Caracterização de um grupo de pacientes em risco para câncer de mama e ovário hereditários quanto a presença e frequência de rearranjos gênicos em BRCA

O câncer de mama é uma das neoplasias malignas mais comuns que afetam mulheres de todo o mundo. No Brasil, o Estado do Rio Grande do Sul tem índices de incidência e mortalidade por câncer de mama que situam-se entre os maiores do país. Aproximadamente 5-10% dos diagnósticos são causados por mutações germinativas em genes de predisposição entre os quais estão BRCA1 e BRCA2, associados à Síndrome de Câncer de mama e Ovário Hereditários (Hereditary Breast and Ovarian Cancer Syndrome ou HBOC, OMIM #114480).A identificação dos casos hereditários de câncer de mama é importante porque indivíduos afetados apresentam risco cumulativo vital muito superior ao da população para o desenvolvimento de câncer, porque familiares de um afetado podem estar igualmente em risco porque há medidas de rastreamento intensivo e intervenções preventivas que podem diminuir significativamente o risco de câncer em portadores de mutação. O diagnóstico molecular da síndrome HBOC é laborioso e caro devido à heterogeneidade molecular da doença. Famílias que apresentam características indicativas de uma síndrome de predisposição ao câncer de mama e ovário hereditários, mas que são negativas para mutações pontuais em BRCA1/2 vêm sendo testadas para grandes rearranjos visto que essas anormalidades têm sido consideradas como respondendo por, no mínimo, 10% do todos os casos HBOC com mutação identificável, incluindo grandes deleções ou duplicações. Um estudo recente de Portugal, demonstrou que um rearranjo fundador no exon 3 de BRCA2 ocorre em por 8% das famílias HBOC do Norte do país. Os objetivos deste trabalho incluíram a verificação da freqüência e caracterização de rearranjos gênicos nos genes BRCA1 e BRCA2, incluindo a mutação fundadora c.156_157insAlu no exon 3 de BRCA2 em famílias brasileiras dealto risco para a síndrome HBOC. Em um grupo de 145 indivíduos em risco nãorelacionados rastreados para a mutação fundadorac.156_157insAlu no exon 3 de BRCA2 foram encontrados 3 portadores da mutação (prevalência de 2%). Em um grupo de 145 indivíduos de risco não-relacionados rastreados para rearranjos gênicos em BRCA1 e BRCA2 pela técnica de MLPA (multiplex ligation-dependent probe amplification) foram identificados 4 portadores de mutação germinativa, sendo a mutação em dois deles um rearranjo gênico no gene BRCA1 (1,4%) envolvendo sequencias Alu. Rearranjos gênicos em BRCA1 e BRCA2 são responsáveis por uma parcela das mutações em famílias HBOC Brasileiras. O presente estudo, envolvendo uma série grande de famílias com o fenótipo da síndrome HBOC, não identificou novos rearranjos fundadores, no entanto, demonstrou a presença de rearranjos tanto em BRCA1 quanto em BRCA2, reiterando a importância da busca ativa por estas alterações, que dificilmente são identificadas por técnicas convencionais de sequenciamento gênico. A técnica de MLPA associada a um protocolo específico para detecção da mutação fundadora Portuguesa c.156_157insAlu podem ser utilizadas como estratégia inicial de rastreamento de mutações em famílias Brasileiras com a síndrome. Families that have characteristics indicative of a cancer predisposition syndrome of hereditary breast and ovarian cancers, but are negative for mutations in BRCA1/2 have been tested for large rearrangements because these abnormalities have been identified as accounting for at least 10 % of all cases HBOC identifiable mutation, including large deletions or duplications. A recent study from Portugal, the founder showed that a rearrangement in exon 3 of BRCA2 occurs in 8% of HBOC families of the north. The objectives of this work included the verification of the frequency and characterization of gene rearrangements in BRCA1 and BRCA2 genes, including c.156_157insAlu founder mutation in exon 3 of BRCA2 mutations in Brazilian families at high risk for HBOC syndrome. In a group of 145 individuals at risk unrelated traced to c.156_157insAlu founder mutation in exon 3 of 3 found BRCA2 mutation carriers (prevalence 2%). In a group of 145 individuals at risk unrelated screened for gene rearrangements in BRCA1 and BRCA2 by the technique of MLPA (multiplex ligationdependent probe amplification) identified four carriers of germline mutation, and two of the mutation in a gene rearrangement in the gene BRCA1 (1.4%) involving Alu sequences. Gene rearrangements in BRCA1 and BRCA2 account for a portion of HBOC mutations in Brazilian families. This study, involving a large series of families with HBOC syndrome phenotype, no new rearrangements identified founders, however, showed the presence of rearrangements in both BRCA1 and BRCA2, reiterating the importance of active search for these changes, which hardly are identified by conventional techniques of gene sequencing. The technique of MLPA protocol associated with a specific mutation detection founder Portuguese c.156_157insAlu strategy can be used as initial screening for mutations in families with Brazilian syndrome. The results presented here, however, indicate mutations that will be identified in less than 10% of the cases using this strategy.

Genomic Rearrangements in Autism Spectrum Disorders: Identification of Novel Candidate Genes

There is evidence from family studies for the importance of genetic factors in the development of autism spectrum disorders (ASDs) but the identification of major genes has not been achieved to date. There are several reports of deletions and duplications in individuals with ASDs, some of which are not unique to an individual. In most cases, the frequencies and relevance of these abnormalities are unknown, as they have been identified serendipitously in one or a few individuals. My overall hypothesis was that such rearrangements would facilitate the identification of “culprit” genes associated with ASDs by identifying a small chromosomal region for candidate gene testing. I molecularly characterized two overlapping 2p15-2p16.1 deletions detected in unrelated individuals with confirmed autistic disorder (Subject 1) or autistic features (Subject 2), a 1.4Mb deletion on chromosome Xp22 (Subject 1) and a duplication of chromosome 7q11.23, reciprocal to the Williams-Beuren Syndrome (WBS) deletion, in one individual with an ASD (Subject 3). Using real-time semi-quantitative PCR, I screened a total of 798 individuals with an ASD and 186 healthy controls for the presence of similar abnormalities. No additional cases were identified in either group. Subsequently, I selected 6 genes [Orthodenticle homolog 1 (OTX1), Variable charge, X-linked (VCX), Neuroligin 4, X-linked (NLGN4X), Syntaxin 1A (STX1A), Cytoplasmic linker 2 (CYLN2) and General transcription factor IIi (GTF2i)], based on their function and localization within or in the vicinity of the rearrangements and tested them for association with ASDs. Although there was no evidence for association for any marker or haplotype in most of the genes tested, this was not so for GTF2i. Haplotype transmission disequilibrium testing revealed an increased transmission, from healthy parents to their affected offspring, of the common alleles of one marker and one haplotype in GTF2i (P = 0.0010 and 0.0005, respectively). This gene encodes a brain-expressed transcription factor previously implicated in the mental retardation associated with WBS. Based on these findings, I propose that, although the genomic rearrangements reported herein are not a common cause of ASDs, the GTF2i gene within the WBS critical region is important in the aetiology of autism. / Thesis (Ph.D, Physiology) -- Queen's University, 2009-11-20 00:35:11.727

Etude clinique et génétique de l’albinisme oculocutané : développement d’outils de diagnostic moléculaire et recherche de nouveaux gènes / Clinical and molecular study of oculocutaneous albinism : development of molecular diagnosis tools and search for new genes

Notre travail s’est intéressé à l’albinisme oculocutané en étudiant ses aspects clinico- moléculaires. Malgré l’analyse approfondie des gènes connus d’albinisme oculocutané, 15 % des patients restent sans mutation identifiée indiquant que les mutations sont situées dans des régions géniques non analysées par les techniques classiques de diagnostic moléculaire, ou qu’il existe d’autres gènes d’albinisme oculocutané. Nous avons établi une base de données clinico- biologiques décrivant les caractéristiques de plus de 400 patients analysés. Des outils de diagnostic moléculaire ont été développés à la recherche de mutations situées dans les introns et les régions régulatrices et de réarrangements géniques. Différentes stratégies ont également été utilisées pour rechercher des gènes candidats. La puce à façon a permis l’identification de grands réarrangements dans les gènes TYR, OCA2 et SLC45A2 et un réarrangement complexe du gène OCA2 chez 2 patients non apparentés. L'analyse de gènes candidats nous a permis d'identifier, chez 5 patients non apparentés présentant un albinisme oculocutané non syndromique, des mutations dans le gène SLC24A5, très récemment associé à l’AOC6. Le séquençage d’exome de 6 patients a mis en évidence des gènes candidats pour lesquels des analyses complémentaires sont poursuivies afin de confirmer leur implication dans la pathogenèse de l’AOC.Les résultats de ce travail permettent de redéfinir les aspects cliniques et moléculaires de l’AOC, d’identifier de nouveaux mécanismes moléculaires à l’origine de l’AOC ainsi que des gènes candidats dont la fonction dans le développement pigmentaire reste à élucider. L’identification de nouveaux gènes impliqués dans l’AOC pourrait permettre de mieux comprendre et de mieux prendre en charge les patients avec un AOC. / Our work focused on oculocutaneous albinism (OCA) by studying its clinical and molecular aspects. Despite a thorough analysis of the known genes involved in oculocutaneous albinism, 15% of patients remain without diagnostic at the molecular level indicating that mutations are located in unexplored regions and are undetected by standard techniques or that other genes are involved in albinism. We established a clinicomolecular database describing more than 400 patients and developped molecular tools in order to improve molecular diagnostic including a custom high resolution array-CGH dedicated to the four OCA genes (TYR, OCA2, TYRP1 and SLC45A2). We also used different strategies to identify new genes. Array-CGH allows us to detect large deletion in TYR, OCA2 and SLC45A2 and a complexe rearrangement in OCA2 in 2 unrelated patients. We identified, in 5 patients presenting with a non syndromic OCA, mutations in SLC24A5, recently associated with OCA6. Exome sequencing of 6 different patients allows us to identify candidate genes, for which further studies are required to confirm their involvement in OCA pathogenesis. The results of this work allowed us to delineate clinical and genetics aspects of more than 400 OCA patients and to identify new molecular mechanisms leading to OCA and candidates genes for which exact nature of their functions has to be understood. Giving the complexity of pigmentary system development and its regulation, identification of new genes leading to OCA could help to better understand OCA and take care of patients

Comme tous les ciliés, P. tetraurelia réarrange son génome à chaque génération sexuelle pendant le développement de son macronoyau somatique ¿ partir du micronoyau germinal. Les réarrangements incluent l’excision précise de courtes séquences dérivant de transposons et appelés IES (Internal Eliminated Sequences) dont la majorité sont intragéniques. L’excision d’une fraction d’entre elles dépend de petits ARN maternels (appelés scnARN) qui sont produits à partir de tout le génome germinal pendant la méiose. Ce mécanisme pose un problème lors d’une conjugaison entre deux souches présentant des polymorphismes d’insertion : une cellule sera théoriquement incapable d’exciser une IES portée par l’allèle paternel reçu si cette IES est absente de l’allèle maternel ou si la séquence est trop divergente. Mes résultats montrent cependant que les allèles paternels divergents sont correctement excisés en utilisant les scnARN produit par la cellule paternelle. Dans le cas d’un polymorphisme absence/présence, l’IES que j’ai étudié est excisée chez 70 % des hétérozygotes F1, également via les scnARN paternels. Nous avons exploré deux hypothèses pour expliquer comment ils pouvaient agir. Il pourrait s’agir d’une programmation précoce des noyaux gamétiques ou alors d’un échange cytoplasmique des scnARN. Finalement, j’ai montré qu’un défaut de scnARN maternels n’est pas une cause possible de dysgénésie hybride. Cependant, 30 % des hétérozygotes F1 présentent une rétention variable de l’IES étudié via un mécanisme inconnu. Si cela est généralisable à toutes les IES homozygotes, alors ce mécanisme aurait un effet délétère sérieux sur les F1 et pourrait contribuer à l’isolement reproductif. / Like all ciliates, P. tetraurelia entirely rearranges its genome during development of the somatic macronucleus from the germline micronucleus, in each sexual generation. Rearrangements include the precise excision of IESs (Internal Eliminated Sequences), single-copy intervening sequences likely derived from transposon insertions. At least for a fraction of IESs, correct excision, which is required to reconstitute functional genes in the macronucleus, is thought to depend on their recognition by Piwi-bound small RNAs (called scnRNAs) produced from the maternal germline genome during meiosis. This raises a problem during conjugation between strains presenting insertion polymorphisms: a cell will be theoretically unable to excise an IES from the incoming (paternal) allele if that IES is absent from the maternal allele, or if its sequence is too divergent. Our results, however, indicate that divergent paternal alleles are correctly rearranged, using scnRNAs produced by the paternal cell. In the case of an absence/presence polymorphism, the IES we studied is excised in 70% of heterozygotes, also using paternal scnRNAs. We explored two hypotheses to explain how they can act. It could be either an early programming of the gametic nuclei or through cytoplasmic exchange of scnRNAs. My results seem to favor the latter. Overall, I showed that the lack of maternal scnRNAs is not a possible cause of hybrid dysgenesis. However, 30% of heterozygous F1 display a variable retention of the IES through an unknown mechanism. If this is true for all hemizygous IESs then it will have a strong deleterious effect on hybrid F1s and may contribute to reproductive isolation.

Évolution des génomes par mutations locales et globales : une approche d’alignement

Durant leur évolution, les génomes accumulent des mutations pouvant affecter d’un nucléotide à plusieurs gènes. Les modifications au niveau du nombre et de l’organisation des gènes dans les génomes sont dues à des mutations globales, telles que les duplications, les pertes et les réarrangements. En comparant les ordres de gènes des génomes, il est possible d’inférer les événements évolutifs les plus fréquents, le contenu en gènes des espèces ancestrales ainsi que les histoires évolutives ayant menées aux ordres observés. Dans cette thèse, nous nous intéressons au développement de nouvelles méthodes algorithmiques, par approche d’alignement, afin d’analyser ces différents aspects de l’évolution des génomes. Nous nous intéressons à la comparaison de deux ou d’un ensemble de génomes reliés par une phylogénie, en tenant compte des mutations globales. Pour commencer, nous étudions la complexité théorique de plusieurs variantes du problème de l’alignement de deux ordres de gènes par duplications et pertes, ainsi que de l’approximabilité de ces problèmes. Nous rappelons ensuite les algorithmes exacts, en temps exponentiel, existants, et développons des heuristiques efficaces. Nous proposons, dans un premier temps, DLAlign, une heuristique quadratique pour le problème d’alignement de deux ordres de gènes par duplications et pertes. Ensuite, nous présentons, OrthoAlign, une extension de DLAlign, qui considère, en plus des duplications et pertes, les réarrangements et les substitutions. Nous abordons également le problème de l’alignement phylogénétique de génomes. Pour commencer, l’heuristique OrthoAlign est adaptée afin de permettre l’inférence de génomes ancestraux au noeuds internes d’un arbre phylogénétique. Nous proposons enfin, MultiOrthoAlign, une heuristique plus robuste, basée sur la médiane, pour l’inférence de génomes ancestraux et d’histoires évolutives d’un ensemble de génomes représentés aux feuilles d’un arbre phylogénétique. / During the evolution process, genomes accumulate mutations that may affect the genome at different levels, ranging from one base to the overall gene content. Global mutations affecting gene content and organization are mainly duplications, losses and rearrangements. By comparing gene orders, it is possible to infer the most frequent events, the gene content in the ancestral genomes and the evolutionary histories of the observed gene orders. In this thesis, we are interested in developing new algorithmic methods based on an alignment approach for comparing two or a set of genomes represented as gene orders and related through a phylogenetic tree, based on global mutations. We study the theoretical complexity and the approximability of different variants of the two gene orders alignment problem by duplications and losses. Then, we present the existing exact exponential time algorithms, and develop efficient heuristics for these problems. First, we developed DLAlign, a quadratic time heuristic for the two gene orders alignment problem by duplications and losses. Then, we developed OrthoAlign, a generalization of DLAlign, accounting for most genome-wide evolutionary events such as duplications, losses, rearrangements and substitutions. We also study the phylogenetic alignment problem. First, we adapt our heuristic OrthoAlign in order to infer ancestral genomes at the internal nodes of a given phylogenetic tree. Finally, we developed MultiOrthoAlign, a more robust heuristic, based on the median problem, for the inference of ancestral genomes and evolutionary histories of extent genomes labeling leaves of a phylogenetic tree.
