Use of formative research to inform family-based approaches to prevent prescription opioid misuse among MississippiansRobertson, Mary Nelson 13 December 2019 (has links)
The United States is in the midst of an opioid epidemic, which is leading to approximately 130 deaths each day. While research on family-based approaches for substance misuse prevention, such as alcohol and tobacco prevention, has been conducted, few if any studies have focused on prescription opioid misuse prevention. Previous literature suggests that a comprehensive family-based approach can be effective in preventing substance misuse at the family-level. Considering the multiple age groups the sandwich generation cares for, the sandwich generation may have greater access to reaching multiple age groups to prevent prescription opioid misuse. Therefore, the purpose of this study is to use formative research findings to inform family-based approaches focused on preventing opioid misuse. A dual method approach that includes qualitative focus groups and quantitative surveys is used to explore adults’ perceptions of prescription opioid misuse, factors perceived as influencing opioid misuse prevention, and perceived predictors of prescription opioid misuse prevention. Participants were adults, 30 to 59 years of age, which is the average age range of the sandwich generation. Extension agents recruited focus group participants (n = 55) and Qualtrics recruited survey participants (n = 335) for this study. Focus group transcripts were coded based on common ideas that arose during the focus groups, previous literature, and the PRECEDE-PROCEED model. Focus group findings indicate that participants view the opioid crisis as a family problem, in which they have a role in preventing, and identified predisposing, reinforcing, and enabling factors that influence whether family members take a role in preventing prescription opioid misuse. Univariate frequencies and multiple linear regression analyses results of the survey data indicate that Theory of Planned Behavior determinants are predictive of the intention to talk about opioids with friends and family. In addition, comfort predicts intention to talk about opioids with friends and family, suggesting that Theory of Planned Behavior determinants and comfort predict intention. Extension agents, family life educators, and other community-health professionals can collaborate and use these findings to develop family-based approaches, such as family communication training and brief strategic family therapy, combined with community-based approaches such as motivational interviewing and media campaigns.

L'hospitalisation de la femme en raison d'une grossesse à risque élevé : l'expérience de couplesLavallée, Émilie 09 1900 (has links)
Attendre un enfant est généralement un événement heureux pour un couple. Toutefois, lorsqu’une hospitalisation de la femme survient en raison d’une grossesse à risque élevé, l’expérience des futurs parents peut se transformer en véritable cauchemar. Jusqu’à maintenant, les savoirs disponibles suggèrent que cet événement imprévu ait de nombreux impacts sur les futurs parents. Néanmoins, ce sujet n’a été abordé que selon une perspective maternelle ou paternelle, sans s’intéresser au caractère systémique de la situation. Cette étude qualitative de cas multiples propose d’explorer l’expérience de couples vivant l’hospitalisation de la femme en raison d’une grossesse à risque élevé, dans une perspective systémique et constructiviste. Des entrevues semi-dirigées ont été réalisées auprès de 7 couples dont la femme était, au moment de l’entrevue, hospitalisée en raison d’une grossesse à risque élevé. L’analyse qualitative des données s’est inspirée de la thématisation et a tenu compte des cadres théoriques et épistémologique choisis, étant respectivement l’approche systémique familiale de Wright et Leahey (2013) et le constructivisme. Les résultats suggèrent que l’hospitalisation est marquée par l’intensité des émotions ressenties. Les futurs parents ressentent presque constamment des sentiments d’inquiétude, d’incertitude et de solitude. Il est toutefois intéressant de constater que le couple lui-même subit des changements au cours de l’hospitalisation. En ce sens, cet événement requiert une immense réorganisation, laquelle touche plusieurs systèmes gravitant autour du système conjugal. Enfin, afin de faire face à l’intensité et à l’imprévisibilité de la situation, les couples sont amenés à solliciter un soutien externe et à puiser dans leurs propres ressources internes. En lien avec les cadres théoriques et épistémologiques choisis, ces résultats conviennent de la nécessité d’adopter une vision systémique et contextuelle afin de développer des soins infirmiers concordants avec l’expérience des couples. Cela ramène à la nécessité de réaliser d’autres études afin de perfectionner la compréhension de l’expérience. Certaines pistes d’interventions infirmières sont néanmoins énoncées, afin d’amener les infirmières à considérer la situation du point de vue de l’expérience conjugale, dont les défis de réorganisations extérieurs et des relations conjugales. Enfin, il est proposé que l’infirmière puisse agir afin de renforcer la capacité des couples à composer avec les conséquences de l’hospitalisation anténatale. / Planning to have a child is most of the time a joyful event for a couple. However, the same event might becomes a real nightmare when the woman is hospitalized because of an at high risk pregnancy. To date, researches exploring this abrupt event have suggested that it may conduct to numerous impacts on future parents. Even though, the subject has only been explored using a maternal or paternal perspective, without considering the systemic aspect of the situation. This qualitative multiple case study suggests to explore the couples’ experience regarding the hospitalization of the woman because of an at high risk pregnancy, in a systemic and constructivist approach. Semi-directed interviews have been conducted with seven couples with a woman hospitalized because of an at high risk pregnancy, at the moment of the interview. Qualitative data analysis was inspired of thematization and took into account the theoretical and epistemological approach, respectively the Wright and Leahey (2013) family approach and constructivism. Results suggest the hospitalization is touched by intense emotions, such as feeling almost all the time worries, uncertainties and loneliness. It is however interesting to note that even the couple himself is touched by changes resulting of the hospitalization. According to that, this event requires a huge reorganization, which implies loads of systems gravitating around the couple’s system. Lastly, in order to face the intensity and the unpredictability of the situation, couples are asking for external support, while using their own internal resources. Concerning the theoretical and epistemological approach chosen to realize this study, results confirm the necessity of adopting a systemic and contextual vision in order to develop nursing cares appropriate with couples’ experience. Besides, it becomes essential to duplicate this type of study to perfection the actual comprehension of the event. However, nursing interventions relating to the external reorganization and the couple relation reorganization are proposed, in order to reinforce nurses’ consideration of the couple’s experience. Only then, it is suggested that nurses can influence couples’ capacity to face the consequences of the antenatal hospitalization.

Previous issue date: 2016-10-26 / The Family Health Strategy (FHS), reorganizing axis of the Unified Health System, recommends in its actions that the health teams, among other things, to target the focus of attention for families and use technology / appropriate tools to work with families. The purpose of this study was to understand the practice of FHS professionals in the perspective of the use of family approach tools. This is a descriptive study with a qualitative approach. The empirical material was obtained through semi-structured interviews with characterization of questions of the participants and the knowledge and practices about the family approach tools. Graduated professionals attended the study who compose the FHS teams in a health facility. Data were collected in September 2016. The technique of Content Analysis was used, resulting in thematic categories: the concept of family, focus of attention, training to work with families and knowledge and practice of the family approach tools. It was observed in the study that all the professionals were female, had average time of action in the strategy of 13 years, most of the participants had a specialization course in family health. Professionals showed the predominant form, a concept of family is not restricted to consanguinity, they reported that although hearken to families, in practice the focus of attention is still the individual reported that the training and further training, were not content that addressed the family assessment, as the knowledge and use of tools approach: genogram, ecomap and family life cycle, all said to ignore and not use such tools. The study indicates the need for qualification and instrumentalization of professionals and the importance and use of familiar tools approach, which could address the identified weaknesses, contributing to the improvement of attention and care to families and communities. / A Estratégia Saúde da Família (ESF), eixo de reorganização do Sistema Único de Saúde, preconiza em sua atuação que as equipes de saúde, dentre outros aspectos, direcionem o foco da atenção para as famílias e utilizem tecnologias/ ferramentas apropriadas para o trabalho com famílias. Este estudo, teve por objetivo compreender a prática dos profissionais da ESF na perspectiva do uso das ferramentas de abordagem familiar. Trata-se de um estudo descritivo, com abordagem qualitativa. O material empírico foi obtido, por meio de entrevistas semiestruturadas com questões de caracterização dos participantes e sobre o conhecimento e práticas a cerca das ferramentas de abordagem familiar. Participaram deste estudo, profissionais de nível superior que compõem as equipes da ESF de uma unidade básica de saúde. Os dados foram coletados em setembro de 2016.Utilizou-se a técnica de Análise de Conteúdo obtendo como resultado as categorias temáticas: conceito de família, foco da atenção, capacitação para trabalhar com famílias e conhecimento e prática sobre as ferramentas de abordagem familiar. Observou-se no estudo, que todos os profissionais eram do sexo feminino, tinham tempo médio de atuação na estratégia de 13 anos, a maioria dos participantes possuía curso de especialização em saúde da família. Os profissionais, apresentaram de forma predominante, um conceito de família não restrito a consanguinidade, relataram que, apesar de atentarem para as famílias, na prática o foco da atenção ainda é o indivíduo, informaram que na formação e nos treinamentos posteriores, não tiveram conteúdos que contemplassem a avaliação familiar, quanto ao conhecimento e utilização das ferramentas de abordagem: genograma, ecomapa e ciclo vital familiar, todos afirmaram desconhecer e não utilizar as referidas ferramentas. O estudo indica a necessidade de qualificação e instrumentalização dos profissionais quanto a importância e utilização das ferramentas de abordagem familiar, o que poderá suprir as fragilidades identificadas, contribuindo para a melhoria da atenção e cuidado às famílias e comunidades.

Processus de facilitation d'une pratique réflexive, de type codéveloppement, auprès d'infirmières pivots en oncologie et son influence sur leur pratique professionnelleTurcotte, Annie 07 1900 (has links)
Deux catégories de compétences influenceraient toute pratique professionnelle, les compétences disciplinaires et relationnelles. Selon le cadre théorique de ce projet, la pratique réflexive (PR) aiderait le professionnel à développer les compétences relationnelles, grâce à l’approfondissement de sa conscience et de son autocritique des composantes tacites de sa pratique, telles que ses valeurs et principes, ses perceptions et ses stratégies d’action, afin de les rendre plus efficaces et satisfaisantes lors de la résolution de problèmes. Dans cette étude, la PR souhaitait aider un groupe d’infirmières pivots en oncologie (IPO) à réfléchir et agir sur les composantes de leur pratique qui contribuaient à certaines difficultés relationnelles persistantes vécues auprès de gestionnaires, collègues et familles et qui, selon les IPO, avaient un impact sur l’expérience de santé de familles touchées par le cancer.
Toutefois, la recension des écrits effectuée dans cette recherche soutient qu’il y a un nombre insuffisant d’études qui a trait à l’influence d’une PR sur la pratique infirmière et, plus encore, sur le développement des compétences relationnelles des infirmières. Les écueils théoriques et méthodologiques suivants ont entre autres été rapportés: la PR étudiée ne reposait pas toujours sur un modèle théorique clair et le but poursuivi par la PR n’était pas toujours énoncé; aucune recherche n’a documenté de quelle manière la PR s’était développée et, une fois acquise, de quelle façon elle avait contribué aux résultats; et, enfin, aucune étude recensée n’a exploré l’influence d’une PR intégrée par l’infirmière, en tenant compte de la perspective de familles qui bénéficiaient de leurs soins.
Considérant ces principaux écueils, cette étude a privilégié la recherche-action participative afin de documenter la facilitation d’une PR et son influence sur les compétences relationnelles d’IPO et sur d’autres dimensions à la fois personnelles, organisationnelles et cliniques. Six IPO ont pris part à l’intervention de l’étude d’une durée d’un an. Cinq collègues des équipes de soins et douze membres de familles touchées par le cancer ont participé à l’évaluation de son influence.
Les résultats de cette recherche portent à croire que les IPO qui développent une PR plus profonde, c’est-à-dire jusqu’à leurs sentiments et besoins internes, parviennent à établir des relations plus mutuelles auprès de collègues et de familles, grâce à des changements importants réalisés sur le plan de leurs valeurs et principes, de leurs perceptions et de leurs stratégies d’action. Les résultats de l’étude soutiennent aussi que ces changements au sein de la pratique infirmière génèrent des effets positifs. Les IPO ont parlé d’une plus grande satisfaction liée à leur travail et de bienfaits sur leur santé. La majorité des IPO et des participantes-collègues de l’étude ont rapporté un climat de travail plus calme et des processus d’équipe plus satisfaisants lors, par exemple, de la résolution de problèmes. According to the conceptual model of this study, reflective practice (RP) helps the professional develop the latter, by way of a deepening of consciousness and a self-critique of the tacit components of one’s practice, such as one’s values and beliefs, perceptions and action strategies, in order to render them more effective and satisfying when problem solving. In this study, RP sought to help a group of oncology pivot nurses (OPNs) reflect and act on the components of their practice that contributed to certain persistent relational difficulties experienced with nursing leaders, colleagues and families, which in turn, they felt had an impact on the health experience of families affected by cancer.
The literature review conducted in this study however, supports that there are an insufficient number of studies regarding the influence of RP on nursing practice and on the development of relational competencies by nurses. The following theoretical and methodological gaps have, amongst others, been reported: the RP studied was not always based on a clear conceptual model and the aim sought by RP wasn`t always stated; no study has documented in what way RP had been developed and, once acquired, in what manner it had contributed to the results; and finally, none of the identified studies explored the influence of the integration of RP by the nurse, and took into account the perspectives of the families that benefited from their care.
Taking these gaps into consideration, this study used a participatory action research approach in order to document the facilitation of RP and its influence on the relational competencies of OPNs’ as well as on other areas like personal, organizational and clinical ones. Six OPNs took part in the intervention of the study for a one year period. Five colleagues from care teams and a dozen family members affected by cancer participated in the evaluation of its influence.
The study results suggest that the OPNs who develop a deeper RP, that is one that involves their feelings and internal needs, are able to establish more mutual relationships with colleagues and families because of important changes made with regards to their values and principles, perceptions and action strategies. The results of the study also support that these changes in nursing practice yielded positive effects. The OPNs spoke about improved work related satisfaction and of benefits on their health. The majority of OPNs and participant colleagues in the study reported a calmer work climate and more satisfactory team processes, for example, when problem-solving. Finally, when OPNs integrate a more mutual approach, families reported a reduction of their concerns and of their psychological distress, a greater capacity to make sense of their cancer experience, a higher quality of communication within their family or with the health care team.

Research has indicated that offering support and services for people who experience mental health problems and their families is a complex and contested area. Despite the controversies surrounding therapeutic interventions with families, it has now been recognised that relatives and other supporters of people with mental health problems should be included in their care. Whole- family interventions and partnership working with carers and families is now central to secondary care UK mental health policies and clinical practice guidelines. However, for many families/ carers this remains an aspiration rather than a reality. The way in which we successfully developed family focused mental health practice, as well as specialist family interventions (FI) for people who have been given a diagnosis of psychosis, has therefore aroused considerable interest. The Somerset Partnership NHS Foundation Trust has adopted a Strategy to Enhance Working Partnerships with Carers and Families, developed best practice guidance and has established two complementary workforce development projects - the development of specialist family intervention services and the widespread training of mental health staff to create a ‘triangle of care’ with service users and their families. This has resulted in widespread adoption of systemically informed, ‘whole-family’ practice. In response to the widespread difficulties experienced following other staff- training initiatives we developed specialist family interventions (FI) services by means of an innovative one-year course delivered in partnership with Plymouth University. This training initiative has been widely acknowledged for its novel integration of psycho-educational and systemic approaches and the effective in-situ, multi-disciplinary service development model. An advantage of this approach is that by the end of the course a local FI Service has been established and staff experience fewer difficulties in applying their new skills than people trained in other programmes. We then ensure the continued development of clinical skills by means of a service structure that emphasises on-going supervision. Regular audits of the service and in-depth research studies clearly indicate that the service is effective and highly valued by users. Our ‘cognitive-interactional’ approach, which integrates systemic therapy with psychosocial interventions (individual- and family-CBT) within a collaborative therapeutic relationship, enables us to meet the needs of families in a flexible, tailored manner. The FI teams are able to deliver early interventions for people with first episode psychosis, as well as meeting the NICE guidelines for people with longstanding symptoms. Recognising that many families do not require formal family interventions/ therapy, we also have been designing ‘stepped-care’ family intervention services. We have developed, and extensively evaluated, short training packages to enhance working partnerships with families throughout our mental health services. We have used this three-day package to train a range of community and inpatient teams. We have also encouraged family- inclusive practice with the establishment of a trustwide steering group, practice guidelines and the establishment of ‘family liaison’ posts to facilitate family meetings on inpatient units, as part of the assessment process. Both training initiatives explicitly focus on developing systemic thinking, by integrating CBT and systemic therapy. The involvement of families/ carers in the design and delivery of both training initiatives is also crucial.
