Hypermobility syndrome and its connection with nerve entrapment syndromes, the example of the thoracic outlet syndrome

 ABSTRACT  Background: Since its first mention by Kirk et al. in 1967 and its recognition as a full- fledged rheumatologic disorder, the hypermobility syndrome (HMS) has been increasingly investigated and reported in the scientific literature. Expeditiously renamed benign joint hypermobility syndrome in the patent absence of life-threatening complications, its relatively innocuous character has been progressively reconsidered. In fact, the HMS tends to date to be considered analogous to the Ehlers-Danlos syndrome-hypermobility type, a heritable disease of connective tissue, and therefore emerges as a chiefly rheumatologic disorder with possible widespread reverberations in practically all organs and systems. The condition thence goes beyond the sole involvement of the musculoskeletal system and is recurrently associated with seemingly-unrelated and more or less severe conditions (cardiovascular, pulmonary, gastro- intestinal…). However, neurologic implications of the hypermobility syndrome remain poorly documented, particularly those regarding the peripheral nervous system. Stroke can cause motor activity impairment and also senzoric and cognitive disorders which lead into restriction of participation of activities of daily living and also reduction of social and community activities. Functional improvment of upper and lower extremity after stroke is a very complex proces which has to be solved in multidiscplinary and multifactorial approach in result of every stroke survivor to live in the most independent way as possible. Robot-assisted therapy for motor recovery could be a successful way to improve motor activity of stroke survivors. Amadeo Instrument is one of robot-assisted therapy for hand rehabilitation of stroke survivors. The title of my thesis is "Historical development of rural settlements in the chosen lokality". As the name mentions I deal with the development of rural settlements in my work. I chose the villages Horní Dvořiště and Pištín for processing. The theoretical part deals with the historical development of rural settlements through different periods. Furthermore, I have described the factors that affect the settlement of our landscape. In the other chapters, the typical building elements of folk architecture are described. The penultimate part is dedicated to folk buildings by regions and at the last part, I have devoted to the South Bohemian folk architecture which I have described according to individual areas. The practical part contains description of both villages, Horní Dvořiště and Pištín. The development is described from the perspective of cadastral territory, therefore I have also mentioned Czech Herslak in the chapter about Horni Dvoriste because it belongs to Horni Dvoriste. The methods of description and collection of facts that I have been used for my work are recorded subsequently.

This thesis deals with the influence of microclimatic conditions (air temperature near tree/shrub top, temperature of the stem and root zone) on the wood phenology of trees (Picea abies) and shrubs (Pinus mugo) growing on the same site in the alpine tree line (ATL) ecotone. Phenological and microclimatic research was carried out in the Giant Mountains at 1,370 m a. s. l. in the 2017 vegetation period, with the aim to compare the course of wood formation depending on the microclimate, to determine the temperature thresholds of wood growth and differences in the morphology of trees and shrubs. Based on the microclimate analysis of trees and shrubs, individual variables were determined (average, average minimum, average maximum, daily amplitude of air, stem and root zone temperatures) characterizing the growing season. The collection of micro-drills of trees and shrubs took place from April to October in an interval of 6 - 11 days, which were subsequently processed and examined in the laboratory. The following phenophases of wood development were distinguished: cambial, enlarging, wall-thickening, mature cells and cells of the previous ring. Interpretation of changes of humeral asymmetry during the Holocene period was based on the analysis of upper limb muscle activity during cereal grinding using the saddle quern and rotary quern. The aim of the diploma thesis was to test if the dominant upper limb muscle activity during cereal grinding using the saddle quern and rotary quern might be estimated by measuring the activity of musculus deltoideus (pars clavicularis), musculus infraspinatus, musculus pectoralis major and musculus triceps brachii (caput longum). Using surface electromyography, we have analyzed activity of musculus biceps brachii, musculus deltoideus (pars clavicularis), musculus deltoideus (pars acromialis), musculus deltoideus (pars spinalis), musculus pectoralis major, musculus infraspinatus, musculus triceps brachii (caput laterale) and musculus triceps brachii (caput longum) during cereal grinding using the saddle quern and rotary quern in 25 subjects. Consistent with our prediction, musculus biceps brachii was the least active muscle during saddle quern grinding and clockwise rotary grinding, therefore it is possible to exclude musculus biceps brachii from the sample of analyzed muscles. The diploma thesis deals with the problem of muscle imbalances of middle school aged children and its relationship with their free time activities. The selected exercises were tested for the muscles involved in the lower and upper cross syndrome. On the basis of the measured results, the frequency of shortened / weakened muscles and muscles in the physiological standard were compared. A questionnaire survey was used to identify preferences of children concerning leisure activities. Based on the data obtained from the comparison of the data found in the questionnaire and the results measured during the testing of the muscles, the following topics were addressed: Whether the muscular condition is influenced by the sex, the frequency of sport activities performed and the type of sport practiced. Introduction: Multiple sclerosis is a chronic autoimmune neurodegenerative affection of the central nervous system with various neurological symptoms. Disability of upper extremities with other symptoms leads to impaired ability to work and ADL-activities of daily living. The purpose of the pilot study was to evaluate the effect of hand training with the Hand Tutor® rehabilitation system. Methods: The study involved 9 probands, completed 6 probands (2 females and 4 males) at an average age of 36 (SD 4), with a mean disease duration of 13 years (SD 4). Patients received a series of 10 Hand Tutor® therapies, 1-2 times a week. The effect of the therapy was evaluated using the Nine Hole Peg Test (9HPT), the Modified Frenchay Scale and the subjective evaluation of the probands. Results: Before the therapies, the average performance in the 9HPT test was 85 seconds and 83 points in the Modified Frenchay scale. After the therapies, the average 9HPT performance was 67 seconds and 86 points in the Modified Frenchay scale. According to the subjective evaluation of probands, the therapy had a positive effect on the tremor of the upper limb and on the use of the upper limb in the ADL. In metric spaces, we can replace distributional gra- dients, whose de nition depends on the linear structure of Rn , by upper gradients that control the functions' behavior along all recti able curves. is gives rise to the so-called Newtonian spaces. e summability condition, considered in the thesis, is expressed using a general Banach function lattice quasi-norm and so an extensive framework is built. Sobolev-type spaces (mainly based on the Lp norm) on metric spaces, and Newtonian spaces in particular, have been under intensive study since the mid- s. Standard toolbox for the theory is set up in this general setting and Newto- nian spaces are proven complete.
