Primary Health Care (PHC) Registered Nurses (RNs) as facilitators of healthcare access for recent immigrants in Ontario: An interpretive descriptive study

Background and Objectives: Recent immigrants experience unique challenges when accessing health services. Registered Nurses (RNs) working in primary health care (PHC) have the competencies and are well-positioned to facilitate healthcare access for immigrants. Aim: This study explored how PHC-RNs in Ontario support healthcare access and address barriers among recent immigrants. Methods: This study used a qualitative, interpretive descriptive approach and was informed by Levesque et al.’s (2013) access to care framework. Semi-structured interviews were conducted with 10 PHC RNs practising in Toronto, Ottawa-Gatineau, Hamilton, Kitchener-Cambridge-Waterloo, and London. Data were collected and analyzed concurrently using an inductive and deductive approach. Results: Findings show that PHC-RNs play instrumental roles in supporting recent immigrants through facilitating healthcare access across 9 dimensions of Levesque et al.’s (2013) access framework: (1) appropriateness, (2) availability and accommodation, (3) ability to perceive, (4) ability to engage, (5) ability to seek, (6) ability to reach, (7) affordability, (8) ability to pay, and (9) acceptability. RNs addressed significant challenges to health service access for recent immigrants, including language, geographical, and financial barriers. Several opportunities were identified to enhance the PHC RN clinical practice role to improve the delivery of PHC for recent immigrants, such as increasing organizational resources, utilizing their expertise and role beyond the PHC clinic, and promoting culturally appropriate care. Conclusions: PHC-RNs are key facilitators of healthcare access for recent immigrants by coordinating their care, educating, and connecting this population to services across the health system. However, there are opportunities to optimize nursing roles and more effectively utilize their scope of practice within interdisciplinary teams to promote the health of immigrants. / Thesis / Master of Science in Nursing (MSN)

Social Supports, Stress and Birth Outcomes among Latina Mothers in Pinellas County, Florida

Social supports are linked in public health research to improved birth outcomes. This study explored the relationship of social supports, stress and birth outcomes among pregnant Latinas in Pinellas County, Florida. A sample of 411 Healthy Start women at risk of poor birth outcomes participated in this study (99 Latinas, 142 Black, and 158 White). Study methods included ANOVA, Principal Component Analysis, multivariable regression, logistic regression, and structural equation modeling to identify significant associations between social support scores, stress scores, demographics and health risk factors with infant birth weight, preterm and small for gestational age by ethnic group. Study findings indicated there was a direct association between social support and stress across all ethnic groups. However, many confounding variables did not have an effect in the study sample. Latina study participants exhibited significantly lower mean social support scores compared to White and Black participants (p=0.000). Latinas also presented higher stress scores that were significantly different from White and Black participants (p=0.000). The study also found ethnic differences in stress level perceptions using the Perceived Stress Scale. Recommendations for public health included conducting additional studies to assess if the study variables have an impact on a different population, exploring different ethnic interpretations of stress, using repeated measures to assess stress in high risk populations and considering using alternate stress measures such as biological markers and stress life event scales to assess social support, stress and birth outcomes.

Retirement and the Healthy Immigrant Effect Among Older People : A comparison of health outcomes using SHARE data

While the healthy immigrant effect (HIE) has been seen throughout the western world in the adult population; research on the older population shows an inverse relationship between self-reported health and immigration status – i.e., in many instances, a health disadvantage for older immigrants. Explanations for this vary from a lack of selection into population, among older immigrants, to a duration effect; such that perceived health advantages seen in early life dissipate upon reaching older age. While these are the widely accepted reasons for the disappearance of the HIE among older people, not all mitigating factors have been examined in detail. This study aims to identify if there is an association between retirement and the healthy immigrant effect seen in Europe. It uses SHARE data for 27 European countries to examine self reported health (SRH) outcomes & presence of longterm illness’ (LTI) for population groups aged 55-74. The findings indicate the presence of a greater health disadvantage for retired immigrants (versus native-born retirees) as compared with those who are not retired. Additionally, better health outcomes (compared to native-born individuals) were seen for immigrants who retired late (after the age of 65) versus those who retired early. Retirement due to poor health was examined as an explanation for these findings, but it did not appear to affect the results, suggesting the observed patterns are most likely caused by an alternative factor relating to retirement. Further research is recommended to identify these factors.

Uncovering the Role of Community Health Worker/Lay Health Worker Programs in Addressing Health Equity for Immigrant and Refugee Women in Canada: An Instrumental and Embedded Qualitative Case Study

“Why do immigrants and refugees need community health workers/lay health workers (CHWs) if Canada already has a universal health care system?” Abundant evidence demonstrates that despite the universality of our health care system marginalized populations, including immigrants and refugees, experience barriers to accessing the health system. Evidence on the role of CHWs facilitating access is both lacking and urgently needed. This dissertation contributes to this evidence by providing a thick description and thorough analytical exploration of a CHW model, in Edmonton, Canada. Specifically, I examine the activities of the Multicultural Health Brokers Co-operative (MCHB Co-op) and its Multicultural Health Brokers from 1992 to 2011 as well as the relationship they have with Alberta Health Services (AHS) Edmonton Zone Public Health. The research for this study is based on an instrumental and embedded qualitative case study design. The case is the MCHB Co-op, an independently-run multicultural health worker co-operative, which contracts with health and social services providers in Edmonton to offer linguistically- and culturally-appropriate services to marginalized immigrant and refugee women and their families. The two embedded mini-cases are two programs of the MCHB Co-op: Perinatal Outreach and Health for Two, which are the raison d’être for a sustained partnership between the MCHB Co-op and AHS. The phenomenon under study is the Multicultural Health Brokers’ practice. I triangulate multiple methods (research strategies and data sources), including 46 days of participant and direct observation, 44 in-depth interviews (with Multicultural Health Brokers, mentors, women using the programs, health professionals and outsiders who knew of the work of the MCHB Co-op and Multicultural Health Brokers), and document review and analysis of policy documents, yearly reports, training manuals, educational materials as well as quantitative analysis of the Health Brokers’ 3,442 client caseload database. In addition, data include my field notes of both descriptive and analytical reflections taken throughout the onsite research. I also triangulate various theoretical frameworks to explore how historically specific social structures, economic relationships, and ideological assumptions serve to create and reinforce the conditions that give rise to the need for CHWs, and the factors that aid or hinder their ability to facilitate marginalized populations’ access to health and social services. Findings reveal that Multicultural Health Brokers facilitate access to health and social services as well as foster community capacity building in order to address settlement, adaptation, and integration of immigrant and refugee women and their families into Canadian society. Findings also demonstrate that the Multicultural Health Broker model is an example of collaboration between community-based organizations and local systems in targeting health equity for marginalized populations; in particular, in perinatal health and violence against women. A major problem these workers face is they provide important services as part of Canada’s health human resources workforce, but their contributions are often not recognized as such. The triangulation of methods and theory provides empirical and theoretical understanding of the Multicultural Health Brokers’ contribution to immigrant and refugee women and their families’ feminist urban citizenship.
