Cerebral palsy (CP) is one of the most frequent neurodevelopmental deseases. The brain lesion may disturb upper extremity functioning in children with cerebral palsy (CP). This thesis aims to create a therapeutic program focused on movability of upper limbs in school-aged children with CP and and verify the efficiency of this therapeutic program. Another goal is to consider the applicability of the Jebsen Taylor Test of Hand Function for assessment of hand function in children with CP. The theoretical part deals elaborately with cerebral palsy, therapeutic interventions in children with CP and its efficiency according to the evidence based medicine. This part also deals with standardized assassment tools and pays close attention to the Jebsen Taylor Test of Hand Function (JTTHF) and the Manual Ability Classification System (MACS). The practical part presents six case studies of children with cerebral palsy who attended the occupational therapy three times a week over three weeks. The created therapeutic program includes three areas focused on the bimanual training, grips and grafomotorics. This program was appropriate for four of six children patients. This study concerns the possibilities of ergodiagnostics (pre-work rehabilitation), appraisal diagnostics, in the area of employment of medically handicapped people. It emphasises the value of work and being employed for the medically disadvantaged. It familiarises those interested with this matter with the means of a complete system of rehabilitation where occupational rehabilitation is irreplaceable. The study informs where legislation on (pre-work) ergodiagnostics can be found, through vocational rehabilitation we discover more detailed information and furthermore what a medically disadvantaged person should do to be able to participate actively in ergodiagnostics. It also offers a thorough overview of ergotherapy which plays an important role in diagnostics, particularly ergodiagnostics of medically handicapped people. It leads you through individual evaluations which are currently used by ergotherapists in complete rehabilitation centres where ergodiagnostics is practised. It is not only ergotherapists, however, who participate in vocational rehabilitation but a whole multi-discipline team which cooperates with job centres. The practical part is oriented on the field of ergodiagnostics or else vocational rehabilitation. Two suppositions were made for the research. The first supposition explores how well-informed people with a medical handicap are about ergodiagnostics. The second then assumes that ergodiagnostics plays a significant role in searching for suitable vacancies for the medically handicapped. The study should serve as informative material for all interested in the problem concerned. It should be especially useful to those who work with the medically handicapped and who want to have an integral view of (pre-work) ergodiagnostics, vocational rehabilitation.

Increasingly better living and working conditions, higher quality healthcare and social conditions are resulting in higher life expectancy more than ever. This is called population ageing. Illness or injury in older age causes various and specific problems which affect all personal areas of the elderly such as physical, emotional, social and spiritual status. It is the aim of rehabilitation to improve or restore those areas following personal injury or illness. One of the various professional disciplines contributing to the rehabilitation process is Occcupational Therapy. The main aim of the thesis is to clarify the role of Occupational Therapy in the area of elderly rehabilitation. It also aims to incorporate the World Health Organisation{\crq}s International Classification of Functioning, Disability and Health (ICF) into clinical work of Occupational Therapy and to find its practical use in this discipline.

Funkční elektrická stimulace paretické horní končetiny u pacientů po cévní mozkové příhodě / Functional electrical stimulation of paretic upper extremity in patients after stroke

This study aims to evaluate the effects of functional electrical stimulation on motor and functional abilities of paretic upper limb in patients after stroke. Specifically detects the immediate effect of functional electrostimulation on the increase of the active range of motion, reduction of the hand spasticity degree and the improvement of functional performance. The research covered a total of 6 patients after first stroke in the basin aretria middle cerebral artery. The research sample had a homogeneous distribution of the paresis heaviness from light, moderate, to severe. The research was conducted in total of three weeks, every working day, 60 minutes a day, with 30 minutes of indicated electrostimulation and 30 minutes of testing of monitored parameters. Patients were evaluated using the Modified Frenchay scale, Modified Tardieu scale and the range of motion goniometer. Research showed in 60 observations that the functional electrical stimulation has immediate and significant effect on increasing the active range of dorsal flexion motion of the wrist (p-value 0.043). The average improvement on 95% interval estimation is 0.08. Ženy po operaci karcinomu prsu v produktivním věku a jejich návrat do pracovního procesu z pohledu ergoterapie / Woman after breast cancer surgery in productive age and their return to employment from the view of occupational therapy

Breast cancer in women of productive age is a huge problem of the current population. Approximately 52% of women, which were diagnosed with breast cancer in 2013, were aged 20-64. The aim of this thesis is to discover the influence of surgical solution of breast cancer on women return to their original profession. There were 68 suitable participants, who received surgical treatment, were aged 20-64, with finished treatment and who went back to their original employment. The results show that amongst manually working women, 60,9% return to their original employment, in administrative 78% return, which, in acceptance region of 0,05 (p = 0,142), is not a statisticaly significant difference. Furthermore, it was not found that age and lenght of treatment are connected (p=0,040). It appears that the time it takes to return to employment is significantly connected to the type of operation (acceptance region 0,05, p=0,046). This research shows how important it is for women after breast cancer treatment to receive timely rehabilitational intervention and to return to employment and how this affects their quality of life. The aim of this diploma thesis is to create a working version of the standardized questionnaire Sensory Profile 2 School companion. Another aim is to verify the clinical applicability of this standardized questionnaire by using it to do a screening on school aged children to identify those with a sensory processing disorder within the classroom. The theoretical part of the thesis is focused on the diagnosis of sensory processing disorders (SPD), their diagnostics and the roles of an occupational therapist in SPD. INTRODUCTION: By the age of 65, every driver holding a valid driving license is required to undergo a compulsory medical examination according to Act 361/2000 Coll. The purpose of the examination should be to examine not only physical, but also psychosocial and cognitive factors that can affect the ability to drive safely. However, the extension of the validity of a driving license may have a considerable impact on the preservation of the existing self-sufficiency. GOALS: To determine the relationship of anamnestic data on the state of driving a motor vehicle with the state of cognitive functions. As a secondary goal, the work asks whether there is a correlation between the results of the Montreal cognitive test (MoCA-CZ1) with newly created memory screening tests (POBAV, ALBA and DOZNAT). METHODOLOGY: The research was based on a qualitative examination of the 39 drivers older than 65 years. Drivers were assessed by the state of cognitive function according to the Montreal Cognitive Test, and were then examined by "new short cognitive tests" targeting different types of memory. The Action Research Arm Test (ARAT) is a standardized tool, which is used for fine and gross motor skills evaluation of hemiparetic patients after stroke. It is widely used abroad thanks to its excellent psychometric properties and easy administration. It is also used in many foreign studies to evaluate the function of upper extremity, especially for neurological patients. Unfortunately, so far it has been used only sporadically in the Czech Republic. In the theoretical part of this thesis the ARAT and the Modified Frenchay Scale (referred as "MFS") is described. In the practical part, the MFS is compared with the ARAT. The main aim of this diploma thesis is a pilot comparison of sensitivity of upper limb motor skills evaluation done by ARAT and MFS. Thirty patients after stroke were tested in the Rehabilitation Center Kladruby in three weeks. They were further divided into three intervention groups according to the severity of the paresis (light, medium, heavy). The analysis of the results shows that the sensitivity of both diagnostic tools is statistically comparable. According to the Spearman correlation coefficient, the ARAT and the MFS correlate very strongly in range of 0.96-0.98. The thesis aims at the elaboration of a business plan for establishing an Occupational Therapy private practice, which would provide Occupational Therapy service and rehabilitation for people with lasting functional deficit in their own social environment or in an Occupational Therapy outpatient department. The theoretic part focuses on the theoretic basis of business, requirements for establishing of a private practice and concept of strategic management and analysis.
