectively involve the myoprosthesis in the locomotor chain in the performance of individual tasks. The aim of the thesis is to find out clinical usability of tablet application Cognitive for training of memory and attention of patients after stroke, in occupational therapist's view. Occupational therapists train cognitive functions (for instance memory and attention) of patients in various ways. Therapy using modern technologies, particularly tablet, is involved. Whereas in foreign countries tablets are used in occupational therapy in many means, in the Czech Republic their use is still minimal. The theoretical part contains a summary about this area. The application Cognitive, which is currently being developed, might help change the situation in the Czech Republic. Cognitive application contains two games, one for memory and the other one for attention. Is this application usable in occupational therapist's practice, though? Within the practical part, a study has been created, that found out the usability of new application Cognitive in practice with six patients after stroke.
