Avledning vid procedursmärta hos barn och ungdom : Sjuksköterskors kunskap, användning och inställning till metoden

Syfte: Att beskriva och analysera sjuksköterskors kunskap, användning av och inställning till  avledning som en smärtlindrande omvårdnadsmetod vid procedursmärta hos barn.  Metod: Fyrtioen sjuksköterskor, varav sexton specialistutbildade, från tre olika pediatriska  slutenvårdsavdelningar besvarade en enkät som sedan analyserades med hjälp av innehållsanalys  samt i statistikprogrammet SPSS där t-tester och Spearman’s rangkorrelationskoefficient beräknats.  Resultat: En stor andel utav sjuksköterskorna hade hört talas om avledning och använde sig  regelbundet av metoden. Två tredjedelar, främst specialistutbildade, hade hög kunskap kring  avledning och ansåg sig använda metoden i hög utsträckning. Flertalet, främst specialistutbildade,  var positivt inställda till metoden. Specialistutbildade sjuksköterskor hade högre kännedom och  kunskap än grundutbildade sjuksköterskor. Ett samband fanns mellan positiv inställning till  avledning och hög användning av metoden.  Slutsats: De flesta sjuksköterskorna kände till avledning, men kunskapsnivån var dock inte lika  hög. Deltagarnas självrapporterade användning utav avledning var hög, men fler  observationsstudier behövs kring ämnet. Trots att flertalet var positiva till avledning fanns ett antal  sjuksköterskor som var negativa till metoden och för att höja sjuksköterskors kunskapsnivå och  inställning till avledning bör man belysa och implementera de forskningsresultat som finns till  klinisk verksamhet. / Aim: To describe and analyze nurses’ knowledge, use of and attitudes towards distraction therapy  as an analgesic nursing method during procedural pain in children.  Method: Forty-one nurses, including sixteen pediatric nurse specialists, from three different  pediatric wards completed a questionnaire which was analyzed using content analysis and the  statistical program SPSS where t-tests and Spearman's rank correlation coefficient was calculated.  Results: A large proportion of the nurses had heard of distraction therapy and used it regularly in  their practice. Two thirds of the participants, mostly pediatric nurse specialists, had a high level of  knowledge about the distraction therapy and reported that they used the method to a large extent.  The majority, primarily pediatric specialists, had a positive attitude toward the method. Pediatric  nurse specialists had higher awareness and knowledge than regular nurses. A correlation was found  between positive attitude to distraction therapy and high use of the method.  Conclusion: Most nurses had a high awareness about distraction therapy, but the level of  knowledge was not as high. The participants self-reported use of distraction therapy was high, but  there is a need of further observational studies regarding the subject. Although a majority were  positive toward distraction therapy, a number of nurses had a negative attitude toward the method  and to increase nurses’ knowledge and attitudes current research results should be emphasized and  implemented into clinical practice.

BAKGRUND: HIV (Humant Immunbrist Virus) är en infektion som är spridd över hela världen. År 1981 då HIV beskrevs för första gången associerades smittan med homosexualitet och bisexualitet bland män samt intravenöst missbruk. Trots mer kunskap om hur viruset sprids förekommer fördomar och diskriminering av patienter med HIV från både allmänheten och vårdpersonalen. Tidigare forskning visar att det finns en ovilja bland vårdpersonal att vårda patienter med HIV. SYFTE: Studiens syfte var att beskriva HIV-patienters upplevelser av mötet med vårdpersonal. METOD: En litteraturöversikt av befintlig forskning inom det specifika området genom granskning av 12 vetenskapliga artiklar. RESULTAT: Studien resulterade i tre teman; kränkning, avståndstagande och rädsla samt professionellt bemötande. Många patienter har negativa upplevelser av mötet med vårdpersonal såsom stämpling, skuldbeläggning, nekad vård och ignorans. Även många positiva erfarenheter framkom där patienterna upplever en god relation med sin vårdgivare. KONKLUSION: Trots mer kunskap om HIV upplever patienterna ett osympatiskt bemötande från vårdpersonalen. Kunskap om patienternas upplevelser av mötet med vårdpersonal har betydelse för att förbättra förståelsen för patientens situation och därmed även omvårdnaden. Many patients have negative experiences of the encounter with health personnel such as labeling, blame, denied care and ignorance. The positive experiences showed that patients experienced a good relationship with their caregivers. CONCLUSION: Despite more awareness about HIV, patients experienced unsympathetic treatment from health care provider. Knowledge of patients' experiences of the encounter with health professionals is important to improve understanding of the patient's situation and the care. It is therefore important that health professionals obtain knowledge about how their treatment affects the patient’s satisfaction of care.

Mental health nurse prescribing : using a constructivist approach to investigate the nurse patient relationship

Background: The interpersonal relationship between nurses and clients is seen as the central element or core activity of mental health nursing. Without this relationship therapeutic alliance cannot take place. Concern has been expressed that nurse prescribing could have a negative impact on the nurse patient relationship and result in the nurse sacrificing nursing skills for the prescribing role. Aim: The aim of this study was to explore the nurse patient relationship in the mental health setting when the nurse is a prescriber. In order to do this a comprehensive literature review was undertaken and views of participants were explored and relationships described. Methodology and methods: Nurse prescribers were sent questionnaires to gather demographic data and basic qualitative data. Focus groups and interviews were undertaken within a large NHS Foundation Trust. A constructivist approach was used with 57 participants including nurse prescribers, pharmacist prescribers, nurse managers, clients and doctors. A discussion guide and an iterative approach were used to clarify findings. Data analysis was guided by a Framework approach. Findings: The majority of clients preferred to have their nurse prescribe for them. Trust was highly valued within the pre-established relationship and clients found nurses easier to talk to about their medication than doctors. Nurse prescribers placed high importance on being able to reduce and discontinue medication for the client, terming this ‘un-prescribing’. Nurse prescribers were uncomfortable with the concept of power, preferring to use the term ‘empowerment’. All groups of participants were unanimous that nurse prescribers continued to provide care and that they had not moved from a traditional ‘caring’ role to a ‘medical’ curing role and importance was placed on the therapeutic alliance between nurse prescribers and clients. Conclusion: Rather than detracting from the nurse patient relationship, results from this study suggest that nurse prescribing enables the mental health nurse prescriber to provide more holistic care than previously. The action of ‘un-prescribing’ may indicate a new culture around mental health nurse prescribing
