The tenn Acute Coronary Syndrome (ACS) encompasses a spectrum of patients who present with chest discomfort or other symptoms caused by myocardial ischemia or infarction. Since critical or acute care nurses care for such patients, they should have a thorough knowledge of ACS pathophysiology and current treatments for ACS The purpose of this research study is to explore and describe the knowledge level that the critical care nurses in a state hospital in East Texas feel they have regarding ACS. This study was quantitative, descriptive and contextual in design, in which a sample survey was performed, using a questionnaire based on a literature study. The response of most ofthe critical care nurses tested was that they felt they had insufficient knowledge. Este estudo teve como objetivos, construir um aplicativo (software) com o conteúdo do Nursing Activities Score (NAS), e utilizá-lo para caracterizar a carga de trabalho de enfermagem da UTI, diagnosticando as necessidades assistenciais dos pacientes da unidade. Tratou-se pesquisa de delineamento misto, sendo prospectivo na análise da evolução da carga de trabalho de enfermagem e estudo do tipo série histórica na análise diária na UTI. A construção do aplicativo com o NAS resultou em um sistema com coleta e administração de dados dos pacientes e permitiu realizar a interface destes em composições gráficas. Na caracterização da amostra de pacientes, 54,5% eram do sexo feminino, a média de idade do estudo foi de 51 anos. Foram mais freqüentes as internações para tratamento cirúrgico eletivo (58,5%). O tempo médio de internação foi de 5,9 dias e a taxa de mortalidade foi de 23,6%. Quanto à caracterização da carga de trabalho de enfermagem, a média foi de 60,3. A média do NAS foi maior em pacientes do sexo masculino (61,5 pontos), menores de 18 anos (65,9 pontos), em pacientes submetidos ao tratamento clínico (66,1 pontos) e em pacientes com mais de 6 dias de internação (63,3 pontos), com p = 0,0122, e em pacientes que evoluíram á óbito (74,1 pontos) com p < 0,0001. A média de horas de assistência de enfermagem por paciente foi de 14,5 horas por paciente por dia. O maior valor do NAS em dias da semana foi aos sábados (64,0 pontos) e nas quintas-feiras (62,8 pontos). M.A. (Clinical Psychology) / The purpose of this study was to establish whether a cognitive intervention with patients in an intensive care unit in a general hospital would lead to a decrease in the commonly observed "intensive-care syndrome", which previously led to a deterioration in patients’ condition irrespective of their physiological condition. In order to effect this study an intervention focused at the alleviation of cognitive factors possibly causing the intensive care syndrome was constructed and rendered on audio-tape. An experimental and control group were randomly selected and pre-tests were performed on these groups. After the pre-tests were performed, the cognitive intervention was applied and post-tests on the same measures were performed. The results were subjected to separate analysis of covariance where baseline levels of the dependent variables were used as covariates. No significant results were found, implying that cognitive factors might not be playing the role they were expected to play, and that organic-physiological factors might play a more important role in the causation of the intensive-care sydrome. In addition, it was hypothesised that patients used their own "cognitive intervention" in order to alleviate the severity of symptoms observed in the intensive care unit. Further research is suggested in order to determine the relevant factors inherent in such a condition.

D.Cur. (Intensive General Nursing) / Critical care nursing is cost-effective quality nursing to the critical ill patient. This nursing is inftuenced by certain expectations as well as the professional-ethical authorisation. The aim ofthis study is to describe and explore the expectations and the professional-ethical authorisation of the functions of the critical care nurse to enable the formulation ofguidelines for the scope of practice for the critical care nurse within South African context. Phase I was to determine the expectations of the critical care nurse, the nursing service managers and the doctors with regard to the functions of the critical care nurse. A focus group interview was held with a group of experts in the field of critical care. The results were used to compile a questionnaire. This questionnaire was sent to the critical care nurses, the nursing service managers and the doctors in South Africa for completion. From these results the functions of the critical care nurse were determined. Phase 2 is to analyse the expectations (Phase 1) according to the professional-ethical authorisation of the critical care nurse. Firstly, literature was explored and described to determine the professional ethicalauthorisation regarding the accountability ofthe critical care nurse. Secondly, the functions were analysed according to the professional-ethical authorisation. The analysis showed that except for prescribing medication, altering medication dosages and drawing blood samples from the critical ill patient, the critical care nurse is practicing within her professional-ethical authorisation. Phase 3 was to formulate guidelines for the scope of practice for the critical care nurse within a South African context. Through usage of the data (Phase I and 2) the scope of practice was formulated. Guidelines were formulated for the practise, education and research regarding the limitations of the professional-ethical authorization and the implementation of the scope of practice for the critical care nurse.
