Diabetic bullae: A case series and a new model of surgical management

No / Bullosis diabeticorum is considered a rare skin manifestation of diabetes mellitus. Tense blisters appear rapidly, mostly on the feet, the cause of which is unclear, with multiple pathophysiologies hypothesised. This is a retrospective review of 4 diabetic patients who presented over six months with diabetic bullae; the condition may therefore not be as rare as commonly believed. All the patients had early surgical debridement followed by topical negative pressure wound dressings. A multidisciplinary team that included vascular surgeons, diabetologists, diabetic foot care team, wound care team, physiotherapists and occupational therapists managed the patients and none of them required amputations. We propose an alternative way of managing these patients with early surgical debridement followed by topical negative pressure wound dressing.

Title: Effect of therapeutic physical program in patients with type 2 diabetes mellitus Objectives: The aim of my work was to examine the effect of 12 weeks therapeutic physical program on distribution of plantar pressure, muscle strength in plantar flexion, ankle range of motion and physical abilitity by patients with type 2 diabetes mellitus and diabetic foot syndrome. Methods: It was a quantitative research, randomized controlled trials. It was chosen 10 patients with type 2 diabetes mellitus and with diabetic foot syndrome. The patients were devided into 2 groups. In each group were four males and one female, aged from 54 to 73 years old. The first group included exercise patients and the second group nonexercise patients. The intervention took place 5 - 7 times a week and contained two types of training units. The first had 11 exercises, the second self mobilization of leg joint and walking. Basic anthropometric figures - height, weight, girth of waist and hipline (tailoring meter,weighing machine), ankle range of motion (goniometer), muscle strenght in plantar flexion (dynamometer), pressure distribution in stand (MobileMat), physical ability (Senior Fitness Test) were measured and compared in two groups after 12 weeks. As lesões no pé diabético é uma das complicações mais devastadoras do Diabetes Mellitus, devido a altas taxas de morbi-mortalidade e que provocam seqüelas de incapacidade causando amputações de membros inferiores. O objetivo deste estudo foi identificar os fatores de risco em trabalhadores portadores dessa doença, quanto aos aspectos sócio-demográficos, clínicos e o grau de risco para o desenvolvimento de lesões. Os dados foram coletados com 117 trabalhadores, de um ambulatório que pertence a uma Instituição Pública de Londrina-PR, por meio de um roteiro semiestruturado. Os resultados demonstraram predomínio de que 53% dos trabalhadores com idade de 51 a 60 anos, do sexo masculino (52,1%), casados (71,8%), os quais, em grande parte, trabalham em serviços gerais (36,8%) e com mais de 21 anos de tempo de serviço (32,5%). Quanto aos dados clínicos, o tempo de diagnóstico de 2 a 5 anos (45,3%), a maioria dos trabalhadores são diabéticos tipo 2 (98,3%) e fazem uso de antidiabéticos oral (57,3%), 55,6% são hipertensos. Entre as complicações, 40.8% tiveram acuidade visual diminuída, a grande maioria não fumam (79,5%) e não bebem (85,5%). Quanto ao índice de massa corporal (52,1%) tinham sobrepeso. No exame clínico dos pés os trabalhadores apresentaram boas condições de higiene (80,3%), faziam uso de calçados adequados (75,2%). A maioria dos trabalhadores não apresentaram comprometimentos graves relacionados a doença vascular periférica com presença de pulsação pedioso (88,5%), tibial posterior (74,1%) e apresentaram boa perfusão periférica (88,9%). Quanto à neuropatia diabética, a maioria apresentou reflexos positivos para aquileu (93,2%) e patelar (93,2%), e para sensibilidade vibratória (93,2%), protetora (100%), térmica (87,2%), dolorosa (87,2%) e tátil positiva (98,9%). A presença de úlcera se fizeram presentes em 4,27% dos trabalhadores sendo que 3,41% sofreram amputações a nível falangiano. Quanto ao grau de risco encontram-se 71,8% em grau 0. They reveal senior workers' prevalence (53%), male (52,1%), married (71,8%), the ones which, largely, they work in general services (36,8%) and he/she has more than 21 years of time of service (32,5%). As for the clinical data, his/her time of diagnosis has from 2 to 5 years (45,3%), most is diabetic type 2 (98,3%) and they make use of antidiabetic oral (57,3%), 55,6% are hipertensos. Among the current complications of the referred diabetes, 40.8% were affected in the visual sharpness, the great majority doesn't smoke (79,5%) and they don't drink (85,5%). As for the index of corporal mass (52,1%) they are overweight. In the clinical exam of the feet the workers are in good hygiene conditions (80,3%), they make use of appropriate shoes (75,2%). They still don't present related serious compromisings the outlying vascular disease with presence of pulsation pedioso (88,5%), subsequent tibial (74,1%) and good outlying perfusion (88,9%). As for the diabetic neuropatia, most presents positive reflexes for aquileu (93,2%) and patelar (93,2%), and for vibratory sensibility (93,2%), protecting (100%), thermal (87,2%), painful (87,2%) and tactile positive (98,9%).
